Abstract:
Context Whereas the serratus anterior (SA) and the upper trapezius (UT) work as a force couple for scapular motion, weakness of the SA and overactivation of the UT are often present in overhead athletes with shoulder dysfunction. Therefore, researchers addressing an intramuscular imbalance between the SA and UT have focused on finding exercises that target the weak SA and minimally activate the UT. Objective To compare the effectiveness of push-up plus (PUP) exercise variants based on the electromyographic (EMG) activity of the SA and UT. Data Sources A systematic search of PubMed and Scopus between January 1, 2000, and March 31, 2008. Study Selection Studies of PUP exercises that involved EMG analysis. Data Extraction We assessed study quality using the Critical Appraisal Skills Program. For the systematic analysis, the following data were extracted: (1) author, year, and study design; (2) participant characteristics; (3) type of PUP intervention; (4) EMG outcome measures; and (5) main results. For the meta-analysis, the EMG data of the SA and UT were calculated using the mean difference of EMG activity with a 95% confidence interval. Data Synthesis Based on 19 studies with 356 participants, different hand positions (the distance between the hands, shoulder-flexion angle, and elbow-flexion angle) and different lower extremity positions variably affected the activation of the SA and UT during the PUP exercise. Also, when participants performed the PUP on an unstable surface compared with a stable surface, UT activity increased 2.74% (95% confidence interval = 0.07%, 5.41%). Conclusions The standard PUP exercise elicited high EMG activity of the SA. Participants generated higher SA and lower UT EMG activity when they performed the PUP exercise on a stable surface in full elbow extension, with the hands placed shoulder-width apart, shoulder-flexion angles of 110° or 120°, and the ipsilateral lower extremity lifted.

Abstract:
Recently, studies on cell surface markers of spermatogonia in combination with germ cell transplantation techniques have made possible the functional analysis of germline stem cells (GS cells). The GS cells are downstream of the stem cells such as ES cells and embryonic germ cells (EG cells), which are derived from primordial germ cells (PGCs). Therefore, GS cells are expected to be useful in the production of genetically modified animals. In this study, we examined the enrichment and cultivation of mouse GS cells by magnetic cell sorting (MACS). Testicular cell suspensions were collected from C57BL/6J cryptorchid adult testes at 2 to 3 months after surgery and ICR pup (6 to 8 dpp) testes. They were digested by 0.1% collagenase followed by 0.25% trypsin with gentle shaking. Dissociated cell suspensions were filtrated through a glass-wool column followed by a Falcon cell strainer (40-m mesh). They were then treated with biotin-conjugated anti-mouse CD9 antibody, whose antigen, CD9, is localized on the GS cell surface, followed by the streptavidin-microbeads treatment. The cell suspension was passed through a MACS-separation column. In Experiment I, MACS-treated fractions were analyzed by flow cytometry (FCM) on the rates of recovery and enrichment and their cellular characteristics. In Experiment II, CD9-positive (CD9+) cells were cultured on gelatin-coated MultiDish (176740, Nunc) with 4-5 105 cells/well in StemPro34-SFM supplemented with 1% fetal bovine serum, leukemia inhibitory factor, GDNF, bFGF, EGF, insulin, transferrin, putrescine, MEM vitamin solution, MEM-NEAA and some other reagents at 32C or 37C under 5% CO2 in air. They were examined for their proliferation and cytological changes such as CD9, 6-integrin and Oct-1 expression by immunohistochemistry. In Experiment I, MACS selection effectively enriched CD9+ cells from mouse testes. However, FCM analysis revealed that the CD9-negative (CD9-) cells partially remained in MACS-selected fraction from cryptorchid adult testes. In contrast, the CD9+ subpopulation could be successfully separated from CD9- subpopulation from pup testes. Therefore CD9+ subpopulation from pup testes was used for the following cultivation. In Experiment II, the cells proliferated in the first few days in suspension. Then they attached to the dish and formed colonies after 5 days or 3 days of culture at 32C or 37C, respectively. Immunohistochemical analysis showed that the cells maintained the expression of CD9 for at least 14 days, but their expression of 6-integrin gradually diminished. It was demonstrated by immunohistochemistry and FCM analysis that the cells in colonies expressed Oct-1, and its expression level was stronger in culture at 37C than at 32C. These findings indicate that the CD9+ cells collected from mouse pup testes have stem cell properties. Abstract:
Regulatory studies of developmental and reproductive toxicity (DART) studies have remained largely unchanged for decades, with exposures occurring at various phases of the reproductive cycle and toxicity evaluations at different ages/times depending on the study purpose. The National Toxicology Program has conducted studies examining the power to detect adverse effects where there is a prenatal exposure, but evaluations occur postnatally. In these studies, examination is required of only 1 male and female pup from each litter beyond weaning. This provides poor resolving power to detect rare events (e.g., reproductive tract malformations). If an adverse effect is detected, there is little confidence in the shape of the dose–response curve (and the Benchmark Dose or No Observed Adverse Effect Level [NOAEL]). We have developed a new protocol to evaluate DART, the modified one generation study, with exposure commencing with pregnant animals and retention of 4 males and females from each litter beyond weaning to improve statistical power. These animals can be allocated to specific cohorts that examine subchronic toxicity, teratology, littering, and neurobehavioral toxicity in the same study. This approach also results in a reduction in animal numbers used, compared with individual stand-alone studies, and offers increased numbers of end points evaluated compared with recent Organization for Economic Cooperation and Development proposals.

Abstract:
With the rapid development of P2P technology, P2P IPTV applications have received more and more attention. And program resource distribution is very important to P2P IPTV applications. In order to collect IPTV program resources, a distributed multi-protocol crawler is proposed. And the crawler has collected more than 13 million pieces of information of IPTV programs from 2009 to 2012. In addition, the distribution of IPTV programs is independent and incompact, resulting in chaos of program names, which obstructs searching and organizing programs. Thus, we focus on characteristic analysis of program resources, including the distributions of length of program names, the entropy of the character types, and hierarchy depth of programs. These analyses reveal the disorderly naming conventions of P2P IPTV programs. The analysis results can help to purify and extract useful information from chaotic names for better retrieval and accelerate automatic sorting of program and establishment of IPTV repository. In order to represent popularity of programs and to predict user behavior and popularity of hot programs over a period, we also put forward an analytical model of hot programs.

Abstract:

Abstract:
Abstract Abstract 4353 BACKGROUND Hypersensitivity reactions to factor VIII (FVIII) concentrates are rare but well known complications of hemophilia treatment with FVIII concentrates. Occasional reports have suggested IgE mediation. Many cases have presented circumstantial evidence for the implication of FVIII but have failed to clearly identify FVIII-targeting antibodies (Abs). The absence of evidence for IgE contribution to those reactions, but the presence of FVIII specific IgM and/or IgG1 and/or IgG3, could support the hypothesis of complement mediation. These reactions have been called “complement activation-related pseudoallergy” (CARPA). As we did not study complement we call these reactions CARPA-like. We report 3 cases of CARPA-like reactions to FVIII which we believe could contribute to alert clinicians to this potential serious side effect of FVIII replacement therapy and could shed some light on its pathophysiology. PATIENTS / METHODS Subject A, 5 y. old Caucasian previously untreated patient (PUP) with mild hemophilia A (FVIII 0.12 IU/mL, 6955C>T exon 26 P2300S, H1 haplotype, no inhibitor with Nijmegen < 0.4 unit), developed a CARPA-like reaction with his sixth injection of a von Willebrand factor-containing FVIII concentrate (Wilate®, Octapharma) for traumatic knee hemarthrosis. He never needed further FVIII replacement. Subject B, 5 y. old African PUP with severe hemophilia A (FVIII < 0.01 IU/mL, intron 22 inversion, H1 haplotype, no inhibitor with Nijmegen < 0.4 unit) developed a CARPA-like reaction with his sixth injection of Wilate®. He was switched to a B-domain deleted recombinant FVIII concentrate (Xyntha™, Pfizer) and has yet to relapse with CARPA-like reaction after more than 100 exposure days. Subject C, 15 y. old Caucasian PUP with severe hemophilia A (FVIII < 0.01 IU/mL, intron 22 inversion, H1 haplotype, inhibitor with Nijmegen 10 units) developed a CARPA-like reaction with his second dose of a full length recombinant FVIII concentrate (Kogenate®,Bayer) while on ITI induction program. Blood samples were obtained from each patient shortly before and after their CARPA-like reaction. ELISA assays (AM. Vincent, Haemophila 2009) were used to test the presence of IgG, IgM, IgE Abs reacting with different types of FVIII (Baxter’s Advate, Wilate®, Kogenate®, Xyntha™) in patients’ plasma. The x-MAP technology was used to confirm the presence of IgG and IgM Abs, and to better define their isotypic profile. For that purpose, Abs specific for the heavy chain (HC; MAb 8860) or light chain (LC; MAb ESH8) of the FVIII were first immobilized on magnetic fluorescent beads, and then incubated with EDTA-dissociated FVIII. After incubation with patients’ plasma samples, binding of IgG subclasses (IgG1 to IgG4) and IgM were revealed by specific anti-human phycoerythrin-labeled Abs. RESULTS For patients A and B (with negative Bethesda titre), we identified an immune response characterized by the presence of IgM, as well as IgG (patient A IgG1; patient B IgG1, IgG3), simultaneously directed against the HC and the LC. ELISA assays on these 2 patients showed the presence of IgM and IgG, as well as the absence of IgE compatible with a CARPA-like reaction. Similar results with the 4 different FVIII concentrates are in favour of an immune response specific to FVIII rather than an immune response to anything else than FVIII. To further support this contention, ELISA assays were all negative for patient A’s mother and for his PUP hemophilic brother, as well as for subject B’s mother and for six normal control plasma tested on the same ELISA plate. Patient C (with positive Bethesda titre) presented anti-FVIII IgG (IgG1 to IgG4) and IgM Abs also targeting the HC and the LC simultaneously. These 3 patients developed a CARPA-like reaction associated with injection of FVIII concentrates. Subjects A and C were not rechallenged with FVIII after their reaction and progressively lost their anti-FVIII Abs. Subject B was rechallanged over 100 times with the B-domain deleted Xyntha™ without reaction and with progressive lost of his anti-FVIII Abs, suggesting that his reaction might have been mediated by Abs reacting to the B-domain of FVIII. CONCLUSION CARPA-like reactions to FVIII concentrates can be associated with anti-FVIII Abs. ELISA and x-MAP are useful technologies to investigate this complication of hemophilia treatment and could help select an optimal choice of FVIII concentrate for further replacement therapy. Abstract:
Family planning (KB) program is an effort to increase awareness and community participation through maturing marriage age (PUP), birth regulation, fostering family resilience, improving the welfare of small families, happy and prosperous. This study attempts to determine factors related to the knowledge and attitudes of family planning acceptors about uterine contraception in utero against the use of uterine contraception in the Mijen health center in Semarang. This research is a quantitative research with non experimental research design, and classified in observational survey research conducted to explain the relationship and influence between independent variables and dependent variables. The sample of this study were 60 respondents using simple random sampling. Data analysis techniques with univariate analysis, bivariate analysis by cross tabulation before statistical tests were done by Chi square test and multivariate with logistic regression analysis. The results of the study showed that the respondents of high school education (43.3%), with private employment (56.7%), distribution of IUD use (65%), the level of knowledge about the IUD was sufficient (80%) and attitudes in the use of the IUD agreed (71.67%). The bivariate results showed that there was a relationship between maternal knowledge of family planning acceptors about IUD with IUD use and there was a relationship between the attitude of mother of family planning acceptors and the use of IUD. Multivariate results show that there is an influence between knowledge and attitudes in IUD use.

Abstract:
We present EUV light curves for a number of AM Her systems observed either as guest observer targets or with the EUVE Right Angle Program. We have formed light curves for eight AM Her stars and show in our presentation the similarities and differences present. We draw some conclusions by grouping the systems by inclination, magnetic field strength, and accretion region geometry. In order to understand the physical structures responsible for the EUV emissions, we have developed a software model to generate synthetic light curves. We find that the EUV accretion regions in the systems UZ For, VV Pup, and AM Her cannot be fit with a flat spot confined to the white dwarf surface, regardless of its shape or brightness profile. Rather, a small, symmetric, raised spot is the only shape consistent with the data. The light curves for systems EF Eri, RE1149+28, AN UMa, and V834 Cen show evidence for additional structure that precedes the primary accretion region in phase. Our model indicates that a large portion of the light curve in each system is seen in absorption. Finally, in three systems, we detect a very gradual rise and fall in the EUV flux (<5% of the peak flux) at phases when the spot is completely obscured behind the white dwarf (WD) limb. We attribute this detection to emission from the accretion column that decreases exponentially with distance from the WD and detect emission at heights up to 15% of the WD radius.

Abstract:
Methionine availability during overall amino acid limitation metabolically reprograms cells to support proliferation, the underlying basis for which remains unclear. Here we construct the organization of this methionine-mediated anabolic program using yeast. Combining comparative transcriptome analysis and biochemical and metabolic flux-based approaches, we discover that methionine rewires overall metabolic outputs by increasing the activity of a key regulatory node. This comprises the pentose phosphate pathway (PPP) coupled with reductive biosynthesis, the glutamate dehydrogenase (GDH)-dependent synthesis of glutamate/glutamine, and pyridoxal-5-phosphate (PLP)-dependent transamination capacity. This PPP-GDH-PLP node provides the required cofactors and/or substrates for subsequent rate-limiting reactions in the synthesis of amino acids and therefore nucleotides. These rate-limiting steps in amino acid biosynthesis are also induced in a methionine-dependent manner. This thereby results in a biochemical cascade establishing a hierarchically organized anabolic program. For this methionine-mediated anabolic program to be sustained, cells co-opt a “starvation stress response” regulator, Gcn4p. Collectively, our data suggest a hierarchical metabolic framework explaining how methionine mediates an anabolic switch.

Abstract:
Abstract Human exposure to boron occurs primarily through diet and drinking water sources. Animal studies have found that reduced fetal weight following gestational exposure to boron (as boric acid) is the most sensitive toxicological effect. However, recent studies suggest that newborns in areas with elevated boron in drinking water may receive levels of exposure that exceed the U.S. EPA oral reference dose for B. Currently, there are no data to inform a boron risk assessment accounting for this developmental window. To address this knowledge gap, the National Toxicology Program evaluated developmental toxicity following pre- and postnatal boron exposure. Time-mated female Sprague Dawley (Hsd: Sprague Dawley SD) rats were administered 0–20 mg B/kg/day (as boric acid) via gavage from gestation day 6 to 21; offspring were dosed via gavage at the same respective dose level from postnatal day (PND) 1 to 28. There were no dose-related effects on dam bodyweight, bodyweight gain, or feed consumption. Clinical findings were limited to low incidences of umbilical hernia in the 20 mg B/kg pups which resolved by study completion. Pup plasma boron concentrations increased in dose-proportional manner and were similar between PND 4 and PND 28. Postnatal weight gain was significantly reduced at 20 mg B/kg, with male and female pups weighing 23% less than the controls on PND 28. These findings demonstrate that postnatal growth in the Sprague Dawley rat is sensitive to boron exposure and highlights the importance of evaluating the potential toxicity of agents with known human exposures during early life stages.

Abstract:
As software is getting more valuable, unauthorized users or malicious programmers illegally copies and distributes copyrighted software over online service provider (OSP) and P2P networks. To detect, block, and remove pirated software (illegal programs) on OSP and P2P networks, this paper proposes a new filtering approach using software birthmark, which is unique characteristics of program and can be used to identify each program. Software birthmark typically includes constant values, library information, sequence of function calls, and call graphs, etc. We target Microsoft Windows applications and utilize the numbers and names of DLLs and APIs stored in a Windows executable file. Using that information and each cryptographic hash value of the API sequence of programs, we construct software birthmark database. Whenever a program is uploaded or downloaded on OSP and P2P networks, we can identify the program by comparing software birthmark of the program with birthmarks in the database. It is possible to grasp to some extent whether software is an illegally copied one. The experiments show that the proposed software birthmark can effectively identify Windows applications. That is, our proposed technique can be employed to efficiently detect and block pirated programs on OSP and P2P networks.

Full text
Abstract:
In 2019, a wild-born dingo pup named Wandi was taken from the Victorian high country to the Australian Dingo Foundation to become a part of their breeding program. Wandi was chosen because he was identified as a ‘rare’ ‘alpine’ dingo. At the point at which Wandi was handed over to the ADF, he became a captive dingo and will likely never be released. Wandi is one of thousands of dingoes who are bred and sold each year by the dingo breeding industry in Australia – both for zoos and wildlife parks to exhibit, and as privately owned ‘pets’. None of these dingoes can ever be released. Dingo captivity is often justified by dingo breeders as a necessary part of ‘essential’ conservation to combat the possible ‘extinction’ of the dingo. In this article, I question this assumption and demonstrate how it perpetuates and energises historically constructed distinctions between dingo ‘types’ (such as ‘alpine’ and ‘pure’). Here, I mobilise Thom van Dooren’s concept of ‘violent-care’ to better understand the contradictory ways in which dingoes experience life and captivity in Australia: ‘rare’ but a ‘pest’, charismatic and newsworthy but also imprisoned, evincing popular sentiments of affection and forced into captive breeding. I work with these contradictions every day as the founder of Sydney Fox and Dingo Rescue (SFDR). As dingo advocates, we have a responsibility to examine the violence dingoes experience as a result of captivity and the ‘logics’ and discourse that drive that violence, as van Dooren writes: ‘[w]hen the ‘logics’ that structure violence (or care for that matter) go unexamined, they become both invisible and commonsensical’ (van Dooren, ‘A Day with Crows’ 3).

Abstract:
In light of data indicating military personnel are more likely to reach out to peers during times of need, peer-to-peer (P2P) support programs have been implemented for military suicide prevention. Often designed to reduce suicidal thoughts and behaviors by reducing mental health symptom severity, existing data suggest that P2P programs have little to no effect on mental health symptoms. Conceptualizing suicide prevention from an occupational safety and injury prevention perspective to promote positive health-related behavior change at both the group and individual level may enhance the effectiveness of P2P programs and military suicide prevention efforts more broadly. To illustrate these concepts, the present article provides an overview of the Airman’s Edge project, a P2P program design based upon the occupational safety and injury prevention model of suicide prevention, and describes a program evaluation effort designed to test the effectiveness of this approach.

Abstract:
A program package has been assembled for the analysis of protein coordinates which are in the Brookhaven Protein Data Bank (PDB) format. These programs can be used to make two types of φ–ψ plots: a Ramachandran-style scatter plot, and a plot of φ and ψ values as a function of the linear sequence. Programs are also available for the display of distance diagonal plots for proteins. Two protein structures can be compared and the resulting r.m.s. differences in the structures plotted as a function of sequence. Temperature factors can be analyzed and plotted as a function of the linear sequence. In addition, various utilities are supplied for splitting PDB files which contain multiple subunits into individual files and also for renumbering PDB files. A utility is also provided for converting Amber-style PDB files into standard PDB files. Priestle's program RIBBON [J. Appl. Cryst. (1988), 21, 572–576] has been converted to run in a stand-alone mode with interactive rotation of the three-dimensional ribbon picture. Programs are Silicon Graphics four-dimensional level and have been tested on 4D70/GT and personal Iris workstations, although programs which give Postscript output have been converted to run on Digital Equipment Corporation VAX computers and Sun workstations.

Abstract:
Purpose – Numerous studies to date have demonstrated the public-private partnership (PPP) project procurement method's failure to deliver low-cost housing (LCH) to low-income groups (LIGs) in developing countries. The purpose of this paper is to investigate critical failure factors (CFFs), and how they cause the failure of PPP-LCH program. Design/methodology/approach – Grounded Theory methodology was used to gather and analyze the data in order to identify, categorize, and develop the logically causal relationships among CFFs that cause PPP-LCH program failure. Findings – Ten CFFs in various phases of PPP-LCH project life cycle caused PPP-LCH program failure. Some CFFs resulted from ineffective PPP policy and strategy, while some were beyond the control of the project/program management team. These CFFs were inter-/intra-related to one another in a particular way. Originality/value – Despite the increase in PPP-LCH projects/programs for LIGs in practice and the prevalence of failure, the studies of PPP-LCH project/program failure still suffer from insufficient conceptual clarity about the causes of these failures. The lessons learned, to some extent, help decision makers in both public and private sectors to reduce the probability of the PPP-LCH project/program failure by clearly explaining the nature of each CFF.

Smart Indonesia Program (PIP) is an enhanced government support program for Poor Students (BSM). This PIP program is expected to help economically disadvantaged students and can ease the burden of disadvantaged families, as well as to recall dropped students due to delinquent fees. It is hoped that there will be no reason for not continuing education to a higher level because of cost issues. For Use of funds to be used as effectively and efficiently as possible. Thus the PIP program can support a 12-year compulsory government program. Both of these programs can go hand in hand with the national goal of the Indonesian nation that is Introduce the Life of the Nation.

Disturbances in physical self-perceptions (PSP) can lead to inappropriate attitudes and eating behaviours (IAEB) including eating disorders (ED) and obesity. This exploratory study assesses the effects of a blind integrated prevention program—the SILENCE Program—with high school students. No significant effect of the SILENCE Program on the IAEB of the 61 adolescents was observed between pre- and post-test, but a positive evolution of specific PSP components (global self-esteem, perceived physical appearance, etc.) during 14 consecutive weeks was demonstrated compared to an ED-only program and the control condition. Promising avenues for integrated prevention programs (like SILENCE Program) are discussed.

Abstract:
Context.— Misinterpretation of high-grade squamous intraepithelial lesion (HSIL) is an important problem in daily practice and in the College of American Pathologists (CAP) PAP Proficiency Test (PAP-PT). Objective.— To investigate factors related to misinterpretation of HSIL through responses in a proficiency test versus an educational environment. Design.— We retrospectively evaluated 28 000 responses in the PAP Education Program (PAP-Edu) and 59 140 responses in PAP-PT from 1147 field-validated HSIL slides from 2007 to 2014. The related factors, such as program types, preparation types, participant types, and program years, were evaluated. Results.— Overall, 4.0% (2379 of 59 140) of responses for HSIL slides from PAP-PT were misinterpreted as either low-grade squamous intraepithelial lesion (LSIL) or negative, significantly more than those from PAP-Edu (3.2%; 898 of 28 000). However, the false-negative rate (misinterpreted as negative) was 0.9% (519 of 59 140) for PAP-PT, lower than that for PAP-Edu (1.0%; 266 of 28 000). The misinterpretation rates in PAP-PT trended down with time. Misinterpretation rates did not vary significantly by preparation methods. The misinterpretation rate for HSIL in the pathologists' responses was lower than that in cytotechnologists' responses in PAP-PT. More HSIL was misinterpreted as LSIL than as benign in both programs. Cytotechnologists interpreted HSIL as LSIL twice as much as pathologists. The most common false-negative misinterpretations were negative for intraepithelial lesion or malignancy and reparative change. Conclusions.— The higher LSIL misinterpretation rate by cytotechnologists may be related to the differences in reporting responsibilities and proficiency test grading criteria. The trend of gradually decreasing misinterpretation rates of a reference diagnosis of HSIL in the PAP-PT program may be related to higher test-taking confidence and better preparation through educational programs. The fact that pathologists performed better than cytotechnologists in PAP-PT, but not in PAP-Edu, may reflect a heightened approach and attentiveness in the test-taking environment for pathologists.

Abstract:
Pay-for-performance (P4P) programs which reward clinical providers with incentive payments based on one or more measures of quality of care are now common in the United States and the United Kingdom and it is likely they will attract increasing interest in Australia. However, empirical evidence demonstrating effectiveness of such programs is limited and many existing programs have not had rigorous outcome evaluation. To maximise success, future P4P programs should incorporate the lessons and insights obtained from previous experience. Based on a review of published trials, program evaluations and position statements, the following principles that may guide future program design and implementation were synthesised: 1) formulate a rationale and a business case for P4P; 2) use established evidencebased performance measures; 3) use rigorous and verifiable methods of data collection and analysis; 4) define performance targets using absolute and relative thresholds; 5) use rewards that are sufficient, equitable and transparent; 6) address appropriateness of provider responses and avoid perverse incentives; 7) implement communication and feedback strategies; 8) use existing organisational structures to implement P4P programs; 9) attribute credit for performance to participants in ways that foster population-based perspectives; and 10) invest in outcomes and health service research. Recommendations flowing from these principles relevant to Australian settings are provided.

Pay for performance (P4P) has become a popular approach to performance improvement in health care. Most of the P4P literature has focused on the United States and there is limited insight in the characteristics of major programs initiated in other countries. This article systematically describes and reviews P4P programs outside the United States. Our literature search identified 13 programs initiated in 9 countries. Although the programs share many similarities, they differ in several important respects, also when compared with the typical P4P program in the United States. In addition, there are clearly possibilities to increase incentive strength and minimize incentives for undesired behavior. In part, observed heterogeneity will be a consequence of contextual differences, but design choices often also seem to be made arbitrarily. In designing their programs, purchasers are hampered by limited knowledge of the influence of specific design choices and effective strategies to mitigate undesired behavior.

Abstract:
In this paper, program logic will be used to ‘map out’ the planning, development and evaluation of the general practice Pap nurse program in the Australian general practice arena. The incorporation of program logic into the evaluative process supports a greater appreciation of the theoretical assumptions and external influences that underpin general practice Pap nurse activity. The creation of a program logic model is a conscious strategy that results an explicit understanding of the challenges ahead, the resources available and time frames for outcomes. Program logic also enables a recognition that all players in the general practice arena need to be acknowledged by policy makers, bureaucrats and program designers when addressing through policy, issues relating to equity and accessibility of health initiatives. Logic modelling allows decision makers to consider the complexities of causal associations when developing health care proposals and programs. It enables the Pap nurse in general practice program to be represented diagrammatically by linking outcomes (short, medium and long term) with both the program activities and program assumptions. The research methodology used in the evaluation of the Pap nurse in general practice program includes a descriptive study design and the incorporation of program logic, with a retrospective analysis of Australian data from 2001 to 2009. For the purposes of gaining both empirical and contextual data for this paper, a data set analysis and literature review was performed. The application of program logic as an evaluative tool for analysis of the Pap PN incentive program facilitates a greater understanding of complex general practice activity triggers, and also allows this greater understanding to be incorporated into policy to facilitate Pap PN activity, increase general practice cervical smear and ultimately decrease burden of disease.

Backdoor or commonly also known as web shell is one of the malicious software that hackers use to maintain access systems that they have entered. Relatively few programs like Anti Web-Shell, PHP Backdoor Scanner circulating on the Internet, and can be obtained free of charge to deal with the issues above. But most of these programs have no actual database of signature behavior to deal with PHP backdoor / Shell nowadays. Then comes the contemporary Anti Web-Shell program that can deal with today's backdoor shell. This study uses an experimental method concerning previous similar studies and is implemented directly into the world of cyber security professional industries. By enriching the Regex dictionary signature and String Array Matching the actualized Anti Web-Shell program can detect more backdoor than similar programs that have existed in the past. The results of this study are in the form of a web application software in PHP extension. The application can minimize 100% of false positives and is twice as fast in scanning files because it is more specific in heuristic analysis scan.

Abstract:
Abstract Background The prevalence of Pneumocystis jirovecii Pneumonia (PJP) is increasing among vulnerable Persons Living with HIV (PLWH) in rural communities. In such communities, optimum clinical management of PJP in PLWH, is often limited by sub-optimal coordination of healthcare programs and inadequate human resources. The study evaluated the impact of interventions implemented by public health programs, to improve the clinical management of PJP among PLWH in rural Nigerian communities. Methods The longitudinal study was done at 30 randomly selected rural communities in Nigeria. Program interventions included: decentralization of comprehensive Anti-Retroviral Therapy (ART) to primary healthcare facilities; and strategic private–public partnerships to ensure provision of free medications for ART and prophylaxis/treatment of PJP, (according to National Treatment Guidelines). Additional interventions included: training of indigenous healthcare workers on clinical management of PJP; task-shifting; routine monitoring and evaluation of PJP indicators to ensure program quality, among others. Real-time data from Patients’ Care Cards were utilized for pre- and post-intervention assessments in January 2014 and January 2019, respectively. Chi-square was applied as inferential statistics; P < 0.05 indicated statistical significance. Results The number of PLWH on ART, who received primary PJP prophylaxis with Trimethoprim-Sulfamethoxazole (TMP-SMX), increased from 2,280 (pre-intervention) to 14,461 (post-intervention). PLWH (55 females; 40 males), exhibited TMP-SMX intolerance and were offered Dapsone for primary PJP prophylaxis. PJP diagnosis and treatment for female PLWH increased from 5% (pre-intervention), to 82% (post-intervention). PJP diagnosis and treatment for male PLWH increased from 3% (pre-intervention), to 79% (post-intervention). Incidences of PJP increased with age, viral load, ART interruption, and late presentation to HIV care. Secondary PJP prophylaxis for PLWH (after the completion of pneumonia treatment) increased from 2% (pre-intervention) to 90% (post-intervention). Conclusion The interventions significantly improved the clinical management of PJP in PLWH. They are recommended for implementation by health programs in rural communities. Abstract:

Abstract:
Parent training programs (PTPs) have been used extensively in Anglo-Saxon countries, but less so in Southern Europe. Several characteristics of families have been linked to effective parenting and positive development of children, but few studies have examined the social determinants of the effectiveness of PTPs. The Parenting Skills Program for families (PSP) is a PTP from Spain. This study aimed to identify the social characteristics (sex, age, country of birth, marital status, educational level, and employment status) of parents that determine the success of the PSP in relation to social support, parenting skills, parental stress, and negative behaviors among children. A quasi-experimental study with a prepost design with no control group was used. We conducted a survey before (T0) and after the intervention (T1). Sample size was 216. We fit multiple logistic regression models. Parenting skills increased more among parents with a lower educational level. Parents’ stress decreased more among parents who had a lower educational level, were unemployed, and were men. Social support increased among parents who were younger, unemployed, or non-cohabiting. We found no significant differences in the effect on children’s negative behaviors according to the social factors evaluated. The PSP is effective for socioeconomically diverse families, but the success differs according to the parents’ social profile. Unlike most previous studies, the results were better among more socially disadvantaged people, highlighting the potential of this kind of intervention for reducing the social inequality gap between groups.

Abstract:
LAY SUMMARY Young people in public safety personnel (PSP)-connected families are affected by the distinctive stress related to having a parent with mental health issues, including operational stress injuries (OSIs) and the existing strain connected to the COVID-19 pandemic. This increased vulnerability makes it important to be innovative in opportunities for prevention in the PSP community. After reviewing available resources aimed at helping young people affected by a parental OSI, the authors uncovered a gap in prevention and intervention programs accessible to all Canadian PSP families. A new virtual six-week program offered to Canadian young people in military and PSP families affected by an OSI creates potential for developing resilience. The online platform opens access to young people who may be isolated from joining a traditional face-to-face mental health support program with peers. The virtual program aims to empower young people in their knowledge of mental health and enable them to learn about the complex nature of parental OSIs, such as posttraumatic stress disorder. More research on this program is needed to ensure interventions created for this target population are effective in the Canadian context and meet the unique needs of PSP-connected young people.

Abstract:

Abstract:
In late 2003, the regional campus of the University of South Australia initiated a peer-mentoring program aimed at assisting the smooth transition of new students to university life. In particular, the Nursing and Rural Health unit envisaged a program that would be effective and rewarding for both student mentees and mentors. This paper presents an analysis of the peer-mentoring program initiated. It begins by discussing the concept of mentoring and the advantages and disadvantages of peermentoring programs in educational institutions. It then introduces the program, describes how it was conceptualised, implemented and strengthened and how the program developed into a unique ‘pop-up’ model of mentoring that fitted the needs of mentees and mentors. The paper evaluates the experiences of mentees and mentors and concludes with some suggestions for improving the program, which others may learn from. Key words: nursing education, first year academic experience, nursing students’ transition to university, peer-mentoring, mentee-mentor relationship.

Abstract:
In recent years there has been substantial development of computer programs which permit the computation of elemental concentration in a variety of samples from basic principles of X-ray absorption and emission. These prog rates are generally called "fundamental parameter" programs. For many years these programs required large main frame computers for execution. An example is the well known NRLXRF program. For the past few years, programs have been available for minicomputers such as the PDP/11 OR LSI/11 systems. The XRF-11 program from Criss Software, the SAP3 program from Batelle Pacific Northwest Laboratory. and a variety of programs from the X-ray instrumentation vendors are examples. Some fundamental parameter programs have been available for several years that can be executed on microcomputers or the so-called personal computers. Some programs use an "effective wavelength" approximation of the tube output to simplify computations.
