Communication Skills of Novice Psychiatric Nurses with Aggressive Psychiatric Patients by Rose L. Moss MS, University of Hartford, 1996 Project Submitted in Partial Fulfillment of the Requirements for the Degree of Doctor of Nursing Practice Walden University March 2015 The transition from novice nurse to a competent psychiatric staff nurse is often associated with major communication challenges, primarily when caring for aggressive patients. Guided by Peplau's theory, this quantitative study assessed the communication skills of novice psychiatric nurses (N = 25) who worked 24 months or less in the state psychiatric hospital with aggressive psychiatric patients. Additionally, certain demographic data such as gender, age range, level of education, and length of time working were analyzed to determine their impact on communication skills. The survey consisted of 20 questions which assessed demographic data, communications skills, and hospital-based orientation. Based on ANOVA, novice nurses did not differ on hospital-based orientation based on gender, age, level of education, or length of time working. Novice nurses' communication skills did not differ by gender, age or level of education; however, novice psychiatric nurses who had worked 19-24 months had stronger communication skills than those working less time with aggressive patients (F = 6.9, p < 0.005). A communication skills class during hospital orientation to prepare novice nurses to communicate effectively with aggressive patient was recommended to nursing leadership and staff. A communication skills class held during hospital orientation could enhance the nurse-patient relationship, cultivate a safer and secure milieu, and improve patient outcomes. The findings have implications for positive social change for staff development to improve the hospital orientation for novice psychiatric nurses to become better equipped as effective communicators with aggressive psychiatric patients.

Abstract:
Many psychiatric nurses working in the community are changing their occupational base. They are working increasingly as members of a mental health multi-disciplinary team. This is a report on aspects of the role of the psychiatric nurse working in such teams. Freidson's professional-dominance thesis is used as a theoretical framework to assess the occupational status of psychiatric nurses working in the community mental health team. In particular, the levels of clinical autonomy experienced by the nurse are explored. Four community mental health teams are examined, using Diary-interview Schedules to record how new clients are processed by the psychiatric nurse. The other members of the teams were interviewed (as were the managers to whom the nurses were accountable) using Focused-interview Schedules. Furthermore, Field-notes were made of substantive, methodological, and pre-analytical observations made during visits to the team centres. The report concludes that although there is an occupational hierarchy and inter-disciplinary rivalry in the teams, the psychiatric nurse enjoys a large amount of de facto clinical autonomy. The psychiatric nurse has also a dynamic and invariably unsupervised influence on the creation and pathway of psychiatric careers for those who are referred to her or him. Recommendations include the need to affirm authoritative leadership in the team, and for formal supervisory procedures to be installed. It is also recommended that psychiatric nurses in the community should re-assess their occupational strategy of professionalisation, with a view to a re-alignment with medical practitioners.

Abstract:
According to recent research, more than 75% of newly employed nurses leave employment within 6 months. Changes in organizational training programs are needed in order to improve psychiatric nurse retention, nationally and globally. A healthy work environment (HWE) has been described as an environment that includes mentoring for good communication skills; it is also one that explores collaborative strategies for safe practice; professional advancement; and empowerment through research, education, and skill development. Examination of the impact of this HWE psychiatric nurse orientation training program on nurse retention was done using Rogers' theory of diffusion. Study variables included the length of training and retention rates pre- and post-training to help identify the impact of the HWE training program on retention using different methods of policy review. The project included 88 nurses and outcomes were measured through review of hire and termination rates for the targeted facility 3 months pre- and post-intervention. An independent samples t test revealed that the average retention rate for nurses trained in the current versus HWE program differed significantly by program (p > .05). Training also was shown to improve clinical practice and increase awareness of existing evidence about strategies to improve psychiatric nurse retention. Social change implications include potentially decreasing the costs associated with hiring and training nurses, improving clinical practice, adding to the nursing knowledge base and skill set, and enhancing workflow processes for quality outcomes.

Magister Curationis - MCur<br>Background: Nurses, because of their close contact with these patients, are frequently victims of assault. The aim of the study is to explore and describe the lived experiences of nurses, who have been assaulted by patients at a Western Cape psychiatric hospital. Research Design: A qualitative approach, using a phenomenological research design was used to achieve the aim of the study. The target population was nurses, who are employed at the hospital under study. A purposive sampling, consisting of six nurses, was selected to participate in this study. Data were collected by means of unstructured interviews with nurses who met the inclusion criteria. Data saturation was reached after the sixth individual interview. The interviews were audio taped and transcribed, verbatim, and field notes were taken, as well. Collaizi’s method of data analysis was used to analyse the data and to identify themes and categories. The major themes that emerged were: Self-care incongruent to intrapersonal interest; Personal responses to trauma; Incongruence between patient behaviour and participant work experience; unprotected staff vulnerable to patient aggression; required and received supportive interventions. Ethical clearance was obtained from the Senate Ethics Committee at the University of the Western Cape. Permission to conduct the study was obtained from the Research Ethics Committee at a psychiatric hospital under study, as well as from the Department of Health, Western Cape. Participants were drawn from different units of the hospital. Findings: The findings revealed that nurses working at this psychiatric hospital frequently encounter assault and violence by patients, while providing care, to the extent that they view the workplace environment as unsafe and insecure. The way they experienced the assaults had some similarities; they all complained that they felt neglected by management after the assaults and that they experienced multiple bodily reactions to trauma. Recommendations: From the data gathered, it appeared that nurses were calling for support from those in authority, be it supervisors or hospital management. Some of the recommendations made by the researcher were: regular refresher courses on self-awareness training; counselling/debriefing; skills development workshops on management of aggression; and the feasibility of paying a special allowance to staff. Conclusion: The study revealed that lack of management support perpetuates the cycle of violence experienced, which, in most incidents, has a spill over effect in the personal lives of the nurses, who have been assaulted by patients. It is the researcher’s belief that nurses should be supported in this stressful environment.

Bakgrund: I Sverige beräknas 14-20% av befolkningen lida av psykisk ohälsa. Egenvård är handlingar som utförs i syfte att upprätthålla hälsa och är central inom den psykiatriska omvårdnaden. Sjuksköterskor möter personer med psykisk ohälsa i olika vårdsituationer och därför krävs kompetens om hur egenvård bör tillämpas i den psykiatriska omvårdnaden. Syfte: Att beskriva sjuksköterskors erfarenheter av hur egenvård tillämpas inom psykiatrisk omvårdnad. Metod: Kvalitativ intervjumetod med induktiv ansats tillämpades. Studien genomfördes inom öppen- och slutenvårdspsykiatrin i en mellanstor svensk kommun. Materialet bestående av nio intervjuer transkriberades och analyserades därefter med hjälp av konventionell innehållsanalys. Resultat: Sjuksköterskorna arbetade utifrån ett professionellt förhållningssätt i syfte att utbilda patienten och individanpassa vården. De beskrev att egenvård som begrepp sällan användes men att de tillskrev många omvårdnadshandlingar till just egenvård. Sjuksköterskorna beskrev att insikt, eget ansvar, motivation och delaktighet var förmågor som var avgörande hos patienten för en fungerande egenvård. Sjuksköterskorna erfor dock att en allt för stor tilltro till medicineringen och bristande kompetens hos vårdgivarna bidrog till att patienternas egenvård försummades då problem ofta löstes kortsiktigt och patienterna blev kvarvarande inom psykiatrin. Konklusion: Bland de intervjuade sjuksköterskornas var den generella uppfattningen att egenvårdsfilosofin inte har anammats inom den psykiatriska omvårdnaden, vilket resulterar i att patienterna blir kvar inom den psykiatriska vården under en obefogat lång tid genom att patienternas sjukdomsförlopp förlängs.<br>Background: In Sweden, 14-20 percent of the population has reduced psychiatric health. Self-care is actions done to maintain health and is crucial in psychiatric nursing care. Nurses meet people with reduced psychiatric health in care situations and therefore knowledge about how self-care should be used in the care of the psychiatric patient is important. Aim: The aim of this study was to describe nurses’ experience of how self-care is applied in psychiatric nursing care. Method: A qualitative interview study was conducted in out- and incare psychiatric care in a middle-size Swedish municipality. Nine interviews was transcribed and thereafter analyzed with a conventional content analysis. Result: The nurses was found to work from a professional approach in order to educate and individualize care. They described that the concept of self-care was rarely used but that they ascribed many actions of nursing care to self-care. The nurses described that insight, responsibility, motivation and participation were the determinant abilities of the patient for a functional self-care. Yet, nurses believed that a greater faith for the medication and lack of competence of the care givers contributed to neglection of the patients’ self-care when problems often were short-term solved and the patients remained in psychiatric care. Conclusion: In the group of interviewed nurses, the general opinion was that the concept of self-care not has been embraced in psychiatric nursing care. According to the nurses, it results in patients remaning in psychiatric care during an unjustified long time and that the course of illness becomes extended.

Abstract:
Syftet: Syftet med studien var att belysa specialistutbildade sjuksköterskors erfarenheter av transkulturell omvårdnad inom psykiatrisk vård. Metod: Studien genomfördes med en kvalitativ metod med enskilda semistrukturerade intervjuer. Specialistutbildade sjuksköterskor som arbetat inom psykiatrisk vård minst ett år fick berätta om sina erfarenheter av transkulturell omvårdnad.  Resultat: I resultatet framkom det att specialistutbildade sjuksköterskor inompsykiatrisk vård upplever språket som ett hinder för omvårdnaden eftersom språket är så centralt inom psykiatrisk vård då samtal utgör en stor del under vårdtiden. Det visar också att religiösa och kulturella seder kan ha betydelse i möten med patienter från olika kulturer. Möjligheter att förstå varandra mellan personal, patienter och anhöriga som är delaktiga i vården lyfts fram. Anhöriga kan bidra med information och även stötta patienterna vilket är värdefullt för omvårdnaden. Tolk används ofta för att lösa kommunikationsproblem men de används inte i så stor utsträckning man skulle önska på grund av kostnader. Slutsats: De specialistutbildade sjuksköterskorna har i stor utsträckning, kunnat ta hänsyn till patientens önskemål och erbjudit patienten det bästa möjliga alternativ till god omvårdnad. Studien visar att det finns specialistutbildade sjuksköterskor inom psykiatrisk vård som anser sig behöva mer kunskap inom transkulturell omvårdnad.<br>Aim: The study focuses on advanced trained psychiatry nurse’s experiences of transcultural nursing in psychiatric care. Method: The study was conducted using a qualitative method of individual semistructured interviews. Nurses working in psychiatric care could share experiences of transcultural nursing. Results: In the result, one finding was that nurses perceive differences oflanguage as a barrier in the care, because communication is central in psychiatriccare. Religious and cultural beliefs and practices may also have an impact on the meeting with a patient from a different culture. There is opportunity for enhanced mutual understand when relatives are involved in the care. They can provide more information as well as support to the patient. Interpreters are used to solve communication problems; however they are not engaged as often as they should, due to the cost. Conclusion: That nurses, to a large extent, have been able to take into account patient’s wishes and thereby been able to offer the best possible treatment optionsto the patient. The study finds that nurses in general need a wider knowledge of transcultural nursing.

Bakgrunden visar att sjuksköterskor tenderar att lämna den psykiatriska vården när de upplever att deras kompetens inte tas tillvara, vilket i förlängningen även drabbar patienterna. Syfte: Syftet var att beskriva hur sjuksköterskor yrkesverksamma inom psykiatrisk vård uppfattar sin yrkesidentitet Metod: Fem sjuksköterskor intervjuades med hjälp av ostrukturerade intervjuer. Kvalitativ design användes i studien. Resultat: Resultatet presenteras i temat en komplex yrkesidentitet samt tre kategorier: att vårda unika människor, att vara arbetsledare och den egna utvecklingen samt åtta underkategorier Diskussion: Resultatet diskuteras utifrån Imogene Kings begrepp roll, organisationer, auktoritet, makt, status och stress, samt utifrån genusteoretiskt perspektiv.<br>Background:Nurses working in psychiatric care experience difficulty in articulating their professional identity. The background describes the role of the nurse in psychiatric care from a historical and contemporary perspective. The background shows that nurses tend to leave the psychiatric care when they feel their skills are not utilized, which in turn also affects the patients. Aim: The aim of the study was to describe how nurses working in psychiatric care understand their professional identity Methods: Five nurses where interviewed with the help of unstructured interviews. A qualitative design was used for the study. Result: The results are presented in the theme a complex professional identity and the three categories: nurturing unique people, to be supervisors and their own development as well as eight subcategories Discussions: The results are discussed from Imogene King's conceptual role, organizations, authority, power, status and stress, and on the basis of gender theory perspective.

<p> Workplace incivility is becoming commonplace in all work environments including health care. Research highlights the dangerous, distressing, and costly side effects of lateral workplace incivility (LWPI) including nursing staff's overall health, organizational commitment, and intent to stay. Historically, organizations have been considered safe work environments but LWPI has increased over the last several decades. There has also been limited research related to LWPI as it affects psychiatric healthcare staff and no study recommending a needs assessment or developing a position to educate and assist victims. The purpose of this project was to assess psychiatric healthcare staff's understanding and exposure to lateral workplace incivility, develop and provide education on LWPI, and develop a position for a LWPI Nurse Liaison to develop zero tolerance policies, provide immediate intervention when LWPI occurs, and develop processes of progressive action in response to repeated acts of incivility.</p><p> <i>Keywords:</i> lateral workplace incivility, horizontal incivility, bullying, nurses, job satisfaction, job commitment, intent to stay, role development, zero tolerance policy.</p>

Abstract:
A literature search was carried out on the role of the Community Psychiatric Nurse (CPN) in counselling within the Primary Health Care Team (PHCT) which highlighted a lack of theoretical knowledge for the psycho-social issues and that CPNs were not trained to provide counselling. The aim of this thesis was centred around the role in counselling the CPNs in Mid-Surrey Health District had in PHCTs, and whether General Practitioners (GPs) perceived CPNs as CPNs or counsellors. Two hypotheses were devised at the beginning of the study and a methodology was pursued in proving or disproving these hypotheses. Results indicated that CPNs were aware of the meaning of the term ‘counselling’ and that all but one felt they had not enough knowledge to carry out more in depth counselling/psychotherapeutic interventions. They all felt that counselling training was inherent to their work as CPNs within the PHCT. This therefore supported the hypothesis that CPN counselling in the PHCT needs to be trained. The GPs result was that they were unaware of qualifications held by CPNs or counsellors. They felt that CPN was invaluable and it would be an advantage if CPNs had counselling training. However, some felt that CPNs needed to retrain the ‘multipurpose’ aspect of their work. From the results it was apparent that there was a misperception of the role of CPN and counsellor as some GPs felt that CPNs were already counsellors. However, the result was inconclusive in proving or disproving the hypothesis. Most GPs introduced the CPN as a CPN and not as a counsellor, although some GPs believed that CPNs were also trained in counselling. It is argued that although these findings indicate that CPNs need counselling training and counselling was perceived as an important part of their work, in depth counselling training was not regarded as necessary to carry out their work as CPN.

Depression is often difficult to diagnose in the elderly. Elders and their families may misinterpret symptoms of depression as symptoms of co- morbid medical illnesses. These illnesses may include: diabetes, stroke, heart disease, arthritis, Parkinson's or Alzheimer's disease (National Mental Health Association, 1996). The Diagnostic and Statistical Manual of Mental Disorders (DSM- IV -TR) assists the health care provider in the diagnosis of depression. These include: Major Depressive Disorder (MDD), Depressive Disorder NOS, Bipolar I and II, and Mood Disorder due to General Medical Conditions. The elderly experience of depression involves loss, including: loss of interest in activities, decrease in energy, loss of control of their life, difficulty with concentration, difficulty with memory and the ability to make decisions (National Mental Health Association, 1996). In fact, one study found that depressive symptoms in the elderly are an important risk factor for developing cognitive impairment and triggering functional decline (Bassuk, Berkman, & Wypij, 1998.) This paper will concentrate on the loss of the decision -making ability in elders that is due to their diminished capacity and the knowledge and skills the Psychiatric Mental -health Nurse Practitioner (PMhNP) offers in assisting families and other providers to help depressed elders.

Cirka 800 000 människor världen över avslutade sina liv genom suicid 2014. En psykiatrisk diagnos är en vanligt förekommande bakomliggande orsak. De vanligaste diagnoserna är depression, bipolär sjukdom, post traumatiskt stressyndrom samt substansmissbruk. Tidigare suicidförsök är den enskilt största riskfaktorn för ett fullbordat suicid. Det är komplext att arbeta med suicidnära patienter i öppenvården och det ställs höga krav på specialistkompetens inom psykiatrisk omvårdnad och goda kunskaper kring det suicidpreventiva arbetet. Syftet med studien var att beskriva psykiatrisjuksköterskans erfarenhet av att bedöma suicidrisk på patienter på en vuxenpsykiatriskmottagning. En kvalitativ metod användes med induktiv ansats där fyra intervjuer med  psykiatrisjuksköterskor genomfördes. I resultatet framkom att psykiatrisjuksköterskan dagligen möter och behandlar suicidnära patienter på en öppenvårdsmottagning. Resultatet visade att psykiatrisjuksköterskan trots år av erfarenhet upplevde arbetet med suicidnära patienter som komplext och ansvarsfullt. Det framkom att psykiatrisjuksköterskan arbetade självständigt och stod ofta ensam med att bedöma suicidrisken och vidta åtgärder. Psykiatrisjuksköterskan har en viktig del i det suicidpreventiva arbetet och behöver känna sig trygga och säkra i omvårdnaden kring suicidnära patienter och i att bedöma suicidrisken. Vidare forskning skulle kunna ge fördjupad kunskap om psykiatrisjuksköterskan roll och behov för att ge omvårdnad till suicidnära patienter.

Aggression or violence by patients towards psychiatric nurses is a global issue. These nurses, therefore, face the huge challenge of providing nursing care to aggressive psychiatric patients. Without the necessary knowledge and skills, the nurses are vulnerable to all kinds of injuries, given the time spent managing aggressive patients. Purpose and objectives: The purpose of the study was to determine the level of knowledge and skills that professional nurses possess to manage the aggression of psychiatric patients. The objective of the study was to ascertain whether the knowledge and skills of professional nurses were sufficient to manage aggressive psychiatric patients. Method: A quantitative approach and descriptive design was used to conduct this study at a Psychiatric Hospital in the Western Cape, South Africa. The target population consisted of 149 professional nurses employed at the Psychiatric Hospital. The sample for the study was all-inclusive i.e. all professional nurses employed at the hospital. Only 70 participants were available for the study. Structured questionnaires were distributed to the participants for data collection and the response rate was 77% (n=54). Data analysis was done with the aid of a statistician using a Statistical Package for Social Science, version 22, and nominal as well as ordinal data was analysed using descriptive analysis. The process and purpose of the study was explained to the participants, who gave their consent, prior to the distributing of the questionnaires. The researcher obtained permission from the necessary authorities before commencing with the study. Findings: The study found that nurses were more likely to be exposed to verbal aggression as opposed to sexual aggression. It also revealed that nurses with less years of experience had more knowledge than their counterparts who had more years of experience in same position. Furthermore, the study revealed that those who had training in aggression management reported that the training did not meet their needs. Overall, the findings revealed that nurses had a fair knowledge of managing aggressive psychiatric patients. The overall findings also revealed that professional nurses had the required skills to manage aggressive patients. Recommendations: There is a need for on-going in-service training and refresher courses in the management of aggression. There should also be a needs analysis before commencing with these training courses which should be made compulsory for all staff to attend. Further studies should be conducted on the different categories of nurses, and other disciplines within nursing, to ascertain their knowledge of the management of aggression. Qualitative studies should also be conducted on nurses‟ experience of aggression in different settings.

<p>Syftet med studien var att undersöka psykiatrisjuksköterskans självrapporterade utförda arbetsuppgifter och självupplevda kompetens. Studien bygger på individuellt genomförda intervjuer med 19 psykiatrisjuksköterskor verksamma inom den psykiatriska slutenvården. Författarna har använd ett proportionellt stratifierat urval. Resultatet presenteras i fem kategorier; psykiatrisjuksköterskans kompetensområde, specifika arbetsuppgifter i kompetensområdet, arbetsuppgifter utanför kompetensområdet, arbetsuppgifter som psykiatrisjuksköterskan saknar samt rollkonflikter med tillhörande subkategorier. I psykiatrisjuksköterskans kompetensområde ingår arbetsuppgifter som arbetsledare och omvårdnadsansvarig att planera fördela arbetet på avdelningen, vårdplanera, dokumentera samt läkemedelsadministration. Det specifika i kompetensområdet visade sig vara bland annat vara stöd samtal, motiverande samtal och undervisning. Sjuksköterskorna skulle också ha en övergripande helhetssyn samt göra observationer. Arbetsuppgifter utanför kompetensområdet visade att de hjälper doktorerna med olika administrativa uppgifter samt en del kuratorsarbete. Arbetsuppgifter som sjuksköterskorna saknar främst; är mer aktiviteter för patienterna, mer patientkontakt, bättre samarbete med vårdgrannar och anhöriga samt en ökad etisk medvetenhet i omvårdnads och dokumentationsarbetet. Sjuksköterskorna upplevde även rollkonflikter i sitt arbete som mest berodde på olika otydligheter inom arbetsgruppen. Författarna anser att en tydligare arbetsbeskrivning, bättre ansvarsfördelning mellan de olika yrkesgrupperna samt en mer strukturerad vård av patienterna skulle minska otydligheten och osäkerheten för psykiatrisjuksköterskorna.</p><p> </p><br><p>The purpose of the study was to examine self-reported psychiatric nurse performed tasks and self-perceived competence. The study is based on individually conducted interviews with 19 psychiatric nurses working in psychiatric inpatient ward. The authors have used a proportional stratified sample. The results are presented in five categories; psychiatric nurse competence, specific tasks in the areas of competence, work outside the areas of competence, tasks psychiatric nurse lack and role conflicts and related subcategories. In psychiatry nurse competence includes duties as supervisor and nursing manager to plan the work in the department, care planning, documenting, and pharmaceutical administration. The specific competence in the field proved to be, inter alia, support calls, motivational talks and teaching. The nurses would also have a comprehensive approach and make observations. Tasks outside the competence demonstrated that they are helping the doctors with various administrative tasks, and some counselors work. Tasks that nurse do mainly lack; are more activities for patients, more patients, better cooperation with neighbors and family care, and increased ethical awareness in the nursing care and documentation work. The nurses also experienced role conflict in their work as most were due to various ambiguities in the working group. The authors conclude that a better working instruction, better division of responsibilities between the different professions and a more structured care of patients would reduce the lack of clarity and uncertainty of psychiatry nurses.</p><p> </p><p> </p>

It happens only in cases where the suspicion arises or is confirmed. It is individually whether and how the nurse asks the woman if she is subjected to violence in a close relationship, depending on patient and nurse. Nurse handles the violence exposed woman by supporting her to share their experiences. Then consult and discuss with their nurse colleagues and other professionals for further follow-up and approach.</p><strong><strong><p>Conclusions:</p></strong><p> </p><p>The study has in accordance with previous research further highlighted the need for more education for nurses on how to handle women who are victims of violence in close relationships. Det saknas studier hur handledarna upplever handledning på avancerad nivå inom psykiatrisk omvårdnad. Rollen handledarna har är komplex, de ska både fungera som en mentor och som den som bedömer studenternas prestationer samt ge god vård åt patienten. Syftet med denna studie är att belysa hur handledning upplevs av den handledande sjuksköterskan under Verksamhetsförlagd utbildning (VFU) för specialistsjuksköterskor i psykiatri. Att få kunskapen studien genererar skapar förutsättningar för organisationen och universitetet att anpassa utbildning och återkoppling som gynnar handledarna och studenternas utveckling samt höjer vårdkvaliteten för patienterna. En explorativ kvalitativ ansats enligt fokusgruppsintervjuer har ansetts lämplig för studien då målet är att fånga vidden och komplexiteten i handledning. När deltagarna dras med i diskussionen skapas ett reflekterade klimat som på ett omfattande sätt belyser individens upplevelser, tankar och känslor. Studien kommer att utföras inom psykiatriska kliniken på Länssjukhuset Ryhov efter tillstånd från etisk kommitté.

New research findings, changes in legislation and the use of information technology in nursing, requires continuous in-service training of psychiatric nurses (Booyens, 2004:381; Muller, 2004:294) to cope with, reflect on, evaluate and modify their clinical practice based on emerging knowledge (Cowen et al.,2009:2; Polit & Beck, 2006:4). Many studies have been conducted on the importance of in-service training, and authors agree that in-service training improves the quality of nursing care. However few studies on in-service in South Africa and psychiatric nursing science have been conducted, of which the last study conducted on psychiatric nursing science in South Africa was 1986 (Nel, 1986:1-125). Personal observations found that some psychiatric hospitals in Gauteng do not conduct in-service training at all and informal discussions held with nursing managers highlighted that when in-service training programmes are compiled that are poorly followed, and it is unclear what the needs of psychiatric nurses are in terms of in-service training to keep them updated and empowered to perform to the best of their ability. The objectives of the study were to explore and describe the needs and benefits of in-service training for psychiatric nurses in a specific psychiatric hospital in Gauteng and to formulate recommendations for in-service training for psychiatric nurses in this hospital. The research project followed a qualitative, explorative, descriptive and contextual approach. Six focus group interviews were conducted and relevant data obtained. The findings of this research included seven main themes, namely the need for in-service training, career advantages, social advantages, physical advantages, psychological advantages of in-service training, recommendations regarding in-service training and factors that hinder in-service training. These findings were enriched with direct quotations from the transcriptions as verbalized by psychiatric nurses during focus group discussions. The findings were compared with available relevant literature for confirmation. The prevalence of mental health disorders in the United States is estimated at 1 in 5 persons in any given year, with a lifetime prevalence of approximately 50% (National Institute of Mental Health, 2016a, 2016b). Despite scientific progress towards effective behavioral and psychopharmacological treatment, nearly 50% of Americans with mental illness do not receive treatment (Substance Abuse and Mental Health Services Administration [SAMHSA], 2014). This project will identify key geographical and logistical factors restricting recruitment and retention of psychiatric providers (MD or NP) for an underserved region in California. The results of the study will be used to develop a model for effective partnerships aimed at a “grow your own” approach to addressing the shortage of psychiatric providers. In California, the Mental Health Services Act (MHSA) was approved by the voters as Proposition 63 in 2004, funding expansion of mental health resources and specifically, workforce development (California Department of Health Care Services [DHCS], 2017). National funding also supports the expansion of PMHNP training and capacity through Health Resources and Service Administration (HRSA) and SAMHSA. Despite administrative and funding resources from MHSA and HRSA, the shortage of psychiatric providers continues in rural and remote areas. While few national studies have included nurse practitioners in the findings, a growing body of evidence suggests that nurse practitioners can serve similar if not the same function as physician colleagues (DiCicco-Bloom & Cunningham, 2014; National Governors Association, 2012; Newhouse et al, 2011). Barriers to mental health services in rural communities include challenges around a) availability, b) accessibility, c) affordability, and d) acceptability (Wilson, Bangs, & Hatting, 2015). Using a descriptive design, this project explores these factors necessary for patient access to psychiatric services in rural areas. A logic model is used to create a summary of the findings, which will be used to propose a clinical training partnership between an urban university-based PMHNP education and a rural clinic-based training to be completed upon conclusion of the project.

Earlier studies show that when psychiatric out- and in-patient care units work together the risk for hospitalization decreases, leads to increased flexibility and shorter in-patient periods. At a psychiatric clinic in western Sweden there are routines in place regarding the transfer of patients from in-patient to out-patient care. The aim of this study was to describe nurses' experience of how out-and in-patient clinics collaborate during patients in-patient care. This is a qualitative study with an inductive approach. Ten nurses participated through semi structured interviews. The result ended up in two domains and eight subthemes and one theme, the diffuse responsibility – makes us not understand each other. Nurses in both out- and in-patient care experience uncertainty as to who has the responsibility for patients' treatment-plans and also uncertainty in how communication between the two parties works. Nurses in in-patient care experience that the out-patient care are uninterested, and nurses in out-patient care experience that in-patient care does not follow treatment-plans. In Conclusion both nurses in out- and in-patient care describe a need for an improved partnership. Some find that they are unsure of their role and their responsibility in the partnership. Structures for an improved partnership need to be implemented from the staff leadership, to be able to live up to the national guidelines and ensure that patients receive the treatment which serious psychiatric illness needs.

Specialistsjuksköterskor inom vårdformen sysslar huvudsakligen med grundläggande psykiatrisk omvårdnad. Deras breda kompetens underskattas och osynliggörs när slutenvården saknar omvårdnadsfokus. Forskning påvisar behov att tydliggöra specialistkompetensens betydelse inom psykiatrisk slutenvård, eftersom ökad förståelse inom området kan medföra bättre vård för patienten. Design. En kvalitativ design med fenomenografisk ansats användes. Metod. Fjorton semistrukturerade intervjuer med specialistsjuksköterskor verksamma i patientnära omvårdnadsarbete inom psykiatrisk slutenvård vid tre psykiatriska kliniker i norra Sverige genomfördes under tiden november 2015 – januari 2016. Intervjuerna transkriberades och analyserades sedan med kvalitativ analys. Resultat. Specialistsjuksköterskans kompetens möjliggör personcentrerad psykiatrisk omvårdnad och är betydelsefull för kunskapsspridning i personalgruppen. Betydelsen kan ökas genom att utrymme för mer patientnära arbete och utvecklingsarbete ges. Specialistkompetensen inom slutenvården behöver synliggöras och omvårdnad framhävas som behandlingsalternativ. Åtgärder för att öka specialistkompetensens betydelse kan påverka den psykiatriska slutenvårdens kvalitet och den enskilde patientens vård positivt. Slutsats. Specialistsjuksköterskans kompetens är betydelsefull för omvårdnadsarbetet inom psykiatrisk slutenvård, men får inte alltid nödvändigt utrymme för att möjliggöra vård av högsta kvalitet. A qualitative design with a phenomenographic approach. Method. Fourteen semi-structured interviews with specialist nurses in patient-oriented inpatient nursing at three psychiatric clinics in north Sweden were conducted during November 2015-January 2016. The interviews were analysed by using a qualitative analysis. Result. The specialist nurse’s competence enables a person-centred psychiatric nursing and shares knowledge within the team. Enabling more patient-oriented and development related work could increase the significance of specialist competence. The specialist competence needs to be visualized and nursing highlighted as a treatment alternative. Increasing the significance of the specialist competence could affect the quality of care and influence the patient’s care positively. Conclusion. The specialist nurse’s competence is important within the inpatient care but needs more space to enable high quality care. The result demonstrates the significance to stress the specialist nurse’s position within psychiatric inpatient care. This could attract specialist nurses to stay within inpatient care and enable the competence to come to use for the patients who needs it the most.

Introduktion: Att någon gång drabbas av psykisk ohälsa blir allt vanligare, därför är betydelsen av forskning inom området stor. Sjuksköterskor med sin specifika omvårdnadskompetens utgör den största personalgruppen inom psykiatrisk öppenvård men sjuksköterskans roll inom psykiatrisk öppenvård är begränsat beskriven i vetenskaplig litteratur. Detta innefattar aspekter kring sjuksköterskans egen roll samt i relation till andra yrkesgrupper inom psykiatrisk öppenvård. Syfte: Att beskriva sjuksköterskans upplevelse av sin egen roll inom psykiatrisk öppenvård samt upplevelsen av interaktionen med andra yrkesgrupper i sitt arbete. Metod: Studien var en deskriptiv intervjustudie med kvalitativ ansats. Sju sjuksköterskor arbetandes i psykiatrisk öppenvård inkluderades i studien. Data insamlades genom semistrukturerade intervjuer och analyserades sedan med kvalitativ innehållsanalys. Resultat: Sammanlagt skapades 25 kategorier: positiva respektive negativa aspekter i arbetet som sjuksköterska inom öppenpsykiatri, samarbete mellan de olika yrkeskategorierna i teamet, bemötande och uppskattning för sjuksköterskans kompetens samt sjuksköterskans bidragande kompetens i vården av patienten. Slutsats: Sjuksköterskans upplevelse inom psykiatrisk öppenvård är komplex och visar bland annat på känslor av hög arbetsbelastning, en odefinierad roll i interprofessionellt samarbete samt både undervärdering och uppskattning för sin kompetens. Uttryck för ett bidragande omvårdnadsperspektiv samt en bredare syn på patienten i vården visar dock på tydliga uppfattningar om sin kompetens som sjuksköterska. Allt detta är relaterat till flera faktorer såsom organisation, arbetskollegor samt den egna uppfattningen om sin yrkesroll i arbetet.<br>Introduction: At some point suffer from mental illness is becoming more common, therefore, research in the field is of great importance. Nurses with their specific nursing skills form the largest staff group in psychiatric outpatient care, but the nurse's role in psychiatric outpatient care is limited as described in scientific literature. This includes aspects of the nurse's own role and in relation to other professions in psychiatric outpatient care. Aim: To describe the nurse's perception of their role in psychiatric outpatient care, and the experience of interaction with other professionals in their work. Method: The study was a qualitative, descriptive interview study. Seven nurses working in psychiatric outpatient care were enrolled in the study. Data were collected through semi-structured interviews and analyzed with qualitative content analysis. Results: A total of 25 categories concerning positive and negative aspects of work as a nurse in outpatient psychiatric care, collaboration between the professions in the team, treatment and appreciation to the nurse's competence and nurses contributory expertise in caring of the patient were created. Conclusion: Nurse's experiences in psychiatric outpatient care are complex and show feelings of heavy workload, an undefined role in inter-professional collaboration and both under-valuation and appreciation of their skills. Though, expression of a contributory caring perspective and a broader view of the patient show views oft the nurse's competence. All this can be related to several factors such as organization, work colleagues and their own perception of their professional role in their work.

Bakgrund: ACT- modellen växte fram under 70 och 80-talet i samband med att många psykiatriska kliniker stängdes i USA. Den ursprungliga ACT- modellen kan ses som en fullservicemodell men har sedan modifierats och anpassats efter samhällets och sjukvårdens rådande förhållanden. Som det ser ut idag så har flera modeller utvecklats med utgångspunkt från ACT-modellen men kan särskiljas av vilka komponenter som ingår. Modellen har visat positiva resultat avseende patienters delaktighet och återhämtning. Sjuksköterskan har en bred yrkesbakgrund och anses ha en viktig roll i teamet. Med sin kompetens har de grundläggande kunskaper för att utföra medicinsk och psykiatrisk omvårdnad men flera studier har visat att det finns ett behov av specialistutbildade sjuksköterskor då många av patienterna är svårt sjuka. Resultat: Innehållsanalysen resulterade i två huvudkategorier, Kompetensen med in i teamet samt Den mångsidiga sjuksköterskan, med tillhörande sex underkategorier. Diskussion: Resultatet diskuterades utifrån de två huvudkategorierna Kompetensen med in i teamet och Den mångsidiga sjuksköterskan. Som stöd i diskussionen användes till ämnet relevanta artiklar och litteratur samt Phil Barkers tio förpliktelser som utgångspunkt.<br>Background: The ACT model developed in the 70's and 80's when many psychiatric clinics were closed in the United States. The original ACT model can be seen as a full service model but has since been modified and adapted to the current conditions of society and health care. As it appears today, several models have been developed based on the ACT model but can be distinguished by which components are included. The model has shown positive results regarding patient participation and recovery. The nurse has a wide professional background and is considered to be an important part of the team. With their skills, they have the basic knowledge to perform medical and psychiatric care, but several studies have shown that there is a need for specialist educated nurses, as many of the patients are severely ill. Aim: The aim of this study was to describe the psychiatric specialist nurse's experience of their competence and role in teams working under the ACT model. Method: A qualitative design has been selected. Data collection was done with semi-structured interviews. A qualitative content analysis with inductive approach was used to process the material. Results: The content analysis resulted in two main categories, skills in the team and the versatile nurse, with six associated subcategories. Discussions: The result was discussed based on the two main categories, skills in the team and the versatile nurse. In support of the discussion, relevant topics and literature were used as well as Phil Barker's ten obligations as a starting point.

Magister Curationis - MCur<br>The provision of mental health care, treatment and rehabilitation of the acutely ill mental health care users (MHCUS) poses a major challenge to the nurses working in acute units. Nurses spend long hours ensuring that acutely ill psychiatric patients receive quality patient care in acute admission units in different psychiatric hospitals in South Africa. With few studies showing a rise in the prevalence of mental disorders in the South Africa and the Western Cape Province, acute psychiatric inpatient units across the province have experienced intense pressure and persistent rise in the number of acute patient admissions. Dealing with this group of patients is a difficult task particularly for nurses who spent prolonged hours caring for them. Despite the continuing provision of care to MHCUS by nurses in acute admission units, very little is known about the lived experiences of nurses in acute admission units. The aim of this study was to explore and describe the lived experiences of nurses who care for the acutely ill MHCUS in an acute male admission unit at a psychiatric hospital in the Western Cape. Acutely ill MHCUS in acute psychiatric units show severely disturbed behaviour at times, aggression, hostility, acute psychotic symptoms and many other symptoms related to psychiatric illness. Methods: a qualitative, descriptive phenomenological research design was used to explore and describe the lived experiences of nurses who care for acutely ill patients in an acute admission unit. A purposive sample of eight nurses was selected. Individual, semi structured phenomenological interviews were used to collect data from nurses caring for MHCUS in an acute admission unit. Data saturation was reached after carrying out the eight interviews. These interviews were audio taped and transcribed verbatim and Collaizi’s (1978) seven steps method of qualitative data was applied to analyse the collected data. Findings: The study found that nurses in the acute admission unit experienced several challenges while caring for MHCUs. Nurses reported both negative and positive experiences. Positive experiences included MHCUs recovery, teamwork and passion for caring while negative experiences were feeling unappreciated and unsupported by authorities. Furthermore, they reported physical assault by MHCUs which led to fear. Challenges experienced included shortage of staff and increased workload which led to burnout amongst nurses in acute admission units.

It necessitates the urgency for professional nurses who are working in specialty units at hospitals, to study further in advanced post-graduate nursing sciences programmes, e.g. advanced psychiatric nursing. Professional nurses were not informed about the implications of the OSD for practice, prior to implementation. It was unclear how advanced psychiatric nurses were experiencing their practice in an OSD hospital setting. In this study, the experiences of advanced psychiatric nurses who were practising at an OSD psychiatric public hospital led to the description of guidelines for supporting these nurses during their practice in an OSD ward. A qualitative, exploratory, descriptive and contextual design was followed. The study population consisted of advanced psychiatric nurses (N = 50). Purposive sampling was conducted until data saturation was reached. Eight participants were included in the sample. In-depth unstructured individual interviews were conducted with each of these participants. Field notes were kept and voice recordings of all interview sessions were captured. The researcher conducted a pilot study with one participant in order to detect possible flaws that could occur during the data collection process. The data analysis where themes were identified was based on Tesch‟s method of qualitative analysis. A literature control supported the findings of this study. Subsequently, guidelines were described from the findings according to the method of Muller (2001:204-205). Trustworthiness was maintained by using the criteria of Guba‟s model; that is credibility, transferability, confirmability, and dependability. The ethical principles of the right to self-determination, withdrawal from the research study, privacy, autonomy and confidentiality, fair treatment, protection from discomfort and harm, and obtaining informed written consent was adhered to. Four themes emerged from the data that indicated: the under-utilisation of the full scope of advanced nursing skills, role conflict and overload, organisational structural barriers that delayed the implementation and practice of advanced nursing skills, and failure to conceptualise / clarify advanced nursing role that resulted in unrealistic and / or unmet expectations.

People living with a mental health issue is elevated both nationally and internationally, and the likelihood of a person interacting with someone who has a mental health issue is high. It is even higher for those who work in the health care profession, so understanding their attitudes on this matter is important. As a review of current literature reveals, there is not only stigma in the health care system, but there is also stigma among health care professionals and students. One of the groups of students that has not been researched to a great extent is nursing students and the degree of stigma they have for caring for those with mental health issues. A cross-sectional survey was conducted to assess the degree of stigma among these individuals. A voluntary online survey was done was administered to undergraduate nursing students at a university in the southeastern part of the United States using the twenty item Opening Minds Scale for Health Care Providers (OMS-HC). The results indicated that mental illness stigma did exist within the student nurse community with no association between gender, classification, and mental health nursing course completion and total score on the OMS-HC scale.

Recognizing the recurrent co-morbidity between mental and physical health conditions, specific commendations addressing the physical conditions causing the increased morbidity and mortality of people with severe mental illness are needed. In some instances, treatment recommendations for the general population may need to be modified for people with SMI.

Det första mötet med sjuksköterskan är ofta avgörande för hur vårdtiden av patienten utvecklas. Upplever patienten sig inte tagen på allvar, sedd eller hörd kan detta leda till flyktkänslor eller att man drar sig undan. Syfte: Syftet med studien är att utifrån vetenskaplig litteratur beskriva patienter och sjuksköterskors upplevelser av att få samt ge vård efter ett suicidförsök. Metod: Kvalitativ forskningsöversikt inkluderat åtta vetenskapliga studier från databaserna CINAHL och PsychINFO. Data har analyserats med stöd av Evans fyrstegsanalys. Resultat: Resultatet visar att patienter har ett behov att känna sig tagna på allvar för att lindra lidande. Resultatet visar även att sjuksköterskan upplever svårigheter i mötet med den suicidala patienten. Sjuksköterskans bemötande och engagemang i patienten har en central roll för patientens möte med psykiatrisk vård efter ett suicidförsök. Diskussion: Mötet mellan patienten och sjuksköterskan avspeglar sig i sjuksköterskans bemötande och engagemang i patienten, genom att ta patienten på allvar kan lidande lindras och patientens välbefinnande kan ökas. Genom engagemang stöttas patienten och genom eliminering av negativa upplevda känslor kan sjuksköterskan nå patienten på ett sätt som gör att patienten kan inspireras att förändra sin livssituation.<br>Background: Previous researches shows patients who come in contact with a psychiatric ward after suicide attempt often experience feelings of shame and fear. The first meeting is determined by how the time at the ward develops. If a patient experience not being taken seriously or heard it usually leads to feelings of escaping or withdrawing. Aim: The purpose of the study is based on scientific literature describing patients and nurses' experiences in receiving and providing care after a suicide attempt. Method: Qualitative research review included eight scientific studies from the databases CINAHL and PsychINFO. Data were analyzed with the aid of Evans four steps analysis. Results: The results show that patients have a need to feel that they are being taken seriously in order to alleviate suffering. The result also shows that nurses are experiencing difficulties in meeting with the suicidal patient. The nurse's attitude and commitment to the patient has a central role in the patient's encounter with psychiatric care after a suicide attempt. Discussion: The meeting between the patient and the nurse is reflected in the nurse's attitude and commitment to the patient, by taking the patient seriously suffering can be alleviated and the patient's well being can be increased. Through dedication supported the patient and by the elimination of negative emotions experienced nurse can reach the patient in a way that allows a patient to be inspired to change their lives.

Magister Curationis - MCur<br>The South African health care system shifted the focus of treating psychiatric disorders from institutional care level mental health services to facilitate this process of integration into the Primary Health Care (PHC) settings. All the provinces were thus engaged in improving mental health care services at community level by providing training for professional nurses in mental health at PHC settings. Consequently, mental health nursing has also changed considerably by shifting the focus of mental health care to the primary care level. It is however, suggested that the current revolving door syndrome experienced at psychiatric institutions was partly due to inadequate community-based psychiatric services. It was also suggested that the attitudes and knowledge of health professionals towards mental illness has a major impact on service delivery, treatment and outcome of mental illness. The aim of this research study was to assess the attitude of nursing staff working at a Community Health Centre (CHC) towards the mental health care user. A CHC was chosen that renders 24 hour services. The inclusive sample included all the different categories of nurses permanently employed at this CHC. The Attitude Scale for Mental Illness questionnaire was used to collect the data. Descriptive statistics: means, median and standard deviations were calculated for the following variables: separatism; stereotyping; restrictiveness; benevolence; pessimistic prediction and stigmatization. In conclusion it can be said that the nursing staff with more experience irrespective of category of nurse has less of a stereotyping attitude towards mental illness. The longer the nurse worked at the setting and irrespective of their nursing qualification the more positive their attitude towards the MHCU became.

Psykisk ohälsa är ett växande problem i samhället och allmänsjuksköterskan möter i sitt arbete dagligen människor med psykisk ohälsa. Dessa människor upplever sig ofta osynliga i samhället och upplever även förutfattade meningar i mötet med sjuksköterskan. Forskning idag utgår mindre ofta från perspektivet hos människor med psykisk ohälsa, vilket behövs för att omvårdnaden ska kunna utvecklas. Problematiken är att sjuksköterskan många gånger möter människor utifrån deras psykiska diagnos, istället för att möta dessa människor utifrån ett helhetsperspektiv. Examensarbetet beskriver hur vuxna människor med psykisk ohälsa kan uppleva mötet med sjuksköterskan. Två teman framkommer efter sammanställning av befintlig forskning. Vuxna människor med psykisk ohälsa upplever antingen sig sedda i mötet med sjuksköterskan eller en känsla av osynlighet. Känslan av att vara sedd uppkommer när sjuksköterskan bekräftar individen genom att vara öppensinnad. Sjuksköterskan lägger ner tid i mötet genom att lyssna och att försöka förstå personens situation. En känsla av osynlighet i mötet uppkommer när sjuksköterskan förminskar personen till en diagnos. De med psykisk ohälsa upplever hinder i relationen genom att sjuksköterskan utövar makt och behandlar dem som barn. De känner sig ensamma när sjuksköterskan inte har tid för dem och inte tyckts bryr sig om dem. Genom denna studie kan sjuksköterskan få en ökad förståelse för hur vuxna människor med psykisk ohälsa upplever mötet med dem. Ökad förståelse kan förbättra relationen mellan människor med psykisk ohälsa och sjuksköterskan. Det kan även leda till en ökad känsla av hälsa hos människor med psykisk ohälsa.<br>Background: Mental illness has in recent years increased in society. It has since centuries been seen as a shameful disease and mental illness has not been a high priority in health care. Aim: The aim of this study was to describe how adult humans with mental illness experiences the meeting with the nurse.Method: The method that was used was a literature based study with basis in analysis of qualitative research. 11 articles with a qualitative approach was analyzed. Result: The result showed two main theme. The first theme was a feeling of being seen in the meeting when an open dialogue was used and a relationship was created. The second theme was a feeling of invisible because they felt reduced to a diagnosis and encountered obstacles in the relation. Conclusion: Adult humans with mental illness could experience the meeting in different ways. A good meeting with the nurse could increase the human with mental illness trust in healthcare. It could improve the care and relationship between human with mental illness and the nurse.

Bakgrund: Specialistsjuksköterskans profession omfattar kompetens och kunskap som förvärvats genom studier i ämnet på avancerad nivå. För att synliggöra det förvärv ingår det att sjuksköterskan ser på sig själv och sin profession som självständig. Tidigare studier tyder på att trots att specialistsjuksköterskan har sin utgångspunkt i det egna kunskapsområdet är det inte alltid tydligt i möten med andra professioner. Det finns även ett synsätt som lever kvar beträffande sjuksköterskans profession som ett bihang till läkaren vilket gör utgör ett hinder till att arbeta självständigt. Syfte: Att beskriva specialistsjuksköterskans erfarenhet av att självständigt utöva sin professionella kompetens inom psykiatrisk slutenvård. Metod: Halvstrukturerade intervjuer genomfördes med sex kvinnliga specialistsjuksköterskor inom sluten psykiatrisk vård på en och samma klinik. Kvalitativ innehållsanalys valdes som metod vid analysen av materialet. Resultat: Specialistsjuksköterskorna upplevde att de hade möjligheter att påverka sin situation och således kunna utföra ett självständigt arbete. De beskrev att samarbetet med de övriga professionerna upplevdes oftast som stärkande och att det främjade patientarbetet. Respondenterna beskrev också specialistutbildningen som en bidragande faktor till ökad självständighet. Diskussion: Resultatet diskuteras utifrån det vårdvetenskapliga konsensusbegreppet vårdande som teoretisk referensram samt utifrån relevant vetenskaplig forskning.<br>Background: Specialist nurse profession includes skills and knowledge acquired through the study of the subject at the advanced level. In order to highlight the acquisition include the nurse looks at herself and her profession as an independent. Previous studies suggest that although the specialist nurse has its starting point in their own field of knowledge, it is not always clear in meetings with other professions. There is also an approach that persists regarding nursing profession as an appendage to the doctor which is an obstacle to work independently. Aim: To describe the specialist nurse experience to independently exercise their professional skills in psychiatric inpatient care. Method: Semi-structured interviews were conducted with six females specialist nurses in inpatient psychiatric care at the same clinic. Qualitative content analysis was chosen as the method in the analysis of the material. Results: Specialist nurses felt that they had the opportunity to influence their situation and thus be able to carry out and independent work. They described the cooperation with the other professions often perceived as strengthening and it promoted the work with patients. Respondents also described the specialized training as a contributing factor to increased independence. Discussions: The results are discussed from the health scientific consensus term care as a theoretical framework and on the basis of relevant scientific research.

Bakgrund: Få studier har gjorts inom psykiatrisk omvårdnad med fokus på patientperspektivet. Detta perspektiv är dock viktigt då det ger en inblick i hur patienterna upplever den vård de erhåller. Syfte: Syftet var att beskriva hur patienter på en psykiatrisk vårdavdelning upplever det bemötande de får av avdelningens sjuksköterskor. Metod: Semistrukturerade intervjuer som sedan analyserades med hjälp av kvalitativ innehållsanalys. Resultat: Vid analys av materialet framkom fyra huvudkategorier som samlade de aspekter på bra och mindre bra bemötande som respondenterna beskrivit: Betydelsen av att bli sedd och bekräftad av personalen, Personalens agerande och attityder gentemot patienterna, Önskemål och förväntningar på personalen samt Hur icke-verbal kommunikation påverkar hur bemötandet upplevs. Slutsats: Den mest framträdande aspekten av gott bemötande beskrevs som förmågan att kunna ta sig tid att lyssna på patienten, och den mest framträdande aspekten av mindre bra bemötande beskrevs som brist på denna förmåga.<br>Background: There are few studies focusing on the patients’ perspective of psychiatric care. However, this perspective is important, as it offers insight in the care received from the patient’s view point. Aim: The aim of this study was to describe how the patients in a psychiatric ward perceive the caregiving behavior of the ward’s nurses. Method: Semi-structured interviews analyzed through qualitative content analysis. Result: Analysis of the data resulted in four main categories describing aspects of good and less good caregiving behavior as perceived by the patients. The categories were: The importance of being seen and validated by the staff, The staff’s attitudes and behavior towards the patients, Wishes and expectations regarding the staff and How non-verbal communication affects the way the care giving behavior is perceived. Conclusion: The most prominent aspect of good caregiving behavior was described as the nurse’s ability to take the time to listen to the patient, and the most prominent aspect of a less good caregiving behavior was described as insufficiency in this ability.

Många människor i Sverige söker sig till psykiatrisk vård för olika psykiatriska åkommor. Ett antal av dessa människor är mycket psykiskt sjuka att de tvingas vårdas inom sluten psykiatrisk vård. Sjuksköterskan som har ansvaret i att administrera läkemedel, som vid tvångsmedicinering, riskerar att bli sedd som fiende och måltavla för våld från patienterna, vilket kan leda till rädsla samt svårigheter att skapa tillit till sjuksköterskan i vårdrelation. Därför är det av vikt att forska och lyfta fram aspekter och perspektiv på hur man ska gå tillväga för att förstå sjuksköterskors upplevelser, erfarenheter av att hantera aggressiva beteenden och utveckla vården så att patienterna kan få optimal omvårdnad. Syftet med studien var att belysa psykiatrisjuksköterskors upplevelser av våld samt beskriva vilka faktorer som påverkar uppkomster av våld inom psykiatrisk vård. Metoden av litteraturstudien var baserad på 15 utvalda vetenskapliga artiklar skrivna mellan åren 2005 och 2015. Databaserna Cinahl, PsycINFO och PubMed användes i sökning av artiklarna. Det gjordes även manuell sökning av artiklar för att få ett större urval, vilket ledde till inkludering av 3 artiklar. Analysen resulterade i fyra teman: Sjuksköterskors upplevelser av uppkomst av aggression och våld inom psykiatrisk vård, sjuksköterskors upplevelser av hur vård och arbetsmiljö påverka patienter och sjuksköterskor, sjuksköterskors upplevelser av tvångsvård och vidtagna åtgärder för att minimera uppkomst av våld samt sjuksköterskors upplevelser av våld. Slutsats: Psykiatrisjuksköterskornas upplevelser av våld vid vård av patienter med aggressivt beteende varierar. Vissa blir stressade, traumatiserade eller rädda och byter därför arbetsplats medan andra uppfattar våldet som en rutin, tillhörande arbetsplatsen och inte reflekterar mer om saken. Våldet påverkar sjuksköterskor både emotionellt och fysiskt vilket kan ge betydande konsekvenser i deras arbetsförmåga och privatliv. För att sjuksköterskor ska kunna utföra sitt jobb effektivt, behövs insyn i arbetsmiljön, stöd och förståelse för sjuksköterskors arbetssituation från arbetsgivaren, ökad kompetens i att hantera och förebygga våld, och möjlighet för reflektion i händelse av incidenter, där även arbetsgivarrepresentant är närvarande. Arbetsgivarna behöver komma med strategier för att behålla sin personal och kunna rekrytera nya genom att erbjuda sjuksköterskor tryggare arbetsplats.

Trots detta är fysisk aktivitet ett ämne och en komponent i behandling som inte prioriteras tillräckligt. Patienter med psykossjukdom lever ofta ett stillasittande liv där isolering är en del av symtombilden. Samtidigt bidrar patienters läkemedelsbehandling med biverkningar som på lång sikt påverkar den fysiska hälsan. Fysisk aktivitet beskrivs kunna bidra till förbättrad fysisk och psykisk hälsa samt bidra till återhämtning. Något som i ett längre förlopp kan påverka en annars dyster prognos med förkortad livslängd relaterad till fysiska hälsoaspekter. Ett sätt att påverka detta är att kontinuerligt arbeta hälsofrämjande med gruppen, vilket är ett ansvarsområde som faller på sjuksköterskan. Syfte: Syftet med studien var att belysa sjuksköterskors erfarenheter av fysisk aktivitet vid omvårdnad av patienter med psykossjukdom. Metod: Studien genomfördes i form av en kvalitativ intervjustudie där analysen skedde med hjälp av induktiv innehållsanalys. Tretton sjuksköterskor verksamma inom psykos öppenvård intervjuades. Resultat: Resultatet presenterades i ett tema, tre kategorier samt åtta underkategorier. Resultatet visade att ett arbete med fysisk aktivitet för gruppen innebär en viktig roll som lyssnare. Samtidigt behöver sjuksköterskor vara lyhörda i mötet. Ett arbete med fysisk aktivitet är tidskrävande då symtombild och övriga omvårdnadsbehov också behöver uppmärksammas. Det är ett arbete som kan bidra till positiva förändringar för patienter vilket i sig kan leda till ett skapande av nya möjligheter i patienters liv. Slutsats: Fysisk aktivitet är en kompletterande behandling som bör få mer prioritet och ges resurser för att utvecklas i omvårdnadsarbetet för patienter med psykossjukdom.<br>Background: Previous research has shown the positive effects that physical activity has in patients with psychosis, highlighting how antipsychotics induce weight gain and increase the risk of patients developing metabolic syndrome. Despite studies clearly demonstrating the importance of physical activity and the positive effect it has on this patient group, physical activity is still low on the agenda in the healthcare treatment and management of psychosis. Patients with psychosis often live a sedentary life, where isolation is a part of the symptomatology. At the same time, prescribed medical treatments often result in side effects that have detrimental effects on the long-term physical health of patients. Recent research demonstrates that physical activity contributes to patients’ physical health as well as their mental health, significantly contributing to their recovery. One way to improve the prognosis of patients with psychosis, often related to poor physical health, is to work with patients to promote all aspects of good health, including physical activity – a role that often fall upon nurses. Aim: The aim of the study was to illustrate nurses' experiences of physical activity in the nursing of patients with psychosis. Method: This is a qualitative study performed using inductive content analysis and is based on the interviews of 13 nurses working in Swedish outpatient care. Results: The result is presented as an overall theme, consisting of three main categories and eight subcategories. The result shows that the promotion of physical activity for nurses involves active and attentive listening, and demonstrates the time-consuming aspect of this work, especially as symptomatology and other needs still needs to be addressed. It is a work that can contribute to both positive change and new opportunities for patients. Conclusion: Physical activity is a complementary treatment that should be given increased priority and resources to further contribute to the improved healthcare for patients with psychosis.

Bakgrund: Lagen om psykiatrisk tvångsvård och Lagen om rättspsykiatrisk vård ger rätt att vårda patienter mot deras vilja. I psykiatrisk vård förekommer specifika omvårdnadsåtgärder som bältesläggning, tvångsmedicinering och avskiljning. Dessa väcker många känslor och kräver att sjuksköterskan arbetar med hänsyn och respekt. Relationen är en central del inom psykiatrisk omvårdnad och ska baseras på tillit och empati. Syfte: Att belysa sjuksköterskors uppfattning om tvångsåtgärders inverkan på omvårdnadsrelationen. Metod: Semi-strukturerade intervjuer med hjälp av en intervjuguide genomfördes med yrkesverksamma sjuksköterskor på en psykiatrisk klinik. Data bearbetades med en kvalitativ manifest innehållsanalys. Resultat: Studien resulterade i sex kategorier; professionellt förhållningssätt, vårdande samtal, tillitsfull relation, tvång som maktmedel, sjuksköterskans uppfattning av patientens känslor och sjuksköterskans känslor. Slutsats: Tvångsåtgärder väckte flera känslor hos sjuksköterskan. Med hjälp av ett professionellt förhållningssätt och vårdande samtal skapades en tillitsfull relation med patienten. Detta medförde att tvångsåtgärderna inte hade en negativ inverkan på omvårdnadsrelationen.<br>Background: The Compulsory Psychiatric Care Act and the Forensic Psychiatric Care Act gives the right to treat patients involuntarily. Specific nursing interventions in the form of restraint, coerced medication and seclusion occur in psychiatric care. These evoke a lot of emotions and require that the nurse work with consideration and respect. The nurse-patient relationship is an essential part within psychiatric nursing and should be based on trust and empathy. Aim: To illuminate nurses perception of coercive measures impact on the nurse-patient relationship. Method: Semi-structured interviews with the help of a topic guide were conducted with nurses on a psychiatric clinic. The data were processed through a qualitative manifest content analysis. Results: The study resulted in six categories; professional approach, caring dialogue, trusting relationship, coercion as an instrument of power, the nurse´s perception of patients emotions and the nurse´s emotions. Conclusion: Coercive measures raised several emotions within the nurse. With the use of a professional approach and a caring dialogue a trusting relationship was created with the patient. This meant that coercive measures didn't have a negative impact on the nurse-patient relationship.

Bakgrund:  Psykiatrisjuksköterskors yrkesidentitet upplevs som komplex och otydlig då den baseras både på sjuksköterskans egna värderingar om sig själv och sin yrkesroll samt på yttre förväntningar. Tidigare studier har visat att yrkesmässig handledning har betydelse för sjuksköterskors upplevelser av arbetstillfredsställelse. Forskningen är dock bristande gällande handledningens inverkan på psykiatrisjuksköterskors yrkesidentitet.  Syfte: Syftet med studien var att beskriva psykiatrisjuksköterskors upplevelser av yrkesmässig handledning och dess betydelse för deras yrkesidentitet inom psykiatrisk heldygnsvård.               Metod: Studien genomfördes med en kvalitativ design där åtta psykiatrisjuksköterskor intervjuades med semistrukturerade intervjuer. Resultatet analyserades genom kvalitativ innehållsanalys med induktiv ansats.                     Resultat: Resultatet presenteras genom följande tre huvudkategorier och sorteras utifrån Imogene King´s systemteori: Att formas som psykiatrisjuksköterska – personligt system, Att sträva mot samma mål – interpersonellt system samt Handledningens betydelse för sjuksköterskeprofessionen – socialt system.  Diskussion: Resultatet diskuteras utifrån Imogene King´s systemteori samt relevant vetenskaplig forskning.<br>Background: The professional identity of psychiatric nurses is perceived as complex and obscure as it is based both on the nurse´s own values of herself and her professional role as well as on external expectations. Previous studies have shown that clinical supervision is significant for nurses´ experiences of work satisfaction. However, the research is insufficient concerning the impact of clinical supervision on the professional identities of psychiatric nurses.                      Aim: The aim of the study was to describe psychiatric nurses´ experiences of clinical supervision and it´s significance for their professional identity within psychiatric inpatient care units.                    Method: The study was carried out with a qualitative design where eight psychiatric nurses were interviewed through semi-structured interviews. The results were analyzed through qualitative content analysis with an inductive approach.                 Results: The results are presented through the following three main categories and are sorted according to Imogene King´s theory of systems: To be formed as a psychiatric nurse – personal system, To aim for the same goal – interpersonal system and The significance of clinical supervision for the profession of nurses – social system. Discussions: The results are discussed according to Imogene King´s theory of systems as well as relevant scientific research.

Bakgrund:Sjuksköterskors huvudsakliga mål är att respektera patientens integritet och självbestämmande i vården samt att ge omvårdnad så att patienten känner sig trygg. Inom psykiatrisk omvårdnad ska sjuksköterskor dessutom ha fördjupade kunskaper för att främja hälsoresurser hos patienter samt tillgodose patientens omvårdnadsbehov. Sjuksköterskor betonar behovet att förtydliga målen inom psykiatrisk omvårdnad och enligt Socialstyrelsen behöver den psykiatriska vården förbättras. Syfte:Syftet med studien är att beskriva brister relaterat till sjuksköterskans utförande av omvårdnad inom psykiatrisk vård för att förebygga uppkomsten av vårdlidande hos patienten. Metod:En litteraturöversikt bestående av tio vetenskapliga artiklar hämtade från databaserna Cinahl och Pubmed. Artiklarna lästes igenom, analyserades, bearbetades och flera kategorier bildades. Den teoretiska utgångspunkten är begreppet vårdlidande som tagits upp av Katie Eriksson. Resultat:Två huvudkategorier av brister med respektiva subkategorier framkom. Brister som direkt relateras till omvårdnaden av patienter med subkategorier: brister i vårdrelationen, brister i kommunikationen och brister i bemötandet samt brister som indirekt relateras till omvårdnaden av patienter med subkategorier:brister i resurser, brister i gemensam vårdfilosofi och brist på kunskap. Diskussion:Litteraturöversikten visar på att det finns brister i sjuksköterskors utförande av omvårdnad. Bristerna har sin grund i att sjuksköterskor har svårigheter att skapa vårdrelationer med patienter, bristande kunskaper och ointresse samt oklarheter kring sin yrkesroll. Sammantaget ger detta patienten ett lidande i stället för hälsobefrämjande omvårdnad.<br>Background:Nurses' primary goal is to respect patient integrity and autonomy in health care and provide nursing care so the patient feels safe. Nurses within psychiatric care should also have more knowledge to promote health resources for patients and meet patient needs of care. Nurses need to clarify their goals within psychiatric nursing and according to Socialstyrelsen the psychiatric care needs to be improved. Aim:The aim of the studie is to describe the deficiencies in the nurse’s performance of nursing within psychiatric care to prevent the rising of the patient´s care suffering. Method:A literature review based on ten peer-reviewed scientific articles gathered from the databases Cinahl and Pubmed describing deficiencies in nursing care based on patients' and nurses' experiences. Katie Eriksson’s concept of care suffering was used as a theoretical basis. Results:Two main categories of deficiencies with respective subcategories emerged. For the first, deficiencies directly related to the nursing care of patients with subcategories: deficiencies in relationship in care, deficiencies in communication and deficiencies in hospitality and for the second, deficiencies indirectly related to the nursing care of patients with subcategories: deficiencies in resources, deficiencies in common care philosophy and deficiency in knowledge. Discussion:The lliterature review shows that there are deficiencies in nurses' performance of nursing. These deficiencies are based on nurses’ difficulties in creating care relationships with patients, lack of knowledge and disinterest and confusion about their professional role. Overall, it gives the patient care suffering instead of health care promoting.

Abstract:
Aim: The aim of this study is to illuminate psychiatric nurses' experiences of suicide assessment in their daily work of a psychiatric inpatient ward. Background: The majority of all suicides are committed by people with some kind of psychiatric diagnosis and suicides within psychiatric hospital departments are increasing. In more than half of the cases where patients have committed suicide in the psychiatric hospital, the patient's suicidal risk assessment has failed and the patient has been able to leave the department and commit suicide. Previous research suggests that suicide risk assessments are important and nurses need more skills and time to be able to pay attention to patients' behavior. Design: This is a qualitative descriptive study based on eight individual semi-structured interviews that have been analyzed using qualitative content analysis by Lundman and Hällgren Graneheim (2012). Method: Data analysis was based on Lundman and Hällgren Graneheim (2012) description of qualitative content analysis. Semi-structured interviews with the addition of narrative questions was conducted. The participants were eight registered psychiatric nurses from four different psychiatric departments at a hospital in Västra Götaland. A male psychiatric nurse and seven female psychiatric nurses participated with at least one years of experience participated. Results: The results showed that the psychiatric nurses had difficulties defining a general suicide behavior. On the other hand, different behavior patterns were identified as more suicidal. Such patterns could be rapid twists in emotional state, strong anxiety, impulsivity and backwardness. Three categories emerged during the analysis process: Clinical Glance, Alliance Work and Experiences Collaboration and Knowledge, and all three had three related subcategories. Conclusion: The suicidal process is complex and the difficulty lies in identifying where in the process the patient is. Psychiatric nurses have responsibility identifying early signs considered by psychiatric nurses to be an important part of suicide assessment. Experience about the patient and knowledge of suicide is something that psychiatric nurses experience as significant to be able to make good suicide risk assessments.
