Full textMuestra las principales características que se asocian a la luxación de prótesis total de cadera en el grupo de estudio. Determina la incidencia de luxación de prótesis en el periodo enero 2010 y junio 2013. Realiza un estudio descriptivo, retrospectivo, mediante revisión de las historias clínicas de 55 pacientes hospitalizados con el diagnóstico de luxación de prótesis total de cadera. Para ello evalúa la información concerniente a la edad, sexo, grado de instrucción, patología previa, diagnóstico inicial, tiempo de presentación abordaje quirúrgico, posición de la copa, tamaño de la cabeza femoral y tratamiento, experiencia del cirujano. Encuentra que la incidencia de luxación fue del 4.2%. El grupo etario más afectado estuvo entre los 66 a 85 años con 34 casos El factor desencadenante más reportado fue el de movimientos forzados con 20 casos seguido del traumático, espontaneo y otros con 18, 14 y 3 casos respectivamente. La luxación precoz estuvo presente en 30 casos y la tardía en 25 casos. En cuanto al diagnóstico pre quirúrgico, la coxoartrosis tuvo 17 casos, seguido de fractura de cadera con 15, complicación de implante o dispositivo protésico 11 casos y otros 12 casos. El abordaje lateral y la luxación anterior reportaron 37 y 22 de los casos respectivamente.46 casos necesitaron tratamiento quirúrgico y 28 de ellos cirugía de revisión. La luxación fue más frecuente en cirujanos con menos de 30 cirugías al año. Concluye que la luxación fue una de las complicaciones más frecuentes luego de la cirugía de remplazo articular de cadera. La coxoartrosis previa, la presentación precoz, el abordaje lateral, presentación en la tercera edad, el tratamiento quirúrgico y la mayor tasa en cirujanos con menor procedimiento por año, fueron las principales características de la luxación de prótesis total de cadera en el grupo de estudio.

Con este proyecto se busca desarrollar una alternativa que satisfaga aspectos tanto estéticos, emocionales y funcionales, que cambie la imagen negativa que se posee de este tipo de tratamientos ortopédicos, los cuales muchas veces se relacionan conceptos en torno a la discapacidad. La importancia que le otorgan las familias a la imagen que proyecta el bebé se traza socialmente como un indicador de la preocupación que ponen los padres en el cuidado de sus hijos. Es por eso que se contemplan aspectos estéticos propios de la indumentaria y vestimenta infantil, en el desarrollo de una órtesis articulada para el tratamiento de la displasia de cadera.

Full textThe present study examines the level of functionality and quality of life that patients undergoing total hip endoprosthesis surgery reach, for osteoarthritis with severe functional limitations pertaining to the Explicit Health Guarantees Program, operated in the Orthopedic Institute in 2009. The total number of patients who have been operated and treated by the Explicit Health Guarantees plan during 2009 in this institute was 121, of whom 54 cases have took part in this study. For assessment of functionality, we used the Harris Hip Score and Merle D'Aubigné & Postel score. Regarding the assessment of quality of life, we used the Health Questionnaire SF-36. The design is non-experimental, descriptive and transversal. We assessed the incidence and values that are manifested in relation to the test applied to different variables: type of surgical approach (lateral or posterior), age groups, postoperative period. We turned to the computer program SPSS version 17 for descriptive analysis of the data. According to the results obtained according to functionality through the functional test Hip Harris Score, it has been found that 70%, 72% and 68% of patients, regardless of the type of approach, age group and postoperative period respectively, were located within Excellent functional categories, and Good. In regard with the Merle D’Aubigne & Postel score, more than 85% of the general population was grouped in categories Good and Moderate. After analyzing the results obtained in each of the dimensions of the SF-36, we can conclude that the studied population has a level of quality of life similar to U.S. referential population, since the average in every dimension, independent of the variable used in this study, was concentrated within the range of 40 to 60 points. Further research could also be conducted to analyze what is the degree of evolution of functionality and quality of life for a preoperative, in respect of postoperative of diverse extensions. It would also be interesting to look at different health centers and cover a larger number of patients.

Full textChronic Periprosthetic Joint Infection (C-PJI) is a devastating complication expected to worsen due to the increased joint replacement procedures. Develop reliable diagnostic tools and effective methods of treatment is critical and for this we must understand these infections as a classic model of bacterial biofilm. The two-stage exchange approach using an antibiotic-loaded cement spacer has become the preferred treatment for any C-PJI, which implies that preoperative identification of the pathogen and its antibiotic sensitivity profile is paramount because it enables selection of the most appropriate antibiotic treatment. The most commonly used sampling technique has been the joint fluid culture obtained by joint aspiration. But such test has proven to have a broad range of sensitivity, which is set at around 70%. Another drawback is the high frequency of dry taps in such low-grade infections. From a therapeutic point of view, aminoglycoside have been considered the antibiotics of choice to load these spacers, however, aminoglycoside resistance in gram positive cocci (GPC) causing C-PJI seems to be increasing, and may potentially impact the utility of “only-aminoglycoside” spacers. Gentamicin-vancomycin combination has been proposed as the ideal to be added to these local antibiotic carriers. Given the foregoing scenario, each of the items included in this thesis focus on the study of one of the following key points in the management of C-PJI: The evaluation of a new technique for microbiological diagnosis, the percutaneous interface biopsy, in preoperative dry aspiration cases of C-PJI (Int Orthop 2012 Jun; 36 (6): 1281-6); to evaluate the rate of aminoglycoside-resistance among GPC in a cohort of C-PJI and to evaluate whether an increase in amynoglycoside-antibiotic resistance exits among relapsed cases previously treated with a two-stage exchange protocol using gentamicin-loaded spacers (J.Arthroplasty 2014 Aug; 29 (8): 1617-1621) and to compare spacers containing either gentamicin or gentamicin-vancomycin with respect to clinical outcomes (Clin Orthop Rel res 2014 Mar; 472 (3.): 923-32) After assuming the limitations of the studies, our data support the effectiveness of the biopsy technique in cases of dry taps, with a strong sensitivity (88%), specificity (100%) and accuracy (92%) to isolate the infecting organism. In our series of C-PJI a high rate of resistance to the aminoglycoside antibiotics most commonly used for impregnation of cements spacers has been demonstrated. Overall, 32% of the isolates were resistant to gentamicin and 41% to tobramycin. Focusing on coagulase-negative Staphylococcus (CoNS) strains, gentamicin resistance was 39.25%. Almost 58% of the CoNS isolated after an unsuccessful staged exchange procedure were resistant to gentamicin, theorizing that gentamicin-spacers may play a causal role in the develop of gentamicin resistance. Moreover we have found no resistance to vancomycin after using antibiotic cement spacers. This, along with other reasons, might suggest that the use of cement spacers with a combination of vancomycin and gentamicin would be the "ideal" combination; but we could not confirm such hypothesis, finding the same infection eradication rates using either gentamicin-spacers or vancomycin-gentamicin spacers; indicating that aminoglycosides can be still effective due to the high local antibiotic concentration that is achieved with the cement spacers.

Full textAim: To estimate the incidence of surgical wound infection (SWI) in patients undergoing controlled clean surgery and to identify the associated risk factors to the surgical site infection as well as to know the percentage of infections that are diagnosed after hospital discharge. Design: Incidence study of surgical site infection with surveillance and follow-up. Area of the study: Area of Orthopedic Surgery and Rehabilitation of the Vall d'Hebron University Hospital in Barcelona. Subjects of the study: All patients who underwent an elective surgery procedure of total hip prosthesis (THP) and knee (TKP) from January 1, 1998, to December 31, 2012, with follow-up of a year after the intervention by means of the review of clinical history. Instrumentation: A medical and a nurse, experts in infection control and epidemiology realize the vigilance of the patients controlled from the day after the intervention and every three days during the stay at the hospital, and during one year after the discharge. In addition there has been realized an examination of the clinical history later to the discharge to determine if there are signs or symptoms of infection of wound or if this one has been reflected by the surgeon in the leaf of clinical course. An epidemiological survey of nosocomial infection has been completed according to the system PREVINE (Specific Program for the surveillance of Nosocomial Infection in Spain) and the information is registered in a database created to the effect in ACCESS program. Results: There were studied 7991 surgical procedures of THP and of TKP. 31% are men. Mean age 70 years. The global rate of SWI has been of 1,4 %. 1,7 % (IC95 %:1,3-2,1) in THP and 1,1 % (IC95 %:0,9-1,4) in TKP. 42 % was superficial infections and 58 % deep and of organ-space. 51,5 % was diagnosed during hospitalization and the rest after the discharge. In deep wound infections a microbiological culture was realized in 98,7% of the cases and confirmation was obtained in 77%. The surgical antibiotic prophylaxis was appropriate in 92% of the cases. Conclusions: The rate of infection of our study is inside the range of the rates of infection described in other European studies. The follow-up method after discharge realized has been optimal for the high percentage of infections diagnosed.

Resumo: Este estudo avaliou a ocorrência de leveduras do gênero Candida sp além dos principais microrganismos periodontopatogênicos e enterobactérias na saliva, em mucosa e no biofilme aderido à prótese total, correlacionando com aspectos clínicos e condição de higiene bucal de 90 indivíduos edêntulos e portadores de prótese total, por meio de métodos moleculares (PCR). Espécimes clínicos intrabucais foram coletados desses indivíduos após avaliação das condições sócio-econômicas e comportamentais. A microbiota bucal dos pacientes foi caracterizada por meio da obtenção de amostras de biofilme aderido às próteses totais, mucosa e saliva, as quais foram processadas, por meio de PCR. As inter-relações entre os diferentes microrganismos foram determinadas por meio dos testes de Qui-quadrado e Mann- Whitney. Verificaram-se diferenças na ocorrência de Prevotella intermedia e Enterobacteriaceae na saliva dos pacientes edêntulos, o mesmo ocorrendo com Enterobacteriaceae, Camphylobacter rectus e gênero Pseudomonas no biofilme aderido às próteses totais. As condições de higiene bucal e estado de conservação da prótese total precários favoreceram a ocorrência de leveduras do tipo Candida sp, em especial Candida albicans, em níveis estatisticamente significante nas amostras de mucosa e biofilme aderido à prótese total, tornando este dispositivo protético um potencial reservatório de leveduras e bactérias entéricas que podem ser de relevância na patogênese das infecções oportunistas
Abstract: The aim of this study was to evaluate the occurrence of major periodontopathogenic microorganisms and Enterobacteriaceae and biofilm adhered to the denture, mucosa and saliva in 90 edentulous subjects with complete dentures, using molecular methods (PCR). Clinical specimens were collected from these individuals after assessing the socio-economic circumstances and behavioral. The oral microbiota of patients was characterized by obtaining samples of the biofilm adhered to the dental prothesis and saliva, for detection of major pathogens using PCR. The possibility of inter-relationships between different microorganisms was determined using the chi-square test and Mann-Whitney test. There were differences in the occurrence of Prevotella intermedia and Enterobacteriaceae in the saliva of edentulous patients, likewise, Enterobacteriaceae, Pseudomonas, Camphylobacter rectus and the biofilm attached to denture patients. The conditions for oral hygiene and stat of preservation of prosthesis total precarious favored the occurrence of yeasts of the Candida sp, particularly Candida albicans, statistically significant levels in samples of mucosa and biofilm acceded to total prosthesis, prothetic device, making it a potential reservoir of enteric bacteria and yeasts that may be of relevance in the pathogenesis of opportunistic infections
