Full textThe study was aimed to determine prevalence of sleep-related respiratory disturbances (SRRD) in a group of pacemaker patients. It was investigated if SRRD are more prevalent in pacemaker patients compared to corresponding patients without pacemaker. This question was raised in background of the wellknown coincidence of SRRD and arterial hypertension respectively coronary heart disease. References about the prevalence of SRRD in corresponding elderly people served as standard of comparison. 192 hospital outpatients (100 men and 92 women) from rhythmological ambulance of university hospital "Charité" in Berlin were randomly selected and examined for one night by means of MESAM IV device, a Non-laboratory-monitoring-system". The mean age of these patients was 62,2 +/- 12,2 years. A respiratory disturbance index (RDI; the sum of apneas and hypopneas per hour of sleep) equal or greater than 10 was laid down for the existence of SRRD. The RDI built the basis for dividing the patients into three groups: Group A (RDI < 5, no SRRD), Group B (RDI >= 5 and < 10, borderline SRRD) an Group C (RDI >= 10, SRRD). Results (all patients / men / women): Group A: 100 / 44 / 56 patients (52,1 / 44,0 / 60,9 %) Group B: 30 / 16 / 14 patients (15,6 / 16,0 / 15,2 %) Group C: 62 / 40 / 22 patients (32,3 / 40,0 / 23,9 %). Significant differences occured in the frequency of SRRD (Group C) between men and women. Apart from that the mean age (Group A: 58,5 +/- 14,5 years, Group C: 67,3 +/- 6,8 years) as well as the Body-Mass-Index (Group A: 24,8 +/- 3,4 kg/m2, Group C: 27,1 +/. 3,3 kg/m2) were significantly higher in patients with SRRD than in patients without SRRD. No significant differences in the frequency of SRRD could be demonstrated in dependence on underlying rhythm disturbances (e. g. Sick-Sinus-Syndrom, atrioventricular block, Bradyarrhythmia absoluta). In the same manner no evidences regarding significant influences of additionally existing cardiovascular diseases or risk factors (e. g. coronary heart disease, arterial hypertension, smoking) on the appearance of SRRD were shown. References about the prevalence of SRRD in corresponding elderly people without pacemaker vary from 24 % to 73 %. The prevalence of SRRD in pacemaker patients (32,3 %) fits right in the frame of this great variety. A more frequent appearance of SRRD in patients with pacemaker than in patients without could not be proved. Equally, the underlying rhythm disturbance has apparently no correlation to SRRD. Extended studies are necessary.

Full textBackground: The aim of this study was to examine the gingival stippling in children and to characterize it. The prevalence of stippling was a special interest. Stippling is a structure of the gingival epithelium, which causes an orange peel like appearance of the gingiva. Material and methods: In this cross-sectional study 274 children (148 boys and 126 girls; average age 4.98 years) were examined. Oral status, approximal plaque index (API), papillary bleeding index (PBI) and race were noted. Photographs or replicas with a silicone impression material of low viscosity were taken if stippling was found. The replicas were used to make stone casts, which were scanned by a video camera. The obtained pictures were analysed with the software analysis 3.0. Results: 68.2% of the children showed stippling, in 67.2% of the boys and 69.0% of the girls. It was evident from an age of one year. Sex and race did not show any influence on prevalence. There was also no relation between stippling and caries, oral hygiene or gingival inflammation. Stippling was more frequent in mixed dentition than in deciduous dentition. The children showed more stippling in the upper jaw and in the anterior region. It was not spread symmetrically. Usually, it was localized to the attached gingiva with the highest frequency in the interradicular area, followed by the radicular area, and subpapillar area. Only one child showed stippling in the free gingiva, too, but this seemed to be an exception from the rule. The data obtained from photographs and replicas showed a density of 2,54 stipples per mm2, a height of 0.10 mm and a width of 0.09 mm. Older children showed a higher density. Height and width were neither influenced by sex nor by age. But stippling in upper arch and anterior region had a higher density and a larger height and width than in lower arch or posterior region. Conclusion: Gingival Stippling is not a major criterion in clinical assessment of gingival or periodontal inflammation, because missing stippling does not always coincide with presence of gingival inflammation. For a correct diagnosis more criteria, e.g. colour, contour, thickness, consistency, pocket depth and bleeding on probing, have to be considered, too.

Full textThis study examines the prevalence of depressive syndromes among unselected primary care patients as well as doctors' recognition and treatment rates. This nationwide study included a total of 20,421 patients attending their primary care doctors (n=633) on the study's target day. Patients received a self-report questionnaire including the depression screening questionnaire (DSQ) to provide diagnoses of depressive disorders according to the criteria of DSM-IV and ICD- 10.These data were compared to the doctor's evaluation form for each patient to assess the physician's diagnostic decision, assessment of clinical severity, and treatment choices. The findings confirm the high prevalence of depressive syndromes in primary care settings (11.3%) and underline the particular challenge posed by a high proportion of patients near the diagnosis threshold. Although recognition rates among more severe major depressive patients (59%) as well as treatments prescribed appear to be more favourable than in previous studies, the situation in less severe cases and the high proportion of doctors' definite depression diagnoses in patients who do not fulfill the diagnostic criteria still raise significant concerns

Full textIntroduction: In a project, it is investigated whether a newly designed web-information system can better inform medical laymen than traditional search engines and health portals. The evaluation of a system for providing information to headache patients is only useful when in fact many people suffer from headaches in the German speaking area and if these people with their complaints rather consult the Internet than a practitioner. Therefore, in three studies the prevalence of headache and the associated physician consultation rate was examined. Method: About 9000 students of the University of Applied Sciences Bremen, 2000 insurants of the BARMER, and about 1000 students of the University of Health Sciences, Medical Informatics and technology Tyrol, Austria (UMIT) were asked if they suffer from headaches and if so whether they already have a medical diagnosis. A total of 521 persons participated in the investigation. Results: From headaches suffered 292 (56%) participants. A medical diagnosis for these had 52 (18%). All in all, it shows slightly lower headache prevalence than found in previous studies. The medical consultation rate cited in the literature is confirmed. Discussion: The evaluation of the three studies showed that the prevalence of headache remains high and the associated physician consultation rate is still low

Full textQualitiy of life and achievements are impaired by unrecognised iron deficiency. The iron requirement of women during their child-bearing age is high and increases in pregnancy. The aim of this study was to determine the prevalence and risk factors for iron deficiency in young mothers under contemporary German life conditions. Between September 1997 and August 1998 the iron status of 507 mothers of one-year old children was assessed. The data was derived from venous blood and questionnaires. Besides conventional methods, the concentration of soluble transferrin receptor was used as leading indicator of iron status. 9,5 % had cellular iron deficiency and 2,2 % of all mothers had iron deficiency anemia. In addition to absence of menstruation one year postpartum, vegetarian foood, a high number of children and non-German nationality are risk factors for iron deficiency. Associated with low maternal hemoglobin concentration are body mass index below 19,8, age below 25 years, low level of education and high birth weight of the child. In contrast, high alcohol intake and cigarette smoking are associated with a better iron status and higher hemoglobin concentrations. Children of mothers with insufficient iron supply are also at higher risk of iron deficiency.

Full textAlcohol use frequently begins in adolescence. However, only few studies have reported the prevalence of alcohol abuse disorders in adolescents. This paper reports results from a representative study in a sample of 3021 adolescents, aged 14-24 years. The Prevalence of alcohol abuse and dependence according to DSM-IV criteria, as well as the prevalence, frequency and quantity of alcohol abuse and a description of the first occuring symptoms, are presented here. Alcohol abuse was reported by 9.7% of respondents and alcohol dependence by 6.2%. Males were more likely to report an alcohol disorder than females, and the prevalence also increased in the older age cohorts. The most frequent initial symptoms were "hazardous use" (91.9%) for abuse and "tolerance" (34.3%) for dependence. First symptoms have been shown to occur long before the age of 18. These results show that even in adolescents and young adults alcohol abuse and dependence are frequent disorders

Full textBackground and Purpose: The primary care sector is of key importance for the management of patients with diabetes mellitus. The authors investigated (a) the prevalence of diabetes mellitus type 1 and type 2, (b) the type and frequency of non-drug and drug treatment and its association with the presence of diabetic complications, and (c) the quality of metabolic control by HbA1c. Method: Using a nationwide probability sample of 3,188 general practices (response rate [RR] 50.6%), a total of 55,518 (RR 93.5%) patients were assessed in a prospective cross-sectional study by their physicians in September 2003 in a standardized manner using questionnaires, physician interview, and laboratory assessments. In addition to diabetes mellitus, 28 diseases were explicitly screened for, among them typical macrovascular (coronary heart disease, cerebrovascular disease, peripheral arterial disease) and microvascular disease (neuropathy, nephropathy, retinopathy, diabetic foot) complications. Results: The prevalence of diabetes mellitus was 0.5% (type 1) and 14.7% (type 2), respectively. 49.5% (type 1) and 50.2% (type 2) of patients had micro- or macrovascular complications. 6.8% did not receive any treatment, 13.5% received non-drug treatment, and 75.3% received oral antidiabetic drugs and/or insulin (26.6% a combination of two or more). Compared to diabetics without any complications, treatment intensity was significantly higher in patients with microvascular complications (odds ratio [OR] 3.02), but not in those with macrovascular complications only (OR 0.98). An HbA1c value ≥ 7.0% was recorded in 39.6% of patients. Conclusion: Compared to previous studies in this setting, the proportion of diabetics with drug treatment has increased. More patients receive antidiabetic drug combinations. Quality of blood sugar control appears to have improved as well
