Full textAim: The purpose was to investigate the nurses perception in using the assessment instrument in preventative care of elderly within a municipal health care setting. Method: A qualitative approach was used and were performed with individual interviews of nurses. They were working in a municipal health care setting. The collected material was assessed with an inductive content analysis. Results: From the content analysis it emerged three categories, tools for increased patient safety, experience and knowledge is vital, lacking reality abutment and usability and to these sub categories where the purpose of the study where shown. The result showed that the assessment instrument can be used as a tool for a safer and more equal care. Experience and knowledge where seen as vital in order to be able to use the assessment instrument. It also emerged that there could be difficulties to interpret the assessment instrument as well as being misleading and not in touch with reality. Conclusions: The use of assessment instruments can lead to a secure, more equal and fair prevention in the care of the elderly. It is required to get education and knowledge about the assessment instrument to make sure it is being used and by using it frequently it increased use and acceptance. The limitations that were perceived were that it could be hard to understand or misleading. Further research could be the elderly experience of the assessment tools in preventive care and how assessment tools are used in assessing the elderly with cognitive impairment.Keywords: Nursing experiences, assessment tools, municipal health care, preventive care of older people.

Full textBackground: In care it is important to see the whole person, not only the disease. To be able to support the elderly in both their physically och psychological suffer, it is suitable to use person-centred care. It is something that both the elderly and their relatives appreciate. Aim: To describe registered nurses experience of working with person-centred care with elderly. Method: Qualitative literature review with a descriptive analysis of 10 articles. Results: The nurses felt that they had a close relationship to the elderly due to the person-centred caring and they felt appreciated. However, the opportunity to achieve person-centred care was sometimes hard, due to lack of time and staff. The nurses also had a lot of administrative work as well, witch made it hard to sustain continuity with the elderly. Conclusion: The nurses appreciated to work with a person-centred approach, and their experience was that the elderly also liked it. According to the nurses the whole person, including relatives, could be a part of the caring if person centred care was used. There was factors that sometimes made the person-centred work hard, but the positive effects was more, than the negative ones.

Methods for maintaining and promoting health are becoming increasingly important.Aim: Illustrate the effect of physical exercise as a preventive action against cognitive impairment by elderly 55 years and older.Method: The intention was to gather studies, with quantitative approach, which showed connection between physical exercises as a preventive method against cognitive impairment. Articles, with quantitative approach, were searched using the databases of PubMed and CINAHL.Results: The context, in which physical exercise appears positive, as prevention against cognitive impairment, can be seen through the gathered studies' different measurement methods. Complexity, however, is showing the possibility to the influence by other individual factors that cannot be ruled out.Conclusion: Cognitive impairment can be influenced by physical exercise.

Full textABSTRACT Today about 19 per cent of Sweden's population are aged 65 or older. Many of these seniors have a complex clinical picture. This requires that the care provided is holistic; consideration must be given to not only the physical but also to the mental, social and spiritual needs of the patient. Person-centered care is aimed at the whole person and their needs and resources. The concept is currently recognized and something that health care should be based on. Previous studies show health care shortcomings in the person-centered care for older people. Many of them feel that the health services does not see them as a person or in their entirety, they feel that health care workers only see a part of them or a diagnosis. Nurses play a privotal role in the care of older people, and therefore it is relevant to highlight their experiences of person- centered care and what advantages and disadvantages they have experienced. This can lead to a greater understanding of the factors that are central to person-centered care, in order to provide optimal care for the elderly. The aim of this literature review was to highlight nurses' experiences of person-centered care for the elderly. The method used was a literature review. Searches for articles have been made in PubMed, CINAHL, Google Scholar and Web of Science. A total of 16 scientific papers were included, with both quantitative and qualitative methods. Content analysis was used and categories were identified in the articles. The result showed factors that are important for nurses in order to conduct in person-centered care for the elderly. These were working environment, organization, demographic factors and communication. Nurses highlighted many positive experiences of person-centered care of the elderly. Benefits were for example higher quality of care, improved working climate and job commitment as well as less stress. It also lowered levels of bad conscience amongst the nurses and made them less inclined to change workplace. The main disadvantages that the nurses had experienced, was that person-centered care was costly. It required time, adequate staffing, effective teamwork and a supportive management team. Many nurses described that policies and procedures are working against person-centered care, and that it was difficult to put the patient's needs and wishes before those. Additionally, nurses were not always comfortable with giving patients more power and influence. The conclusion to be drawn from this literature review is that nurses' experiences showed that factors such as work environment, commitment and quality of care improved by person- centered care, but that time and resources could affect performance negatively.

Full textAndelen äldre personer kommer i framtiden att öka och behovet av en god och värdig vård blir en utmaning inom den kommunala äldreomsorgen. I samband med att vård utvärderas relateras det ofta till bevarad eller kränkt värdighet. Begreppet värdighet är ett grundbegrepp inom omvårdnad som är något fundamentalt och självklart hos alla människor. Upplevelsen av bevarad värdighet är beroende av i vilken utsträckning viktiga värden kan uppnås. Media skriver om att kränkningar förekommer i vården av äldre som bor och lever på äldreboende. I vilken mån upplevelsen av värdighet kan återfås efter en kränkning är beroende av om en försoning med situationen kan ske. Avsikten med studien var att utifrån litteratur beskriva innebörden av begreppet bevarad värdighet och vidga förståelsen för den äldre personens erfarenheter av vård och hjälp samt hur livet förändras på äldreboende. I de två delstudierna framkommer nya dimensioner av begreppet bevarad värdighet och när vårdaren är inkännande och lyssnar till den äldres berättelse framträder bevarad värdighet. I beskrivningen av hur äldre personer erfar att vara i behov av vård och hjälp de sista levnadsåren synliggörs att de strävar efter att behålla en inre känsla av stolthet. Vården bör vara planerad och utformad så att den äldre kan komma tillrätta när kroppen och livssituation förändras och de blir i behov av vård och omsorg.

Full textBackground: In Sweden conducts public palliative care in all careforms as hospitals, primary care, home care and in carehomes by staff with essential knowledge and competence in palliative care. According to the Swedish health authorities (Socialstyrelsen, 2016) it´s important to always do a clinical assessment to minimize the risk of untreated pain.Therefore it ought to be easy access to routines and guidelines for the staff how to use the different tools that are available for different assessments. And also that the staff has access to education and information how to use the different tolls that are available. Aim: To describe nurse´s experiences of pain assessment and pain relief of elderly patients in palliative care. Method: Descriptive qualitative interview study with inductive content, analyzed according to Graneheim and Lundman´s (2004) content analysis. Result: Four head categories emerged: clinical eye and experience, insufficient routines and missing guidelines, the Palliative advising team´s importance and security in prescription. Conclusion: It emerged that nurses need education and information about the use of different pain assessment tools. And that the Palliative advising team is an important work partner around patients in palliative care. And teamwork is a guarantee of quality of the care that the patient acquire.

Full textStudies show that older is an increased proportion patients in hospital and that it is a patient group that can be difficult to communicate with because of older patients' often multiple disease picture. Relatives to the patient can be help for caregivers in the communication and the care of the patient. The aim with the study was to examine relative's experience of participation, meeting/support, accessibility and information on a geriatrics rehabilitation care unit. The study had a descriptive design and was designed as a questionnaire study. It was implemented on one geriatrics rehabilitation care unit on the countryside in central Sweden, belong to Uppsala University hospital. The number questionnaires that were distributed out were 49 and the reply frequency was 82 %. The result showed that most relatives in the study were satisfied with participation, meeting/ support, accessibility and information. The result showed also on a significance correlation between participation and meeting/ support, accessibility and information. The study did not reveal any difference between men and women concerning the experience of participation. Likewise, no correlation was revealed on between how long time the patient where in hospital and the experience of participation. Relatives experienced to have a relative good contact with the doctor, but a part stated however that they did not have any opinion about it. Our conclusion is that the concept to participation is meeting/ support, accessibility and information.
Studier visar att äldre är en ökande andel patienter i vården och att det är en patientgrupp somkan vara svår att kommunicera med på grund av äldre patienters ofta multipla sjukdomsbild. Närstående till patienten kan vara till hjälp för vårdgivaren i kommunikationen och vården av patienten. Syftet med studien var att undersöka närståendes självskattade upplevelse av delaktighet,bemötande, tillgänglighet och information på en geriatrisk rehabiliteringsavdelning. Studien hade en deskriptiv design och var utformad som en enkätstudie. Den genomfördes på en geriatrisk rehabiliteringsavdelning på landsbygden i centrala Sverige tillhörande Akademiska sjukhuset. Antalet enkäter som delades ut var 49 stycken och svarsfrekvensen var 82 %. Resultatet visade att de flesta närstående i studien var nöjda med delaktighet, bemötande, tillgänglighet och information. Resultatet visade även på en signifikans gällande samvariation mellan delaktighet och bemötande, tillgänglighet och information. Studien visade inte på någon signifikans gällande skillnaden mellan män och kvinnor avseende upplevelsen av delaktighet. Likaså påträffades ingen signifikans mellan hur lång tid patienten var inneliggande och den närståendes upplevelse av delaktighet. Närstående upplevde sig ha en relativt bra kontakt med ansvarig läkare men en stor del uppgav dock att de inte hade någon uppfattning. Vår slutsats är att nyckeln till delaktighet är bemötande, tillgänglighet och information.

Full textBackground: The number of elderly people who live and die in a particular accommodation is increasing, which entails high demands that the nurse has sufficient knowledge and skills to be able to offer a high-quality palliative care. Nurses come in contact with emotional situations associated with nursing of patients in end of life. Aim: The purpose of the study is to compile, integrate and synthesize current knowledge of nurses’ experiences of palliative care for elderly people in home care. Method: Systematic literature study based on 11 original articles with qualitative design. Database searches were performed in the databases PubMed, Cinahl and PsycINFO. The synthesis of the articles was done according to Noblit andHares metaethnography. Result: The results were presented from 4 themes, “Challenging and emotionally oneruos, “The importance of communicating”, “Support relatives” and “The importance of teamwork”. Conclusion: Nurses’ experiences of palliative care for elderly people in home care, many different perspectives are included. Nurses get in touch with daily challenges in palliative care where strong emotions, existential issues, ethical problems and the requirement for cooperation should work.

Full textBackground: Elder care affect many people. As the older population in the world expand, the requirements of the elder care will increase. Participation is a condition for good health, healthy ageing, and is clearly identified in laws and guidelines. Aim: The aim of this study was to describe elderly peoples experiences’ of participation when receiving care in their home. Method: A qualitative literature review where 14 scientific articles were analyzed though a deductive approach. Result: The elders primarily experienced lack of participation when receiving care in their home. Positive experiences was identified occasionally, for example when the care givers asked questions prior to care routines. Discussion: To improve elders experiences’ of participation when receiving care in their home, it requires that the relationship between care taker and care giver get better partly through enlighten the unequal power relation between the two parts. Conclusion: Through improved participation the chances of good health and healthy ageing will increase.

Full textBackground According to the Swedish Association of Local Authorities and Regions should all healthcare professionals within the regions, municipalities and county council work for an introduction of person-centered care (PCC). PCC has three fundamental pillars: partnership, the patient´s story and documentation. Healthcare should be in agreement with the person needing care, it´s the foundation of PCC. Objective The objective of the study was to describe the nurse's application of person-centered care in the municipal home healthcare in the care of people aged 65 years and older. Method The study has a descriptive design with a qualitative approach. Main results The results showed that none of the participating municipalities applied PCC to full extend currently in daily work. The nurses described good conditions for PCC like having focus on the person. The nurses also tried to create a relationship with the patient and their relatives, through continuity, partnership and patient´s life story. The study identified obstacles in the application of PCC like lack of organizational management, continuity and time, unrealistic requests of care from the person and knowledge deficit regarding PCC. Conclusion The nurses within municipal home healthcare lacked knowledge regarding PCC and how to apply PCC in daily work. The study showed that it is the management who characterizes the care given. In the application of PCC a necessity is a management who guides, supports and offers relevant conditions for PCC to the nurses. It should be a right for all people receiving care to obtain PCC.

Full textBackground: Loneliness is a complex phenomenon that occurs in different forms and all humans are likely to experience some form of loneliness during their lifetime. Life changing events can affect the appearance of loneliness and nearly 60% of the older people in Sweden have described themselves as lonely. Since loneliness is a personal experience it is important that nurses work person-centred to identify the patient’s individual needs and encourage health. Aim: The aim of this literature review was to summarize older peoples experiences of loneliness. Method: A literature review with qualitative approach has been made and 10 studies from Cinahl and PubMed have been analysed using a content analysis. The quality of the included studies has been reviewed and then compiled in a result. Results: From the content analysis five main categories appeared; different point of views of loneliness, losses in life, the importance of social relations, a feeling of abandonment and existential questions. The experience of loneliness was affected by life changing events such as the death of a spousal, decreasing social network, after retirement and at times when family members prioritized otherwise than the older people. The result shows the difficulties in describing loneliness and tell others about it. Conclusion: The older people experienced that the phenomenon of loneliness impacted on the well-being, both in a positive way but especially in a negative way. Loneliness emerged to be more than just being alone, it can be understood as a multifaced phenomenon with individual experiences. Therefore it´s important that the nurses have knowledge about loneliness to gain a deeper understanding for the patients experiences and needs.

This places higher demands on the district nurse and increases the complexity of nursing care at home. Purpose: The purpose of the study was to describe district nurses experiences of caring for elderly patients with type 2 diabetes in home care. Method: Qualitative study design has been used. Data was collected through interviews with nine district nurses between the ages 30 and 60 years old with semi-structured interviews. Data was analyzed with qualitative content analysis with a manifest approach. Results: District nurses described the importance of an interdisciplinary approach as central to achieving good and safe home health care and they felt that their work had been facilitated if they also collaborated with a diabetes nurse within the municipality. The district nurses felt that clear guidelines were lacking and that the workload and the lack of time were high and that increased nursing responsibility and competence in diabetes were required at the district nurses in home care. Conclusion: In order to ensure good and safe home health care, it is important that the municipality clarifies guidelines for the care of patients with type 2 diabetes and is investing in strengthening the interdisciplinary cooperation in home care.

Full textBackground: Diabetes type 2 has increased among the elderly. The district nurse has a role by providing safe healthcare treatment and preventing long-term complications among elderly patients’ living at home with diabetes type 2. They even give support and coordinating with other caregivers. Aim: To illuminate district nurse’s experiences of providing home care to elderly patients’ with diabetes type 2. Method: Six district nurses from five different municipalities in Sweden were chosen to do a survey research. Data was analyzed using qualitative content analysis. Findings: The district nurses experienced lack of support from diabetes nurses and general practitioners (GP) from primary care, but received assistance from their colleagues in the municipality, had difficulties accessing patient’s records, and experienced hindrances to further their education. Patients lacked motivation and expected district nurses to take responsibility of their situation. An experience of teamwork and working independently made a positive impact to the district nurses. Conclusion: The district nurses have many areas of responsibility and need good collaboration with other colleagues to offer a quality care. Research on the relationship between the patient and the district nurse, medical record access and patient perspective on the disease is needed.

Full textBackground: The elderly is a group that will be a major part of the population in the future. With increasing age, the risk of suffering from one or more diseases increases, which will place higher demands on healthcare. Swedish law states that healthcare has an obligation to promote patient integrity, participation and self-determination, but this is invalid in the care of the elderly. Many complex illnesses in an elderly person necessitate nursing based on the patient's individual needs. Person-centered nursing is about a partnership between the individual and healthcare staff, where the individual is at the center of care. Aim: To highlight the elderly patient's experience of care based on a person-centered perspective. Method: A literature review was conducted and the data was found in the databases CINAHL and PubMed, 13 articles with qualitative method were selected. Result: The results showed that there were similarities and differences in how the elderly patient experiences their care. These experiences can be divided into three main categories: self-determination, participation and integrity. The elderly patient had a longing for independence and self-determination, however, this was reversed due to a lack of time and competence shortage. The elderly had a desire to be involved in their care, where the relationship and communication were an important basis for creating participation. Many of the elderly did not feel confirmed as a unique individual of nursing staff. Conclusion: In order to promote the role of the elderly in their care, must a holistic perspective and a person-centered approach be applied, where the elderly is respected as a unique individual. With the older individual at the heart of his care, his right to self-determination, participation and integrity is strengthened, which in turn can lead to an increased quality of life.

Palliativ vård erbjuds till människor som är i livets slut samt där botande behandling inte längre är möjligt. Fokus för vården är då istället att lindra patientens lidande och främja patientens livskvalitet. Detta litteraturbaserade examensarbete beskriver sjuksköterskors upplevelser av arbetet inom palliativ vård av äldre. Det föreligger anledning att beskriva sjuksköterskors upplevelser av arbetet inom palliativvård av äldre eftersom åldersgruppen 65 år och äldre är en växande population som dessutom klassas som sårbar. Sjuksköterskor upplevde arbetet inom palliativ vård av äldre som känslomässigt utmanande med situationer som kantats av otillräcklighet, bristande samarbete och frånvaro av stöd. Trots detta belyste sjuksköterskor vikten av att arbeta utifrån ett personcentrerat förhållningssätt genom att skapa en vårdande relation med patienten och arbeta för patientens bästa. För att kunna bedriva en god palliativ vård för äldre är det viktigt att sjuksköterskor ges möjlighet till stöd för att minska känslan av otillräcklighet och för att hantera psykiska påfrestningar.

Full textBackground: Almost 90 percent of the total amount who dies annually in Sweden is 65 or older and their own home is the place that many choose getting palliative care in. The nurse should have a holistic approach in end of life care and the professional responsibility include alleviate the suffering, a dignified treatment of the patient and person-centered care. Aim:To describe the nurse experience of palliative care to elderly patients in their home. Method: A literature review has been carried out in which nine qualitative and one quantitative article were included. They were extracted from the databases CINAHL complete, Ageline and PubMed. The quality of the articles follows the guidelines of Friberg’s analysis method. Results: The analysis resulted in four themes Emotional experiences, Importance of nurse-patient relations, Importance of teamworkand Need of support and education. The nurses described both positive and negative experiences of end of life care to elderly patients in their home. The nurses felt satisfaction in end of life care where they created a strong relationship with the dying patient. The negative experiences were based on external factors that made the nurses feel powerless and vulnerable in their professional role. Discussion: The method discussion raises strengths and weaknesses in the literature review. The authors discussed the results based on the background and previous research, along with parts of Katie Eriksson’s theory of caring, suffering and the concept of health.

Full textBackground: Nurses have for many years cared for multi-sick elderly people whom are suffering from long-term pain. The pain may appear as symptoms caused by inadequate treatment. This results in a gradual impairment of both physical and mental health of the multi-ill elderly. Therefore, it is of great importance to investigate and gain more knowledge about how nurses perceive caring for multi-ill elderly people with long-term pain. Aim: The purpose is to describe how nurses experience caring for multi-sick elderly people with long-term pain. Method: The study has a qualitative inductive approach to elucidate nurses' experience, knowledge and attitude. Data was collected using three phases, eight nurses participated and contributed with their knowledge and experience. The data collected was analyzed by means of qualitative content analysis. Results: The analysis of nurses' experiences resulted in three categories; see the entire person, pain relief, knowledge and competence. The categories were made up by seven subcategories. Conclusion: The importance of having knowledge of what individual adaptation means and having a structured adequate pain relief that helps to reduce the patient's suffering. It is important to achieve an increase in the patient's quality of life through improvement of physical and mental health. As future specialist nurses with a focus on Care for the Elderly, this knowledge can contribute to a way of working that is based on an attitude which means to support and help the patient to increase their quality of life.

Full textABSTRAKT I föreliggande studie har åtta vårdpersonal med lång erfarenhet av arbete på vård och omsorgsboende för personer med demenssjukdom personer intervjuats. Syftet med denna studie var att beskriva vårdpersonals upplevelse av att vårda personer med demenssjukdom och BPSD. Materialet analyserades med kvalitativ innehållsanalys och induktiv ansats. Analysen resulterade i fyra kategorier: Att skapa kontakt, Ingen dag är den andra lik, Vara lugn och ta sig tid samt Alla behövs. Resultatet visade att vårdpersonal i vård och omsorgsboende ställdes inför en mängd svåra och varierande uppgifter i omvårdnaden. Vårdpersonalen bemötte personerna med demenssjukdom genom att anpassa mötet utifrån rådande förutsättningar. Vårdpersonalen pratade lugnt, använde få ord och lät kontakten ta tid. Både verbal och icke verbal kommunikation användes för att få kontakt och i vissa fall humor och musik. Ett gott bemötande förutsatte emellertid ett stödjande sammanhang både på grupp- och organisationsnivå. Vårdpersonalen beskrev betydelsen av ett fungerande team där kontinuerlig behovsbaserad planering och en tydlig målsättning var viktiga delar i vården av personer med demenssjukdom. Specialistsjuksköterskan inom vård av äldre har en central roll i att leda och samordna vårdarbetet i teamet med den demenssjuke personens behov som utgångspunkt. Med sina expertkunskaper kan sjuksköterskan utveckla omvårdnadsarbetet och säkerställa att den teoretiska kunskapen omsätts och kombineras med erfarenhetsbaserad kunskap.
