In the long run, responding to the challenge of climate change is going to require a de-carbonized economy with different energy systems and reconceptualized social-ecological relationships. Given the magnitude of the task, quite how to move in this direction remains a matter of some contention. In this chapter we examine several potentially complementary approaches, though nobody has yet identified the key to transition, and we do not resolve all the big questions about how to proceed. This coverage of a variety of innovations also makes sense in light of the failure to date of established centers of power—be they the global economic system, UNFCCC negotiations, or national governments—to craft effective responses. We group innovations under social resilience, new thinking about economics, new movements that embody this thinking, and new governance. Some developments are incremental, some more radical. These developments give us something more upbeat to end on, showing that transition is already being lived, if only, so far, on the margins of societies. In Chapter 4 we looked at the importance of adaptation, especially in light of current global failure to advance on the mitigation front. Adger et al. (2011a: 757) point out that “adaptive responses are not equal in terms of the sustainability of resource use, energy intensity, reduction of vulnerability, or in the distribution of their benefits.” While the international community has had difficulty in coming to terms with adaptation (even by 2011 the Green Climate Fund adopted at the Durban meeting of UNFCCC covered adaptation but omitted any clear definition of the term), it does hold significant potential. Adaptation does not have to be defeatist, and indeed can be linked to empowerment— we adapt by recognizing “that human beings can protect themselves from damage by living harmoniously with their atmospheric environment” and reduce their vulnerability to climate change (Burton, 1994: 15). In this section we consider how adaptation might contribute to transition. On the academic side, adaptation to environmental change is increasingly understood in terms of the concept of resilience. According to the multinational Resilience Alliance (2013), “A resilient ecosystem can withstand shocks and rebuild itself when necessary.

Abstract: Spinal dural arteriovenous fistulas are a rare cause of congestive myelopathy. Symptoms are insidious in onset and may be confused with degenerative spinal disease. MRI characteristically shows edema of the spinal cord with serpiginous flow voids that follow the surface of the spinal cord. Careful evaluation with spinal angiography is required to ensure accurate diagnosis. Spinal dural arteriovenous fistulas differ from spinal arteriovenous malformations in that most fistulas have only a single fistulous point without a nidus. Spinal dural arteriovenous fistulas may be treated successfully with either surgical resection or endovascular embolization depending on their anatomy. Earlier treatment is associated with better outcomes.

Vascular neurosurgery has long been one of the focal points of neurosurgical practice, and the topic is routinely included in the Oral Board Examination process. Each case involves clinical and diagnostic acumen, a solid grasp of the natural history and treatment risks, consideration of operative approaches, alternative treatment options, and the management of potentially catastrophic complications, as a result of what we have done or what we failed to do. Five cases are discussed in this chapter, and for each case the history and physical symptoms, imaging studies, analyses of the cases and treatment plans, and complications are provided. Key points are also presented after each case. The cases discussed include an aneurysmal subarachnoid hemorrhage (aSAH), an unruptured brain arteriovenous malformation (AVM), an intracerebral hemorrhage (ICH), a spinal dural arteriovenous fistula (dAVF), and symptomatic carotid stenosis.

La psycho-oncologie constitue aujourd'hui un corpus significatif de connaissances empiriques et cliniques permettant de s'intéresser aux difficultés et problématiques liées à la maladie cancéreuse ainsi qu'aux différentes interventions influençant positivement l'ajustement psychologique du patient ou de ses proches. Les travaux en psycho-oncologie s'intéressent également à l'impact psychologique du cancer où l'on retrouve plusieurs angles cliniques de recherche telle que l'impact du cancer sur la qualité de vie du patient ou de ses proches ou encore l'impact physique, psychologique ou psychiatrique du cancer; ceux-ci pouvant se rencontrer tant dans la phase active des traitements que durant la phase de rémission et de l'après-traitement. Le cancer de l'adulte et de l'enfant se distingue d'un point de vue étiologique, médico-clinique et des protocoles de soins mis en place. Cependant, l'expérience du cancer exige tant chez le patient que chez ses proches un ajustement et un processus continu d'adaptation à la réalité du cancer et de l'après-cancer.
