Abstract:
Abstract There is a substantial need for new effective therapies for patients with recurrent or refractory diffuse aggressive NHL. Efficacy and safety of sphingosomal vincristine (SV), a novel formulation of vincristine encapsulated in sphingomyelin liposomes or ‘sphingosomes’, were evaluated in a multinational study in aggressive NHL patients with multiple unfavorable prognostic factors. Methods: 119 pts with relapsed or refractory aggressive NHL were enrolled. SV was administered at 2 mg/m2 without capping the dose as a 1 hr IV infusion every 2 weeks. The objective response rate (ORR) and 95% CI for the proportions and difference in proportions were calculated for the following subgroups: by age (≤ 60 vs. > 60); sensitive disease vs. resistant (refractory or relapsed within 3 months of prior treatment); IPI score 0–2 vs. 3–5; prior therapies 2 vs. >2; bone marrow involvement at baseline; and prior ABMT. Results: ORR in the entire group was 25% (7% CR/Cru and 18% PR) (ASH Abstr. 2353, 2003). A multivariate analysis showed that the two strongest predictors for response were number of prior regimens and sensitivity to the last therapy. The highest level of activity was observed in patients with sensitive disease and 2 prior regimens (ORR 64%). Response to SV in older patients was the same as in younger patients. Prior ABMT did not impact response with ORR being similar in both groups. SV was well tolerated and the safety profile was similar to standard vincristine despite a doubling of dose intensity.

626 Background: The determination of ER and PR has a considerable importance when evaluating the prognosis and the response to treatment in breast cancer (BC). Assessment of HER2 status is also a standard for the management of BC. Hormone receptors (HR), HER2, and increasingly, genomic profiles distinguish at least four major classes of BC: HER2+; HER2-HR+, which can be divided into two classes, favorable and unfavorable; and basal-like that express neither HER2 nor HR. In the clinical practice is possible to divided BC according to ER, PR and HER2: ER-PR-HER2+, ER-PR-HER2-, ER+PR+HER2-, ER+RP+HER2+; and less frequently ER+PR-HER2-, ER+PR-HER2+, ER-PR+HER2+ and ER-PR+HER2-. In the neoadjuvant setting the pathological complete response (pCR) to primary chemotherapy (PC) is associated to negativity for ER and PR receptors, and in patients (pts) with HER2 + BC, the addition of trastuzumab to PC increasing the rate of pCR. The aims of this study were to determine the pCR of PC with anthracyclines (A) and taxanes (T) in pts with LABC grouped according to ER, PR and HER2 status. Methods: Pts with LABC treated with PC including A and T were grouped according to ER, PR and HER2 status, and the pCR rate were analyzed using the chi-squared test and correlations with a p value of ≤0,05 were considered statistically significant. Results: A total of 103 pts were treated. There were 14.6% of ER-PR-HER2+ BC, 23.3% ER-PR-HER2-, 35.0%ER+PR+HER2-, 8.7% ER+PR+HER2+, 11.7% ER+PR-HER2-, 2.9% ER+PR-HER2+, 1.9% ER-PR+HER2+, and 1.9% ER-PR+HER2-. For the analysis of pCR only 100 pts were included. There were18 pts who achieved a pCR. Of these pts, 9/18 (50.0%) were RE-RP-HER2-, 5/18 (27.8%) ER-PR-HER2+, 3/18 (16.7%) ER+PR+HER2+, and 1/18 (5.6%) ER+PR+HER2- (p≤0.01). The table 1 show the pCR rate for each subgroup of BC. Conclusions: In patients with LABC to group BC according to ER, PR and HER2 status can help to predict pCR to PC. [Table: see text] No significant financial relationships to disclose.

Abstract:
Abstract Donor lymphocyte infusion (DLI) is use for relapse after allogeneic stem cell transplant (ASCT). Immune activation with cytokines maybe an alternative to DLI. We administered Granulocyte-Macrophage Colony Stimulating Factor (GM-CSF) and Interleukin-2 (IL-2) at the time of relapse after ASCT in patients (pts) with hematologic malignancies. Pts. received subcutaneous GM-CSF at 500 mcg/day on days 1–14 and IL-2 at 1 × 106 units/m2/day on days 8–14. Pts. were off immunosuppressive therapy and had no prior history of graft versus host disease (GVHD) at the start of treatment. Twelve pts. received IL-2/GM-CSF for treatment of relapse AML (7), ALL (2), CML (1), MDS (2). Median age was 55 (range 8–66). Stem cell sources included: peripheral blood = 9, bone marrow = 2, umbilical cord blood (UCB) = 1. Donor sources were: match-related sibling = 4 and match-unrelated donor = 8 (UCB=1). Nine pts. had resistant relapse or primary resistant disease at time of ASCT. Median time from transplant to relapse was 4 months (range = 1–14). Two pts. had failed DLI and 5 pts. had received reinduction chemotherapy prior to IL-2/GM-CSF. Eight pts. responded to IL-2/GM-CSF (CR = 7, PR = 1). Two pts. remain disease free at 18 and 26 months post IL-2/ GM-CSF. Six pts. developed GVHD and of these 4 were responders. Two pts. had GM-CSF discontinued due to increase in peripheral blood blasts. No other toxicities related to IL-2/GM-CSF except for mild flu-like symptoms. The table below summarizes quantitative analysis of immune activation. Values represent means +/− standard error at day 0 (first day of GM-CSF), day 8 (prior to start of IL-2) and day 14 (last day of IL-2 and GM-CSF). P-values are based on paired t-test analysis of day 8 versus day 0 and day 14 versus day 0, respectively. Flow cytometric analysis showed an increase in the numbers of T-lymphocytes (CD3) and T-cell subsets (CD3/CD8 and CD3/CD4) as well as an increase in natural killer cells (CD16/56). Although no differences were seen in the number of dendritic cell subsets, DC1/DC2 ratios decreased with the administration of GM-CSF/IL-2. Limited (n= 4) CD4/FoxP3 analysis did not show change in absolute numbers with administration of GM-CSF/IL-2 (data not shown). In conclusion, cytokine therapy with IL-2/GM-CSF is well tolerated and is an alternative to DLI for relapse after ASCT. Flow cytometry analysis demonstrated a quantitative increase in immune effector cells and polarization to DC2. Temas relacionados às estratégias competitivas têm sido cada vez mais abordados por pesquisadores que buscam indicativos do comportamento organizacional realizado por empresas frente a um ambiente mutável. A competição entre as organizações faz com que aquelas que não conseguem se adaptar a mudanças tenham dificuldades em sobreviver no mercado. O objetivo deste estudo foi analisar a influência das estratégias competitivas nas formas de gestão das empresas de eventos da cidade de Curitiba-PR e Região Metropolitana. O estudo é do tipo exploratório-descritivo, com análises quantitativas, e se deu por meio de aplicação de instrumento de coleta de dados junto a gestores de empresas organizadoras de eventos baseado nas estratégias competitivas de Porter, Ansoff e Miles e Snow. A estratégia mais indicada pela média de homogeneidade foi a estratégia de diversificação de Ansoff, e a segunda e terceira foram as estratégias de Miles e Snow de atitudes analíticas e atitudes prospectivas. Como principais resultados aferiu-se que as empresas utilizam estratégias de forma aleatória e não vinculadas a um planejamento estratégico formulado e discutido para obter o maior retorno e uma maior aplicabilidade com possibilidade de retorno na resolução de problemas. Percebe-se que devido à concorrência cada vez mais acirrada as empresas que subsidiam suas decisões em estratégias já consolidadas possuem fatores de sobrevivência em longo prazo. ABSTRACT Human immunodeficiency virus type 1 (HIV-1) variants resistant to protease (PR) and reverse transcriptase (RT) inhibitors may display impaired infectivity and replication capacity. The individual contributions of mutated HIV-1 PR and RT to infectivity, replication, RT activity, and protein maturation (herein referred to as “fitness”) in recombinant viruses were investigated by separately cloning PR, RT, and PR-RT cassettes from drug-resistant mutant viral isolates into the wild-type NL4-3 background. Both mutant PR and RT contributed to measurable deficits in fitness of viral constructs. In peripheral blood mononuclear cells, replication rates (means ± standard deviations) of RT recombinants were 72.5% ± 27.3% and replication rates of PR recombinants were 60.5% ± 33.6% of the rates of NL4-3. PR mutant deficits were enhanced in CEM T cells, with relative replication rates of PR recombinants decreasing to 15.8% ± 23.5% of NL4-3 replication rates. Cloning of the cognate RT improved fitness of some PR mutant clones. For a multidrug-resistant virus transmitted through sexual contact, RT constructs displayed a marked infectivity and replication deficit and diminished packaging of Pol proteins (RT content in virions diminished by 56.3% ± 10.7%, and integrase content diminished by 23.3% ± 18.4%), a novel mechanism for a decreased-fitness phenotype. Despite the identified impairment of recombinant clones, fitness of two of the three drug-resistant isolates was comparable to that of wild-type, susceptible viruses, suggestive of extensive compensation by genomic regions away from PR and RT. Only limited reversion of mutated positions to wild-type amino acids was observed for the native isolates over 100 viral replication cycles in the absence of drug selective pressure. These data underscore the complex relationship between PR and RT adaptive changes and viral evolution in antiretroviral drug-resistant HIV-1.

Trata-se de estudo observacional de corte transversal que teve como objetivo analisar a exposição e proteção solar de estudantes de Medicina de Curitiba (PR), através de questionário autoaplicado, comparando os resultados entre alunos de ambos os sexos que tiveram ou não intervenção da disciplina de Dermatologia. Análise estatística pelo Teste Exato de Fisher e Qui-Quadrado. Amostra composta de 398 estudantes com perfil sociodemográfico caracterizado principalmente por mulheres (54,9%), brancas, predominando a média de 23,8 anos. A maioria cursou a disciplina de Dermatologia (72,1%) e possui conhecimentos básicos sobre fotoproteção (65,3%). Quanto às práticas de exposição solar, os horários preferidos foram após 16 horas (55%) e entre 10-16 horas (23,3%), com uso diário de protetor solar (36,5%) e fator de proteção 30 ou superior (65,5%). Em relação aos riscos individuais para carcinomas cutâneos, 61,2% se enquadraram nos fototipos II e III de Fitzpatrick. Histórico familiar de câncer de pele ocorreu em 24,6% nos avós e/ou pais. Entre as mulheres que cursaram a disciplina de Dermatologia, verificou-se aplicação diária de fotoprotetor mais vezes ao dia e com fator de proteção solar mais alto.

<p>Este estudo teve como objetivo analisar o comportamento estratégico das empresas localizadas no município de Vitorino – PR. Conforme a tipologia proposta por Miles e Snow (1978) as empresas podem apresentar quatro tipos de comportamento estratégico, sendo eles o prospector, o defensivo, o analítico e o reativo. Com o intuito de responder ao problema de pesquisa, optou-se pela abordagem quantitativa com característica descritiva. A coleta de dados ocorreu uma amostra de 48 empresas em um universo de 119 empresas existentes no município. Como resultado da pesquisa considerando toda a amostra verificou-se que na área empresarial 42,7% das empresas apresentam comportamento estratégico analítico. Na área de engenharia, tanto o comportamento analítico quanto o reativo foram adotados por 27,8% das empresas, já na área administrativa o comportamento prospector foi o predominante, com 33,3% das empresas. Considerando os resultados inerentes aos comportamentos estratégicos apresentados pelas empresas, não considerando a estratificação por ciclos adaptativos e suas respectivas áreas, os dados evidenciam um equilíbrio entre os quatro tipos de comportamento. Este estudo ressalta a importância de as empresas adotarem um comportamento estratégico bem definido, que seja direcionado aos objetivos empresariais, e que possam potencializar a competitividade das empresas em seu nicho mercadológico.</p>

SummaryOptimal P2Y12 receptor blockade is critical to prevent ischaemic recurrence in patients undergoing percutaneous coronary intervention (PCI). We aimed to compare the level of platelet reactivity (PR) inhibition achieved by prasugrel and ticagrelor loading dose (LD) in diabetic acute coronary syndrome (ACS) patients undergoing PCI. We performed a single-center prospective open-label randomised trial. Patients with diabetes mellitus undergoing PCI for an ACS were randomised to receive prasugrel 60 mg or ticagrelor 180 mg. The primary endpoint of the study was the level of platelet reactivity (PR) assessed between 6 and 18 hours post-LD using the VASP index. We randomised 100 diabetic patients undergoing PCI for an ACS. No difference was observed in baseline characteristics between the two groups. In particular, the rate of patient receiving insulin therapy was identical (25 vs 28.6%; p =0.7). Ticagrelor achieved a significantly lower PR compared to prasugrel loading dose (17.3 ± 14.2 vs 27.7 ± 23.3%; p=0.009). In addition the rate of high on-treatment platelet reactivity, defined by a VASP ≥50%, tend to be lower in the ticagrelor group although the difference did not reach statistical significance (6 vs 16%; p=0.2). The rate of low on treatment PR was identical (60 vs 54%; p=0.8). The present study demonstrates that ticagrelor LD is superior to prasugrel LD to reduce PR in ACS patients with diabetes mellitus. Whether the higher potency of ticagrelor could translate into a clinical benefit should be investigated.

3571 Background: NPI-2358 is a vascular disrupting agent (VDA) that destabilizes tumor vascular endothelial cell architecture resulting in selective collapse of established tumor vasculature. In murine tumor models NPI-2358 produces tumor regression alone and synergistically with agents such as paclitaxel, docetaxel (TXT) and irinotecan, often with decreased toxicity. Methods: The objective was to determine the Recommended Phase 2 Dose (RP2D) of NPI-2358 in combination with TXT. Patients with previously treated advanced NSCLC or other malignancies where TXT use was appropriate were enrolled. Patients received 75 mg/m2 TXT every 21 days. NPI-2358 was administered IV 2 hours after TXT on Day 1, and alone on Day 8. The dose of NPI-2358 was escalated from the single agent biologic effect dose (BED) of 13.5 mg/m2 to the single agent RP2D of 30 mg/m2 using a 3+3 design. PK was assessed on Days 1 and 8. Results: 13 patients were enrolled of whom 10 had NSCLC. At all dose levels adverse events were consistent with those of both agents given alone. Adverse events commonly associated with NPI-2358 were seen, including nausea, vomiting, fatigue, fever, tumor pain and transient blood pressure elevations. One DLT of nausea, vomiting, dehydration and neutropenia occurred at 30mg/m2. PK analysis did not indicate a drug-drug interaction. Of the patients with NSCLC, 8 had measureable disease of which 2 demonstrated a partial response (PR), with 4 others having lesser regressions. The RP2D was thus 30 mg/m2 of NPI-2358 with 75 mg/m2 TXT. Conclusions: The combination of full dose NPI-2358 and TXT is tolerable. Although a limited data set, activity appears favorable relative to the 5–10% response rate reported with TXT alone in this population. Based on these results, efficacy is now being assessed in Phase 2 (the ADVANCE study), a randomized comparison of TXT ± 30 mg/m2 of NPI-2358 in 2nd- line NSCLC. Of note, entry criteria allow patients with squamous cell carcinoma, as VDAs do not appear to result in unfavorable outcomes associated with some other agents in this sub-population. Introduction: Prognosis and management of breast cancer are influenced by the classic variables such as histologic type and grade, tumor size, lymph node status, status of hormonal receptors- i.e. estrogen receptor (ER) and progesterone receptor (PR) of the tumor, and, more recently, HER-2/neu (Human Epidermal Growth factor Receptor-2) overexpression. Expression levels of the estrogen, progesterone and HER2/neu receptors which characterize clinically distinct breast tumours have been shown to change during disease progression and in response to systemic therapies. The interrelationship of ER, PR, and HER -2 has come to have a crucial role in the management of breast cancer. Aims & Objectives: To determine the ER, PR and HER2/neu status in breast malignancies. Place and duration of study: The study was conducted at Department of Histopathology, Federal Postgraduate Medical Institute, Shaikh Zayed Hospital Lahore, for the period of Six months. Material & Methods: 30 samples of diagnosed breast cancer were included in study. Formalin fixed paraffin embedded wax block were taken for immunohistochemical staining of ER, PR & HER-2/neu. Results: Out of 30 cases, racially 29 were Punjabi and 1 was Pakhtun. There were 23 mastectomy samples and 7 needle core biopsies. Out of 30 cases, 29 were Infiltrating Ductal Carcinoma. Grade I, II, III 13.3%, 66. 7% and 20% respectively and only one case was of Carcinosarcoma (p<0.01). Overall immunoexpression for ER, PR and HER-2/neu were 33.3% (p<0.01), 56.7% (p<0.05) and 73.3% respectively. Triple positive (TP) cases were 23.3% and triple negative (TN) were 10 %. Regarding ER expression in infiltrating ductal carcinoma, 63.3% were ER negative and 33.3% were ER positive and comparison was statistically highly significant (p<0.01) .PR expression in infiltrating ductal carcinoma showed 56.7% PR positive and 40% PR negative which was statistically significant (p<0.05).There is negative correlation of HER-2/neu positive and ER positive and positive correlation of HER-2/neu positive and PR positive cases. Conclusion: There is inverse correlation of HER-2/neu positive and ER positive and positive correlation of HER-2/neu positive and PR positive cases.

Abstract Background Studies in adults have shown a relationship between high blood pressure and hyperuricemia, but few studies have investigated this association in children and adolescents. The aim of the present study was to associate urate concentration with systolic blood pressure (SBP) and diastolic blood pressure (DBP) in schoolchildren. Methods This cross-sectional study used a sample of 2335 schoolchildren in basic education between 7 and 17 years old. Blood pressure was classified by percentile according to Brazilian parameters for sex and age, reclassified into two categories: normal and borderline/hypertension. Blood collection to obtain serum for urate concentration analysis was performed after a 12-h fast. Values above 5.5 mg/dL were considered hyperuricemia. Descriptive data were presented in frequency and percentage. Linear regression and Poisson regression (prevalence ratio [PR]) was used to test the association between urate concentration and blood pressure. Results Urate concentration was weakly associated with SBP (β: 0.05; 95% CI: 0.02–0.08) and DBP (β: 0.03; 95% CI: 0.01–0.05) z-scores. Schoolchildren with hyperuricemia had a higher prevalence of high SBP (PR: 1.12; p<0.001) and DBP (PR: 1.08; p<0.001). Conclusions Elevated urate concentration is associated with altered blood pressure in schoolchildren.

Aim: to analyze the endometrial expression of leukemia inhibitory factor (LIF) related to the estrogen/progesterone receptor endometrial status in women with the history of reproductive dysfunctions.Materials and methods. The main group consisted of patients with the history of infertility (n = 81) or early pregnancy loss (n = 40). The control group included 16 women with normal fertility. At days 6–8 after ovulation, endometrial biopsy was performed and peripheral blood samples were taken to assess the estradiol and progesterone levels. Histological and immunohistochemical (to quantify the estrogen (ER)/progesterone (PR) receptors and LIF expression) examinations of the endometrium biopsy materials were carried out.Results. In the middle phase of the secretion, women of the control group showed higher levels of LIF expression in the endometrial glands (94%; n = 15) and stroma (88 %; n = 14) significantly more often than patients with reproductive dysfunctions – 69 % (n = 84) and 44 % (n = 53) respectively (p < 0.05). There was no difference in the endometrial LIF expression between women with infertility and those with early pregnancy loss. In women with a sizable hormone-receptor 'response', a pronounced expression of LIF in the luminal epithelium (87 %; n = 52 out of 60) and stroma (68 %; n = 47 out of 69) was detected more often (p < 0.01) than that in patients with impaired hormone-receptor interactions (61 %, n = 27 of 44; 29 %, n = 20 of 68). High PR expression in the endometrial glands (H-score > 105) indicates the risk of impaired LIF expression in the luminal epithelium (OR =2.6) and stroma (OR = 2.5). Overexpression of ER (H-score > 155) in the endometrial stroma is associated with the risk of sub-normal LIF expression in the endometrial glands (OR = 2.5) and stroma (OR = 2.8).Conclusion. A meaningful connection has been found between the endometrial estrogen/progesterone receptor status and the expression of LIF. Women with well-pronounced endometrial hormone-receptor interactions show high levels of endometrial LIF expression more often. The stromal ER and glandular PR expression levels are considered to be prognostic factors of reduced endometrial LIF expression in the uterine body mucosa.

Endometrial aspiration biopsy was performed on 6–8 days after ovulation; in parallel, a peripheral blood sample was taken to determine the levels of estradiol and progesterone. A histological and immunohistochemical study of the endometrium with determination of estrogen (ER) and progesterone (PR) receptors and e-cadherin expression was carried out.Results. All fertile women (n = 16) and only 44 % (n = 53) of patients in the main group had the normal pattern of hormonereceptor endometrial interactions, i. e., low expression of ER and PR in the endometrial glands as well as reduced expression of ER and high expression of PR in the endometrial stroma. The endometrial e-cadherin expression was assessed in the following cohorts: a) women with normal hormone-receptor interactions (16 women in the control group and 53 women in the main group); b) with abnormal variants of hormone-receptor interactions in the endometrium (68 patients in the main group). The frequency of reduced e-cadherin expression was significantly lower in the cohort of women with normal estrogenprogesterone-receptor endometrial status (a) than in the cohort of women with abnormal variants of the endometrial “response” (b): in the luminal epithelium – in 5 % (3 out of 63) (а) vs. 17 % (8 out of 47) (b) (p < 0.05); in the glands – in 13 % (9 out of 69) (a) vs. 37 % (25 out of 68) (b) (p < 0.05).Conclusion. The estrogen-progesterone-receptor status of the endometrium is significant for the e-cadherin expression in the luminal epithelium and glands of the endometrium.

186 Background: In men with mCRPC receiving chemotherapy, measurable disease response by World Health Organization criteria has been demonstrated to be prognostic for overall survival (OS). We aimed to explore the association of changes in measurable disease by Response Evaluation Criteria in Solid Tumors (RECIST) with OS. Methods: Data from the control arm (n=612) of the phase III VENICE trial receiving docetaxel plus prednisone combined with placebo were available. Data on baseline clinical and laboratory variables were obtained in addition to outcome measures: best RECIST 1.0 response, PSA response and OS. Cox proportional hazards regression was used to evaluate the prognostic ability of RECIST-defined changes for OS using a 90-day landmark analysis. Results: 363 of 612 patients (59.3%) had measurable lesions, of whom 296 were evaluable for landmark analysis. 28 (9.5%) had progressive disease (PD) prior to day 90, while 58 (19.6%) had unconfirmed partial response (PR), i.e. declines ≥30% of the sum of diameters of target lesions. Median OS beyond day 90 for men with PR, stable disease (SD), and PD was 28.3, 23.3, 11.4 months, respectively (P < 0.001). In a multivariable analysis adjusting for pain, PCWG-2 subtype, type of progression (PSA vs. other), Gleason Score (≤ 7 vs. > 7), PSA, derived neutrophil-lymphocyte ratio, ECOG performance status and alkaline phosphatase, the hazard ratio (HR) for OS for patients with PR was 0.64 (95% CI 0.42 – 0.99, P = 0.045) compared to those without PR, and 1.78 (95% CI 1.07 – 2.95, P = 0.026) for those with PD compared to those without PD. PD remained significant (HR = 1.85, 95% CI 1.10 – 3.12, P = 0.020) after adjusting for PSA changes, but PR (P = 0.14) did not. Conclusions: In men with mCRPC receiving first-line docetaxel-based chemotherapy PR and PD by RECIST 1.0 within 90 days were associated with longer or shorter OS, respectively. Given the more frequent detection of measurable disease with current imaging, the accrual of patients with measurable tumors in phase II trials to assess RECIST changes may provide an objective signal of efficacy of new drugs.

A RPPN do Ninho do Corvo, localizada no Município de Prudentópolis, é um destes empreendimentos que possuem relato dos visitantes quanto as atividades de esportes radicais oferecidos pelo estabelecimento. Portanto, o objetivo deste estudo é identificar com base nos relatos dos usuários do Tripadvisor, quais as dimensões da esfera da economia da experiência são estimuladas pelas atividades na RPPN Ninho do Corvo em Prudentópolis. A pesquisa em questão se caracteriza por ser de natureza qualitativa, exploratória e descritiva e se utilizou da netnografia como técnica de coleta de dados e do programa de análise qualitativa MAXQA para auxiliar na apresentação e análise dos dados. Foi constato por meio do estudo que os relatos no site se referem principalmente as dimensões das experiencias de evasão, entretenimento e contemplação, vividas pelos visitantes na RPPN Ninho do Corvo. Esse trabalho teve como objetivo melhorar as condições higiênico-sanitárias dos alimentos comercializados na Feira do Produtor do município de Laranjeiras do Sul-PR para garantir a qualidade dos alimentos e a saúde dos consumidores. Para isso, foi inicialmente realizada uma pesquisa exploratória das condições higiênico-sanitárias dos ambientes de preparo, manipulação e comercialização dos alimentos a partir de visitas técnicas e diagnósticos das condições das propriedades dos agricultores e da comercialização de alimentos na feira. As não conformidades identificadas durante essa etapa do trabalho foram utilizadas para elaborar o curso de BPF baseado na legislação federal vigente focado, porém, na realidade dos feirantes. Os cursos foram ministrados em três locais para grupos de agricultores e participantes da feira, totalizando 101 pessoas. Após a execução dos treinamentos, foi realizado o monitoramento das atividades para observar o atendimento às boas práticas na feira e os resultados foram analisados de forma qualitativa e interpretativa. Como resultado, conclui-se que os cursos foram eficazes, pois melhorias significativas foram constatadas no local do estudo, como a melhoria dos hábitos de higiene praticados pelos feirantes, como práticas adequadas no preparo e comercialização de alimentos.

A Feira do Agricultor fornece uma importante fonte de renda para a agricultura familiar do município de Laranjeiras do Sul, na qual são comercializados principalmente hortaliças, produtos de panificação, massas alimentícias, doces e derivados do leite. O presente projeto de extensão universitária teve como objetivo a elaboração de uma ferramenta de cálculo de informação nutricional de produtos alimentícios. A partir de uma pesquisa exploratória, foi verificado que poucos produtores possuíam produtos alimentícios com rótulos e os mesmos tinham interesse na obtenção dos rótulos para se adequarem às normas municipal e federal. Para isso, uma ferramenta de cálculo foi desenvolvida em planilha eletrônica baseada na metodologia proposta pela Agência Nacional de Vigilância Sanitária (ANVISA) e em tabelas de composição de alimentos (TACO). Os feirantes foram conscientizados da importância de obter produtos alimentícios portando rótulos, e um levantamento da situação atual da feira foi realizado. Com o processo de rotulagem dos alimentos, as receitas tiveram que ser padronizadas, e o rendimento da produção foi medido. O serviço de confecção dos rótulos foi oferecido para os produtores que comercializavam os seus alimentos na feira e depois estendido para a comunidade. Development of a nutritional information calculation tool for labeling the food products sold at the Farmer’s Market from Laranjeiras do Sul-PR Abstract: The Farmer’s Market provides an important income source for family farming in the city of Laranjeiras do Sul,he where vegetables, bakery products, pasta, sweets and milk-based foods are the main products for sale. This university extension project was aimed at developing a nutritional information calculation tool for the food products. From an exploratory study, it was found that only a few producers had food products with labels and they were interested in getting the labels to suit the municipal and federal standards. For this, a calculation tool was developed with electronic spreadsheets based on the methodology proposed by the Agência Nacional de Vigilância Sanitária (National Health Surveillance Agency - ANVISA) and food composition tables (TACO). The food sellers were made aware about the importance of obtaining products with labels and a survey of the current Farmer’s market situation was conducted. With the process of food labeling, the recipes were standardized and production yield was measured. Purpose Melanomas harbor aberrations in the c-Kit gene. We tested the efficiency of the tyrosine kinase inhibitor imatinib in selected patients with metastatic melanoma harboring c-Kit mutations or amplifications. Patients and Methods Forty-three patients with metastatic melanoma harboring c-Kit aberrations were enrolled on this phase II trial. Each patient received a continuous dose of imatinib 400 mg/d unless intolerable toxicities or disease progression occurred. Fifteen patients who experienced progression of disease were allowed to escalate the dose to 800 mg/d. Results Forty-three patients were eligible for evaluation, and the median follow-up time was 12.0 months. The median progression-free survival (PFS) was 3.5 months, and the 6-month PFS rate was 36.6%. Rate of total disease control was 53.5%: 10 patients (23.3%; 95% CI, 10.2% to 36.4%) and 13 patients (30.2%; 95% CI, 16.0% to 44.4%) achieved partial response (PR) and stable disease (SD), respectively. Eighteen patients (41.9%) demonstrated regression of tumor mass. Notably, nine of the 10 PRs were observed in patients with mutations in exons 11 or 13. The 1-year overall survival (OS) rate was 51.0%. The median PFS and OS times for patients who had PR or SD versus disease progression were 9.0 months versus 1.5 months (P < .001) and 15.0 months versus 9.0 months (P = .036), respectively. Imatinib 400 mg/d was well tolerated, and only one of the 15 patients who received dose escalation to 800 mg/d achieved SD. Conclusion Imatinib demonstrated significant activity in patients with metastatic melanoma harboring genetic c-Kit aberrations, with an overall response rate of 23.3%. Escalation to 800 mg/d could not restore disease control.

Objetivos: analisar os efeitos do Método Pilates no equilíbrio, na força muscular e na flexibilidade em mulheres idosas. Método: estudo de natureza intervencional com idosas do município de Maringá/ PR. Foram avaliadas em relação ao equilíbrio, à flexibilidade e à força muscular. Ao término da avaliação, iniciaram os atendimentos fisioterapêuticos que consistiram em 10 sessões, realizadas em grupo, com duração de 60 minutos, utilizando o Método Pilates no solo. A análise estatística foi realizada pelo teste Kolmogorov-Smirnov, seguida do teste t/Wilcoxon, e do teste t/Mann – Whitney, adotando 5% como índice de significância. Resultados: obteve-se melhora de todas as variáveis, sendo: 10,71%, para a flexibilidade; 25,47%, para o equilíbrio; 46,05%, para a força dos músculos abdominais; e 65,44%, para os músculos extensores de tronco. Conclusões: o Método Pilates no solo mostrou-se eficaz na melhora de todas as variáveis estudadas em mulheres idosas.

O cronotipo constitui fator importante para o aprendizado e tarefas cotidianas. Alterações no ciclo sono-vigília podem ocorrer com maior frequência em adolescentes, o que também pode levar à diminuição de seu desempenho. Atualmente, tem se mostrado que quando o estudante está matriculado no mesmo período de acordo com seu cronotipo, seu desempenho escolar é melhor. Este trabalho teve como objetivo avaliar o cronotipo dos alunos do 9º ano e associar com as notas obtidas ao longo de um semestre. A pesquisa envolveu alunos de dois colégios públicos de Curitiba. As informações foram coletadas através de questionário de identificação do cronotipo. Teve também o apoio de um levantamento documental sobre o desempenho acadêmico dos alunos investigados. Observou-se nos resultados obtidos que o cronotipo intermediário é o mais comum entre esses adolescentes e que apenas 7% deles estudam em horário inapropriado ao seu cronotipo. Ficou demonstrado, também, que o rendimento escolar não sofreu interferência significativa do cronotipo, sexo ou idade do aluno, o que pode estar associado ao pequeno universo amostral de estudantes. Com base nos resultados, conclui-se que a aprendizagem destes estudantes não demonstrou relação com seu tipo cronobiológico, ou com a idade e o sexo, ainda que outras pesquisas sejam necessárias para melhor compreensão dessa relação.

Este artigo tem por objetivo analisar as questões relativas às desigualdades entre meninos e meninas nas relações desenvolvidas no brincar das crianças participantes de um projeto de Educação Social desenvolvido no Vale do Ivaí-PR. É uma pesquisa de cunho qualitativo que utiliza técnicas etnográficas. Na pesquisa, realizamos observações participantes e análise dos relatórios produzidos por educadores/as atuantes no projeto, após as intervenções e entrevistas semiestruturadas com as crianças e os/as educadores/as. A análise de dados baseou-se no método de análise de conteúdo de Bardin (1997). Apontamos que ainda há diferenças de gênero no brincar de meninas e meninos participantes do projeto, as quais ocorrem de maneiras sutis, ficando mais evidentes em práticas binárias já enraizadas na cultura dominante, como em jogos/esportes tradicionais. No decorrer da pesquisa, observamos que as crianças ainda reproduzem muitos aspectos normativos, no entanto também conseguem ressignificar algumas normas de gênero, muitas vezes sem se darem conta.

O presente artigo objetiva compreender as possíveis contribuições da resolução de problemas referente ao conceito de função do 1º grau com a utilização do software Scratch. Para tal, desenvolveu-se algumas atividades com estudantes do 2º ano de Licenciatura em Matemática da Universidade Estadual do Norte do Paraná (UENP) – Campus Cornélio Procópio-PR. A partir destas atividades e das respostas ao questionário entregue ao final, tornou-se possível tecer algumas considerações: evidenciou-se que a utilização do Scratch proporciona o ensino de forma lúdica, construindo diálogos, de modo a responder a situação-problema proposta. Outro fator a se destacar é que o Software pode atuar como instrumento motivador para o ensino de conceitos matemáticos, que vão desde as quatro operações básicas até o trabalho com funções.

Foram realizadas entrevistas com produtores de alface em cultivo hidropônico, em Colombo, região metropolitana de Curitiba com objetivo de verificar a situação atual, bem como identificar os pontos de estrangulamento no sistema de produção. Foram identificados 17 produtores na região, a quem foram realizadas várias visitas para coleta de dados no período de novembro de 2002 a março de 2003. Foi elaborado um questionário, onde foram registradas informações sobre a data de implantação (pico máximo em 1998 com 4 produtores); tamanho da área de produção (23,53% com áreas entre 0,5 e 1,0 ha); idade dos produtores (35,30% com idade entre 20 a 30 anos); grau de instrução (41,19% possuem 1o grau); experiência profissional fora da atividade agrícola (53,0% dos produtores hidropônicos não possuem); origem dos produtores (65% são de origem rural e 35% urbana); de quem foi a iniciativa; a origem do capital para investimento (65% capital próprio); diversidade de produtos produzidos (47,06% apenas 1 variedade); nível de processamento (70,59% apenas embalam); locais de comercialização (pequenos mercados 35,30%); gerenciamento e administração (88,24% o proprietário); marketing (5,89% apenas 1 investe) e ampliação da área (64,71% ampliaram). A pesquisa evidenciou que embora a maioria dos produtores seja proprietário da terra e tenha acesso à tecnologia, sua utilização é feita empiricamente; os produtores estão sem assistência técnica e produzindo por meio de tentativas, demandando uma atuação mais efetiva de órgãos de ensino, pesquisa e extensão da região.

Abstract FDG-PET, a functional imaging modality used for staging and monitoring response to treatment of malignant lymphoma, has a higher sensitivity and specificity than conventional imaging. PET/CT may be more accurate than conventional imaging in assessing treatment effects to correctly identify patients with residual disease and predict therapy outcomes. We prospectively investigated that PET/CT may provide additional prognostic information in mid-response assessment prior to completion of chemotherapy. Patients and Method: Eighty-six newly diagnosed patients with malignant lymphoma were enrolled from Aug. 2004 to July 2006. Both CT and PET/CT analysis were performed at the time of diagnosis and after the 3rd or 4th chemotherapy. The clinical stage of the patients was assessed according to International Workshop Criteria (IWC). PET/CT imaging was analyzed according to the combination of morphology using the CT portion and by the uptake and location of the FDG-PET portion. The cut off value of the positive in PET/CT was more than max SUV 3.0. We divided them into four different response groups using IWC and PET/CT (CRu-negative, Cru-positive, PR-negative, PR-positive). The limited-stage patients were treated with chemotherapy and involved field radiation therapy and the advanced-stage patients were treated with eight cycles of chemotherapy. However, advanced-stage patients who were older than sixty, if they had CRu-negative in interim analysis, were treated with only six cycles. Results: Median age was 53.5 years (range: 19 – 85). Seventy five patients had Non-Hodgkin’s lymphoma [Indolent-11 (12.8%), Aggressive-64 (74.4%)] and eleven patients (12.8%) had Hodgkin’s lymphoma. Forty-two patients (48.8%) achieved CRu-negative, 6 patients (7.0%) achieved CRu-positive, 20 patients (23.3%) achieved PR-negative and 18 patients (20.9%) showed PR-positive by interim IWC and PET/CT. The relapse rate is significantly different between PET-positive (58.3%) and PET-negative (8.1%) (P=0.000), especially when 12 of 18 patients in PR-positive group (66.7%) relapsed after or during chemotherapy. Figures showed the Kaplan-Meier estimates of overall and progression free survival of 86 patients depending on the combined evaluation of interim IWC and PET/CT. Conclusion: Interim PET/CT analysis was a significantly predictive value of disease progression and survival. The patients with PR-positive according to interim IWC and PET/CT analysis should be considered for an intensive therapeutic plan. Figure Figure

A metodologia adotada é descritiva, bibliográfica, com enfoque qualitativo, complementada por dados quantitativos. A coleta de dados se deu em 4 colégios estaduais de Francisco Beltrão-PR, sendo 2 colégios abaixo da média e 2 acima da média proposta pelo IDEB, sendo este um critério que foi usado apenas para selecionar as escolas pesquisadas. O público estudado consistiu em 336 adolescentes e 17 professores. O prognóstico evidencia motivo de preocupação e necessidade emergente de conscientização da sociedade quanto aos impactos biopsicossociais de uso das tecnologias digitais. Aponta-se, com isso, a necessidade emergente de mudanças na prática pedagógica, nos métodos de ensino e muito mais na gestão escolar dos recursos disponíveis, sobretudo os tecnológicos.

Introduction. The efficacy and safety of biosimilar follitropin alpha have been demonstrated in randomized blinded prospective clinical trials of phases I and III. Unfortunately, there is a gap between the clinical trials and real clinical practice data. The real-world patient data helps to create an evidence-based background for successful implementation of medicine at everyday practice in a nonselected population.Aim: to investigate the efficacy of follitropin alpha biosimilar therapy (Primapur®) in nonselected real-world population.Materials and Methods. A retrospective observational anonymized cohort study of follitropin alpha biosimilar (Primapur®) as a pre-filled pen injector with a dose adjustment of 5 IU, aimed to investigate its efficacy and safety in a nonselected population with indications to assisted reproductive technologies (ART) was carried out. The ovarian stimulation (OS) protocols included: monotherapy protocols with using only Primapur®; mixed protocols (recombinant and urinary-derived gonadotropins); short protocols with using antagonists of gonadotropin-releasing hormone (GnRH) and long protocols with GnRH agonists. The stimulation protocols were analyzed with Primapur® application for at least 5 days.Results. The overall clinical efficacy of ovarian stimulation cycles (N = 5484) was: oocytes retrieved - 9.5 ± 7.2, mature (MII) - 6.8 ± 6.6, fertilized (2PN) - 6.1 ± 5.8, clinical pregnancy per ET (PR) - 38.4 %. Mixed gonadotropin protocols (N = 2625) vs. monotherapy with Primapur® (N = 2859): oocytes retrieved - 8.6 ± 6.8 vs. 10.3 ± 7.4 (p < 0.001), mature (MII) - 6.7 ± 6.2 vs. 7.7 ± 6.9 (p < 0.001), fertilized (2PN) - 5.8 ± 5.2 vs. 7.2 ± 6.2 (p < 0.001). There were statistically significant differences between oocyte yields in mixed vs. monotherapy protocols due to subgroup differences, including age, body mass index (BMI) and IVF/ICSI attempts. No statistically significant differences were found for PR: 39.3 % vs. 37.6 % (p = 0.314). Monotherapy protocols with GnRH antagonist OS (N = 2183) vs. GnRH agonist (N = 676) revealed: oocytes retrieved - 10.5 ± 7.5 vs. 9.6 ± 7.0 (p = 0.032), mature (MII) - 7.6 ± 6.9 vs. 6.7 ± 5.7 (p < 0.001), fertilized (2PN) - 7.3 ± 6.3 vs. 5.7 ± 5.0 (p < 0.001). There were statistically significant differences between BMI and IVF/ICSI attempts. No statistically significant differences were found for PR: 37.9 % vs. 35.9 % (p = 0.482). All medicines were well tolerated and no serious adverse reactions were reported.Conclusion. This was the largest retrospective observational study conducted in the field of fertility in Russia and involved 5484 ovarian stimulation protocols at 35 IVF clinics. The obtained results demonstrated similar clinical efficacy for follitropin alpha biosimilar Primapur® in different OS protocols in real clinical practice.
