Abstract:
SUMMARYComputer models can be useful in planning interventions against novel strains of influenza. However such models sometimes make unsubstantiated assumptions about the relative infectivity of asymptomatic and symptomatic cases, or conversely assume there is no impact at all. Using household-level data from known-index studies of virologically confirmed influenza A infection, the relationship between an individual's infectiousness and their symptoms was quantified using a discrete-generation transmission model and Bayesian Markov chain Monte Carlo methods. It was found that the presence of particular respiratory symptoms in an index case does not influence transmission probabilities, with the exception of child-to-child transmission where the donor has phlegm or a phlegmy cough.

Abstract:
This review paper summarized the current situation of studies on the essence of phlegm syndrome and relation between phlegm syndrome, diseases, and therapeutics based on published English articles. In studies on the essence of phlegm syndrome, omic technologies were used to explore the molecular basis of phlegm syndrome; in studies on relation between phlegm syndrome and diseases, discovery of markers of phlegm syndrome in diseases becomes a hotspot; the distribution of phlegm syndromes in some common chronic diseases was found; in the therapy of phlegm syndrome, two therapeutic models, treatment with CM formula and treatment with a combination of CM formula and Western medicine, were used most frequently. It is certainly that using one omic technology is not able to deal with the complexity of phlegm syndrome and that the use of a combination of multiple omic methods will be a trend in future studies. Meanwhile, for rapidly increasing clinical research quality of phlegm syndrome, a series of agreed criteria, such as syndrome diagnostic criteria and efficacy criteria clinical studies of phlegm syndrome, needed to be established urgently, and there was an urgent need of standardizing syndrome names in English.

Abstract:
Neuropeptide Y (NPY), which is widely expressed in both the central and peripheral nervous systems, has an important role in a variety of biological fields. In this study, we analyzed the distribution of NPY polymorphisms in dampness-phlegm pattern and non-dampness-phlegm pattern in elderly Korean subjects with cerebral infarction (CI). A total of 1.097 subjects (498 normal subjects and 599 CI patients, including 198 with dampness-phlegm pattern and 401 with non-dampness-phlegm pattern) participated in this study. Genotyping for five SNPs (G-1484A, C-1471T, C-399T, A1201G, and C5325T) was conducted by primer extension. The results were statistically analyzed for genetic association of NPY-polymorphisms with normal versus dampness-phlegm pattern or non-dampness-phlegm pattern subjects. Among the five SNPs tested, the T allele of C-399T has a negative association with the dampness-phlegm pattern and is marked by a decrease in serum cholesterol levels. Furthermore, serum cholesterol levels were significantly higher in dampness-phlegm pattern patients than in non-dampness-phlegm pattern patients.In this study, for the first time, the association of NPY polymorphisms with pattern identification (PI) of traditional Korean medicine (TKM) was analyzed in a large CI patient population.

Abstract:
This thesis is an application of surrealism to architecture. Surrealism is an art & literary movement started in Europe between the world wars. After the chaos of the first world war the surrealist group was disillusioned with the devotion to rationalism that they believed produced the war. The group determined that an opposing philosophy would produce the opposite result. They began to develop an art based on the irrational. Their creativity was derived from chance, group activities, dreams, and other undirected actions. They promoted this as a new way to discover beauty and a foundation for a new way of thinking. The surrealists were prevalent in art and literature, but never integrated themselves into the world of architectural design. This thesis explores the possibilities of using surreal concepts and techniques as an inspiration for design. The site for this exploration is a fifty foot wide infill lot in an urbanizing Washington, D.C. suburb, Falls Church, Virginia. The art schoolâ s program was selected by a Wikipedia random article search and a deck of cards.

Abstract:
This dissertation examines the Chinese conception of phlegm and related body fluids phlegm from the first occurrence of phlegm in Zhang Zhongjing’s 張仲景 (fl. 150-219) Jingui yaolue金匱要略 through the first extended discussion Wang Gui’s 王桂 (1264-1354) Taiding yangsheng zhulun 泰定養生主論 (1338). Following the conceptual development of phlegm and fluids, the study illustrates one of the most important shifts in postclassical Chinese medicine. That is the transformation of the experience of phlegm from an insignificant water pathology that causes indigestion, to a major pathogen in Chinese medicine that is associated with an astonishing range of symptoms, and external and internal etiologies. The history of phlegm also intersects with another major shift in the history of Chinese medicine that is rise of fire, and the link of fire and emotions that was forged during the early Song dynasty. In contrast to the current over-emphasis on pneumatic or energetic aspects of the Chinese imagination of the body, this dissertation focuses on the humoral aspects of Chinese medicine. This focus brings into view distinct parallels in the conception, experience, and treatment of fluids in the Chinese, Greek and Indian medical traditions, such as the concern with maintaining flow, and the fear of blockage, stagnation, and misguided flows. For instance, all of these traditions view phlegm as the result of a disturbance in the flow. These parallels in the Chinese, Greek, and Indian conception of humors, therefore, help us to better understand the history of phlegm not only in the history of Chinese medicine, but also in the Indo-European traditions. The dissertation further sheds light on the history of Sino-Indian and Sino-Persian knowledge transfer, and the influence of Indic and Greek conceptions into Chinese medicine, as it puts forward evidence, which suggests that the similarities between Chinese and Indo-European conceptions of phlegm were due, in part, to historical influences from the Indic and Islamic medical traditions. Āyurvedic conceptions of phlegm reached China through the intermediary of Buddhist translations, where phlegm played an important role in physiology. Islamic medicine was present in the Yuan dynasty, and its concepts show clearly in Wang Gui’s Yuan period treatise. The dissertation’s focus on fluids also brings into view differences in the conception of matter and the experience of fluid in the Chinese and the Greco-Roman medical traditions. In early Chinese medicine, phlegm and stagnant fluids were associated with lumps and tumorous growth, but not with decay. In the Greco-Roman tradition phlegm and stagnations were feared because of their immediate connection with putrefaction and decay. In early Chinese medicine, phlegm and fluids were diagnosed by signs from within the body, such as the sounds of water, but also the subjective feeling of fullness reported by the patients. In the Greco-Roman tradition, as in Wang Gui’s Yuan period treatise, phlegm was diagnosed through the examination of the patients’ outflows.

Introdução: O sinal da aspiração faríngea (SAF) e o sinal do pigarrear (SPIG), manifestações clínicas em geral relacionadas, respectivamente, a doenças de vias aéreas superiores e inferiores, são expressões de mecanismos de defesa de natureza mecânica que atuam na limpeza do trato respiratório, e com freqüência encontram-se associados à tosse crônica. Repercutem especialmente em nível faríngeo, com as correspondentes modificações estruturais desse compartimento. Objetivo: Descrever e analisar as modificações anatômicas e características fisiológicas que servem de base para a ocorrência do SAF e do SPIG. Métodos: Estudo clínico observacional onde, foram selecionados 30 indivíduos adultos de ambos os sexos, portadores de rinussinusite crônica, fora de agudização, os quais foram submetidos a exames de imagem, estáticos (radiográficos) e dinâmicos (fluoroscópicos), de modo a ser possível estudar as alterações anatômicas nos momentos mencionados (SAF, SPIG), e durante a situação de repouso (REP). Os pacientes eram colocados em posição ortostática, de perfil, buscando-se, com isto, reduzir os efeitos da sobreposição de tecidos, objetivando tornar as mensurações mais acuradas. Cada um dos 30 indivíduos foi estudado nas três situações: em repouso (REP), em manobra de aspiração faríngea (SAF), e durante o ato do pigarrear SPIG). Usaram-se testes de média para comparações entre as mensurações (teste T e ANOVA), intervalos de confiança de 95,0 %, adotando-se nível de significância de 5,0 %. Resultados: Encontrou-se a área transversa da faringe medindo 4,13±1,339 cm² na posição de repouso, 1,879±0,950 cm² durante a manobra de aspiração, e 5,280±1,421 cm² durante o pigarrear. Esses três valores foram todos significativamente diferentes entre si (p<0,001). A maior variação com relação ao repouso ocorreu no SAF (120,0% menor), enquanto que no SPIG foi em torno de 30,0% maior. Os valores foram menores entre as mulheres, mas a variabilidade foi similar em ambos os sexos. Conclusão: A área da secção transversa da faringe, mensurada no nível da primeira vértebra cervical de indivíduos adultos, mostrou-se significativamente menor durante a manobra inspiratória (SAF), em comparação com a situação de repouso (REP), enquanto que a do pigarrear (SPIG) foi significativamente maior. As modificações do calibre da faringe, em especial o acentuado estreitamento durante o SAF, associadas a movimentos vibratórios das estruturas locais, devem conferir a esse compartimento maior eficiência na remoção das secreções do trato respiratório.
Introduction: The signs of both pharyngeal aspiration (SPA) and phlegm in the throat (SPT), clinical manifestations generally related, respectively, to either upper or lower diseases of the airways, are expressions of mechanic defense mechanisms for respiratory tract cleaning, frequently associated to chronic cough. They have repercussion mainly at pharyngeal level, with the correspondent structural modifications of that compartment. Objective: To describe the anatomic modifications and physiologic characteristics in which are based the grounds for the SPA and SPT occurrence. Methods: Clinical and observational study in which were selected 30 of both sex adult individuals, all with inflammatory upper airways disease, with the purpose to specially study the alterations in morphology of the pharynx, which occurs during the SPA and SPT expression. For that, the individuals in orthostatic position were submitted to static (radiographic) and dynamic (fluoroscopy) image exams, in profile projections, aiming with this to reduce the tissue superposition and to obtain more accurate measurements. Each of the 30 individuals were studied in three situations: at rest (RST), in SPA and SPT. Tests of media (T, ANOVA) were used for comparisons between measurements, adopting a 5.0% significance level. Results: It was found the transverse area of pharynx measuring 4.133±1.339 cm2 in the rest (RST) position, 1.879±0.950 cm2 during pharyngeal aspiration (SPA), and 5.280±1.421 cm2 in SPT – values that were significantly different (P<0.001). In relation to the rest situation, the most accented variation occurred in SPA (120.0% smaller), whereas in SPT it was 30.0% greater. The values were smaller among females, but their variability was similar in both sexes in all the three situations. Conclusions: In comparison to the rest situation (RST), the transverse section of the pharynx, measured at the first cervical vertebra, was significantly smaller during the aspiration (SPA), and greater on the SPT maneuver. The modifications of the pharynx caliber, specially its accented narrowing associated to vibratory movements of the local anatomic structures, should be confer to that compartment a better efficiency for secretion removal from the respiratory tract.

Le sommeil profond, distinct du rêve qu’il accompagne, a bien souvent été occulté par ce dernier au sein des études philosophiques et historiques. La présente recherche, consacrée aux figures d’endormis et aux théories du sommeil à la Renaissance, propose de combler en partie cette lacune en partant de l'hypothèse selon laquelle le sommeil profond, bien qu’il échappe à la conscience, n'en demeure pas moins chargé de sens et de valeurs. L’imaginaire bifurque pour créer, en lieu et place d’une absence de connaissance, une source vive de métaphores. Placé au confluent des traditions chrétiennes et païennes, l’art de la Renaissance offre à l’interprétation une multiplicité de figures endormies. Métaphores visuelles, pétries par la littérature de l’époque, nourries de théories qu’elles transforment en retour. À travers elles, dormir constitue bien souvent l’indice du péché, car « l’oisiveté est mère de tous les vices ». Mais, la métaphore change avec la pose du dormeur, son lieu et son environnement également. Selon les œuvres, le sommeil devient l’emblème de l’enfance ou de la vieillesse (premiers et derniers sommeils), une métaphore d’ignorance et d’oubli (sommeil léthéen), la figure du secret et de la foi (repos contemplatif), du désespoir et de la mélancolie (sommeil de tristesse), de l’amour céleste et du ravissement (vacance de l’âme) ou bien du rapt et de la volupté (érotisme léthargique). L’énigme du sommeil donne forme aux questions insolubles qui ne cessent de nous mettre en éveil : images léthargiques de bonheur, d’amour et de mort qui n’en finissent pas de hanter l’imaginaire contemporain, « métaphores absolues » dont l’histoire ne cesse de s’inventer
Scores of historical and philosophical studies have been devoted to dreams. However, the notion of deep sleep (as opposed to dreams) seems to have been thoroughly neglected. The present study attempts to fill this gap by analyzing representations of sleepers and theories of sleep in the Renaissance. My working hypothesis is that even though deep sleep appears to be located outside of the realm of consciousness, it amounts nonetheless to a heavily determined notion, both semantically and axiologically. Sleep seems to offer a form of imaginative diversion, as the absence of knowledge gives free rein to fanciful metaphors. At the crossroads of Christian and pagan traditions, Renaissance art offers a cornucopia of representations of sleep. They constitute visual metaphors, deeply influenced by the literature and theoretical writings of the period, on which they had in turn a tremendous impact. Thus, sleep is often equated with sin, as « idle hands are the devil's workshop », but its meaning also depends on the sleeper's attitude, location and context. For instance, sleep can be depicted as an emblem of early childhood or old age (first or last sleep), a metaphor for oblivion (Lethean sleep), a figure of ecstasy and faith (contemplative rest), an image of melancholy (sleep of sadness), of spiritual rapture (vacation of the soul) or sensual ravishment (lethargic eroticism). Whichever the case, sleep always remains an enigma, a Protean concept assuming the shape of virtually unanswerable questions : lethargic images of happiness, love and death that are still haunting the contemporary imagination, « absolute metaphors » constantly reinventing themselves and their history

Massage is one of the oldest and most welcomed therapeutic methods because of its significant effectiveness and simple implement. It can relax muscles, release the body and mind, and improve blood circulation. In recent years, due to the increment of elderly people, the medical care becomes a more and more important issue in the society. There are many institutions committed to developing the medical system automation, in order to solve the shortage of caregivers and enhance healthcare quality. The massage therapy automation is also one of the focuses of development. Kneading, pinching, pressure and tapping are four common massage techniques. This thesis focuses on tapping motions since few theses mention it. Unlike the other three techniques, tapping needs a high-frequency repeated motion whose frequency is about 1-2Hz. This thesis exploits speed control to manipulate its tapping frequency and calculate the force through the human body impedance model. The contact detection is developed to reduce the contact time and ensure the stability in each tapping motion. This thesis also adds tapping phlegm applications by replacing the end-effector of the robot with a phlegm cup, since these two motions are alike. With the phlegm cup, phlegm in the lungs can be loosened by tapping the human back. Furthermore, the author combines the robot with the speech control to improve the human-robot interaction and provide additional safety and convenience for users. In the experiments, the force/torque sensor and the microphone are utilized to collect the force-time and audio data. Then, the similarity between masseur and robot with and without using the phlegm cup while massaging on the subject’s back are calculated through waveform similarity and audio similarity. The results show that the similarity has reached 84 to 89 percent which is almost the same as the masseur massage tapping motion.

Listening examination is one of the major diagnostic methods in traditional Chinese medicine, however, it needs objective documentation and quantitative evaluations. In this study, we assessed the voice in patients with phlegm syndrome (rhinosinusitis) versus non-phlegm controls by means of computerized acoustic analysis system. Eighty two (30 female, 52 male) patients and sixty (30 female, 30 male) controls were enrolled for voice analysis. Subjects were asked to phonate /a/ and /m/ for at least 4 seconds at their conversational pitch and intensity in a voice-isolated room. Approximately 2 seconds from the first second of each recording was subjected to analysis. Thirty acoustic parameters of multi-dimensional voice program (MDVP) and another ten of resonance were assessed. The parameters of MDVP were classified into 5 groups: (1) fundamental frequency information; (2) frequency perturbation; (3) amplitude perturbation; (4) noise and tremor evaluation; (5) voice break, subharmonic, and voice irregularity. The parameters of resonance were classified into 2 groups: (1) formant frequency (F1to F4); (2) differences in formant frequencies. There were 22 significantly different (p<0.05) parameters in female group and 20 in male group. Rhinosinusitis possibly affected the voice quality by altering the shape of voice tract and the viscosity of vocal folds. By using linear discriminant function, we could distinguish rhinosinusitis from non-rhinosinusitis with 86.7% in female group and 93.9% in male group. Voice analysis may be a simple, noninvasive procedure in conventional diagnosis instead of the radiation exposure. Our results suggested that it is a potential method in both diagnosis and outcome evaluation for rhinosinusitis. For other phlegm syndrome in traditional Chinese medicine other than rhinosinusitis, the sensitivity and feasibility of voice analysis allow further evaluation.
