Abstract:
The volume contains the commented edition of the Devisement dou monde based on the Berlin Staatsbibliothek - Preußischer Kulturbesitz Codex, Hamilton 424. The Hamilton 424 Codex, transcribed in Venice in the second half of the fifteenth century, contains the translation of a Latin model (whose features can be partially reconstructed virtually starting from some translation errors) and is the only known witness of V. Due to its characteristics, V represents a crucial point in the definition of the transmission dynamics of Polo’s book: its readings are confirmed, often in a broader form, by the Latin version known as Z. V strengthens the hypothesis that, after returning to Venice, Marco Polo modified the text written with Rustichello da Pisa in Genoa. Actually, version V represents the first step of this long process of rewriting, which probably occurred in several phases. This volume includes an introduction, the text edition, a textual commentary, as well as an index. A second and forthcoming volume will contain the linguistic analysis and glossary.

Abstract:
This article focuses on left-handed metamaterials (LHMs). It begins with a discussion of negative-permeability metamaterials, with particular emphasis on split-ring resonators (SRRs) and SRRr arrays and how magnetic permeability influences the response of materials to the incident magnetic field. It then considers the transmission spectra of SRR, LHM and a composite metamaterial as well as the reflection characteristics of a one-dimensional double-negative material. It also examines the effect of disorder on the transmission and reflection properties of ordered left-handed materials, along with the negative refraction, negative phase velocity, and subwavelength imaging and resolution of LHMs. The article concludes with an analysis of planar negative-index metamaterials.

Abstract:
Approximately 20 million strokes occur in the world each year and over one-quarter of these are fatal. This makes stroke the second most common cause of death, after ischaemic heart disease, and strokes are responsible for 6 million deaths (almost 10% of all deaths) annually. Stroke has major consequences in terms of residual physical disability, depression, dementia, epilepsy, and carer burden. Moreover, around 20% of survivors experience a further stroke or serious vascular event within a few years of the index event. Ischaemic stroke contributes the greatest share of the impact of stroke, with a rate of approximately 1 in 1000 person-years and accounting for between 60% (in Asia) and 90% (in Western ‘white’ populations) of all strokes around the world. Diagnosis and assessment are essentially clinical and confirmed by CT or MRI scanning. Prognostication is difficult in the early phase of haemorrhagic stroke and in ischaemic stroke is affected by the availability and timely use of treatments to recanalize the occluded vessel.

Abstract:
The optical interface between a light source and a detector is important. In most practical realizations the aim should be to collect the maximum light possible. Lens systems seldom do this efficiently, especially where the light source is diffuse. Underlying any attempt at concentrating or guiding light is subject to a fundamental limitation referred to as étendue (phase space cannot be squeezed). Light collection from small volume scintillators of high refractive index may approach 50 %, while collection from large-area scintillators is typically less than a few per cent. Incorporation of wavelength-shifting light guides and fibres leads to enhanced performance. Efficiency measurements by the author in terms of photoelectrons per keV are presented for selected configurations. Optical recycling derived from total internal reflection provides enhancement in effective quantum efficiency by a factor of up to 10. Concepts such as escape cones, adiabatic light guides, and trapped light are covered in detail.

However, bone and joint disorders can cause significant impairment in survivors of critical illness. Return to pre-admission function is often limited by acquired complications such as joint contractures, heterotopic ossification, and altered bone metabolism. Critical care physicians should maintain a high index of suspicion for joint contractures, as they are often asymptomatic but the source of enduring disability once the critical illness had receded. Research is needed to document the effectiveness of alternate positioning, stretching, and bracing which are the current standard practice for prevention of contractures. Heterotopic ossification should be considered in the context of a swollen, warm, painful musculoskeletal site. Early detection with triple phase bone scan and, in some cases, prophylaxis with non-steroidal anti-inflammatory medication or radiation may be warranted. Bone hyperresorption in ICU patients can be caused by immobility, heightened inflammatory status, medication, hormonal changes, and vitamin D deficiency. Laboratory biomarkers can guide treatment, which is important to prevent long-term osteoporosis and stress fractures. Systematic physical examination and early patient mobilization may represent important steps to detect and prevent joint contractures and heterotopic ossification.
