Q fever is a zoonosis caused by Coxiella burnetii, an intracellular Gram-negative spore-forming bacterium, the common animal reservoirs of which are cattle, sheep, and goats. Rats and mice have been implicated as reservoirs in some areas. In French Guiana the three-toed sloth is the reservoir. C. burnetti is trophic for the endometrium and mammary glands of female animals, and during pregnancy the organism reaches very high concentrations in the placenta such that at the time of parturition organisms are aerosolized and contamination of the environment occurs. Inhalation of even one microorganism can result in infection. Diagnosis is confirmed by serological testing: in acute disease antibodies to phase II antigen are higher than those to phase I, whereas the reverse is true in chronic disease.

The first indications that hormonal imbalance during pregnancy may result in abnormal development of the fetus goes back to the 1930s. In 1939 researchers at Northwestern University Medical School reported that when pregnant rats were given an extra dose of external estrogen, the offspring suffered structural defects in their sex organs, both females and males (1) For years, this phenomenon was considered by the scientific and medical community as specifically related to rodents and thus did not concern humans. Furthermore, it had been generally believed that human placenta represented a barrier impenetrable by chemicals to which a pregnant woman was exposed. The myth of the placental barrier was shattered by the thalidomide tragedy. Thalidomide was developed in 1957 and found extensive use in Europe and Australia as a prescription drug to be used in pregnancy as a tranquilizer and against nausea. Soon, however, it had to be withdrawn from the market because some babies of women who took thalidomide were born highly deformed, lacking whole limbs or having underdeveloped limbs. Not all babies of women taking thalidomide suffered deformities There was no relationship between the total dose of the drug and the effect. Rather the effect depended on timing—on the time during the pregnancy during which the drug was taken. The deformities occurred only when thalidomide was taken during the organ-forming period—between the fifth and eighth week. Diethylstilbestrol (DES) was first synthesized as a synthetic estrogen-analog in 1943. In decades to follow it was widely prescribed to pregnant women for prevention of miscarriages. However, in 1952 an epidemiological study conducted at the University of Chicago indicated that there was no difference in the frequency of miscarriages between women who did not take DES and those that did take it. Despite this finding many physicians kept prescribing the drug through the 1960s. In 1971 two independent case-control epidemiological studies had shown that among girls born to women who took DES there was a high frequency of vaginal cancer occurring at unusually young age of 15 to 22 (2,3).

The article provides a review of a landmark study characterizing the risk of repeat cesarean deliveries. This multicenter prospective cohort study of over 30,000 cesarean deliveries demonstrates that risks of cesarean delivery increase sequentially, even in the absence of placenta accreta. The most dramatic increase is noted in the high rates of accreta with placenta previa and multiple cesarean deliveries, greater than 40% with 3 of more cesarean deliveries. This review highlights the key findings, methodology, and impact of this landmark study. It discusses other studies that continue to verify these results as well as contextualize them in the management of placenta accreta with developing centers of excellence. Finally, the article provides a sample clinical case scenario that applies the findings of this study.

During pregnancy the fetus requires an adequate supply of oxygen and clearance of carbon dioxide which is a waste product of metabolism. In fetal life, the placenta provides this function. After birth the lungs are aerated and perform this. If there is a failure to transport these chemicals across the placenta, the fetus can become hypoxic and acidotic, which can lead to permanent brain damage. In the neonate this can manifest in a number of ways, but most seriously cerebral palsy. Assessment of the fetal heart rate has been shown to identify fetuses where this maybe occurring. However the inter observer variation amongst clinicians assessing the heart rate monitoring is high and interpretation skills are often not good. Computer assessment of the fetal heart rate has been used and developed. Expert system techniques have been used to develop a system. The systems developed have been shown in studies to perform better than humans and to be able to identify subtleties not seen by the human eye. Future research is further assessing the value of these systems.
