Abstract:
The clinical outcomes of nicorandil in percutaneous coronary intervention (PCI) are conflicting. We sought to evaluate the effects of nicorandil on periprocedural myocardial injury (PMI) in elective PCI. Eligible studies that reported the effect of nicorandil on PMI in elective PCI were obtained from PubMed, Web of Science, and Cochrane Library (up to October 28, 2019). The outcomes were PMI and major adverse cardiovascular and cerebrovascular events (MACCEs). Ten randomized controlled trials with 1304 patients undergoing elective PCI were evaluated. Nicorandil significantly reduced the incidence of PMI (odds ratio [OR] = 0.48; P = .0003); however, there was no significant difference in MACCEs (OR = 0.80; P = .45) between the 2 groups. Subgroup analyses showed that nicorandil significantly lowered the PMI risk when only patients with stable coronary artery disease (OR = 0.41; P = .0008) were considered and when nicorandil was administered intravenously (OR = 0.41; P = .0007) or orally (OR = 0.33; P = .0001). This meta-analysis suggests that nicorandil could reduce the incidence of PMI without increasing the occurrence of MACCEs in elective PCI. The effect of nicorandil in lowering the PMI risk is associated with the diagnosis of the patients and the route of nicorandil administration.

Abstract:
Aims Periprosthetic joint infections (PJIs) and fracture-related infections (FRIs) are associated with a significant risk of adverse events. However, there is a paucity of data on cardiac complications following revision surgery for PJI and FRI and how they impact overall mortality. Therefore, this study aimed to investigate the risk of perioperative myocardial injury (PMI) and mortality in this patient cohort. Methods We prospectively included consecutive patients at high cardiovascular risk (defined as age ≥ 45 years with pre-existing coronary, peripheral, or cerebrovascular artery disease, or any patient aged ≥ 65 years, plus a postoperative hospital stay of > 24 hours) undergoing septic or aseptic major orthopaedic surgery between July 2014 and October 2016. All patients received a systematic screening to reliably detect PMI, using serial measurements of high-sensitivity cardiac troponin T. All-cause mortality was assessed at one year. Multivariable logistic regression models were applied to compare incidence of PMI and mortality between patients undergoing septic revision surgery for PJI or FRI, and patients receiving aseptic major bone and joint surgery. Results In total, 911 consecutive patients were included. The overall perioperative myocardial injury (PMI) rate was 15.4% (n = 140). Septic revision surgery for PJI was associated with a significantly higher PMI rate (43.8% (14/32) vs 14.5% (57/393); p = 0.001) and one-year mortality rate (18.6% (6/32) vs 7.4% (29/393); p = 0.038) compared to aseptic revision or primary arthroplasty. The association with PMI persisted in multivariable analysis with an adjusted odds ratio (aOR) of 4.7 (95% confidence interval (CI) 2.1 to 10.7; p < 0.001), but was not statistically significant for one-year mortality (aOR 1.9 (95% CI 0.7 to 5.4; p = 0.240). PMI rate (15.2% (5/33) vs 14.1% (64/453)) and one-year mortality (15.2% (5/33) vs 9.1% (41/453)) after FRI revision surgery were comparable to aseptic long-bone fracture surgery. Conclusion Patients undergoing revision surgery for PJI were at a risk of PMI and death compared to those undergoing aseptic arthroplasty surgery. Screening for PMI and treatment in specialized multidisciplinary units should be considered in major bone and joint infections. Cite this article: Bone Joint J 2022;104-B(6):696–702.

Pekerja Migran Indonesia (PMI) adalah Warga Negara Indonesia yang sebelum bekerja, setelah bekerja dan akan bekerja di luar negeri menurut Undang Undang Perlindungan Tenaga Kerja Migran. Badan Pelindungan Pekerja Migran Indonesia (BP2MI) memiliki peran penting dalam penyaluran PMI baik sebelum bekerja,telah bekerja dan akan bekerja di luar negeri, sehingga pada era Covid-19 terdapat berbagai kebijakan yang dikeluarkan oleh pemerintah untuk perlindungan PMI di Provinsi Jawa Tengah. Penelitian ini bertujuan untuk menganalisis peran BP2MI dalam perlindungan PMI di Provinsi Jawa Tengah Sekaligus menganalisis kendala yang dihadapi BP2MI dalam Perlindungan PMI di masa New Normal di Provinsi Jawa Tengah. Metode yang digunakan pendekatan Yuridis Empiris yang diperoleh melalui data primer dan sekunder yang dianalisis menggunakan metode Analisis Kualitatif. Pada hasil penelitian menunjukkan bahwa wabah Covid 19 telah mempengaruhi pengiriman PMI ke luar negeri sehingga pengiriman PMI mengalami penurunan pada rentang waktu tahun 2020 sehingga peranan lembaga BP2MI sangat mendesak dalam melindungi PMI di luar negeri di masa new normal berdasarkan Undang-Undang Perlindungan PMI dan Kpeputusan Menteri Tenaga Kerja tentang masa adaptasi baru (New Normal), Pada rentang waktu tahun 2020 BP2MI harus menjamin perlindungan terhadap setiap PMI yang akan di salurkan melalui Provinsi Jawa Tengah dengan berkoordinasi dengan P3MI sebagai perusahaan penyalur sehingga keamanan dan keselamatan terpenuhi bagi setiap PMI di masa adaptasi kebiasaan baru dewasa ini.

Abstract:
Objective. To investigate the clinical characteristics of patients with unstable angina (UA) who received elective percutaneous coronary intervention (PCI) in a traditional Chinese medicine (TCM) hospital and to analyze the related risk factors of periprocedural myocardial injury (PMI). Methods. On the basis of cross-sectional investigation, the case-control method was adopted. We retrospectively collected clinical data of patients with UA who successfully received elective PCI in Beijing Hospital of TCM from February 2017 to February 2019. Based on the occurrence of PMI, the case-control was formed. The influence of related factors on PMI occurrence was analyzed using the logistic multiple regression equation based on the parameters between the comparison groups. Results. 1. Incidence of PMI and related clinical features: Of the 265 UA patients, the incidence of PMI was 26.4%, nearly one quarter (23.4%) had old myocardial infarction, nearly half (45.3%) had previously received coronary intervention. The prevalence of patients with previous hypertension (75.8%), type 2 diabetes (57%), and high-low-density lipoprotein cholesterolemia (69.3%) exceeded 50%, more than 50% of the patients have triple-vessel disease (50.2%). 2. Features of TCM syndrome elements: The main TCM syndromes of the investigated patients are blood stasis syndrome (81.1%) and Qi deficiency syndrome (77.3%), the others include Phlegm turbidity syndrome (53.2%), Yang deficiency syndrome (50.9%), Yin deficiency syndrome (50.1%), Qi stagnation syndrome (30.1%), and coagulated cold syndrome (17.1%). 3. Factors of PMI occurrence: According to the occurrence of PMI, 265 patients were divided into PMI group (n = 70) and non-PMI group (n = 195). The comparison between groups shows that the preoperative SYNTAX score, the number of stents, and the total length of stents of the patients in the PMI group were higher than those in the non-PMI group ( P < 0.05 ); the patients in the PMI group treated by Shen-Yuan-Dan (SYD), a Chinese medicine prescription with Qi-supplementing and blood stasis-purging, were significantly lower than those in the non-PMI group ( P < 0.05 ). Brought these four factors (preoperative SYNTAX score, number of stents implanted, total length of implanted stents, and treated by SYD) into the binary logistic regression equation, those who were only treated by SYD have statistical significance in the equation as a protective factor (OR 0.327, 95% CI 0.117–0.916, P = 0.033 ). Conclusion. Patients with UA who received elective PCI in TCM institutions may have clinical characteristics including multiple accompanying diseases and high stenosis coronary artery, in which the incidence of poor blood glucose control and high rate of three-vessel coronary disease are particularly significant. The TCM syndromes are mainly Qi deficiency and blood stasis syndromes. The decrease of PMI may be attributed to the application of SYD in the real world. This trial is registered at ChiCTR2100043465.

Peranan Balai Pelayanan Penempatan dan Perlindungan Tenaga Kerja Indonesia (BP3TKI) Nunukan dalam menyalurkan Pekerja Migran Indonesia (PMI) ke Malaysia. Penelitian ini bertujuan untuk mengetahui Peranan serta menjadi faktor pendukung dan penghambat BP3TKI Nunukan dalam menyalurkan Pekerja Migran Indonesia (PMI) ke Malaysia. Hasil penelitian yang di lakukan menunjukkan bahwa peranan BP3TKI Nunukan dapat dilihat dalam beberapa hal seperti: pelayanan yang diberikan kepada PMI sudah cukup memudahkan dalam mengurus segala dokumen dan di dukung dengan sebuah program Layanan Terpadu Satu Atap (LTSA) yang didalamnya terdiri dari beberapa instansi diantaranya; BP3TKI, Disdukcapil, Disnakertrans, Imigrasi, RSUD, dan BPJS Ketenagakerjaan. Dalam hal penempatan yang dilakukan oleh BP3TKI dimana Perusahaan Penempatan Pekerja Migran Indonesia (P3MI) membawa perjanjian kerja (job order) yang dalam ketentuannya mengatur tentang masa kerja, gaji pekerja, cuti pekerja dan asuransi kerja sehingga ketika ditempatkan PMI tahu mengenai yang menjadi hak selama bekerja, ketiga perlindungan yang diberikan BP3TKI dimulai saat PMI berproses kemudian selama bekerja atau ditempatkan dan saat dipulangkan kekampung halamannya, keempat penyelesaian masalah PMI yang di deportasi dari Malaysia kemudian di proses dan di tampung dan ditanyakan terlebih dahulu mengenai keinginan melanjutkan sebagai PMI atau kembali kekampung halaman. Faktor penghambat peranan BP3TKI Nunukan dalam menyalurkan PMI ke Malaysia seperti: sebagian warga Nunukan menjadikan peluang bisnis untuk merekrut CPMI dan menyeberang secara non prosedural atau ilegal, warga negara yang lahir di Malaysia ketika di deportasi karena tidak memiliki dokumen dan saat ingin dipulangkan kekampung halamannya PMI tersebut tidak mengetahui dimana kampung halamannya karena sudah berada dalam waktu yang cukup lama sekitar 3-4 generasi berada di Malaysia, banyaknya akses ke Malaysia yang sering menyulitkan BP3TKI secara bersama-sama dengan satuan Kepolisian dan TNI AL untuk melakukan pencegahan dan waktu menunggu VISA masuk yang lama sekitar 1 minggu. Faktor pendukung peranan BP3TKI Nunukan dalam menyalurkan PMI ke Malaysia ada beberapa hal seperti di dukung oleh media, P3MI yang tertib, dan dibantu dengan satuan Kepolisian dan TNI AL.

Abstract:
The purpose of this experimental study was to evaluate the range of particulate mass (PM) and particulate number (PN) results from gasoline direct injection (GDI) vehicles by using four test fuels with a range of particulate matter index (PMI) from 1.38 to 2.39 and particulate evaluation index (PEI) from 0.89 to 1.92. The properties of four test fuels were analyzed with detailed hydrocarbon analysis (DHA). Two passenger cars with a GDI engine were tested with four test fuels by conducting the China 6 test procedure, which is equivalent to the worldwide harmonized light-duty vehicle test procedure (WLTP). When the fuels could meet the China 6 primary reference fuel standard with PMI from 1.38 to 2.04 and PEI from 0.89 to 1.59, the PM variation of Vehicle B was from 1.94 mg/km to 3.32 mg/km and of Vehicle A was from 2.55 mg/km to 4.15 mg/km, respectively. In addition, the PN variation of Vehicle B was from 1.57 × 1012 #/km to 3.38 × 1012 #/km and of Vehicle A was from 3.02 × 1012 #/km to 4.80 × 1012 #/km. It was noted that the two different cars had a unique response and sensitivity by using the different fuels, but PMI and PEI did trend with both the PM and the PN response. All PM and PN results from the two cars had an excellent correlation R2 > 0.94 with PMI and R2 > 0.90 with PEI. Therefore, PMI/PEI would be the appropriate specification for sooting tendency in reference fuel standards of emission regulations.

Abstract:
Aiming to enhance understanding of visual obstacles inherent in two-dimensional (2-D) sketches used in high school spatial geometry instruction, we propose a measure of visual difficulty based on two attributes of the sketches: potentially misleading geometrical information (PMI) and potentially helpful geometrical information (PHI). The difficulty of 12 normatively oriented cube-related sketches was theoretically ranked according to their ratios, #PHI/#PMI. The ranking was compared to the actual visual difficulty as measured by the percentage of correct or desired comprehension, individual spatial ability, and study-time allocation. This procedure was repeated for unnormatively oriented sketches, obtained by vertically flipping the original sketches. In both cases, the findings substantiate #PHI/#PMI as an a priori measure of visual difficulty. Practical, theoretical, and methodological implications are inspected and discussed.

Abstract:
Purpose. To evaluate the efficacy and safety of nicorandil for periprocedural myocardial injury in patients undergoing PCI through meta-analysis of randomized controlled trials. Methods. We analyzed the clinical data of patients including the incidence of periprocedural myocardial injury (PMI) and major adverse cardiovascular events (MACE) from selected articles. RCTs were retrieved from medical literature databases. RR and 95% confidence intervals (CI) were calculated to compare the endpoints. Results. In total, 15 articles (16 trial comparisons) were retrieved which contained 2221 patients. In general, 1130 patients (50.9%) were randomized to the experimental group, whereas 1091 patients (49.1%) were randomized to the control group. The result showed that nicorandil significantly reduced the incidence of PMI and MACE after PCI compared to the control group. Conclusions. Overall, early use of nicorandil in patients undergoing percutaneous coronary intervention (PCI) was associated with a significant reduction of PMI and MACE.

Abstract:
Percutaneous coronary intervention (PCI), as a method of myocardial revascularisation, is widely and effectively used for the treatment of coronary heart disease (CHD), with immediate success rates of >90%. Depending on the diagnostic criteria, 5–30% of these patients could develop the signs of periprocedural myocardial damage (PMD) or periprocedural myocardial infarction (PMI). PMD predictors, mechanisms of PMD development, and its specific clinical features play an important role in the PMI prevention. At present, there is no universal agreement on the definition and diagnostics of periprocedural myocardial necrosis and PMI, or on their impact on the clinical outcomes. According to the results of the recent studies, which are presented in this review, the current criteria of PMI might need to be modified, due to the increasingly high sensitivity of the modern threshold levels of troponin.

While healthcare industry is striving to achieve e-health systems for improvements in healthcare quality, cost, and access, privacy and security about medical records should be considered carefully. This paper makes a deep study of Public Key Infrastructures (PKIs) and Privilege Management Infrastructures (PMIs) and how they can secure e-health systems. To access resources, e.g. patient records, both authentication and authorization are needed, so public key certificates and attribute certificates are both required to protect healthcare information. From a typical medical scenario, we see not only static but also dynamic permissions are required. Dynamic authorization maybe the most complex problem in e-health systems.

Abstract:
Purchasing Mangers' Index (PMI) is an index published by the Institute for Supply Management. The index is based on a monthly survey answered by hundreds of purchasing managers in the manufacturing business all over the United States. The questions capture the activities of companies when it comes to orders, production, employment, supplier deliveries and inventories. PMI is an important and reliable macro economic factor and has shown a high correlation with the GDP which means that it may be of interest for a bank as Nordea to predict it. The purpose of this thesis is to evaluate whether it is possible to use neural networks, more precisely sequence-to-sequence models and recurrent neural networks, to forecast PMI with a multivariate data set. Simpler methods and models are also evaluated and compared to the neural networks. In total, there was two naive models, one Autoregressive Moving Average (ARMA) model, two linear regression models, and three neural network models that was implemented and compared to each other. All models were evaluated by their mean squared error (MSE) and mean absolute error (MAE).  By analyzing MSE and MAE for the different models it is shown that the ARMA-model predicted only slightly better than the persistence algorithm which assumes that PMI next month equals PMI current month. Only one neural network model performed better compared to the linear regression models, and the same model also gave a better prediction compared to the persistence algorithm.

C. M. (Supply Chain Management) sont hors de portée pour les PME-PMI, comment les organiser pour y accéder? Le design de notre papier est conçu en cinq parties: la présentation de la weltanschauung teilhardienne ; l'axiomatique du système/objet; la réfutation de la pensée conforme; la modélisation de l'objet et, la validation par le satisfacing. Le corpus théorique de notre thèse s'appuie sur les théories du constructivisme, du Système Général de Le Moigne et " administrative " de Simon. Le terrain choisi est celui des Chambres consulaires. The premises of our research can be described through a two-fold professional challenge. The France is under-equipped in terms of removable containers. What should be done to reduce this handicap? The S. C. M. (Supply Chain Management or global logistics) software are out of reach for small and midsize companies. How can they be organized in order to make them accessible? Our paper is designed in five parts: the introduction presents the teilhardian weltanschauung, the axiomatic of our research, the refutations of the "true thought", the modelling leads of the object and, the validation by the satisficing. The theoretical corpus of out thesis is founded on three theories : the constructivism, the General System, Simon "administrative " theory. We chose the field of the consular Chambers. We realized that the construction from scrap of a network is not realistic in the context of our thesis which led us to propose the systemic metaphor of the "Chambre de métiers" of Haute-Savoie

La façon de se comporter face à ce choix fondamental qui est l'investissement ; diffère selon que l'on se trouve dans une grande entreprise ou une petite et moyenne entreprise. Pour cette dernière, l'hétérogénéité de la population qui la compose constitue le fruit d'une grande disparité des attitudes et aussi des valeurs de référence des décideurs. Les valeurs et les attitudes sont susceptibles d'expliquer le comportement des individus, en tant qu'informations cognitives et éléments de la personnalité, elles prédisposent l'homme à un certain comportement. Face aux décisions concernant les problèmes d'investissements le comportement de l'entrepreneur P. M. E P. M. I. Porte généralement sur des considérations subjectives. Il s'ensuit que les autres membres de l'entreprise jouent un rôle toujours moins important dans l'élaboration de telles décisions qui sont pour une part toujours plus grande, réserves. Modern economy characterizes itself by a continuous increase that soon constrains the company to adapt itself to the market and fight against competition. Facing with this fundamental choice, which is investment, the way of behaviouring is different according as you own to a big or small and middle - sized company. For this latter, the heterogeneity in population which composes it does constitute the result of a great disparity in attitudes and also in settlers reference values, values and attitudes are susceptible to justify individual behavior. As far as cognitive informations and personality components, they predispose the man in a certain behavior. Face to decisions regarding investment problems, the P. M. E. P. M. I. Contractor's behavior is generally concerning with subjective concerning with subjective considerations, hence it follows that the company other members are always playing a less important role in the elaboration of such decisions which, always in a great measure are reserved to the company dealer. Si les notions d'intelligence économique et de veille technologique prennent de plus en plus d'importance comme facteurs de compétitivité des entreprises, l'innovation est un autre de ces facteurs. Les modalités de l'innovation sont ici abordées d'après un certain nombre de rencontres que nous avons menées auprès des dirigeants de PMI et de PME d'une zone économique en difficulté. Nous avons dégagé des variables d'innovations de l'analyse des interviews semi-directives recueillies auprès d'un échantillon d'entreprise. Des modèles descriptifs d'intéractions de ces variables sont proposés en vue d'une meilleure compréhension des processus d'innovation. D'autre part nous décrivons une méthodologie d'analyse de discours oral des dirigeants d'entreprises à l'aide d'outils bibliométriques initialement conçus pour l'analyse de grandes quantités de données de type bibliographique. Les deux méthodes, analyse thématique et statistique, donnent des résultats complémentaires. Enfin, l'accent est mis sur la relative imprécision des informations disponibles sur les activités et technologies caractéristiques d'un tissu économique donné en vue d'une opération de transfert de technologie ou de coopération recherche-industrie. Un système d'information est proposé pour établir une corrélation à priori entre les activités d'entreprises et les thématiques de la recherche publique

Abstract:
The objective of reliability-based robust design optimization (RBRDO) is to minimize the product quality loss function subject to probabilistic constraints. Since the quality loss function is usually expressed in terms of the first two statistical moments, mean and variance, many methods have been proposed to accurately and efficiently estimate the moments. Among the methods, the univariate dimension reduction method (DRM), performance moment integration (PMI), and percentile difference method (PDM) are recently proposed methods. In this paper, estimation of statistical moments and their sensitivities are carried out using DRM and compared with results obtained using PMI and PDM. In addition, PMI and DRM are also compared in terms of how accurately and efficiently they estimate the statistical moments and their sensitivities of a performance function. In this comparison, PDM is excluded since PDM could not even accurately estimate the statistical moments of the performance function. Also, robust design optimization using DRM is developed and then compared with the results of RBRDO using PMI and PDM. Several numerical examples are used for the two comparisons. The comparisons show that DRM is efficient when the number of design variables is small and PMI is efficient when the number of design variables is relatively large. For the inverse reliability analysis of reliability-based design, the enriched performance measure approach (PMA+) is used.

Abstract:
Platelet membrane glycoprotein (GP)IIb-IIIa is a component of a common adhesive protein receptor for fibrinogen, fibronectin, and von Willebrand factor. A monoclonal antibody, PMI-1, defines a divalent cation dependent regulation of the surface orientation of the heavy chain of GPIIb. Exposure of the PMI-1 epitope inversely correlates with the capacity of platelets to bind fibrinogen and aggregate. We have now localized and chemically synthesized this epitope. A 1.1 Kb cDNA clone which directs the synthesis of a fusion protein which bears the PMI-1 epitope was isolated from a lambda gt 11 expression library constructed from mRNA from the human erythroleukemia (HEL) cell line. The position of the N-terminal sequence of the light chain of GPIIb in the deduced amino acid sequence of the clone defined the orientation of the light and heavy chains of GPIIb. Analysis of the amino acid sequence corresponding to the heavy chain of GPIIb identified a single region with a high likelihood of containing a continuous epitope. A synthetic 17 residue peptide, corresponding to the predicted antigenic site, inhibited the binding of PMI-1 to platelets. Two uM peptide was required to inhibit binding 50% in the presence of 1 uM PMI-1, indicating an approximate dissociation constant of 1.5 uM for the peptide-antibody complex. This figure should be compared to a Kd of 0.95 uM (JCI 78:1103, 1986) for PMI-1 binding to GPIIb. A second peptide, corresponding to the region immediately adjacent to the predicted antigenic site, failed to inhibit PMI-1 binding. Neither peptide inhibited the binding of two other monoclonal anti GPIIb-IIIa’s to platelets. The peptides had similar effects on PMI-1 interaction with purified GPIIb-IIIa in detergent solution. These data localize the PMI-1 epitope to a 17 amino acid region located near the carboxyl terminal of the heavy chain of GPIIb. Thus, they chemically define a region of GPIIb whose surface expression reflects the competence of GPIIb-IIIa as a component of a platelet receptor for adhesive proteins.

The United States represents the world’s largest market for pharmaceutical drugs. It is also the only advanced economy in the world that does not regulate drug prices. There is no upper threshold for the prices of medicines in the United States. List prices are instead set by manufacturers in negotiation with supply-chain intermediaries, though some federal programs have degrees of discretion in price determinations. In practice, this deregulated system means that drug prices in the United States are generally far higher than in other advanced economies, adversely affecting patient accessibility and system affordability. In this paper, we draw on the “theory of innovative enterprise” to develop a framework that provides both a critique of the existing pricing system in the United States and a foundation for developing a new model of pricing regulation to support safety and effectiveness through drug development as well as accessibility and affordability in the distribution of approved medicines to patients. We introduce a regulatory approach we term “Pricing for Medicine Innovation” (PMI), which departs dramatically from the market-equilibrium assumptions of conventional (neoclassical) economics. The PMI approach recognizes the centrality of collective investments by government agencies and business firms in the productive capabilities that underpin the drug development process. PMI specifies the conditions under which, at the firm level, drug pricing can support both sustained investment in these capabilities and improved patient access. PMI can advance both of these objectives simultaneously by regulating not just the level of corporate profit but also its allocation to reinvestment in the drug development process. PMI suggests that although price caps are likely to improve drug affordability, there remain two potential issues with this pricing approach. Firstly, in an innovation system where a company’s sales revenue is the source of its finance for further drug development, price caps may deprive a firm of the means to invest in innovation. Secondly, even with adequate profits available for investment in innovation, a firm that is run to maximize shareholder value will tend to use those profits to fund distributions to shareholders rather than for investment in drug innovation. We argue that, if implemented properly, PMI could both improve the affordability of medicines and enhance the innovative performance of pharmaceutical companies.
