Full textThe construction of the hospital nurse profession is related to the medicalization of the hospital institution, to the development of a technical and specialized practice and to the growing control of the state into national health management. The feminization of this profession, its moral definition and the subordinate position it holds in the technical division of labour are tightly connected to the monopolization of medecine by the medical profession and to the histroical background of the hospital wages policy. The gradual protection of the diploma and the juridical codification of the professional practice have made the nursing staff a state regulated paramedical profession. These regulations are strongly related to the elaboration of health political measures. The professional group identity is at stake in its position in the hospital division of labour towards doctors on one side and nursing assistants and helpers on the other. The nursing staff activity is indeed defined by an intimate collaboration with the proper medical work but also vy attending sick persons directly. Throuh a corporatist wayn, the nursing staff is trying to elaborate a new representation of their professionality which, for the first time, refers to the erudite profession model. Present changes in the hospital, resting on the reinforcement of management commitments contribute to the internal segmentation of this professional group and tend to oppose a grade of executives, invested with an important organizing power, against a majority of qualified female technicians

Full textFor several decades, the Quebec healthcare system has been confronted with major structural and economic transformations. These transformations have significant social impacts on health system staff, such as repeated job changes, shift work, job insecurity, and sometimes job loss. Possible psychological impacts include an increase of occupational stress. Occupational stress, or work suffering, of male nurses working in hospitals in Quebec is the central element studied in this thesis. Males remains a small minority in the nursing workforce, which may represent a source of stress in the workplace. Overall, sources of pleasure and suffering, ways to cope with suffering at work and male gender are the main concepts examined in this research. To access this information, semi-directed interviews were conducted with 20 male nurses from different hospitals in Quebec. The transcriptions were analyzed with the QDA Miner software, using Dejours'(1993) work psychodynamics theory. To complete the analysis of effective solutions against suffering at work as well as the role of gender, Antonovsky's (1979) theory of salutogenesis and Butler's (199) theory of performativity have been added to the theoretical framework. The results of the analysis show that the notion of care is central to the sources of pleasure for the respondents, precisely the ability to help and make a difference in the lives of patients. On the contrary, the economic and political situation of the management of the health network appears as one of the most stressful elements for the respondents, the latter having far-reaching impacts on daily work. Also, teamwork has a shared role, being both a source of pleasure and a source of potential suffering. Then, the most common solutions used by nurses to cope with suffering at work are sport, recreation, social support and reorganization of work tasks. Finally, regarding the role of the male gender in the dynamics of work-related suffering, the answers of the respondents indicated that the concept of being a man is very diversified. Some similar elements in the answers relate to the practice of nursing, which would be more pragmatic and solution-oriented when practiced by a man. The respondents, by their answers, show that, on the one hand, the clientele sees the entry of men as a good thing for the nursing profession, and on the other hand, that the expectations of the female colleagues and employer on the role that must occupy a male nurse remains strongly anchored in the traditional image of a man for these two actors. According to these findings, an urgent need remains in the employer's openness to collaborate with their employees in the search and the practical application of solutions to cope with the suffering at work.

Full textRecognition at work is based on gratitude judgments in order to reward the good work performed by the employee and positively influences the discretionary behaviors that are not explicitly reward with formal compensation systems.This doctoral dissertation studies the relationship between recognition at work and discretionary behaviors, particularly the helping and innovative behavior.This research aims to find out by which means recognition at work affects helping behavior an innovative behavior. To do so, we suggested and exam this fundamental hypothesis that employee recognition at work has positive effect on both discretionary behaviors namely helping behavior and innovative behavior.We tested this hypothesis by conducting a qualitative study on a sample of 20 nurses that we have completed with a quantitative study conducted among a sample of 215 Tunisians nurses. The results of our study allowed us to identify the most explanatory forms of recognition of the helping behavior and innovation behavior.The results of our doctoral dissertation showed that nurses are constantly in search of recognition and they are looking to make their work more visible in order to ensure different forms of recognition, some of this forms have proved the most explanatory of both discretionary behaviors.Our research is innovative and contributes to study the employee recognition as a factor that promotes the adoption of helping behavior and innovative behavior

Full textPreviously called ward sister because the activity is widely feminine and marked by the presence of nuns to this post, the nurse managers exercise an activity that exists in hospitals from their origin: the organization and supervision of care services. They can also practice in nurses' schools as master trainers. However, the missions and the status of these supervisors evolved a lot, because of medical advances and technology of care activity that generate an increasing specialization of tasks, but also political and organizational changes. If all nurse managers were first worked as a "caregiver", they are now carrying out duties related primarily to management. In addition, they must be trained in an executive’s school for a year and now belong to a unified category which comprises three sectors, together twelve allied trades. Nurse managers are currently still in France from the health paramedical care, care remains the heart of their business. We analyze in this research what is played in the passage of a trade to a nursing management function in construction skills. It is indeed moving from a culture where priority is the patient to a manager posture, accountant of a balanced budget-related activity. Full textThere are more and more constraints weighing on organizations, with more and more rules and procedures to respect. These constraints have a repercussion on employees, in particular through increased pressure and stress. The health sector has not been spared by this societal phenomenon and must face major challenges that lie ahead: contain increased health care costs, face technological and medical advances, deal with public pressure to improve the quality and security of health care and respond to a rising demand due to the ageing population.These challenges may give rise to absenteeism and turnover among caregivers. Absenteeism and turnover among nursing staff constitutes a major public health problem that is both costly and disrupting. Work carried out on absenteeism and departure intentions shows that complicated, intricate concepts are involved. Interpreting these behaviours through the concepts of organisational commitment, job satisfaction and role stressors allows one to grasp the complexity of these phenomena. Previous work done so far on role tensions has never jointly measured the three different forms of role tensions. In fact, the majority have concentrated on role conflict, to the detriment to the two other forms (role ambiguity and role overload). A simultaneous analysis of the three forms of role tensions, however, would be of great managerial interest as it would preferentially orientate management action on the form or forms of role tension which are the true explanatory factors in dependent variables. This would lead to better adaptation of the Human Resources Management practices and, in the end, boost their efficiency. This is the aim of our research. An exploratory qualitative study, supported by rigorous review of the literature, allowed us to construct research models that were tested in a quantitative study involving 300 nurses and around 30 health professionals. Our work contributes to shedding light on the determination of role tensions in the turnover phenomenon, or more precisely on the intent to leave of personnel, as well as job satisfaction and organisational commitment. Our findings are more disappointing in regard to absenteeism, which highlights the difficulty of its study and tends to confirm the complexity of this phenomenon

Full textThe french hospital workforce is aging. The present work aims at proposing and validating a framework accounting for the regulations that take place in relation to the age, health, and work conditions of care workers in a large hospital. Three kinds of regulation are examined : organizational (turnover within the hospital, special adaptation of certain working conditions in the workplace), collective (new task allocation within the team, interindividual help) and individual (changing working methods, use of new strategies). In order to identify these regulations, we carried out three macroscopic studies using the methods of epidemiology, work demography, and professional course analyses ; and three more microscopic studies based on interviews, questionnaires and work analysis. Concerning organizational regulations, results show that moves that occur along the professional career are influenced by the way some work constraints are tolerated. Indeed, the youngest care units (mean age of personnel) were found to be more concerned than older ones by shift-work, physical and emotional constraints. At the beginning of their career, care workers are confronted with high levels of job demands. This is likely to account for health problems and moves towards units were the type and level of jobs demands are better tolerated. Concerning collective regulations, several types of reciprocal help and task re-allocation between nurses were observed in young units, which serve to better manage physical and emotional stress. Individual regulations take various forms and their implementation is closely related to the worker's room for manoeuvre. Thus, the more severe the reported work constraints, the more difficult the implementation of nurse's competencies. The conclusion stresses the need to favour the three types of regulations as a mean to cope with increasing health problems which are themselves partly due to the increasing age of the personnel

Full textSince the seventies, the social relationship between physicians and nurses in public hospitals is construed in different ways according to class, sex and generation. On one hand, this relationship has been modified because of a still going on double process. More women are entering the medicine practice while the male rate in nursing is growing up. On the other hand, male and female physicians or nurses as well don't come from the same class as before. As the relative number of women and men vary in each field, the internal professional hierarchy with its class and gender divisions is reinforced. The work team structured along the class and gender lines are different according to the kind of hospitals and specialized branches of medicine. We realized a general study of social and gender division of labour in two hospitals and monographs on two paediatrics units based each one on the most usual gender configuration : male physicians/female nurses and male and female physicians/female physicians. Changes in that social relationship are related to the changes which occurred in the hospital system mainly because the pervasive presence of the law in the hospital structure itself as in the different medical and nursing practices

Full textThe aim of this work is to analyse the historical process of the hospital nursing professionalization in Portugal by defining its original characteristics, despite the French and British influences. Several factors contributed to the development of nursing as a profession especially the continuous changes in the Portuguese political and religious situation, the progress in medicine and hospitals' improvements. This. Analysis starts by describing the Portuguese historical background, underlining the role played by several religious orders and institutions such as the Misericordias in the care of the sick when nursing becomes a profession. The development of nursing as a profession requires first a training, then the organisation of a restricted group, socially recognized and with a unique identity. The existing original characteristics are revealed by examining the nursing schools from their creation until their standardisation in Portugal, pointing out their organisation as well as the training they provided. The profile and the results of the students of two private nursing schools (managed by Oporto's and Braga'sMisericordias) will be analysed in detail. We will study the evolution of the nursing profession until its regulation and the creation of a system controlling its practice. The nursing associations and their publications will also be taken into account since they contributed to exchange knowledge and to reinforce the group identity. Finally, the analysis of nursing practice at the Hospital GeraI de Santo Antonio (Oporto) enables us to understand how this practice evolved and what changed for the nursing professionals

Full textWe initialised this piece of research in order to explore methods that could humanise the implementation of NPM principles and make changes in hospital environments socially more acceptable. Compatible with a polyphonic management, favourable to change when people with different values and opposing interests have to co-operate around one same issue, Actor Network sociology became an obvious assessment matrix for a change process that we have been observing and conducting via two projects that aim to retain staff. .Lead in a transformative approach, our research and intervention is based on two similar case-studies undertaken in public hospitals that have to face chronic shortages in nursing staff in psychiatric wards. Our ethnographic connection with the field was possible thanks to a CIFRE. It enabled us to obtain all kinds of rich and diversified data. We thereby brought our contribution to identifying what determines the initial intentions of psychiatric nursing staff in France, thanks to the organisation of multi-disciplinary workshops. During the second part of the implementation of our projects, thought-out in total collaboration with the actors on the field, we determined the actual obstacles to change as well as change-facilitators

Full textBackground : Nursing workload assessment tools are widely used to determine nurse staffing requirements in the neonatal intensive care unit (NICU). However, several tools exist and it is unclear if they are equivalent. Objective : We aimed to compare three existing workload assessment tools and assess their association with mortality or morbidity among very preterm infants. Methods : Single-center retrospective cohort study of infants born <33 weeks and admitted to a 52-bed level 3 NICU in 2017 to 2018. Required nurse staffing was estimated for each shift using the Winnipeg Assessment of Neonatal Nursing Needs Tool (WANNNT) used as reference tool, the Quebec Provincial NICU Nursing Ratio (QPNNR), and the Canadian NICU Resource Utilization (CNRU). We evaluated correlation between tools using Pearson R. The association between NICU nursing provision ratio (actual number of nurses / recommended number of nurses per shift according to the tools used) during the first 24h, 7 days of hospitalization and whole hospital stay with mortality / morbidity was assessed using logistic regression models.Results : Median number of nurses required per shift using the WANNNT was 25.0 (interquartile range [IQR]:23.1–26.7). Correlation between WANNNT and QPNNR was high (r = 0.92, p < 0.0001), but the QPNNR underestimated the number of nurses per shift by 4.8 (IQR: 4.1–5.4). Correlation between WANNNT and CNRUwas moderate (r = 0.45, p < 0.0001). Nursing provision ratio during the first seven days of admission calculated using both WANNNT and QPNNR was associated with mortality/morbidity (adjusted odds ratio [aOR] (95% CI):0.92 (0.86-0.99); 0.94 (0.89-0.98), respectively). The association between nursing provision ratios for the first 24h and whole hospital stay and mortality/morbidity was not statistically significant. Conclusion : Lower nursing provision ratio during the first seven days of admission is associated with anincreased risk of mortality / morbidity in very preterm infants.

Full textThe aim of this thesis is to raise awareness of how State Qualified Operating Room Nurses deal with the constant evolutions of their professional activity, such as legislation to reorganize the French health system which impacts strongly on operating rooms, new standards of activities and skills for operating room nurses, validation of work knowledge and experience, exclusive operating room acts, etc. It investigates the potential psychological discomfort felt by the personnel when they consciously avoid respecting good practices by committing routine violations (Bourrier, 1999) either through necessity or convenience (Reason, 2013). These nurses generally extoll the virtues of professional health care, notably “taking care” (Hesbeen, 1997), rigour, safety, quality treatment, and feel uncomfortable when committing any sort of violation which could increase the surgical risks for the patient. The theory of cognitive dissonance (Festinger, 1957) can help to understand such phenomenon insofar as these professionals combine incompatible cognitions such as safety and avoiding risks on the one hand, and violations on the other. Two complimentary methodologies are used here. The questionnaire of the quantitative survey allows the cognitions, values, and perfectible practices to be matched with the emotions felt by the nurses. A scale of 20 selfrelated affects is used to achieve this (Lecrique, 2007). The interviews and their thematic analysis of the qualitative survey bring to light different strategies for reducing dissonance. The final discussion deals with the professional context in which the nurses evolve, a context which is likely to normalize violations by generating an increase in accident-prone zones (Amalberti and al., 2006), at the expense of guaranteeing safe treatment in the service of the patient

Full textThis thesis introduces "cohesion analysis", a composite method involving both the graph analysis approach (cliques) and the classic statistical approach. This method allows multivariate and comparative approaches to data, even if the samples' sizes and data frequencies are unequal. Our data was collected through a questionnaire sent in 1981 to hospital nurses just finishing "ecole de cadres" (n=380). The analysis is centered on nurses' perceptions of the process of information underlying patient care. The general trend of the responses indicate a conception of nursing which goes beyond basic care and faithful execution of doctor's orders and emphazises the cognitive work on information. Nurses' traditionnal auxiliary status is contested, collegial relations within the care unit are deemed necessary, and the sick person's contribution to his diagnosis and choice of treatment is highly desired. Understaffing and the replacement of nurses by "aides-soignantes" for time or tasks requiring nursing skills are widely denounced. Seven sub-groups within the sample were compared. The main differences that were found between groups concerned nurses' perceptions of the cognitive division of labor and the material conditions of work that would make possible their participation in the information process. Full textNursing research and education recently changed in France. In our country, the nursing discipline is currently transforming toward an academic institutionalisation. European development of nursing research is heterogeneous, but the paths of progress seem similar if four sequential stages. Nursing research is characterized by its diversity. However, French nurses implement research projects, in a relative seclusion. In the current professional, disciplinary and intellectual French nursing context, how can nursing research integrate both the international arena and develop its own unique programme? This thesis further aspires to highlight French nurses? scientific values and provide guidelines for nursing research and education in France. The research programme Thot, a multi-method programme, was launched in 2010 to study the development of French nursing research capacity. Thot 1 study was a descriptive cross-sectional analysis of the Hospital Nursing Research Programme 2010 and 2011 scientific activity. Thot 2 study was an ethnography of nursing research practices, combined with clinical practice. Thot 3 was a phenomenographic study of research concepts acquired during a doctoral apprenticeship in France and Sweden. Seven values were derived from the integration of the results of the three studies: reflexivity, dedicated education, coherence, diversity, confidence, transmission and transformation. We hope that this programme, deployed and implemented concurrently to one of the stage of development of nursing in France, will participate in the future integration of the values of French nurses as scientific values

La complexité et le phénomène d’évolution des problématiques rencontrées par les personnes, ainsi que les besoins spécifiques de ces patients justifient essentiellement l'élaboration d'un suivi infirmier adressé spécifiquement aux personnes épileptiques. Selon la méthodologie de Burns et Grove (2005), un modèle de suivi a été élaboré à l’aide de plusieurs sources et différentes méthodes de collecte, soit les entrevues qualitatives auprès des patients épileptiques (n=4) et auprès des professionnels (n=6), en plus de séances d’observation participante et d’une consultation de la littérature. En fin de compte, à la lumière des résultats obtenus, le suivi infirmier proposé cherche à répondre plus spécifiquement aux besoins des personnes épileptiques et ainsi favoriser leur adaptation, selon le modèle de Roy (2009).
Epilepsy is a neurologic disorder that can affect the person in its entirety, perturbing physical, psychological and social dimensions. The complexity and the phenomenon of evolution of problems faced by people with epilepsy, as well as specifics needs of these people justify essentially the development of a nursing follow up addressed specifically for them. According to the methodology of Burns and Grove (2005), interventions that composed the nursing follow up was created using different data sources and different collecting methods, like qualitative interviews with epileptic person (n=4) and professionals (n=6), participant observation sessions, in addition to literature review. Analysis of verbatim was done according to the approach of Giorgi (1997). Ultimately, in the light of obtained results, the proposed nursing follow up is more specific to needs and problems experienced by people with epilepsy and thus, facilitate adaptation, depending on the adaptation model of Roy (2009).

This research attempts to describe the perceptions of violence and suffering within these health professionals through the evaluation of perception of psychosocial risks by nurses in the emergency and hospitalization departments in the governorate of Tunis. It is about multiple deleterious sufferings, which are tinged with stress, and burn out. Fed with anxiety, and amplified by a faulty organizational system, and an inconsistent relational quality. Full textPeople who are diagnosed with advanced stages of lung cancer suffer from different impacts of the disease and treatment of the latter. In consequence, their quality of life is affected and they frequently suffer from distress. Hope, in these patients, may have a positive outcome on their physical and psychologic experience in regards to the disease. Mobilizing hope can constitute an intervention to support the adaptation of patients diagnosed with lung cancer of advanced stages. From a professional standpoint, and by their proximity with the patients and their relational skills; a nurse holds a key position to intervene at this level. The interventions that can mobilize hope in patients are scarce in todays literature and contain very few details. This descriptive qualitative research has as a goal to explore the interventions used by these nurses to mobilize hope in these patients. Two research questions are formulated: How do nurses perceive hope in patients who are diagnosed with advanced cancer and what interventions do they use to mobilize this hope? Ten nurses working with patients diagnosed with advanced lung cancer in a cancer research center in Quebec City were questioned in a semi-directed individual interview. The nurses perceive hope as the wait for a positive outcome. The reported interventions are described as four functional attributes of the Farran, Herth, Popovich (1995) model; the experiential process, the relational process, the spiritual and transcendent process, as well as rational thought process. Although a large number of interventions are focused on the relational process, very few rely on the spiritual and transcendent process. By appealing to different processes, this conceptual method can be useful to guide nurses in their choice of intervention aimed to mobilize hope in their patients.

Full textEmergency management is one of the principal hospital missions. Since the 90s, hospitals have answered, through the emergency structure, an exponential request for not programmed care, which posed the problem of the dimensioning of the structure and its means. This situation provoked enormous organisational problems. In front of this situation, the hospital decision-makers miss tools witch are indispensable to re-size the emergency resources and to choose the new emergency organization. They shall also allow the planning of the emergency management as well as human and material resources allocation. In response to these needs, this thesis proposes an approach of assistance to the management of emergency systems. This approach tackles three principal problems. At first, we focus our attention on the modelling of emergency processes and its main paths. This modelling, coupled with patients flow simulation, will enable us to analyze the functioning of the emergency structure and to determine the parameters which influences the emergency process. We are interested, secondly, in the nursing staff’s allocation by taking into account their profile of competence evolution using the linear programming. In the third time, we shall study the problem of in hospital vital emergencies management. By means of a process modelling combined with a simulation, we proceed to an approach of conception and evaluation of in hospital survival chain. We tackle, in this last stage, the question of appointment of mobile resuscitation emergency team as well as the positioning of semi automatic defibrillator
