Full textApproximately one third of all patients in primary care have a psychosocial component to their illness that requires adequate professional treatment. However, primary care is insufficiently prepared for these patients. The aim of this case-study was to evaluate a pilot project in primary care offering psychotherapy as a treatment alternative. The data collected includes 1) data from 352 referred patients regarding demographics, diagnosis, treatment duration and pre- and post health status, 2) a patient-satisfaction questionnaire and 3) interviews with six therapists and five medical doctors. The treated patients improved significantly and showed a high degree of satisfaction. Doctors and therapists agreed on the advantages of cooperation and of psychotherapy as a treatment alternative in the primary care setting.

Full textBackground: The district nurse triage daily patients with mental illness in primary care. Mental illness is multifaceted and to some extent taboo in society. This requires that the district nurse has a good knowledge of mental illness and how it can express itself in different patients. The district nurse also needs to take a holistic approach with person-centred care in focus. Aim: The aim was the district nurse experiences with triage of patients with mental illness in primary care. Method: A qualitative research approach based on interviews with eight district nurse/nurse active in primary care. Qualitative content analysis. Result: The result was three categories and ten subcategories. The three categories were: Deficiencies in education, Challenges in triage and Deficiencies in collaboration. The results showed that the district nurse needs to have more knowledge about mental illness and have the conditions required to carry out a good triage, such as support tools and security in dialogue about suicidal thoughts in the patient. The district nurse needs to be aware of somatization and its impact on triage. The collaboration between primary care and the open psychiatric clinic needs to be improved. Conclusion: In order for triage of mental illness to improve, the district nurse needs to receive more training in mental illness. There, both employers and education managers have a responsibility to influence the opportunities for triage of mental illness in primary care. The district nurse also needs support tools to ensure equivalent triage of mental illness in primary care.

Full textBackground: Older patients are at higher risk of depression than the general population. Most elderly people suffering from mental illness are treated and followed up through primary care. The district nurse is often the first to meet the elderly patient, who tends to seek help for their physical problems, rather than the mental ones.Purpose: The purpose of this study was to describe the district nurses' experiences and perceptions of meeting older patients with suspected depression.Method: Qualitative method consisting of semi-structured interviews of eight district nurses, working in six different primary care institutions in central Sweden. The material was analyzed by qualitative content analysis with an inductive approach.Main results: The district nurse's view of her role varied from merely seeing herself as a coordinator to being able to play a significant role in patient care, as listener and support giver. Prerequisites for being able to help the patient, were mainly described as having enough time and being able to gain the patient's trust. Challenges in meeting these patients, could be the patient seeing mental illness as a stigmatized subject, that the district nurse perceived their knowledge of depression amongst the elderly as insufficient and that they sometimes felt powerless in relation to the patient's life situation. Work environment challenges involved stress and time-constrained schedules, lack of professional and continuous supervision, and the absence of a specialist trained psychiatric nurse in the workplace.

Full textMental ill health in working life is a major and growing problem in the welfare society. The problem is multifaceted and raises many questions about who is responsible, what kind of phenomenon it is and how it should be managed. This study examines how care professionals manage work-related mental ill health. Focusing on occupational healthcare and primary care, interest is directed towards how care professionals argue about workrelated mental illness, what dilemmas they face and the strategies they rely on in managing them. It also seeks to answer the question of responsibility regarding this complex problem, not least in the light of the privatization of occupational healthcare. The study uses a multidisciplinary perspective, combining concepts from technology and science studies (STS), sociology of professions and organizational theory in order to analyze various aspects of care management of work-related mental ill health. The empirical material is mainly based on interviews with physicians, psychotherapists, counsellors, occupational therapists, psychologists, rehabilitation coordinators and behavioral scientists, but also includes observations in primary care and occupational health care. The management of work-related mental ill health in everyday healthcare practice is characterized by the fact that the cause of the problem is complex, the division of responsibility unclear and that psychosocial causes of disease are controversial. The study discusses the challenges and possibilities of managing this complex problem in a broad sense. The analysis pays attention to the drawing of boundaries by the care professionals regarding both responsibility and the phenomenon of work-related mental illness. The concept of knowledge infrastructure is used to explain and understand the knowledge and material structures that the care professionals work within. The analysis shows that the care professionals have a pragmatic approach and use different strategies to create scope for dealing with work-related mental health.

Full textThe study aimed to investigate differences between client groups in treatment effect with respect to anxiety, depression, burnout, sleep and stress after participating in stress or sleep group treatment. This was investigated by studying whether background factors such as reason for seeking medical treatment, previous treatment, health status before treatment and demographic factors are related to the treatment effect. The stress- and sleep group treatments are psychoeducative, manual-based, aimed at stress or sleep problems. The treatments were implemented in a primary care setting by psychology students under the supervision of a licensed psychologist. The group treatments are based on the cognitive behavioral therapy method (CBT) and consist of four meetings. The study is based on data from 95 individuals who completed the treatment and participated in pre- and post-measurements. To investigate the relationship between background variables and treatment outcome, bivariate correlation analyzes were performed in a first step. Based on the significant correlations, regression analyzes were then performed to examine the predictive effects of the background variables on the treatment outcome. The results showed that few background variables could predict the outcome, which indicates that the treatment is effective for a broad group of patients. The variables that could predict treatment outcomes with regard to insomnia and burnout, respectively, were that burnout and insomnia had been stated as the reason for seeking medical treatment. This two-way link between burnout and insomnia has also been shown in other studies. The results of the study provide support that the treatment has a clinically relevant effect for a broad group of patients, but it also gives an indication that those seeking treatment for burnout and/or insomnia may need additional and longer interventions.

Full textBackground: Mental illness can range from mild anxiety to more serious illnesses such as psychosis, bipolar disorder and more severe depression. Today, primary care has a great responsibility for patients with mental illness. Shortcomings have been identified regarding health care staff not having the appropriate knowledge in order to deal with these patients. Cooperation between different professions is important both inside and outside primary care. A functioning team around the patient is important when creating good care, and the nurse's core competencies are highlighted, which means that the nurse must work towards collaboration in teams and person-centered care. Aim: The aim was to illustrate the nurse ́s coordinating function in the care of patients with mental illness in primary care. Method: Systematic literature study with inductive approach. Results: The results are based on 11 scholarly articles, which illustrate the nurse's caring of patients. The result presents into two categories. Firstly, the meaning of a close collaboration between specialist healthcare and primary care, which also should emerge in the team around the patient. Collaborations also lead to an increase in knowledge about mental illness. It emerges that if the nurse can create continuity and perform follow-up around the patient, a more person-centered care arises. Conclusion: The study shows that care becomes more accessible when the role of the nurse is to provide support in terms of coordinating and continuous care.

Full textThe purpose of the study has been to increase the knowledge of how counsellors within health care view internet based treatment of patients who suffer from mental illnesses, to be able to describe and understand the possibilities and limitations with such forms of treatment. The study has been carried out through qualitative semi structured interviews with ten professional counsellors in eight different primary care units in Stockholm county. The transcripts of the interviews have been analysed by using the theory of symbolic interactionism. The result showed that the counsellors’ view of internet based treatment is ambivalent. The internet based treatment is complicated and the counsellors seeing both pros and cons indicates that there are both possibilities and limitations which such methods. The result also showed that the possibilities that the counsellors can identify in internet based treatment concerns availability and that such methods have the potential to be a valuable tool for those patient who desire such methods of treatment. When it comes to limitations the counsellors can see that treatment via internet make great demands on the patient’s own abilities and that written interaction is far less nuanced than the interactions of traditional treatment work. It is clear that the main reason for the counsellors’ hesitation about these methods is the lack of face-to-face interaction with the patient, as well as difficulties imagining the practical applications of the internet based treatment. Thus the internet based treatment work can not easily be compared with the traditional psychosocial work that occurs face-to-face, but should be considered as a potential complement to traditional treatments or even a complete new way of working.

Full textAllmänläkare i primärvården är den första vårdgivare som människor i behov av psykisk hjälp möter. Det saknas forskning om behandling av psykiskt sjuka inom primärvården i enskilda länder. I syfte att hitta nya frågeställningar och få djupare förståelse av hur primärvården fungerar med avseende på psykiskt sjuka intervjuades elva allmänläkare i en kroatisk stad. Insamlade data från intervjuerna strukturerades och bearbetades genom en induktiv tematisk analys. Resultaten visar att allmänläkare upplever en hög arbetsbelastning. Deras tid för samtal med psykiskt sjuka är väldigt begränsad. Behandling med psykofarmaka blir ofta det enda vårdalternativet. Psykosociala problem och psykotrauma förekom ofta som konsekvens av krig. Sjukpensionering av krigsveteraner med PTSD-diagnos blev en omdiskuterad fråga. Undersökningen visar att det kan bli ansträngande för allmänläkare att behandla psykisk ohälsa. De behandlar dessutom majoriteten av psykiskt sjuka men deras vård kompletteras inte med några psykosociala tjänster. Behovet av psykiskt stöd blir inte bemött hos många patienter.

Diabetes i kombination med psykisk ohälsa är ett stort problem i dagens samhälle. Höga blodsockervärden är en riskfaktor för depression. Depressionens symptom kan i sin tur öka risken för att drabbas av diabetes. Dessa två sjukdomar påverkar livet och hälsan. Syftet med denna litteraturstudie var att belysa män och kvinnors upplevelser av att leva med diabetes och psykisk ohälsa. Studiens design var en litteraturstudie av sju vetenskapliga artiklar i form av primärkällor funna genom sökning i databasen Psychinfo . Texten bearbetades och analyserades med hjälp av kvalitativ innehållsanalys. I resultatet framkom att leva med diabetes och psykisk ohälsa, hade en negativ inverkan på det dagliga livet. Detta påverkade förmågan att sköta sin diabetes på ett bra sätt. Även förmågan att genomföra egenvårdsaktiviteter som är viktiga vid diabetes påverkades negativt Många kände oro inför framtiden på grund av risken för allvarliga komplikationer och relationerna med familj och omgivning blev ansträngda då många upplevde bristande förståelse från familj och vänner. Särskilt kvinnor kände att de saknade det stödet. Preventiva åtgärder och sjuksköterskans perspektiv belystes. Behov av genomarbetade vårdprogram och utbildningsmaterial för att öka kunskaperna hos personal och patienter.

Full textPerfectionism is becoming more and more common in the Western world. Parallel to that, mental ill-health is increasing among young people. It is therefore of interest to investigate whether the parallel increases in perfectionism and mental ill-health are related. Burnout is a form of mental ill-health that has been found to be associated with perfectionism. Statistics show that psychologists report more sick leave due to burnout than other occupational groups. This study investigated the relationship between perfectionism, mental health and quality of life among psychology students in Sweden. Data was collected through an online survey, (N = 270, M = 26.4, SD = 5.67). The survey consisted of three questionnaires; Clinical Perfectionism Questionnaire (CPQ), Depression Anxiety Stress Scales (DASS-21) and Satisfaction With Life Scale (SWLS). Correlational analyzes were conducted to investigate the relationship between perfectionism, mental health and quality of life. Analysis of variance was conducted to investigate whether these relationships differ in strength depending on the number of reported life domains within perfectionism. The result showed that there is a significant positive relationship between perfectionism and mental ill-health as well as a significant negative relationship between perfectionism and quality of life. Furthermore, it was found that participants who reported perfectionism in multiple life domains estimated a higher degree of mental ill-health compared to participants who reported none or one life domain. However, there was no difference between the groups in estimated quality of life.

Full textTwo prevalent public health diseases are obesity and mental illness. Obesity and overweight are associated with diseases such as diabetes, cardiovascular disease, various cancer diagnoses and even psychiatric illness. The risk factors for being overweight or obese are unhealthy diets and physical inactivity.The purpose of this study is to investigate factors associated with obesity and mental illness such as comorbidity in women.The chosen method is a literature study. The author has performed a systematic literature review of scientific articles.The result describes that factors such as socioeconomic status and ethnic background can be associated with overweight or obesity and depression and anxiety.The study shows that it is necessary in an intervention that aims to reduce the weight of individuals also take into account the promotion of mental health and does not expose an individual to feelings of shame and guilt.

Full textThe purpose of the study is to investigate why mental illness increases in primary school and how professionals in the school work to reduce mental illness among students. The empirical data has been gathered through interviews with four professionals at four schools located in southern Sweden. During the course of the study, the Covid-19 pandemic has caused some limitations and difficulties. The interviews were conducted through telephone interviews on the recommendation of the university to not visit any schools. Because of the pandemic, the number of interviews became less than planned. The study has a qualitive approach with semi-structured interviews. The results from the interviews shows that mental illness is increasing among students, where stress and performance anxiety are the main factors. According to the interviews, stress and performance anxiety develops because of the requirements that the parents and school has on the students. The results have also shown that the collaboration between the school, authorities, parents and the student are according to the study. According to the results, the mental illness starts to show already in students who are in third grade.

Full textThe purpose of the study was to explore Somali young adults’ perception of mental illness and how they look on seeking support and help from people in the surrounding environment. The method that has been used is a qualitative method using semi-structured interviews. The participants in the interviews were young adults with a Somali ethnic background. There were three men and women and their ages vary between 19 to 29 years old. The analysis was done using a qualitative manifest content analysis. The result of the study shows that young adults Somalis’ perception of mental illness looked different and difficult to define. Young adult Somalis also feel that it is not possible to talk openly about mental illness with people in their surroundings, especially with members of the family. Their perception of support and help is that it is avoided, religion plays a major role among it and there is a lack of knowledge regarding where they can seek help and support for mental illness.

Full textSammanfattning Denna kvalitativa studie syftar till att undersöka yrkesverksamma personalvetares uppfattning om arbetsrelaterad psykisk ohälsa och om det arbetsmiljöarbete som bedrivs i förebyggande syfte mot arbetsrelaterad psykisk ohälsa på deras arbetsplats. Arbetsrelaterad psykisk ohälsa är ett växande fenomen som orsakas av den psykosociala arbetsmiljön. Arbetsmiljöarbetet är till stor del lagreglerat i arbetsmiljölagen, som genomgick en korrigering under 2016. Forskning visar dock att de riktlinjer som framtagits är svåra att implementera och utvärdera i det verkliga arbetslivet, att det råder brist på engagemang från arbetsgivaren, samt ett ökat krav på individer att förvärva kunskaper för att främst öka organisationers effektivitet. De teoretiska ramverk som använts för studien är det livslånga lärandet, och ämnar att förklara hur förvärvandet av kunskaper om arbetsrelaterad psykisk ohälsa gått till, och dessa kunskaper används i arbetslivet. Studien utgick från åtta intervjuer och resultaten presenterades genom en tematisk analys. Det visade sig att formell utbildning många gånger är svårt att sätta i en verklig kontext, att fenomenet arbetsrelaterad psykisk ohälsa ofta är svårbemött, och att det är genom det informella lärandet som informanterna lärde sig att bemöta det. Det visade sig även att särskiljandet av den fysiska och den psykosociala arbetsmiljön upplevdes som svåra att separera, och att den psykosociala arbetsmiljön då ofta inte synliggjordes i samma utsträckning som den fysiska. Resultaten visade även på att samarbetet med företagshälsovården på arbetsplatser hjälpte till att synliggöra ett arbetsmiljöarbete. Chefens roll var avgörande många gånger för synliggörandet samt känslan av ett inkulderande i arbetsmiljöarbetet. Det informella lärandet visade sig vara av stor betydelse för informanternas möjlighet att kunna hantera faktorer i arbetslivet, men att det finns en avsaknad av icke-formellt lärande eller formellt lärande i arbetslivet, som bekräftar den kunskap som kommer från det informella lärandet.

These have been systematically revived and graded. The articled have been searched and found in following databases: Blackwell Synergy, Cinahl, PsycInfo and Science Direct.Result: The result of the study shows that the nurses through different cultural nursing tools are able to intervene against mental illness among patients from different cultural contexts. The study also shows how mental illness express itself is determined by cultural context. The clinical practice of the study is that it pinpoints the need of cultural competence among nurses´.

Full textBeing a student is a crucial investment for future work opportunities, but it is also related to increased levels of stress with risk for anxiety and depression. The main aim of this study is to explore if self-esteem has a relation with students´ levels of stress, anxiety and depression, even after controlling for demographic data. The secondary aim is to investigate if differences in depression, anxiety, stress and self-esteem, are related to various fields of study. Participants consisted of university students (N= 514) from three different fields of study: Computer Science and Information Technology, Health care, and Culture, Media & Design. Three validated instruments were used to examine: Self-esteem (Rosenberg Self-esteem Scale, RSES), Stress, (Perceived Stress Scale, PSS) and Anxiety and Depression (Hospital Anxiety and Stress Scale, HADS). The result from three multiple hierarchical regression analyses shoved that low self-esteem had a strong relation to higher levels of anxiety, stress and depression, in the whole sample.The result from a MANOVA showed that there was a statistically significant difference in stress and mental health according to field of study. The Group of Culture, Media & Design had statistically significantly higher degrees of stress and anxiety than the groups for Computer Science and Health care. There was no statistically significant difference in depression related to field of study. The group of Computer Science had statistically significantly higher self-esteem than the group for Culture, Media & Design. This paper shows that mental illness is common among students and especially among students of Culture, Media & Design. It is therefore important to prevent mental illness and to improve the wellbeing of students.

Full textBakgrund: Ungdomar utsätts ständigt för riskfaktorer som kan påverka den psykiska ohälsan. Men genom kunskap och rätt omvårdnad kan flera riskfaktorer elimineras och det psykiska måendet förbättras. Syfte: Syftet med denna studie var att belysa riskfaktorer och omvårdnadsåtgärder vid psykisk ohälsa bland ungdomar samt granska de valda artiklarnas dataanalysmetod. Metod: Denna beskrivande litteraturstudie grundades på 16 vetenskapliga artiklar som analyserats och granskats för att urskilja teman samt vilken dataanalysmetod som använts. Huvudresultat: Flera olika riskfaktorer visade ge ökad risk för psykisk ohälsa bland ungdomar. Det som stod ut mest var ungdomar vars familjer hade sämre socioekonomiska förhållanden, ungdomar som bevittnat eller blivit utsatt för våld samt utanförskap. Överlag led fler andel tjejer av psykisk ohälsa. I omvårdnaden kring ungdomar som drabbats av psykisk ohälsa sågs ett stort behov av stöd. Det var också viktigt att sjuksköterskan tog sig tid och byggde upp ett förtroendefullt förhållande gentemot ungdomen. Genom att sjuksköterskan motiverade och utbildade ungdomar med psykisk ohälsa ökade medvetenheten och stigmatiseringen minskade. Slutsatser: Denna litteraturstudie belyste flera riskfaktorer för psykisk ohälsa bland ungdomar, vanligaste var familjens inverkan, socioekonomiska förhållanden, att utsättas eller bevittna våld, att vara tjej och stress. Litteraturstudien lyfte även omvårdnadsåtgärder vid psykisk ohälsa bland ungdomar där de belyste vikten av att ge stöd, lyssna, vara tillgänglig, arbeta för att bygga en förtroendefull relation samt samverka med föräldrar och andra professioner. Denna kunskap möjliggör för sjuksköterskor att identifiera psykisk ohälsa bland ungdomar i tidigt stadie och sätta in omvårdnadsåtgärder i tid.

Full textBackground: A problem in society for people with mental illness is stigma and negative attitudes. Primary care is first-line health care, where many turn first to get help for mental illness. As stigma can be a reason not to seek help for mental illness, it is important that the healthcare professionals do not contribute to this. Preconceived notions and negative attitudes towards patients also counteract the building of good caring relationships, which hinders the patient's recovery and results in a worsened care experience. Aim: The purpose of this literature review is to study how adults with mental illness experience care meetings in primary care and which aspects are considered important in the meeting. Method: A descriptive design with general literature review as method was used and 13 scientific articles with a qualitative or mixed method were retrieved from the databases PubMed, CINAHL and PsycINFO. After quality review, the results of the articles were analyzed according to Graneheim and Lundman's (2004) qualitative content analysis. Results: Five themes emerged with 17 associated subcategories which described people's positive and negative experiences. These themes were: Treatment in primary care, Primary care staff's attitudes, Primary care staff's competence, Cooperation and participation and access to care and resources. Conclusion: The study showed that a good treatment and collaboration with care staff contributes to a positive care experience and a willingness to get involved in their own care. Poor treatment, on the other hand, with lack of cooperation and negative attitudes, increases the stigma of mental illness and provides a negative care experience. To counteract negative attitudes towards this patient group, healthcare professionals need more knowledge about mental illness and work to improve these attitudes.
