OBJECTIVES: The precise identification of the bleeding point is important to the efficient management of epistaxis. Our hypothesis was that the septum was the commonest bleeding site. STUDY DESIGN AND SETTING: This was a prospective clinical study of 50 consecutive adult idiopathic posterior epistaxis patients carried out in a busy teaching hospital. METHODS: The nasal cavity was inspected with endoscopy to identify the site of bleeding before any intervention. The location of bleeding sites was recorded on a proforma. RESULTS: Of bleeding sites posterior to the piriform fossa, 94 percent were

Introduction: Posterior epistaxis brings us with a more challenging condition that can even threaten the life of a patient. Approaching the posterior nasal cavity is not an easy task because of its anatomical complexity and impending vision by turbinate. Increase endoscopic anatomic details of lateral nasal wall and endoscopic expertise with high definition camera leads to a higher success rate of endoscopic sphenopalatine artery ligation. Methods: This prospective study was conducted in patients with refractory posterior epistaxis between January 2017 to December 2019. Five patients needed further cautery in order to control the bleeding. Three patients required temporary balloon tamponade, and a single patient had internal maxillary ligation. Epistaxis is a common reason for seeking emergency medical care. As a rule, these visits are associated with the recurrent bleeding, which is typical for epistaxis from the posterior parts of the nasal cavity due to their difficult accessibility and anatomical features of the vascular system. The review article provides an algorithm for treatment of the patient with posterior epistaxis including the sequential use of methods to stop bleeding: electrocoagulation of the bleeding vessel, tamponade of the nasal cavity, coagulation of vessels along the length, embolization.

AbstractPosterior epistaxis is usually treated by repeated nasal packing and in failed situations by ligation of feeding arteries with considerable morbidity and mortality. The most logical approach should be location of the bleeding site and arrest of haemorrhage by local treatment. The exact location of the bleeding area can be identified in actively bleeding noses with the fibreoptic naso-laryngoscope and the bleeding arrested by chemical, or thermal cautery and in failed situations by using small nasal packs confined to the bleeding site. Abstract Intracranial hemorrhage is any bleeding within the intracranial vault, including intracerebral hemorrhage (ICH) and intraventricular hemorrhage (IVH). Most ICH is associated with chronic hypertension; superficial ICH in older patients is often associated with cerebral amyloid angiopathy. Immediate reversal of anticoagulation and moderate blood pressure control with medical management are the standard of care. For patients who have ICH or IVH with symptomatic hydrocephalus, an external ventricular drain for cerebrospinal fluid diversion can be considered. ICH has only a few indications
