Full textThe purpose of this study is to analyze and compare two complete denture reline technique, one occluding and one digital, to determine which resulted in less displacement of tissue-complete denture base interface during impression making. Materials and methods. The experiment included 10 completely edentulous subjects. For each subject, three mandibular casts were obtained : (1) a reference cast of existing denture base, (2) a cast made with an occluding reline impression technique, and (3) a cast made with a digital reline impression technique. With the use of an optical 3-dimensional measurement system, the corresponding casts in a common coordinate system were analyzed geometrically. For each cast, the coordinates of the barycentric point and high point were determined. For the same subject, the difference between the coordinates of the barycentric points of the three casts were calculated 2-by-2. The same calculation was performed for the high points. To determine whether these differences, which represent the displacements of the barycentric points and high points, were statistically significant, the Wilcoxon signed-rank test for paired group comparisons was applied (P<0,05). Cartographic differences among the casts from the same subjects also were compared. Results. In the maxillary , the analysis of the displacement of the tissue-denture base interface was mainly vertical and more important under digital impression technique than under occluding impression technique. In contrast, in the mandibular arch, the displacement of the tissue-denture base interface was roughly of the same magnitude regardless of the impression technique used. Conclusion. The digital reline impression technique is recommended at the mandible while the occluding impression technique has to be prefered at the maxillary

Full textThe increase in life expectancy, the requests for prostheses renewal and the local or general factors responsible for progressive tooth loss lead to the fact that a constant number of complete removable prostheses are still produced. Therefore, the Complete Removable Prosthesis remains a major therapy to this date. The use of digital tools for the complete removable prosthesis and their marketing on the international market have been recently developed. Today, they constitute an alternative therapeutic response for toothless patients. However, these new tools remained to be validated in a clinical setting.The validation of a care practice involves checking whether the expected result of the care is actually obtained. Total removable prostheses essentially aim to satisfy a dual social and functional objective, and while it is customary for clinicians to consider patient satisfaction to verify the achievement of the aesthetic objective, few have a routine practice of functional assessment. With a view to being able to propose a method for the evaluation of the total prostheses produced by CAD/CAM, this work undertakes a critical analysis of the methods for the functional evaluation of total prosthesis rehabilitation.The literature search conducted in the first part of this work reports that functional assessment could be carried out by psychometric tests, in the form of questionnaires that could be implemented in clinical practice, and by physiological or biological tests more often implemented in research than in clinical practice. In Part 2, the limitations of functional assessment by psychometric tests are discussed. The 3rd part aims to verify whether physiological assessment methods of mastication are suitable for the evaluation of the outcome of functional rehabilitation by complete prosthesis. In particular, it was verified on the one hand which variables must be controlled during a granulometric food bolus analysis, and on the other hand which test foods could be used for the evaluation of the result of functional rehabilitation by Removable Complete Implanto-Retained Prosthesis.Future prospects involve the validation of a test food that can be chewed by edentulous subjects at all stages of their prosthetic rehabilitation and which, at the same time, allows its collection at the swallowing threshold in a physiological mastication framework

Full textThe aim of this study was to define the characteristics of muscular activity in complete denture wearers during mastication of model foods differing in hardness but similar in terms of rheological properties. The foodstuffs used in this study were laboratory-developed gumdrops demonstrating viscoelastic properties. All masticatory parameters were modified by the increase in food hardness for dentate subjects. Chewing rate was not modified by hardness of model food or ageing. Greater muscle activity was required by complete denture wearers than for dentate subjects to prepare the same food bolus for swallowing. Complete denture wearers took a greater number of masticatory cycles, and thus a longer duration of mastication and a higher level of EMG activity per cycle to chew the food samples. In addition, chewing rate was lower for complete denture wearers. This study also aimed to validate several parameters of video evaluation versus electromyography (EMG) during mastication of complete denture wearers. Analysis of video recording is a simple way to routinely assess chewing parameters such as number of chewing cycles and masticatory duration, along with other parameters not seen by EMG recordings, for example open mouth cycles or onset of swallowing

Full textProblem: There is a theoretical and practice knowledge gap in regard to immediate loading of unsplinted implants in edentulous individuals. Objectives: This study aimed to: (1) determine the implant survival rate of 2 unsplinted implants supporting a mandibular complete overdenture following an immediate loading protocol, (2) assess marginal bone level and implant stability changes of these immediately loaded implants in a 4-month period and compared them to a control implant, and (3) describe the clinical complications associated with this mode of loading. Methods: In 18 edentate individuals (mean age 62±7 years), this phase-I trial with a pre/post design assessed the clinical outcomes following the immediate loading (<48 hours) of 2 unsplinted implants supporting a mandibular overdenture. Periapical radiograhs, bone probing measurements and resonance frequency analysis were used to assess marginal bone losses and implant stability changes of these two implants inserted in the mandibular parasymphyseal region, at baseline (T0) and 4-month follow-up (T1). A non-submerged and non-loaded implant inserted in the mandibular symphysis was used as a control. Data were analyzed using linear mixed models, adjusted Tukey tests, Friedman’s analysis of variance and Wilcoxon signed-ranks tests. Results: From T0 to T1, 3 immediately loaded implants failed in 2 patients. This resulted in an implant survival rate of 91.7% (33/36), and, per patient, of 88.9% (16/18). No control implant failed. The marginal bone losses around loaded implants were -0.2 ± 0.3 mm for radiographic measurements and -0.5 ± 0.6 mm for probing measurements. There was no statistically significant difference between immediately loaded and control implants in regard to the supporting marginal bone losses. The implant stability levels increased of 5 ISQ units regardless of loading. There was no statistically significant difference in the mean ISQ levels between immediately loaded and control implants at T0 or T1. Five out of 18 patients experienced no clinical complication, while 9 of them had at least two. Apart from implant failures, none of these clinical complications led to changes in the protocol. Conclusion: The short-term results suggest that: (1) the implant survival rate following the immediate protocol is similar to those reported during a conventional loading protocol, (2) marginal implant supporting bone and stability changes are not different compared to those of a control implant, (3) a high level of clinical and surgical experience is required to perform the procedures and to manage the associated complications. These preliminary results should be confirmed in a phase II clinical trial.

Full textABSTRACT The superiority and stability of the effect of mandibular two-implant retained overdentures (IODs) over conventional dentures (CDs) in relation to the oral health-related quality of life (OHRQoL) remain to be clarified. Furthermore, the influence of psychological factors, such as the sense of coherence (SOC), on the perception of treatment outcomes is still unknown. The aim of this study was to determine the magnitude of the effect of IODs and CDs on the OHRQoL and assess the stability of the treatment over time taking into account the level of SOC. METHODOLOGY: Edentulous participants (n = 172, mean age 71, SD = 4.5) wearing CDs or IODs were followed up for two years. The OHRQoL was assessed at baseline and both follow-ups using the « Oral Health Impact Profile (OHIP-20) » questionnaire. The SOC was only assessed at both follow-ups using « The Orientation to Life (SOC-13) » questionnaire. Statistical analyses were conducted to evaluate between and within treatment differences (descriptive, bivariate and multivariate statistical analyses). RESULTS: A significant pre-/post-treatment improvement in the OHRQoL for both treatment groups was found (Wilks’s Lambda = 0.473, F (1,151) = 157.31, p < 0.0001). The magnitude of the effect in the IOD group was 1.5 times larger than that in the CD group. These outcomes were stable over the two-year study period and the SOC did not influence any of these results. CONCLUSION: IODs provide better long-term OHRQoL than CDs regardless of SOC level. The results are clinically meaningful and confirm the superiority of IODs over CDs.

Full textProblematic: Tooth loss is one of the chronic diseases associated with several anatomical and functional alterations in the oral cavity and the upper airway tract. These changes result in the deterioration in oral health-related quality of life (OHRQoL), particularly among the elderly. Complete dentures can enhance the appearance and restore some impaired masticatory functions, creating a positive impact on OHRQoL. However, continuous nocturnal wear of dentures may raise oral health problems. In contrast, some studies have found that the impact of sleeping with dentures is positive with regard to the quality of sleep and quality of life. Objectives: The aim of this Master’s study is to investigate the impact of sleeping with dentures on OHRQoL in edentulous elderly patients who suffer from obstructive sleep apnea (OSA). Methodology: This master research project is nested in a randomized, crossover trial study of 70 edentulous elderly patients diagnosed with moderate to severe OSA (AHI ≥10). The participants were randomly assigned to one of two groups (sleeping with or without the prosthesis in opposite order), for two periods with a 30-day interval between them. Data on the effect of nocturnal wearing of dentures on OHRQoL and health-related quality of life (HRQoL) were collected at the baseline and during the two follow-up visits, using the Oral Health Impact Profile (OHIP-20) and Short form (SF-36) questionnaires respectively. Data analysis was performed using mixed effect models appropriate for repeated measures including types of intervention (sleeping with or without dentures), sequence and visit number. Results: Results reported below are based on data collected from 63 patients. The overall OHIP-20 score was slightly higher among patients who wore their dentures while sleeping than in those who did not, but not at a significant level (p=0.08). The scores of all OHIP-20 subscales were increased among patients who slept with prosthesis, except for the social disability which slightly decreased, and the handicap remained at the same average. However, the significant negative impact was reported only for two OHIP scores: psycho-discomfort (p=0.04) and physical disability (p=0.05). OHIP-20 scores were influenced neither by the assignment sequence nor by the period, with an exception for handicap scores which slightly increased among elders assigned to sleep with dentures in the first period of this study. In contrast, SF-36 results showed an improvement in the mean scores in all subscales among the elderly when participants were sleeping with dentures, but the only significant impacts were found on social function and health change aspects (p <0.01, and p=0.01, respectively). The assignment sequence had a significant effect on physical function (p=0.03), pain (p=0.02), and health change (p=0.02). The follow-up visit number had a significant impact only on the physical function domain (p=0.03). Conclusion: While sleeping with dentures could have a negative impact on psycho-discomfort and physical discomfort aspects of OHRQoL, it could have a positive impact on the social function and health change aspects of HRQoL. The results of this Master’s research should be considered as preliminary since the total data analysis has not completed yet.
