Abstract:
Prostate cancer is among the most common malignancies in males in the United States and adenocarcinoma accounts for 95% of all malignancies of prostate. Rarely prostate cancer can also present as small cell carcinoma. Pure small cell carcinoma is rare at time of initial diagnosis (<2%) however neuroendocrine differentiation into small cell carcinoma may emerge in men who have had previous treatment with ADT for prostate adenocarcinoma. These tumors, sometimes called treatment-related neuroendocrine prostate cancers or aggressive-variant prostate cancers, are increasingly recognized in the cast

Urological Chronic Pelvic Pain Syndrome (UCPPS) was long thought to be the direct result of prostate infection. This unidimensional perspective is not supported by current research suggesting a biopsychosocial perspective. Contemporary views take a multifactorial approach to defining UCPPS, and recognize a heterogeneous presentation of symptoms in men with UCPPS. The present thesis examines and describes this heterogeneity and attempts to differentiate UCPPS subtypes. Obesity and metabolic syndrome are associated with increased risk of advanced prostate cancer, more aggressive high grade disease and increased risk of death from prostate cancer. Androgen targeted therapies can rapidly induce symptoms similar to metabolic syndrome in patients, affecting health and quality of life. The development of therapeutics which could target metabolic dysregulation in addition to cancer growth is urgently needed. This thesis established a new mouse model of metabolic syndrome and assessed components of the ghrelin axis as treatments of prostate cancer. Importantly, an a

Advanced prostate cancer is a common and generally incurable disease. Androgen deprivation therapy is used to treat advanced prostate cancer with good benefits to quality of life and regression of disease. Prostate cancer invariably progresses however despite ongoing treatment, to a castrate resistant state. Androgen deprivation is associated with a form of metabolic syndrome, which includes insulin resistance and hyperinsulinaemia. The mitogenic and anti-apoptotic properties of insulin acting through the insulin and hybrid insulin/IGF-1 receptors seem to have positive effects on prostate tumo

L’obésité, le diabète ainsi que le syndrome métabolique ont été associés à la survenue de plusieurs cancers. Néanmoins, le rôle de ces facteurs dans la survenue du cancer de la prostate est encore largement discuté. Prostate cancer (CaP) is the most commonly diagnosed cancer in males in Australia, North America, and Europe. If found early and locally confined, CaP can be treated with radical prostatectomy or radiation therapy; however, 25-40% patients will relapse and go on to advanced disease. The most common therapy in these cases is androgen deprivation therapy (ADT), which suppresses androgen production from the testis. Lack of the testicular androgen supply causes cells of the prostate to undergo apoptosis. However, in some cases the regression initially seen with ADT eventually gives way to a growth

AIM OF THE THESIS Since Metabolic Syndrome (MetS) is essentially based on increased adiposity and it is associated with male hypogonadism, erectile dysfunction, psychological disturbances, and BPH/LUTS, and all these factors might, in different ways, affect reproductive capacity, we investigated their possible correlations with MetS. Hence, we performed two studies. In the first study (study 1) we evaluated possible associations between MetS, semen and hormonal parameters, as well as clinical characteristics, including sexual, ultrasound and psychological characteristics, in a cohort of men
