Full textBackground: Nearly sixty percent of stabilized bipolar patients suffer from important cognitive impairments that lead to significant functional disabilities. Without proper treatment, these impairments remain throughout lifespan. However, cognitive deficits in bipolar disorders have been overlooked and only a few studies investigated treatments to improve cognitive functioning for bipolar patients. Method: We developed ECo, an individual ecological cognitive remediation intervention that was designed for mood disorders. We conducted experimental studies to investigate psychological correlates of cognitive impairments, and the cognitive, functional and psychological improvements of bipolar patients after either cognitive remediation or individual psychotherapy.Results: Our results suggest that metacognitive impairments lead to an increased frequency of everyday life activities that can create a cognitive and emotional overload. We observed that cognitive remediation, and in particular the ECo program, can improve problem solving skills in our population. The ECo program can improve impaired cognitive functions and metacognitive regulation, as well as coping skills, emotional control, openness to relationships and self-esteem.Conclusion: An ecological, individualized cognitive remediation program, targeting metacognition and self-efficacy, can contribute to an improvement of functional health components in bipolar disorders

Full textQuality methods of many different types have been used in hospitals. Some specific projects have shown improved results, but there is no strong evidence of the effectiveness of institutionwide quality improvement (QI) programs. This research deals with a specific category of QI programs: those recognised by experts as world class. Three such programs are examined in a case study, investigating their content, implementation, perception and results. The study shows that QI programs can lead to improved clinical results. It documents improvements measured on process indicators for each of the cases and one or a few improved clinical outcomes in two of the cases. The concept of an investment threshold is proposed and explained, a zone of noise delaying evidence of improved results to show, as well as a set of factors increasing or decreasing the effectiveness of QI programs

Full textClinical practice guidelines help practitioners to offer evidence-based practices to patients with cancer. The number of oncology guidelines produced keeps growing but guideline quality varies widely calling into question their validity. An appraisal instrument for clinical practice guidelines has been developed and tested internationally. The quality of cancer guidelines has been compared with guidelines of other disease domains. Predictors for high-quality guidelines in oncology have been identified. The quality criteria of the appraisal instrument are consistent with the quality criteria that must be met to prevent problems of liability of guideline authors. Guideline users consider guideline quality and validity important, but they prioritize the guideline’s source, its user-friendliness and applicability. Adopting common international rules for practice guidelines facilitates cooperation among guideline developers to address duplication of effort and variation in guideline quality

Full textIn France, therapeutic patient education programs (ETP) to adolescents with type 1 diabetes are designed to make the teenager to self-manage their disease and its treatment. Based on social cognitive theory of Bandura, the effects of educational activities should lead to the strengthening of self-efficacy, coupled with other socio-cognitive factors, favoring the therapeutic adherence of teenager, a better quality of life and improved glycemic control.The thesis work was broken down into three main goals:1) Perform concept clarification on the specificitiess of TpE in a pediatric context, from a literature analysis method according to Rogers (2000).2) To conduct a systematic review aimed to describe the content and effects of educational programs from 2009 to 2014 on glycemic control, disease management, psychosocial criteria of children and adolescents with type 1 diabetes, and assess the consistency of these programs with the recommendations of the ISPAD.3) Describe the evolution of the socio-cognitive and clinical profile on three months of a diabetic adolescent cohort participated in an ETP in France.This work helped to develop knowledge on the concept of TPE in the pediatric context, the state of research on structured educational programs in type 1 diabetes in children and adolescents, but also implement a pilot study in the context of the ETP in France
