Chapter 7 is concerned with the presence of autism spectrum disorder (ASD) in individuals with Fragile X Syndrome (FXS). It is estimated that 21–50% of individuals with FXS meet the criteria for autism or autism with pervasive developmental delay not otherwise specified. Importantly, recent findings indicate that approximately 2–6% of individuals with ASDs have a mutation in the FMR1 gene, making it one of the most significant single genes associated with the presence of ASD.

Abstract:
There is very good evidence for a strong genetic component to the autism spectrum disorders (ASDs), which include autistic disorder, Asperger syndrome, pervasive developmental disorder not otherwise specified, and Rett syndrome. At the same time, identifying the loci contributing to ASD risk has proven difficult because of extreme heterogeneity. However, in spite of these difficulties, many ASD loci have been identified and, even using current clinical measures, an etiological diagnosis can be given in upward of 20% of cases. With the introduction of “second-generation” sequencing, gene discovery in ASDs will accelerate. As genes are being discovered, functional analyses are leading to potential novel therapeutics, and there is great optimism for more effective treatments in ASDs arising from gene discovery. In the current review, some of the important findings in ASD genetics will be outlined, as will the next steps in ASD genetics.

Personality pathology—which is characterized by a pervasive, maladaptive, and inflexible pattern of thoughts, emotions, and behaviors—has long been defined as a set of categories that are distinct from each other a “normal” personality. Research over the past three decades has challenged that assumed separation and instead suggested that abnormal personality is merely a maladaptive extension of the same features that describe the personality of all humans. This volume surveys cutting-edge research on the science of basic personality and demonstrates the application of these ideas and methods to conceptualizing pathology. It first provides a historical overview as well as the present state of the personality disorder literature. Ensuing chapters then highlight critical issues in the assessment and conceptualization of personality, its development across the life course, and biological underpinnings. In this way the chapters are inherently useful as a primer of the present knowledge concerning the basic science of personality from specific genes to complex social interactions. Furthermore, each chapter aims toward not only elucidating current understandings of personality but demonstrating its direct application to clinical diagnosis and conceptualization.

Pediatric psychiatric conditions are rising in estimated prevalence, and these disorders place an enormous burden on parents, educators, and the health care system. This rise in prevalence likely contains elements of diagnostic changes, greater awareness of these disorders, and true changes in incidence. It has been estimated that there is nearly a 50% lifetime childhood prevalence of one or more mood, anxiety, or behavioral disorders (excluding eating and substance abuse disorders) and that more than 20% of children meet the definition of severe impairment. This chapter focuses on epidemiology, heritability, and implied genetic architecture in representative pediatric neuropsychiatric conditions. We consider five major diagnostic categories and highlight major diagnosis within each, specifically, intellectual disability, pervasive developmental disorders (autism spectrum disorder [ASD]), hyperactive and inattentive behavior (attention deficit/hyperactivity disorder [ADHD]), obsessive compulsive disorder (OCD) and tic disorders (TD) (which includes Tourette Syndrome [TS] and other chronic tic disorders).

Autism spectrum disorders (otherwise known as pervasive developmental disorders) are neurodevelopmental conditions characterized by impairments in reciprocal social interaction and communication, and the presence of restricted and repetitive behaviors. In order to provide a framework for appreciating the advances that basic science and model systems have made to the field, this chapter will outline the behaviorally defining features of the disorder and describe the phenotype of autism. We will present the current diagnostic conceptualization and criteria for autism spectrum disorders, discuss the diagnostic assessment process, explore frequently co-occurring problems in individuals with autism, and introduce various types of treatments that are frequently employed in this population.
