En réanimation, environ 10 à 15 % des patients ne peuvent pas ventiler seuls après l'extubation du ventilateur mécanique. Ainsi, entraînant un échec de l'extubation. Le problème est que les cliniciens ne disposent pas d’outils efficaces pour suivre le patient une fois qu'il est extubé. En effet, avant l’extubation, iIs utilisaient principalement le ventilateur pour recueillir des informations sur l'état ventilatoire du patient. Après quoi, ils s'appuient sur une évaluation visuelle de toute détresse ventilatoire, ou ils peuvent demander une analyse de sang ou une échographie pulmonaire qui sont des mesures qui ne peuvent pas être effectuées en continu. Ainsi, notre objectif serait d'utiliser la tomographie par impédance électrique pendant la phase de sevrage après l'extubation, pour prédire le résultat de l’extubation. Pour présenter brièvement |'EIT, il s'agit d'un dispositif de surveillance non invasif, qui peut être utilisé en continu au chevet du patient pour surveiller sa ventilation, et il est utilisé en soins intensifs pour mieux configurer le paramètre du ventilateur mécanique. Des recherches ont déjà été menées par Longhini et ses équipes pour prédire le résultat de l'extubation avec l'EIT. Cependant, ils n'ont testé que 4 fonctionnalités EIT et ont réalisé la mesure avant l'extubation. Nous voulons donc approfondir notre compréhension de la capacité ElT dans une telle situation, en incluant davantage de fonctionnalités EIT et de surveillance pendant la phase de sevrage<br>In ICU, there are around 10 to 15% of patients that cannot ventilate on their own after the extubation from the mechanical ventilator. Thus, resulting in an extubation failure. The problem is that clinicians do not have efficient tools to monitor the patient once he is extubated. Indeed, before the extubation, they primarily used the ventilator to gather information about the patient's ventilation state. After which, they rely on visual assessment of any ventilation distress, or they can ask for a blood analysis or lung ultrasound which are measures that cannot be done continuously. So, our goal would be to use the Electrical Impedance Tomography during the weaning phase after the extubation, to predict the extubation outcome. To briefly introduce the EIT, it is a non-invasive monitoring device, that can be used continuously at the bedside of the patient to monitor his ventilation, and it is used in ICU to better setup the parameter of the mechanical ventilator. Research has already been done by Longhini and his teams to predict the extubation outcome with the EIT. Though, they only tested 4 Elt features, and realized the measure before the extubation. So, we want to push our understanding of the ElT capability in such situation, by including more EIT features and monitoring during the weaning phase
