The World Health Organization (WHO) launched the Global Hearts Initiative and its HEARTS Technical Package in 2016. Its aim is to improve clinical preventive services in primary health care (PHC) using highly effective, scalable, sustainable, and proven interventions. As the WHO Regional Office for the Americas, the Pan American Health Organization (PAHO) is developing and coordinating the HEARTS in the Americas Initiative with a clear vision: By 2025, HEARTS will be the institutionalized model for cardiovascular disease risk management, including hypertension, diabetes, and dyslipidemia in primary health care in the Americas. The HEARTS in the Americas Initiative is entering its sixth year of implementation, having expanded from the original cohort of 4 countries to 22 current countries implementing the HEARTS model. The accumulation of knowledge, practices, and experiences from the field and from different levels of implementation has been compiled in this new Guide and Essentials for Implementation. This manual complements the WHO HEARTS technical package Implementation Guide. Expanding on specific lessons learned from the systematic implementation of HEARTS in the Americas as it spans various roles and requires coordination across ministry of health (MOH) departments, stakeholders, partner agencies, scientific societies, and academic institutions. Documenting the implementation and scale-up experiences of countries in the Americas is timely to propel forward the institutionalization of the HEARTS model for cardiovascular disease (CVD) risk management, including hypertension, diabetes, and dyslipidemia in PHC in the Americas by 2025. This manual is designed to be practical and user-friendly. It aims to guide implementers at national and subnational levels to navigate throughout different stages of implementation and to ensure longtime sustainability. The manual is written for national focal points, managers and coordinators at subregional, provincial, district, municipal level, and health facilities implementing HEARTS. It is intended to answer frequently asked questions about the premises, objectives, components, and steps for implementation of the HEARTS in the Americas Initiative.

[INTRODUCTION]. Epilepsy is one of the most common neurological disorders in the world and in our hemisphere, affecting millions of people. However, it is estimated that more than half of people with epilepsy in Latin America and the Caribbean (LAC) do not receive care of any kind from health services. One of the key problems is a limited ability to identify, manage, and monitor epilepsy cases in primary health care (PHC) services. Furthermore, in most LAC countries, the secondary (specialized) care level either has serious limitations or is nearly nonexistent (PAHO, 2011; PAHO/ILAE/IBE, 2013)... This document offers guidelines and a frame of reference to help countries design their national epilepsy programs and should be regarded as complementing the Strategy and Plan of Action on Epilepsy (PAHO,2011) adopted by all the ministers of health of the Americas at the Directing Council of PAHO, and also complementing the resolution and document approved by the World Health Assembly (WHO, 2015). [INTRODUCCIÓN]. La epilepsia es uno de los trastornos neurológicos más comunes en el mundo y en nuestro continente, que afecta a millones de personas. Sin embargo se estima que, en América Latina y el Caribe (AL&C), más de la mitad de las personas con epilepsia no reciben ningún tipo de atención en los servicios de salud. Uno de los problemas clave son las limitaciones en la identificación, manejo y seguimiento de los casos de epilepsia en la atención primaria de salud (APS). A esto, se adiciona el hecho que en la mayoría de los países de América Latina y el Caribe el nivel secundario o especializado tiene serias limitaciones o es casi inexistente ... El presente documento es una guía o marco de referencia para ayudar a los países a diseñar sus programas nacionales sobre epilepsia y debe considerarse como un complemento a la Estrategia y Plan de Acción sobre la Epilepsia adoptado por todos los ministros de salud de las Américas, en el marco del Consejo Directivo de la OPS; así como de la Resolución y documento aprobado por la Asamblea Mundial de la Salud.

Abstract:
Cardiovascular disease (CVD) is the leading cause of disease burden globally. According to the 2017 Global Burden of Disease estimates, there were 14 million new cases of CVD, 80 million people living with this condition, and nearly 1 million deaths attributed to CVD in the Americas. Hypertension is the major risk factor for CVD, causing half of the cases, and is highly prevalent, affecting one in four adults, including 40% of those over age 25 years.To appropriately detect hypertension, accurate measurement of blood pressure is critical, and inaccurate measurement of BP has important consequences for policies to address hypertension, as well as for patient safety and quality of care. The Fourth World Health Organization (WHO) Global Forum on Medical Devices identified several critical issues related to medical devices. Among those issues were recommendations for increased regulation of medical devices in low- and middle-income countries and development of technical specifications to optimize procurement of priority medical devices. World Health Assembly Resolution 67.20 (Regulatory system strengthening for medical products) stresses the importance of regulation of medical devices for better public health outcomes and to increase access to safe, effective, and quality medical products. This publication seeks to contribute to meeting these recommendations by providing a practical tool for governments to improve their national regulatory frameworks to improve accuracy of blood pressure measuring devices (BPMDs), in turn contributing to the exclusive use of accuracy validated automated BPMDs in primary health care (PHC) facilities by 2025. This publication can also guide the development of procurement mechanisms that will ensure exclusive availability of BPMDs in PHC facilities. Specifically, this publication will provide a brief background on the importance of using validated BPMDs and highlight key elements of regulations related to pre-market approvals to promote accurate BPMDs.
