Abstract:
Schwindel ist ein häufiges Symptom und oft nicht einfach zuzuordnen. Wir beschränken uns im Folgenden auf gerichtetete Schwindelformen, d. h. Schwindel als Illusion einer Bewegung. Hauptaufgabe des Neurologen ist es, zentralnervöse, oft lebensbedrohliche Störungen von peripher-vestibulären zu unterscheiden. Eine potenziell auch lebensbedrohliche Erkrankung auszuschließen bedeutet dabei oft, eine benigne peripher-vestibuläre Erkrankung sicher diagnostizieren zu können. In Anlehnung an J. C. Kattah [1] und Kevin A. Kerber [2] schlagen wir mit dieser Übersicht eine diagnostische Vorgehensweise vor, welche durch klinische Beobachtung eine periphere Erkrankung zu erkennen oder den Verdacht auf eine zentrale Genese zu erhärten erlaubt.

Abstract:
Zusammenfassung. Schwindel ist nach Kopfschmerzen eines der häufigsten Symptome, die zu einem Arztbesuch führen. Ausgehend von dieser Tatsache stellen sich ca. 25 % der Patienten mit akuten Schwindelbeschwerden in einer Notfallaufnahme vor. Die Herausforderung der betreuenden Kollegen besteht darin, ein akut zentral-vestibuläres Syndrom nicht zu übersehen, was verheerende Folgen mit sich bringen könnte. Eine sorgfältige und strukturierte Anamneseerhebung zusammen mit der klinischen Untersuchung trägt bereits zu ca. 90 % zur Diagnosestellung bei und ist sensitiver als die diffusionsgewichtete Kernspintomografie. Der Fokus der Anamnese liegt bei der Dauer der Symptome und deren Triggerfaktoren. Ein Bedside-Test, bestehend aus Kopfimpulstest, Nystagmusprüfung, Test auf Skew und zusätzliche Hörprüfung mittels Fingerreiben, hilft bei der differenzialdiagnostischen Eingrenzung im Hinblick auf die Differenzierung zwischen akutem zentralen und peripheren Schwindelsyndrom.

Zusammenfassung: Gegenstand und Ziel: Der Fallbericht beschreibt die serielle Messung und den Vergleich von peripher venösem Hämatokrit mit dem Hämatokrit der Abdominalflüssigkeit bei einem Hund mit traumabedingtem Hämoabdomen. Diese Methode half bei der Entscheidung zwischen konservativer und operativer Therapie. Material und Methoden: Bei einer 18 Monate alten Mischlingshündin wurde nach einem Autounfall mit Hämoabdomen vor und während der Stabilisation mit Flüssigkeitstherapie regelmäßig der abdominale und peripher venöse Hämatokrit bestimmt. Ergebnisse: Bei Präsentation betrug der peripher venöse Hämatokrit 39% und der abdominale Hämatokrit 42%. Nach Flüssigkeitstherapie sank der peripher venöse Hämatokrit auf 20%, während der abdominale Hämatokrit bei 40% stabil blieb. Die Hündin wurde konservativ behandelt. Schlussfolgerung und klinische Relevanz: Der Vergleich von Hämatokritwerten in abdominaler Flüssigkeit und venösem Blut kann hilfreich sein bei der Unterscheidung zwischen einer inaktiven abdominalen Blutung, die sich konservativ behandeln lässt, und einer ungestoppten abdominalen Blutung, die eine chirurgische Intervention notwendig macht.

Schwindel und Gleichgewichtsstörungen, beides häufige Symptome, können oft anhand einer exakten Anamnese unterschieden werden. Diese ist auch richtungsweisend, um systematischen von unsystematischem Schwindel zu unterscheiden. Peripher vestibuläre Funktionenstörungen äußern sich typischerweise durch systematischen Drehschwindel, der je nach Ursache unterschiedlich lang persistiert und in der Regel von typischen Augenbewegungen, den Nystagmen, begleitet wird. Der Nystagmus ist beim Vorliegen von peripher vestibulären Funktionsstörungen richtungsbestimmt und meistens horizontal oder rotatorisch, wohingegend ein rein vertikaler Nystagmus an eine zentral vestibuläre Störung denken lassen muss, ebenso wie begleitende Kopfschmerzen oder zusätzliche Hirnnervenläsionen. Beim Vorhandensein von letzteren ist eine neurologische Untersuchung und gegebenfalls eine Bildgebung indiziert, um eine zentrale Pathologie auszuschließen. Typische peripher vestibuläre Funktionsstörungen wie der benigne, paroxysmale Lagerungsschwindel, die Neuronitis vestibularis und der Morbus Menière werden besprochen; ihre Diagnose und Therapie, ebenso ihre Therapie und die Diagnosestellung.

Abstract:
The aim of the present project was to analyse the link between peripheral posture, optics, optics and refractive error progression. Preliminary studies were conducted to ensure that peripheral aberrometry is valid for further analysis. The repeatability of the IRX-3 for peripheral aberrometry was asgood as for central measurements and the recalculation of elliptical pupils did not seem to be necessary for measurements up to 20 degrees eccentricity. Higher order aberration measurements werecomparable to other studies. Eye and head movements as well as working distance did not differ significantly between myopes and non-myopes. However, there was some evidence, that forward bending of the head during reading increases in association with higher refractive error progression rates. The link between central higher order aberrations and refractive error development was analysed by comparing higher order aberrations between isometropes and anisometropes. This analysisdid not show any significant association of higher order aberrations on the development, as no major differences were found between the two groups. For central vision, changes in biometric parameters during accommodation were analysed. It was found that biometric parameters change similarly in myopes and non-myopes. Peripheral accommodation was found to differ between myopes and emmetropes indicating that there might be an influence of peripheral refraction on myopisation. However, associationbetween peripheral refraction or peripheral aberrations and refractive error progression were not significant. The reason for this observation might be the low refractive error progression (0.04± 0.29 D in myopes and -0.12 ± 0.38 D in emmetropes) during one year in the study population.

The design of the APU (Auxiliary Power Unit) for the F-35 JSF (Joint Strike Fighter) focused on minimizing size and weight while meeting stringent performance goals. To help realize that goal, a unique turbine scroll was designed. The scroll design delivers air from the combustor to the turbine inlet with minimal loss and flow distortion while minimizing design space. CFD (Computational Fluid Dynamics) results of scroll total pressure loss and exit peripheral distribution of total pressure, Mach number, and flow angle are presented. Rig tests were utilized for measuring and validating the computed total pressure and Mach number distributions around the periphery of the scroll exit. Comparisons of the CFD simulations and test data indicate strong correlation in values of average total pressure loss, local total pressure loss and Mach number around the exit periphery.

Abstract:
A widely held view of thrombopoiesis is that platelets arise from fragmentation of the periphery of mature megakaryocytes (MK’s). Evidence against this concept was provided by Zucker-Franklin, who showed that platelet plasma membrane is different from that of the MK (freeze-fracture, membrane antigens). Several authors have described contractile processes in MK’sWe have performed detailed electronmicroscopic studies of the numerous small MK's of a subject with congenital macrothrombocytopenia. In the young granular MK’s a central zone with organelles and a thick peripheral zone without organelles can be observed. The absence of elements of the demarcation system in the peripheral zone argues against derivation of the demarcation system from the megakaryocyte plasma membrane. In the mature granular MK’s the heart of the central zone is not occupied by the nucleus, but by an area, free of organelles and membranes, containing fibrillar structures and the centrioles. Elements of the demarcation system, delineating platelet territories, radiate from this fibril-rich area, but do not extend into the very thin peripheral zone. In the platelet producing MK’s the peripheral zone is thicker and the central fibril-rich area is surrounded by separated platelet territories. The peripheral zone shows several openings. In the old MK’s the nucleus is only surrounded by a markedly thickened peripheral zone, which seems to result from contraction of the more extended peripheral zone of the platelet producing MK's.From these observations we conclude that, at least in this patient, platelets are formed inside the MK and are extruded through openings of the peripheral zone. Further studies with cytoskeleton markers are required to confirm that (!) the fibril-rich heart governs the organisation of platelet territories and (2) that platelet extrusion results from contraction of the peripheral zone, the latter however not giving rise to platelets.

Abstract:
Abstract This paper presents a novel process that uses RF plasma spraying and premixed elemental powders to produce intermetallic matrix composites in situ without having to add reinforcement fibers or particles. Splats were collected on a stainless steel substrate and were analyzed to determine if nitrides had formed in metal droplets during flight and how it affected splat morphology. The typical splat morphology of impinged Ti droplets is disk-type with an outer peripheral fringe. Aluminum splats, on the other hand, are classified into two categories: a disk-type with an irregular outer periphery and a semi-massive-type. Other composites produced and examined include TiAl, AlN, and Ti2AlN.

Abstract:
This work relates to an experimental, theoretical and numerical study of a turbulent flow with separated boundary layers between a rotating disc (rotor) and a stationary one (stator) without any superposed radial flow. The originality of this study is that the pre-swirl ratio at the periphery of the cavity (Kp) is lower than the core swirl ratio (KB) corresponding to the solid body rotation, as predicted by Batchelor in the case of infinite discs: this is what the authors call a rotor-stator system with low pre-swirl ratio. Under these conditions, recent works have shown that the core swirl ratio (K) is a decreasing function of the radius, at least in the peripheral region. This behaviour has rather been observed in the cavities with superposed centripetal radial flow. In the present paper, this flow property is explained starting from an asymptotic approach which leads to step-by-step analytical computation method of the radial distribution of the core swirl ratio (K) and of the static pressure on the stator side p*. The validation of the theory is based both on experimental and numerical results. The experimental tests are carried out in a rotor-stator cavity for different values of the pre-swirl ratio, which is done by geometrical changes of the periphery of the system. The experimental data mostly include the velocity measurements and the turbulent correlations by hot-wire anemometry in interstice between the discs, as well as the static pressure measurements on the stator side. Comparisons with predictions from the CFD code Fluent are also provided. The numerical simulations are performed using the two-equation k – ω SST turbulence model, assuming a 2D-axisymmetric steady flow, in a domain corresponding to the inter-disks spacing and the peripheral outer region of the cavity. Computed and experimental values are in good agreement with the theoretical results.

Abstract:
In some engines, corotating gas–turbine discs are cooled by air introduced at the periphery of the system. The air enters through holes in a stationary peripheral casing and leaves through the rim seals between the casing and the discs. This paper describes a combined computational and experimental study of such a system for a range of flowrates and for rotational Reynolds numbers of up to Reϕ = 1.5 × 106. Computations are made using an axisymmetric elliptic solver, incorporating the Launder–Sharma low–Reynolds–number k–ε turbulence model, and velocity measurements are obtained using laser–Doppler anemometry. The stationary peripheral casing creates a recirculation region: there is radial outflow in boundary layers on the discs and inflow in the core between the boundary layers. The radial extent of the recirculation region increases as the flow rate increases and as the rotational speed decreases. In the core, the radial and tangential components of velocity, Vr and Vϕ, are invariant in the axial direction, and the measured values of Vϕ conform to a Rankine–vortex flow. The agreement between the computed and measured velocities is not as good as that found for other rotating–disc systems, and deficiencies in the turbulence model are believed to be responsible.

Abstract:
The research is based on the statement that retina produces the proper level of electrical activity, sourcing visual system. I started the research with partial darkening of different parts of the visual fields of humans to register possible psychological and physiological changes. The tested showed dramatically increasing variability and number of changes within just four exact types of darkening. More, emotional and physiological aspects of those changes were polarized into general acceptance and general rejection of a certain type of darkening in most of the individual tests. Thus the tested formed two opposite groups within every one of those types of darkening: a group with general negative reactions and a group with general positive ones. Further, those types of darkening turned out combined in pairs. General tune of reactions of most of the tested changed to strictly reverse within a pair of upper-lower types of darkening of peripheral vision and outer-inner ones as well. Between the pairs of types of darkening, there was no correspondence. The tested showed stability of their reactions during at least several months. Thus I may state a possibility of existence in the visual system of humans of two independent neuropsychological structures both having two alternative modes of functioning with a stable preference of just one of them in every individual case. If it is true, there may be a vision-based typology.
