Full textIntroduction: The Laboratoire d’Organogenèse Expérimentale (LOEX) has developed a unique production technique of self-assembled autologous skin substitute (SASS). This method allows the generation of permanent skin equivalents that display a structure and a function similar to normal human skin. Problematic and objectives: This thesis presents the first results of SASS clinical use in the treatment of chronic complex wounds to demonstrate the clinical efficacy and safety. The need to develop a faster production method in order to improve burn patient prognosis is also outlined. Indeed, the delay before definitive skin coverage to insure proper thermoregulation and protection from the external environment is directly associated to the vital and morbid prognosis of this population of severely traumatised patients. The current eight weeks SASS production delay must be reduced in order to further improve burn patients quality of care and survival. Method: This work describes the development and refinement of a faster production method for SASS using decellularized dermal matrices (SASS-DM) that generated skin substitutes produced in only 4 weeks and a half. These faster produced skin substitutes where compared in vitro to the standard SASS in regard to the histological characteristics, cellular differentiation and the presence of a functional basement membrane. The faster produced SASS-DM were then compared in vivo to standard SASS by following the evolution of grafted mice in order to complete a preclinical trial of this innovative technique. Results: The faster production method for the autologous self-assembled bilayered skin substitutes was shown to be equivalent to the standard production method in vitro and in vivo. Conclusion: This work presents the clinical use of bilayered skin substitutes for the treatment of chronic venous ulcers, which was shown to be efficient as well as safe. Afterwards, the preclinical development of a new faster production method of autologous bilayered self-assembled skin substitutes is presented, allowing the culture of skin grafts in four weeks and a half instead of the previous 8 weeks long protocol, with equivalent quality and characteristics as the standard cultured skins. This innovation represents a major adjunct to severely burned patients treatments and could possibly change their surgical planning and their survival.

Full textRoundup 3 plus®, a glyphosate-based herbicide, is widely used in agriculture, but its extensive use is responsible for a health risk in human. The aim of this thesis was double: to investigate and compare the antioxidant imbalance induced by glyphosate alone or included in Roundup 3 plus®, the cell cycle and apoptosis of human cutaneous ceIls, and to modulate these toxic effects with antioxidants or vectorisation. An experimental design was used to choose concentrations, and effects were measured on keratinocyte cultures. Our results showed. That glyphosate, alone or formulated, induced significant changes in antioxidant status and increased cell death and caspase 3 activities. Antioxidant supplementation modulated these enzymatic activities, reduced apoptosis in herbicide-treated keratinocytes. The cutaneous toxicity of a glyphosate-based herbicide might aIso concretely be reduced by the presence of antioxidants in the formulations

Full textDespite the high number of formulations available for a drug on the pharmaceutical market, it can arise that a patient needs a treatment for which none pharmaceutical form or dosage is available. In this case, pharmacy department of the hospital may have to prepare this form, in accordance with health agency.The goal of this work was to develop a semi-solid transdermic formulation from ready for use vehicle and containing three antiemetic drugs for the treatment of chemotherapy induced nausea and vomiting. Hence, ondansetron, dexamethasone and aprepitant have been formulated in five commercial vehicles of liposomal (PLO®, Lipovan®, Pentravan® and Pentravan Plus®) or phytosomal (Phytobase®) composition. After development of a formulation protocol, quality control was carried out on every finished preparation It focused on the evaluation of the content of active ingredients in formulations and their homogeneity. For this purpose a separation method using HPLC-UV and a method based on PLS-UV have been developed and validated according to the recommendations of the French Society of Pharmaceutical Science and Technology to assay the three antiemetics simultaneously.Two in vitro studies were then implemented on a Franz cell. The first, using cellulose acetate membranes, has shown that the PLO® and the Pentravan Plus® are the most efficient vehicles for releasing the three drugs formulated separately. The second one was performed to assess the permeation of the same drug through pig ears skin epidermis. It highlighted the performances of Pentravan Plus® as a vehicle for the simultaneous transdermal administration of ondansetron and dexamethasone. Nevertheless, as permeation of aprepitant is too poor to consider its transdermal administration, it has not been included in the final formulation. Different chemical enhancers were evaluated to improve the transcutaneous passage: tween 20 appeared to be as powerful as ethanol conventionally used for the incorporation of drugs in these vehicles
