Despite these complicating factors, India has to engage this challenge because a pathway to development innocent of climate change is no longer possible. This volume brings together leading researchers and practitioners—negotiators, activists, and policymakers—to lay out the emergent debate on climate change in India. The discipline of geography is built around four key concepts—environment, place, space, and scale—that form a matrix for exploring and appreciating many aspects of contemporary society. The environment is the ultimate source of human sustenance; people have created places to realize that potential; and a spatial structure—nodes, routes, surfaces and bounded territories—has been erected within which human interactions are organised.The relationships between human societies and their environments—now very much changed from their pre-human “natural” state—involve competition for and conflicts over resources, of increasing intensity. Resolution of all but the smallest scale of those conflicts requires a body that is independent of the actors involved and can ensure that agreements are reached and then implemented. Such a body is the state, a territorially bounded apparatus that, through the operation of territoriality strategies, can ensure conflict resolution among its citizenry and thereby resolve environmental problems.Many of those problems—the most severe being global climate change resulting from anthropomorphically induced global warming—are not contained, and cannot be contained, within an individual state’s territory, however. Tackling them requires inter-state co-operation, at a global scale, but the absence of a super-national body with the power to require actions by individual states is a major constraint to problem resolution.

Traumatic brain injury (TBI) is a common, clinically complex, heterogeneous global public health problem. Neuroprotection strategies focus on preventing secondary injury by creating a physiologic environment devoid of extremes while targeting normal physiologic parameters. Careful attention must be paid to aggressively avoid and treat hypoxia, hypotension, hypoglycemia, intracranial hypertension, and cerebral hypoperfusion (low cerebral perfusion pressure). Aggressive management of intracranial pressure and cerebral perfusion pressure through optimal patient positioning, appropriate use of sedation and analgesia, and administration of hyperosmolar therapy remain the hallmark for the care of the TBI patient. Surgical decompressive craniectomy and hypothermia hold promise but remain controversial and should be used in carefully selected clinical situations. Early identification of injury progression is aided through careful monitoring by clinical examination and cerebral physiological monitoring. Multimodal monitoring provides an early warning system to guide appropriate clinical responses to identified deranged physiology.

China’s Three Gorges Dam is the largest hydroelectric megadam in world history and the subject of Jia Zhangke’s digital film Still Life (2006), Liu Xiaodong’s oil paintings, and Yang Yi’s digital photographs. Built in part to address the problems of global warming, the dam has had a number of surprising effects on the environment and has led to massive displacement of people to make way for the water. Jia populates his film with migrant workers, refugees, tourists, visitors, and even aliens from outer space, none of whom are at home in this world. Jia captures the city at the moment of its undoing, a place that is changing, he remarks, too fast for film. Reading Jia’s film together with Liu and Yang’s artwork and through the genre of still life painting, the chapter argues that Jia’s film envisions a kind of minimal hospitality that emerges when the world is past.

Topical pharmacological agents such as non-steroidal anti-inflammatory drugs (NSAIDs) and capsaicin are widely recommended as first-line analgesics in the treatment of osteoarthritis (OA) of the knee, hand, and potentially other peripheral joints in view of their safety and efficacy. Although initial studies were short in duration (2–4 weeks), recent randomized controlled trials have confirmed the efficacy of topical NSAIDs over longer (12-week) study periods. Systematic reviews demonstrate that their efficacy can be equivalent to oral NSAIDs for OA pain, but they have a significantly better systemic toxicity profile than the corresponding oral formulations. Topical capsaicin is less well studied than topical NSAIDs but has been demonstrated to be effective in several placebo-controlled clinical trials. Local warming and an uncomfortable burning sensation is a common problem with initial applications, but this subsides with continued treatment and can be minimized by using a low-strength preparation (e.g. 0.025%) initially. Several other topical treatments such as drug-free transfersome gel and local lignocaine patches have been shown to be effective in controlling pain due to OA. However, they have been studied in relatively few studies and currently are not recommended for general use.
