Abstract:
Evidence from controlled clinical trials supports the prescribing of clozapine for patients with treatment-resistant schizophrenia (TRS). Early studies focused on severely ill TRS patients. More recent studies indicate that clozapine can be effective for patients who are relatively stable but are burdened by persistent psychotic symptoms. Clozapine treatment is associated with a substantial side effect burden, including sedation, orthostasis, weight gain, constipation, and seizures. In addition, because of a risk of potentially fatal agranulocytosis, clozapine patients require regular monitoring of their neutrophil count. Measuring clozapine plasma concentrations can be helpful in managing patients with severe side effects and those with an inadequate clinical response. A trial of clozapine should consist of a minimum duration of 12 weeks.

Clozapine is licensed in the UK for use in treatment-resistant schizophrenia, treatment-intolerant schizophrenia, or psychosis associated with Parkinson’s disease. As with many drugs, it is also used outside of these licensing parameters for other conditions or clinical situations—often referred to as ‘off-label’ prescribing. These off-label indications have varying degrees of theoretical support, peer-reviewed evidence, and practical experience associated with them. This chapter discusses the use of clozapine for children and adolescents, older adults, and in the treatment of aggression and mood disorders. The use of supramaximal doses of clozapine to achieve therapeutic plasma concentrations is also off-label, although adding interacting medication to reach the same result is not; these contrasting approaches are also debated. Finally, rechallenging with clozapine in patients who have previously had serious adverse events to the drug is also considered.

Pregnancy is characterized by unique physiological changes within the kidney, resulting in a marked increase in renal blood flow and glomerular filtration, which are associated with successful pregnancy outcomes. Early in normal pregnancy there are increases in plasma volume and cardiac output, but a lowered peripheral resistance leads to average blood pressures being lower. A pregnancy-associated respiratory alkalosis occurs. Protein excretion tends to increase slightly in women without kidney disease. Kidney size is increased, and pelvicalyceal system dilatation is noticeable in most women in the third trimester, right greater than left.

Normal pregnancy represents a haemodynamic challenge for the heart. The main changes are increases in blood (plasma) volume and cardiac output, and a decrease in systemic vascular resistance. These change start early during pregnancy and will be maximal around the 24th week of gestation. This translates into echocardiographic changes such as increase in ventricular volume, stroke volume, and changes in geometry. Peripartum cardiomyopathy is a left ventricular dysfunction without any underlying cause, arising near the end of the gestation or in the early postpartum period. Echocardiography is the key to confirm the diagnosis and also to exclude other causes of cardiomyopathy or to explain clinical signs.

Neuronal membrane potential depends on the distribution of ions across the plasma membrane and the permeability of the membrane to those ions afforded by transmembrane proteins. Ions cannot pass through a lipid bilayer but enter or exit neurons through ion channels. When activated by voltage or a ligand, ion channels open to form a pore through which selective ions can pass. The ion channels that support a resting membrane potential are critical to setting a cell’s excitability. From the distribution of an ionic species, the Nernst potential can be used to predict the steady-state potential for that one ion. Neurons are permeable to potassium, sodium, and chloride ions at rest. The Goldman-Hodgkin-Katz equation takes into consideration the influence of multiple ionic species and can be used to predict neuronal membrane potential. Finally, how synaptic inputs affect neurons through synaptic currents and changes in membrane resistance is described.

Glomerular filtration, the first step leading to the formation of primitive urine, is a passive phenomenon. The composition of this primitive urine is the consequence of the ultrafiltration of plasma depending on renal blood flow, on hydrostatic pressure of glomerular capillary, and on glomerular coefficient of ultrafiltration. Glomerular filtration rate (GFR) can be precisely measured by the calculation of the clearance of freely filtrated exogenous substances that are neither metabolized nor reabsorbed nor secreted by tubules: its mean value is 125 mL/min/1.73 m² in men and 110 mL/min/1.73 m² in women, which represents 20% of renal blood flow. In clinical practice, estimates of GFR are obtained by the measurement of creatininaemia followed by the application of various equations (MDRD or CKD-EPI) and more recently by the measurement of plasmatic C-cystatin. Under physiological conditions, GFR is a stable parameter that is regulated by the intrinsic vascular and tubular autoregulation, by the balance between paracrine and endocrine agents acting as vasoconstrictors and vasodilators, and by the effects of renal sympathetic nerves. The mechanisms controlling GFR regulation are complex. This is due to the variety of vasoactive agents and their targets, and multiple interactions between them. Nevertheless, the relative stability of GFR during important variations of systemic haemodynamics and volaemia is due to three major operating mechanisms: autoregulation of the afferent arteriolar resistance, local synthesis and action of angiotensin II, and the sensitivity of renal resistance vessels to respond to NO release.

Living in the Merry Ghetto reframes how people use music to build resistance. To do so, Hagen addresses the social context of illegal music-making in Czechoslovakia during state socialism, asking “How Do Aesthetics Nurture Political Consciousness?”. He tells the story of a group of rock ’n’ rollers who went underground after 1968, building a parallel world from where they could flourish: the Merry Ghetto. The book examines the case of the Czech Underground, the politics of their music and their way of life, paying close attention to the development of the ensemble the Plastic People of the Universe. Taking in multiple political transitions from the 1940s to the 2000s, the story focuses on non-official cultural practices such as listening to foreign radio broadcasts, seeking out copied cassette tapes, listening to banned LPs, growing long hair, attending clandestine concerts, smuggling albums via diplomats, recording in home-studios, and being thrown in prison for any of these activities. Drawing on ethnographic interviews with Undergrounders, archival research, and participant observation, Hagen shows how these practices shaped consciousness, informed bodies, and promoted collective action, all of which contributed to an Underground way of life.
