Full textOur team had discovered a new method to obtain extracts of Bacteroides thetaiotaomicron (E) which preserved its viability. This intestinal symbiont was anaerobically grown on an agar medium poorly supplemented in growth factors. After exposure to air, the bacterium seemed to possess and generate in E all the equipment able in vitro to detoxify reactive oxygen species. It let us expect a therapeutic power referred to anti-inflammatory properties.Objectives and methods: The aim was first to characterize E, in terms of carbohydrates, lipids and proteins. To achieve this last-mentioned goal, proteins contained in E coming from living bacteria were separated by two-dimensional electrophoresis and identified by the peptide mass fingerprinting technique. The gels (n ≥ 6) were statistically analyzed (PDQuest®, Bio-Rad). To find the origin of these proteins in bacteria, they were compared with those obtained by destruction of B. thetaiotaomicron (BT) and identified in the cell fraction containing the bacterial outer membrane proteins. Electron microscopy work was also undertaken to visualize any event occurring during extraction.The antioxidative effect of standardized E extracts was checked in vitro. E safety was also controlled in cell models using polymorphonuclear neutrophils. An E anti-inflammatory effect was then searched in animal models. E was first evaluated using a skin irritation mouse model. Inflammation was induced by benzalkonium chloride on ears of anesthetized mice. Positive and negative controls were treated in parallel. The ear thickness was measured every hour for 5 h and histological ear sections were performed after 2h for some animals. Two different staining methods enabled the enumeration of degranulating mast cells in ear sections.The effect of the bacterial extract was next tested locally by intrarectal (IR) instillations in mice undergoing the early stages of inflammation in a dextran sodium sulfate (DSS)-induced colitis. This acute model evolved over 8 days. In parallel, positive and negative animal controls underwent or not the colitis and were treated or not. Clinical and colonic histological severity scores were daily determined. Inflammation markers were measured in mouse colonic tissues after animal autopsy. [...]

Full textArt leads to a kind of revelation which allows us to rediscover the world but above all to rediscover the individual. This rediscovery and the wonder Art brings about call on two central aspects: Subjectivity and Expressiveness. This could be the cause of a certain Art’s transforming power as a space of revelation and expressiveness that contributes, moreover, to meet oneself, the other and the world. We consider that this transforming ability of Art constitutes, precisely, a foundation of its therapeutical power. The main purpose of this investigation is to create an approach to this therapeutical power of Art. The interest of this study lies on the fact that we start from an aesthetical point of view. The aesthetical approach of therapeutics in Art dismisses a psychological and psychoanalytical angle - that is a clinical view - from two aspects: the first one is the consideration of Art as a phenomenon per se instead of a means of psychological and psychoanalytical therapy; the second one resides in the emphasis of expressiveness. The aesthetical approach of Art therapeutics finds its justification, sense and raison d’être within a society and humanity that have become subjects of a lifestyle which has been imposed by a complexe dominant system. This system is characterised by a serious ecological crisis mainly resulting from the destructive action of what Felix Guattari calls « Global Capitalism » (CMI : Capitalisme Mondial Intégral)

Full textThe production of reactive oxygen species is strictly kept under control in the Human body. However, several conditions are characterized by the over-production or the uncontrolled production of these species, promoting damage to the host tissue. Among oxygen species producer, the myeloperoxidase/hydrogen peroxide/chloride system is a key element. This the consequence of the large quantity of myeloperoxidase found in neutrophils and that can be released rapidly during an inflammatory process. Moreover, hypochlorous acid, synthesized by this system, is a powerful oxidant.
We have studied the impact of non-steroidal anti-inflammatory drugs (oxicams, nimesulid and flufenamic acid) on three reactive oxygen species, namely, hydroxyl radical, hydrogen peroxide and hypochlorous acid. The first results showed the weak antioxidant properties of these molecules and the necessity to focus on the myeloperoxidase/hydrogen peroxide/chloride system. During the study of the myeloperoxidase inhibition, it appeared that flufenamic acid was an efficient inhibitor that modulated the hypochlorous acid production and was directly oxidized by the enzyme.
Due to the efficacy of flufenamic acid, this molecule was tested in a model of myeloperoxidase-dependent low-density lipoprotein (LDL) oxidation and compared to thiol-containing molecules like N-AcetylCystein and its lysinate salt, glutathione and captopril. The results showed that flufenamic acid lost part of its inhibiting effect while thiol-containing molecules demonstrated an interesting inhibiting activity in this model. A potential explanation could be the ability of myeloperoxidase to bind lipoproteins, masking the entry of its catalytic pocket. In these conditions, the inhibitor size becomes a key parameter in the inhibition of the MPO-dependent low-density lipoprotein oxidation and N-AcetylCystein appears as a powerful inhibitor in this context. These results render N-AcetylCystein an excellent candidate for studies that focus on the reduction of cardiovascular pathology risk.
Le corps humain est le siège constant de la synthèse d’espèces oxygénées réactives dont la production contrôlée est indispensable au bon fonctionnement de l’organisme. Cependant dans de nombreuses pathologies, il arrive qu’une production exagérée et/ou incontrôlée de ces espèces aboutisse à des dégâts oxydatifs. Parmi les mécanismes de production d’espèces oxydantes, le système myéloperoxydase / peroxyde d’hydrogène / chlorure détient une place importante. Ceci est notamment la conséquence de la grande quantité de MPO présente dans les neutrophiles, pouvant être libérée très rapidement lors de l’inflammation. De plus l’acide l’hypochloreux synthétisé par ce système est un très bon oxydant.
Nous avons étudié l’impact des anti-inflammatoires non-stéroïdiens (oxicams, nimésulide et acide flufénamique) sur trois espèces oxygénées réactives :le radical hydroxyle, le peroxyde d’hydrogène et l’acide hypochloreux. Les premiers résultats montrent le faible pouvoir anti-oxydant des molécules testées et la nécessité de concentrer notre recherche sur le système myéloperoxydase / peroxyde d’hydrogène / chlorure, responsable de la synthèse de l’acide hypochloreux. Lors de l’étude de l’inhibition de ce système, il est ressorti que l’acide flufénamique est un bon inhibiteur de la myéloperoxydase car il inhibe la synthèse de l’acide hypochloreux en étant directement oxydé par l’enzyme.
En raison de l’efficacité de l’acide flufénamique, cette molécule a été testée dans un modèle d’oxydation des lipoprotéines de basse densité (LDL) par le système myéloperoxydase / peroxyde d’hydrogène / chlorure en comparaison avec des thiols tels que la N-acétylcystéine et son sel de lysine, le glutathion et le captopril. Les résultats montrent une perte importante du pouvoir d’inhibition de l’acide flufénamique dans ce modèle alors que les thiols et la N-acétylcystéine en particulier, présentent une efficacité non négligeable. Ce phénomène pourrait être attribué à la capacité de la myéloperoxydase de se fixer sur les lipoprotéines, ce qui pourrait masquer l’entrée du site catalytique. Dans ces conditions, la taille de la molécule devient un facteur crucial dans l’inhibition de l’oxydation des lipoprotéines de basse densité et la N-acétylcystéine apparaît dès lors comme un inhibiteur puissant dont les résultats en font un excellent candidat dans des études d’intervention visant la diminution du risque de pathologies cardiovasculaires chez certains patients.

Full textIn radiotherapy, treatment planning is the optimization of the ballistics to administer the prescribed dose to the treated lesions while minimizing collateral doses received by the healthy tissue. The algorithm of the dose calculation is at the heart of this numerical simulation. It must be precise and computationally efficient. The antagonism of these two features has led to the development of rapid analytical algorithms whose improvement in dosimetric accuracy has nowadays reached its limit. The accuracy of the dose calculation algorithm is particularly important in proton therapy to fully exploit the ballistic potential of protons. The Monte Carlo proton transport method is the most accurate but also the least efficient. This thesis deals with the development of a Monte Carlo dose calculation platform that is sufficiently effective to consider its use in clinical routine. The main objective of the project is to accelerate the Monte Carlo proton transport without compromising the precision of the dose deposition. To do this, two lines of research have been exploited. The first was to establish a new variance reduction technique called the equivalent restricted stopping power formalism (formalism Leq). This technique significantly improves the algorithmic time complexity made constant (O(1)) instead of linear (O(n)) for the current Monte Carlo algorithms. The second line of research focused on the use of graphics processing units to improve the execution speed of the proton Monte Carlo transport. The developed platform, named pGPUMCD, transports protons on graphic processors in a voxelized geometry. In pGPUMCD, condensed and discrete interaction techniques are considered. The inelastic low-range interactions are modeled with a continuous proton slowing down using the Leq formalism and the energy straggling is considered. The elastic interactions are based on the multiple Coulomb scattering. The discrete interactions are the inelastic interactions, the nuclear elastic and the non-elastic proton-nuclei interactions. pGPUMCD is compared to Geant4 and the implemented physical processes are validated one after the other. For the dose calculation in a clinical context, 27 materials are defined for the tissue segmentation from the CT scan. The dosimetric accuracy of the Leq formalism is better than 0.31% for various materials ranging from water to gold. The intrinsic efficiency gain factors of the Leq formalism are greater than 30, between 100 to 630 for a similar dosimetric accuracy. Combined with the GPU acceleration, the efficiency gain is an order of magnitude greater than 10⁵. Dose differences between pGPUMCD and Geant4 are smaller than 1% in the Bragg peak region and below 3% in its distal fall-off for the different simulation configurations with homogeneous phantoms and clinical cases. In addition, 99.5% of the dose points pass the criterion 1% and the prescribing ranges match with those of Geant4 at less than 0.1%. The computing times of pGPUMCD are below 0.5 seconds per million of transported protons compared to several hours with Geant4. The dosimetric and efficiency performances of pGPUMCD make it a good candidate to be used in a clinical dosimetric planning environment. The expected medical benefit is a better control of the delivered doses allowing a significant margin and toxicity reductions of the treatments.

Full textThis research explores the reconfiguration of psychiatric intervention in the context of the decolonization of New Caledonia. To do so, it looks at the representations, histories of patients, care practices and discourses that frame mental disorders on the island of Lifou.In the context of the greater autonomy given to the institutions of New Caledonian, how are psychiatric practices which are strongly marked by the colonial legacy opening up and changing? How has the subjective experience of mental disorders and their care been changing?This dissertation is based on 14 months of fieldwork working at the intersection of various ethnographic entry points and angles such as family and institutional spaces of care, the rural villages of Lifou versus the urban space of Nouméa, narratives of people suffering from mental disorders and their families, but also the narratives and stories of healers, customary chiefs, pastors, nurses, psychologists and psychiatrists. Through a participant-observation led method of research that was employed in a decentralized pedopsychiatric institution on the island of Lifou, I analyzed the changes in European and Kanak discourses on mental health as well as their means of resolving mental disorders, all within the context of larger changes in techniques of governmentality. A few case studies help unfold the various patient histories that exist between these different spaces and sheds light on the subjective experience of mental disorders as well as the way in which families negotiate care in new treatment facilities. In order to better understand the healthcare institutions and actors, I also retrace the history of colonial psychiatry in New Caledonia – from an asylum-based psychiatry which was conceived within the prison colony, to its opening up in the frame of decentralized structures which take charge of care provision in addressing social suffering.This dissertation works to unravel the different dimensions of the political space of mental health, from the subjective experience of mental disorders to therapeutic power, for care institutions to the socio-sanitary actors who work within them. It shows how this space is criss-crossed by tensions between the ethics of relationality on the one hand, the techniques of discipline carried out through colonial history, and the institutional reconfigurations linked to the post-colonialism context and the ensuing neoliberalization of the economy, on the other
