Abstract:
The paper presents an analysis of results of surgical treatment of patients with chronic lung disease. To predict the probability of postoperative complications, duration of treatment and the final outcome after surgical treatment for lung the authors used artificial neural networks (ANN). Currently in thoracic surgery practically there are no universally accepted prognostic systems, allowing with high degree of confidence to make the right decision in the treatment strategy for various lung diseases. The complexity of forecasting in this situation due to the fact that the most information is a subjective expert evaluation by a physician based on his knowledge and experience in the treatment of patients with lung disease. The results of the research proved that the modeling method based on ANN allows to solve problems of classification, optimization and forecasting and to give higher prediction accuracy in comparison with multivariate statistical analysis methods. The article shows that the use of ANN methods enables more accurately predict the risk of postoperative complications. This accelerates the work of specialists and facilitates to plan hospitals with high surgical activity.

Background: Glioblastoma multiforme (GBM) is the most common and aggressive primary brain tumor. Previous studies on GBM biomarkers focused on the effect of the biomarkers on overall survival (OS). Until now, no study has been published that evaluates the performance of biomarkers for prognosing OS. We examined the performance of microRNAs, gene expressions, gene signatures, and methylation that were previously identified to be prognostic. In addition, we investigated whether using clinical risk factors in combination with biomarkers can improve the prognostic performance. Methods: The Cancer Genome Atlas, which provides both biomarkers and OS information, was used in this study. The time-dependent receiver operating characteristic (ROC) curve was used to evaluate the prognostic accuracy. Results: For prognosis of OS by 2 years from diagnosis, the area under the ROC curve (AUC) of microRNAs, Mir21 and Mir222, was 0.550 and 0.625, respectively. When age was included in the risk prediction score of these biomarkers, the AUC increased to 0.719 and 0.701, respectively. The SAMSN1 gene expression attains an AUC of 0.563, and the “8-gene” signature identified by Bao achieves an AUC of 0.613. Conclusions: Although some biomarkers are significantly associated with OS, the ability of these biomarkers for prognosing OS events is limited. Incorporating clinical risk factors, such as age, can greatly improve the prognostic performance.

Abstract Background Asthma is a common disease with increasing prevalence in children and adults. The WHO estimates that annually 15 million disability-adjusted life-years are lost, and 250,000 asthma deaths are reported worldwide. Approximately, 500,000 annual hospitalizations are due to asthma (1). Aim In our study, we aimed to evaluate the endothelial function in children with asthma in remission and the prognosis of severe asthma. Materials and methods The study involved examination of 91 children, aged 6–17 years, with persistent asthma in the remission period. Indices of endothelial function (soluble vascular molecule of intercellular adhesion-1 [sVCAM-1], concentration of stable metabolites of nitric oxide in blood serum [NO2 +NO3 ], thickness of the intima-media complex [IMC] of the common carotid artery (CCA), and endothelium-dependent dilatation of the brachial artery [FMD%]). Statistical analyses were performed with StatSoft STATISTICA Version 8 (Tulsa, OK). To determine the relation between qualitative characteristics, the criterion χ2 was used, and the procedure of multiple logistic regression analysis was performed. Results The endothelium parameter levels (FMD% [H = 46.02], IMC [H = 60.75], NO2 + NO3 [H = 40.82], and sVCAM-1 [H = 76.57, p = 0.0000]) depend on the severity of the disease. The study showed that the factors that should be taken into account in prognosis of the formation of the severe course of asthma include positive family allergic history, serum sVCAM-1 and NO2 + NO3 levels, and the thickness of IMC CCA. Conclusions All the children with asthma in the remission period were found to have endothelial dysfunction. The degree of disruption of the function of the endothelium depends on the severity of the course of asthma. An algorithm for predicting the severe course of asthma in children has been developed.

Objective. Criteria specification and prognosis establishment of intraocular pressure compensation in primary open-angle glaucoma (POAG) patients at medical and surgical treatment based on integrated clinical and instrumental examination. Methods. 128 POAG subjects (234 eyes) receiving medical treatment were included in the study, surgery was performed in 42 patients (52 eyes). Results. The use of multiple clinical parameters into the statistical analysis leads to an increase in the probability of IOP compensation and correct prognosis (up to 84.0-98.4 % of cases) in medical treatment, and does not alter the prognosis accuracy at surgical treatment. In analysis of lower variables number, correct prognosis of IOP compensation at medical treatment can be established in 75.4-75.7 % of cases, and at surgical treatment - in 100 % of cases. The results of present study are the basis for a computer program development that could allow a practicing ophthalmologist to determine the prognosis of IOP compensation in a POAG-patient.

Maintaining tick colonies under laboratory condition, as well as carrying out experimental studies on these arthropods may be more successful with better knowledge of some basic biological parameters. In this study, we evaluated how the weight, engorgement period and premolt period of Amblyomma auricularium nymphs (N=600) correlated with the sex of the adults that subsequently emerged. The experiment was carried out in the W.O. Neitz Parasitological Research Station, Ixodology Laboratory of the Department of Animal Parasitology, Federal Rural University of Rio de Janeiro, located in Seropédica, state of Rio de Janeiro. The average weight of the nymphs was 5.1±1.51mg among those that originated males and 8.2±2.5mg among those that originated females. The engorgement period for the nymphs that originated males was 8.3±1.5days while it was longer for the nymphs that originated females, with an average of 8.8±1.8days to complete their diet. The premolt period was shorter for nymphs that originated males (17.2±0.93days) than for nymphs that originated females (17.7±1.00days). Despite an area of overlapping between the weights of engorged nymphs that originated adult males and females, this is the most reliable parameter for predicting the sex of adults of A. auricularium.

Abstract:
Aim. The purpose of the article is to provide a clinical and physiological assessment of adaptive variability of the psychophysiological status in schoolchildren and to obtain prognostic markers for forming success during health enhancement and sports activities. Materials and methods. 187 male schoolchildren of the first and second health group from secondary educational institutions in Tyumen participated in the study. A set of clinical and physiological methods was used. Habitual physical activity (HPA) was measured on a daily basis using Huawei Band 2 Pro (China) fitness tracker provided with accelerometer and gyroscope. The band is available for both Android (version 4.4 or later) and iOS 8.0 or later, which allows registering data at any time period. The type of partial body constitution was established according to a three-component scheme and a HPA level (low HPA – FTC-1, medium HPA – FTC-2, high HPA – FTC-3). Results. Based on the concept of typological variability of physiological individuality, a systemic relationship between the indicators of anxiety, aggressiveness and depression was established in primary schoolchildren with different levels of habitual physical activity (low – LHPA, medium – MHPA, high – HHPA) and of different functional types (FTC-1, FTC-2, FTC-3). Compared with other groups, primary schoolchildren with LHPA – FTC-1 showed an increase in general anxiety and a total indicator of depression, a decrease in mood, a negative assessment of their own inefficiency, and the lowest indicator of aggressiveness with an auto-aggressive orientation. In children with HHPA, there was a decrease in the general level of depression and mood, a negative assessment of generally accepted rules, aggressive behavior, a negative assessment of their own inefficiency combined with an increased level of aggressiveness of a heteroaggressive orientation. Conclusion. Clinical and physiological characteristics of three physiological norm variants (functional constitutional types, children with low (FTC-1), medium (FTC-2) and high (FTC-3) habitual physical activity) determined the selection of psychophysiological markers of stress reactivity in primary schoolchildren, which provides an objective basis for prognostic assessment and targeted success formation in sports and health enhancement activities, as well as the creation of effective monitoring of transition states and preventive forms for correcting dysregulation.

Complex network theory has been used, during the last decade, to understand the structures behind complex biological problems, yielding new knowledge in a large number of situations. Nevertheless, such knowledge has remained mostly qualitative. In this contribution, I show how information extracted from a network representation can be used in a quantitative way, to improve the score of a classification task. As a test bed, I consider a dataset corresponding to patients suffering from prostate cancer, and the task of successfully prognosing their survival. When information from a complex network representation is added on top of a simple classification model, the error is reduced from 27.9% to 23.8%. This confirms that network theory can be used to synthesize information that may not readily be accessible by standard data mining algorithms.

Methods. We studied prospectively and partly retrospectively 149 patients, 109 female and 40 male patients, 5-78 years old, in the period 1982-2007. There were 96 patients with GD. The control group consisted of 53 patients, 21 with hyperthyroidism of second etiology and 32 patients on amiodarone therapy, with or without thyroid dysfunction TRAb was measured by radioreceptor assay (TRAK Assay and DYNO Test TRAK Human Brahms Diagnostica GMBH). Results. According to the results the sensitivity (Sn) of TRAb test was 80%, specificity (Sp) 100%, positive predictive value (PP) 100% and negative predictive value (NP) 83%. Also, the Sn of hTRAb test was 94%, Sp 100%, PP 100% and NP 94%. Our results show that an increased level of TRAb/hTRAb at the beginning of the disease and the level at the end of medical therapy is associated with an increased number of GD relapses and a shorter remission duration. Conclusion. Detection and measurement of TRAb in serum is a very sensitive method for diagnosing GD and very highly specific in vitro method for differential diagnosis of various forms of hyperthyroidism. Clinical significance of differentiating various forms of hyperthyroidism, using this in vitro assay, lays in adequate therapeutic choice for these entities.

Abstract:
The epidemiological features and social factors of HIV-infection were studied. The clinical picture of disease was estimated. The laboratory researches included detection of contents CD4-lymphocytes and blood lipid structure. The patients with HIV-infection in the 3rd stage were divided into 2 groups according to duration of a course of the stage. The number of females involved in a HIV-infection as a result of the sharp increase of heterosexual way of transmittion was enlarged considerably. Pronounced changes of lipid metabolism in patients with HIV-infection in a 3rd stage were observed. As lisophospholipids indices in HIV patients with duration of the 3rd stage more than 5 years were several times lower, than in the HIV patients with the smaller experience, it is possible to use these parameters for prognosing the course of the disease.

AbstractThe article presents a parametric method applicable in assessing the suction force of a slender body to the seabed, and prognosing the body extrication time. Along with the body weight in water, the information on the suction force is essential for assessing the force needed to lift the object from the seabed. Based on the Foda theory and the resulting integral equation, which relates the maximum suction force with basic parameters of the issue of concern, the parametric method has been developed which can be widely applied in engineering practice of lifting objects. A set of discrete values of parameters selected from the range of typical values for engineering practice was the basis for developing algebraic formulas approximating the integral Foda equation for slender bodies. The included results of tests present the variability of suction force characteristics and the envelopes of maximum forces needed to extricate the body from the seabed.

Abstract:
The article aims to identify the latest trends in research on renewable energy, sustainability and the environment. A total of 92,873 publications from 123 Scopus sources for 2020–2021 are compared using the scoping review method. The results show that the most cited works in this sample are those by authors from the Asian region. The research of these authors focuses on the security, efficiency and reliability of separate elements in energy systems. Besides, the paper considers the problems regarding COVID disease along with the renewable energy sources, perovskite and organic solar panels, nanostructured materials and high energy density. Finally, the paper analyses applications of computer science methods in research on renewable energy, sustainability and the environment. The findings evidently show that recent advancements in computer science methods were not extensively used in the discussed research domain and give a great room for novel strategies of prognosing, simulation and processes optimisation.

AbstractNeuropsychiatric diseases, such as schizophrenia, bipolar disorder (BD), major depressive disorder (MDD) and autism spectrum disorder (ASD), are a huge burden on society, impairing the health of those affected, as well as their ability to learn and work. Biomarkers that reflect the dysregulations linked to neuropsychiatric diseases may potentially assist the diagnosis of these disorders. Most of these biomarkers are found in the brain tissue, which is not easily accessible. This is the challenge for the search of novel biomarkers that are present in various body fluids, including serum or plasma. As a group of important endogenous small noncoding RNAs that regulate gene expression at post-transcriptional level, microRNAs (miRNAs) play a crucial role in many physiological and pathological processes. Previously, researchers discovered that miRNAs contribute to the neurodevelopment and maturation, including neurite outgrowth, dendritogenesis and dendritic spine formation. These developments underline the significance of miRNAs as potential biomarkers for diagnosing and prognosing central nervous system diseases. Accumulated evidence indicates that there are considerable differences between the cell-free miRNA expression profiles of healthy subjects and those of patients. Therefore, circulating miRNAs are likely to become a new class of noninvasive, sensitive biomarkers. Despite the fact that little is known about the origin and functions of circulating miRNAs, their essential roles in the clinical diagnosis and prognosis of neuropsychiatric diseases make them attractive biomarkers. In this review we cover the increasing amounts of dataset that have accumulated in the last years on the use of circulating miRNAs and their values as potential biomarkers in most areas of neuropsychiatric diseases.

Abstract:
The aim of the study was to determine the functions influencing the recovery of the motor and cognitive functions and general functional state in cerebral stroke patients during the second rehabilitation stage, and their prognostic value. The contingent and methods. The studied contingent consisted of 226 cerebral stroke patients: 109 men and 117 women who had undergone the second stage rehabilitation in Viršužiglis Rehabilitation Hospital, a branch institution of Hospital of Kaunas University of Medicine. The mean age of the patients was 67.8±10.4 years. The greatest proportion of the patients (88.5%) was with cerebral infarction. The mean duration of rehabilitation was 38.8±8.9 days. The functional state of the patients was assessed by functional IndependenceMeasure; the cognitive function, by the short mental function study method; the severity of the stroke, in accordance with the National Institutes of Health Stroke Scale. The prognostic value of the factors was evaluated by the logistic regression method. A study analyzed the influence of patient’s gender, age, social factors, clinical symptoms and signs, risk factors for stroke, comorbities, the character and localization of the stroke, and psychoemotional state on the recovery of capacities related to motor and cognitive functions, and restoration of general functional state. Results. During the second stage of rehabilitation, the score of the functional state of the patients assessed by the Functional Independence Measures significantly improved from 65.9±20.3 to 93.5±20.9 (P<0.0001). At the end of rehabilitation, good efficacy of rehabilitation was determined in 64.2% of the rehabilitees; moderate, in 19.4%; insufficient, in 16.4%. In prognosing insufficient recovery of general functional state during the second stage of rehabilitation, the following factors had a significant influence: extremity hemiplegia, disturbed cognitive functions, urination impairment, joint and heart diseases. In cases of insufficient recovery of capacities linked with motor function in the second stage of rehabilitation, exerted extremity hemiplegia, neglect of the affected side of the body, urination impairments, joint and heart disorders had a significant influence, whereas localization of the stroke lesion in the left hemisphere of the brain, impairment of cognitive functions (disturbance of speech and perception), urination disturbances – on the insufficient recovery of capacities related to cognitive functions. Conclusion. Insufficient efficacy of rehabilitation of the rehabilitees with cerebral stroke in the second stage of rehabilitation was influenced by impairment of motor and cognitive functions, urination disturbancies, and comorbidities. In forming individualized inpatient rehabilitation programs and prognosing the further health care of patients with cerebral stroke, it is important to pay attention to factors, influencing insufficient efficacy of rehabilitation.

This interdisciplinary article presents a concept of the 21st century and phenomena that are products of the 4th industrial revolution – big data and Artificial Intelligence technologies – as well as the opportunities of their application in public governance and social policy. This paper examines the advantages and disadvantages of big data, problems of data collection, its reliability and use. Big data can be used for the analysis and modeling of phenomena relevant to public governance and social policy. Big data consist of three main types: a) historical data, b) present data with little delay, c) prognostic data for future forecasting. The following categories of big data can be defined as: a) data from social networks, b) traditional data from business systems, c) machine-generated data, such as water extraction, pollution, satellite information. The article analyzes the advantages and disadvantages of big data. There are big data challenges such as data security, lack of cooperation in civil service and social work, in rare situations – data fragmentation, incompleteness and erroneous issues, as well as ethical issues regarding the analysis of data and its use in social policy and social administration. Big data, covered by Artificial Intelligence, can be used in public governance and social policy by identifying “the hot spots” of various phenomena, by prognosing the meanings of variables in the future on the basis of past time rows, and by calculating the optimal motion of actions in the situations where there are possible various alternatives. The technologies of Artificial Intelligence are used more profoundly in many spheres of public policy, and in the governance of COVID-19 pandemics too. The substantial advantages of the provided big data and Artificial Intelligence are a holistic improvement of public services, possibilities of personalization, the enhancement of citizen satisfaction, the diminishing of the costs of processing expenditure, the targeting of adopted and implemented decisions, more active involvement of citizens, the feedback of the preferences of policy formation and implementation, the observation of social phenomenas in real time, and possibilities for more detailed prognosing. Challenges to security of data, necessary resources and competences, the lack of cooperation in public service, especially rare instances of data fragmentation, roughness, falseness, and ethical questions regarding data analysis and application can be evaluated as the most significant problems of using big data and Artificial Intelligence technologies. Big data and their analytics conducted using Artificial Intelligence technologies can contribute to the adequacy and objectivity of decisions in public governance and social policy, effectively curbing corruption and nepotism by raising the authority and confidence of public sector organizations in governance, which is so lacking in the modern world.

Data indicate significant phenotypic and genotypic overlap, plus a common pathogenesis, between two groups of inherited disorders, autosomal dominant polycystic kidney diseases (ADPKD), a significant cause of ESRD, and autosomal dominant polycystic liver diseases (ADPLD), which result in significant PLD with minimal PKD. Eight genes have been associated with ADPKD (PKD1 and PKD2), ADPLD (PRKCSH, SEC63, LRP5, ALG8, and SEC61B), or both (GANAB). Although genetics is only infrequently used for diagnosing these diseases and prognosing the associated outcomes, its value is beginning to be appreciated, and the genomics revolution promises more reliable and less expensive molecular diagnostic tools for these diseases. We therefore propose categorization of patients with a phenotypic and genotypic descriptor that will clarify etiology, provide prognostic information, and better describe atypical cases. In genetically defined cases, the designation would include the disease and gene names, with allelic (truncating/nontruncating) information included for PKD1. Recent data have shown that biallelic disease including at least one weak ADPKD allele is a significant cause of symptomatic, very early onset ADPKD. Including a genic (and allelic) descriptor with the disease name will provide outcome clues, guide treatment, and aid prevalence estimates.

In this paper, an analysis of a three-dimensional state of strain and stress in the case of the hot cogging process of X32CrMoV12-28 die steel with the application of the finite element method is presented. The results of the investigations connected with the simulation of the kinematics of metal flow and thermal phenomena are presented, accompanied by prognosing the formation of ductile fractures in the course of the hot cogging process conducted with the application of three different shape tools and of a proposed deformation criterion of the loss of cohesion. The applied anvils were found to be highly effective in the aspects of distribution of effective strains and stresses, absence of tensile stresses in the axial zones of a forging, and also of a significant thermal stability in the internal layers of a deformed material. The developed course of changes in the deformation of the damage factor in the case of forging in the investigated anvils renders it possible to predict the situation and the phase of deformation in which the loss of cohesion by a deformed material will occur. The comparison between the predicted and the experimental results showed a good agreement.

It has been more than 50 years ago, when in 1965 Florence Mahoney and Dorothea Barthel published an article entitled "Functional evaluation: the Barthel Index." Since then, Barthel scale, also known as a Basic ADL Index (BI), Barthel Score and Maryland Disability Index, is one of the most well-known scales ADL (Activities of Daily Living) and is still popular. This simple scale is still used by the representatives of a number of medical workers for many pur-poses, including the assessment of the results of treatment, rehabilitation, prognosing, assess-ment of self-reliance, assessment of needs for care and for the purposes of the judgment. De-spite its simplicity, the administration of the original Barthel questionnaire takes about 5-10 minutes; that`s why there is a demand for a shortened form of BI. Based on a review of the literature the advantages and disadvantages of the abbreviated BI versions in assessing activities of daily living has been presented.
