Abstract:
The interaction between mystically inspired beguines and nuns on the one hand and the friars as their confessors, on the other, that is, the male authorities in the late Middle Ages, certainly requires careful assessment because many different factors come into play here. In her monograph, Claire Taylor Jones pursues a host of different aspects pertaining to this complex issue in order to gain a grasp of those female writers particularly in the female Dominican monasteries in the Southwest of Germany and their male colleagues, or spiritual confessors, especially Heinrich Seuse and Johannes Tauler. She draws heavily from the Nuremberg Dominican convent of St. Katherine’s library (15th century), but this actually depends on the various chapters included here. It becomes very clear, however, that the notion of women’s lack of Latin needs to be reviewed carefully considering that that library contained ca. 726 manuscripts, of which 161 were in Latin.

Introduction: Preexposure prophylaxis (PrEP) is an effective biomedical intervention that has the potential to dramatically decrease the incidence of HIV but remains an underutilized method of HIV prevention. The Philadelphia Department of Public Health administered an online survey to health care providers in the Philadelphia area with the aim of characterizing PrEP attitudes, knowledge, and prescribing practices. Methods: Online surveys were distributed to 1000 providers who were recruited through distribution lists of Philadelphia medical providers between September and December 2017. A Likert-type response scale was utilized to analyze participant self-reported responses. Participant practice settings included HIV/ID, family and internal medicine, women’s health, and pediatric/adolescent clinics. Results and Discussion: The response rate of the survey was 9%. Of 81 complete responses, 75% (N = 61) felt comfortable providing PrEP and 77% (N = 62) had ever written a PrEP prescription. Compared with primary care providers, HIV care providers were significantly more knowledgeable about required laboratory testing for prescribing PrEP ( P = .03) and were more likely to have prescribed PrEP to more than 10 patients ( P = .006). Women’s health and pediatric providers reported feeling less comfortable providing PrEP to their patients ( P = .0003). Conclusion: The majority of health care providers in the Philadelphia area who responded to the survey reported experience with providing PrEP to their patients. In the present study, HIV care providers were significantly more comfortable and knowledgeable about prescribing PrEP compared with providers in primary care, women’s health, and/or adolescent/pediatric medicine. Results were limited by sampling bias, as providers who responded to the survey may have prior experience with PrEP. Future Health Department educational trainings need to target primary and preventive care providers, providers who have never prescribed PrEP, and providers who see few patients living with HIV.

Abstract Background Ending the HIV Epidemic (EHE) requires prompt diagnosis and treatment of HIV to reduce transmission. Delayed HIV diagnosis and late entry into care remain challenging. Strategic deployment of testing resources may leverage both targeted and universal testing to accomplish the timely diagnosis of HIV infection. Methods We extracted data from the City of Philadelphia’s Enhanced HIV/AIDS Reporting System for 3,856 individuals diagnosed with HIV infection in Philadelphia, PA from 2012-2018, to determine characteristics associated with delayed diagnosis, defined as: AIDS diagnosed within 90 days of HIV or date of AIDS diagnosis prior to HIV diagnosis. Independent variables included: time since HIV diagnosis, age category, birth sex, current gender, race/ethnicity, transmission risk, insurance status, and receipt of care from Ryan White medical provider. We used Chi-square and multivariate logistic regression to assess factors associated with delayed diagnosis. Results From 2012 to 2018, the number of HIV diagnoses declined from 731 to 422; those with delayed diagnosis declined from 28% to 18%. Age category of 25-34 years comprised the majority of HIV diagnoses N=1402 (36%). The majority were: born male (78%), current gender male (76%), black (69%), MSM (51%), insured (54%), and participating in Ryan White care (71%). In multivariate regression, current gender male, heterosexual transmission, race/ethnicity Asian, American Indian, Alaska Native, or Multi-race, unknown insurance status, and receipt of care from a Ryan White medical provider were 3.7 (95%CI, 1.2-11.4), 1.3 (1.0-1.7), 1.8 (1.2-2.8), 5.9 (4.9-7.1), and 1.4 (1.2-1.7) times as likely to have delayed diagnosis, respectively, after adjustment for time since diagnosis, age category, and birth sex. Participants’ Characteristics and Logistic Regression Results Conclusion EHE will only be successful by reaching all people living with HIV and creating opportunities for early diagnosis. Routine opt-out universal screening combined with repeated, targeted testing will allow for identification and early treatment of HIV infection. As a medical care safety net, Ryan White program provides care to a disproportionate number of people with delayed diagnosis of HIV. By diagnosing HIV as early as possible, we may eliminate delayed diagnosis and reduce the risk of AIDS-related events or death. Disclosures All Authors: No reported disclosures

On October 31, 1994, it was announced that a confidential settlement had been reached in Doe v. Kohn, Nast & Graf, P.C., et al. (No. 93-4510 (E.D. Pa. filed Aug. 19, 1993), 862F. Supp. 1310 (E.D. Pa. 1994)). The settlement in ths widely publicized AIDS discrimination case came three weeks after the trial began in the District Court for the Eastern District of Pennsylvania (“Philadelphia Lawyer Settles AIDS Discrimination Claims Against Firm,” Daily Lab. Rep. (BNA), Nov. 2, 1994, at 10).Plaintiff Doe, an associate employed at Kohn, Nast & Graf, a prominent law firm in Philadelphia, filed an AIDS discrimination case under the Americans with Disabilities Act (ADA) against his employer claiming that the firm fired him in March 1993 because he was HIV-positive. Doe's complaint also cited claims under ERISA and other Pennsylvania employment laws.

Abstract:
This study investigated the geographic variation and the clustering of lung cancer incidence rates in Philadelphia and the surrounding areas using addresses at the time of diagnosis. Using 60,844 cases from Pennsylvania Cancer Registry, we calculated and mapped the age-adjusted incidence rates for five Pennsylvania (PA) counties near Philadelphia between 1998–2007 and 2008–2017. We identified ZIP codes with significantly higher incidence rates than the state rates and examined their demographic and exposure characteristics. Further, we tested for spatial autocorrelation and identified spatial clusters using Moran’s I statistic. Our results showed that approximately one in four ZIP codes had an incidence rate that was significantly higher than the PA state rate in each period studied. Clusters of higher incidences were detected in the southeastern part of PA bordering New Jersey. These areas tended to be more populated, of lower socioeconomic status, and closer to manufacturing facilities and major highways. Possibly driven by the community and environmental factors, the observed differences in disease incidence suggest the importance of including residential location in risk assessment tools for lung cancer.

Abstract:
The 1988 reauthorization of the 1965 Elementary and Secondary Education Act Chapter 1 legislation introduced several significant changes in both program and evaluation features, many of which have benefited students and schools in the School District of Philadelphia. However, some provisions of the legislation, and the accompanying regulations, have unintended consequences which are hampering efforts to provide the best possible program for students. This article addresses the implementation of schoolwide projects in Philadelphia, discusses some of the conceptual problems which exist in interpreting results of those programs, and offers suggestions for alternative assessment methods. As a result of the 1988 reauthorization, schoolwide projects have increased from 37 sites in 1988–1989 to 103 for the 1991–1992 school year. This expansion has occurred because of positive results in test scores and report card grades and because of the satisfaction expressed by school staffs and communities at having the flexibility to design a program tailored especially for the needs of their own students. Problems with the current legislation, as noted in the article, have stimulated the following attempts at solutions: how to discern which schools are in need of program improvement, how to ensure that schools with successful programs have the opportunity to continue implementing them, how to determine the intent of program improvement, and how to find alternatives to standardized testing. Finally, the article offers suggestions for alternative ways to use standardized test data, as well as performance-based measures of student learning.

Topographic map evidence is used to interpret Pennypack Creek drainage basin erosion history in and north of the City of Philadelphia, Pennsylvania (USA). Southwest and west-southwest oriented through valleys crossing the south oriented Pennypack Creek drainage basin, barbed Pennypack Creek tributaries, and significant valley direction changes are used to determine that the Pennypack Creek valley eroded headward across massive southwest oriented floods. Initially floodwaters flowed on a low gradient topographic surface at least as high, if not higher, than the highest Pennypack Creek drainage basin elevations today. Shallow low gradient diverging and converging flow channels were eroded into the underlying bedrock surface predominantly along fault lines and other zones of easier to erode materials. Headward erosion of the much deeper Pennypack Creek valley across this anastomosing channel complex captured southwest oriented floodwaters and flow on northeast ends of beheaded channels was reversed so as to move toward the newly eroded and deeper Pennypack Creek valley. These reversed flow channels captured southwest oriented floodwaters still moving north of the actively eroding Pennypack Creek valley head. This captured water then moved in a northeast direction and eroded deep northeast oriented valleys headward from the newly eroded Pennypack Creek valley. These valleys today account for northeast and east oriented Pennypack Creek valley segments and northeast oriented (barbed) tributaries flowing to south oriented Pennypack Creek. The floodwater source cannot be determined from Pennypack Creek drainage basin evidence, but was from the northeast. Melting of a continental ice sheet could produce floods of sufficient volume and duration to overwhelm whatever drainage system previously existed and to erode new drainage basins in a manner similar to how the Pennypack Creek drainage basin was eroded.
