Full textWe are currently experiencing an aging of the population in the province of Québec. Even though many elders are food insecure, very few of them resort to food aid programs. However, the reasons behind this low take-up are unknown. This study aims to document the food access strategies used by food insecure lone elders living in the Limoilou neighborhood and to better understand the motives behind their use and non-use of several types of food aid programs (food banks, collective kitchens, etc.). We carried out two waves of fourteen individual interviews with food insecure lone elders from Limoilou, which deepened our understanding of their experience. The results of this study suggest interesting avenues for research and intervention to facilitate their access to food and adapt food aid programs to the elders’ needs and realities.

Full textMany studies have shown that social participation benefits elderly people’s health and well-being. However, the way in which social participation is presented in social policies and programs implies a kind of productive social participation that does not always correspond to the needs of elderly people with physical disabilities. The objective of this qualitative study is to document the perceptions that elderly people with physical disabilities have of their social participation. More specifically, it is to identify the factors that can facilitate or hinder their social participation. To do this, 11 semi-structured interviews were carried out with people who consider themselves to be elderly and who have physical disabilities affecting their mobility and/or agility. The interviews made it possible to gather the participants’ perceptions of social and environmental factors that can affect their social participation. The data were then analyzed using thematic content analysis within the analytical framework used in this thesis project: the Human Development Model – Disability Creation Process. The results of this research make it possible to identify the factors that can influence participants’ social participation. Personal factors include perceptions concerning the loss of physical abilities, ability to assert oneself and express one’s needs, adaptability and sociability. The most important environmental factors are friends’ and relatives’ attitudes, adaptation of community organizations’ services, climate, technology, adapted transport and societal attitudes toward people with disabilities. Recommendations are made regarding sensitization and adaptation strategies and avenues for further research based on the findings, with the aim of better supporting social participation by elderly people with physical disabilities.

Full textBackground: In Canada and elsewhere, community-based seniors living with frailty, pre-frailty, or experiencing some limitations in their ability to pursue their daily activities often go unnoticed. Systematic assessment of frailty and of its risk factors in primary care would better meet their needs and prevent their decline. Purpose: The objective of this study is to identify the factors influencing the implementation of a new primary health care trajectory to adapt and eventually implement it in different family medicine groups in Quebec. Method: In this qualitative descriptive study, we conducted 47 semi-structured interviews and 8 focus groups with 32 health care professionals, 27 patients over 70 years of age, 8 health care decision makers from three family medicine clinics in the province of Quebec, 16 representatives of community organizations and 9 representatives of specialized geriatric clinics in surrounding areas. We conducted an inductive/inductive thematic analysis based on the Consolidated Framework for Implementation Research. The data were analyzed using Nvivo12. Results: The various study participants provided several facilitators and barriers to the implementation of the trajectory. The complexity of adding the screening tool to electronic medical record, the lack of communication between primary care practices and community services, as well as public policy effects on community support services' capacity to serve their clients were identified as barriers. These findings support adapting the intervention by 1) allowing the integration of screening into the electronic medical record, 2) defining clinical roles for screening, and 3) sustaining partnership between different health system sectors. Conclusions: Eventually, this project will focus on the determinants influencing implementation over the next few years to ensure that older adults’ health care is improved and adapted to their needs according to a preventive approach.

Full textAs the population ages, projections suggest that elder abuse will increase significantly. According to the latest studies on the prevalence of all types of abuse, financial abuse is the type most often reported by Quebec seniors. The purpose of this master’s thesis is to interview several types of employees who meet seniors in the course of their work in order to identify the factors that influence their susceptibility to experience financial abuse.These factors can be divided into three types: vulnerability factors, risk factors and protective factors. The vulnerability factors examined in this research are cognitive and physical health problems, gender and trust in others. The risk factors are family tensions, isolation and the use of personal financial management tools, i.e.,powers of attorney and joint accounts. Also, the protective factors are financial literacy and the support of employees. The results obtained in this research show that although the vulnerability and risk factors have long been known,seniors are still at the mercy of abusers who take advantage of their vulnerability. However the employees' vision makes it possible to understand how these factors have an impact on the lives of victims. As for protective factors, the employees confirm the low level of financial literacy of many seniors and highlight their multiple roles within their senior clientele to protect them from financial abuse. A particularly important issue according to the employees is the non-reporting of financial abuse. Teamwork among all types of employees and increased support from financial institutions in uncovering financial abuse are ways to better protect seniors from situations of financial abuse.

Full textHeart failure (HF) patients should be exposed to drugs reducing mortality and hospitalization (standard treatment), but should not be exposed to drugs potentially inappropriate in HF. This thesis presents population-based studies performed with the Quebec public health administrative databases. Among seniors newly diagnosed with HF, the probability of exposure to standard treatment decreases with increasing age. Patients aged 90+ and those with chronic obstructive pulmonary disease are less likely to initiate a β-blocker. Exposure to a β-blocker is associated with a decreased risk of death and hospitalization. Exposure to potentially inappropriate drugs (nonsteroidal anti-inflammatory drugs, thiazolidinediones, nifedipine and non-dihydropyridine calcium channel blockers) is associated with an increased risk of hospitalization. Since HF quality treatment improves health outcomes in HF seniors, treatment guidelines should be extended to seniors.
