Abstract:
This chapter discusses the aetiology of depression. It begins with an overview of reasons why the aetiology of depression has been and still is difficult to define, including its heterogeneity and the wider philosophical and methodological challenges involved. It then considers life events and social determinants of depression as well as psychological factors relevant to depression, including childhood development and early experiences, parenting style, and personality and personality disorders. It also examines cognitive behavioural theories of depression, neuropsychology, and psychodynamic theories, along with the role of unconscious processes and ego psychology in the aetiology of depression. Finally, the chapter describes the biological determinants of depression, paying attention to genetics, neurochemical changes in the brain, and stress and thyroid hormones.

Abstract:
Of the many composers in the Western classical tradition who celebrated the marriage between psyche and sound, those explored in this book followed the lines diverging from Wagner in philosophizing the nature of desire in music. This book offers two new theories of tonal functionality in the music of the first half of the twentieth century that seek to explain its psychological complexities. First, the book further develops Riemann’s three diatonic chord functions, extending them to account for chromatic chord progression and substitution. The three functions (tonic, subdominant, and dominant) are compared to Jacques Lacan’s twin concepts of metaphor and metonymy, which drive the apparatus of human desire. Second, the book develops a technique for analyzing the drives that pull chromatic music in multiple directions simultaneously, creating a libidinal surface that mirrors the tensions of the psyche found in Schopenhauer, Freud, and the post-Freudians Lacan, Lyotard, and Deleuze. The harmonic models are tested in psychologically challenging pieces of music by post-Wagnerian composers. From the obsession with death and mourning in Suk’s Asrael Symphony to an exploration of “perversion” in Strauss’s Elektra, from the post-Kantian transcendentalism of Ives’s Concord Sonata to the “Accelerationism” of Skryabin’s late piano works, and from the Sufi mysticism of Szymanowski’s Song of the Night to the failed fantasy of the American dream in Copland’s The Tender Land, the book cuts a path through the dense forests of chromatic complexity and digs deep into the psychological makeup of post-Wagnerian psychodynamic music.

Abstract:
Chapter 3 addresses the implications of age and developmental stage of the youth on the therapist’s behavior and how the therapist conducts treatment. The authors provide a theoretical overview across development as it relates to child and adolescent anxiety psychodynamic psychotherapy (CAPP) (Piaget, Bowlby, Erikson, Mahler, Winnicott, among others), and the authors explain how making therapeutic contact is adjusted to the stage related competence for understanding. Particular attention is paid to language, development of communicative abilities in general, and the capacity to treat nonverbal play as well as verbal story telling symbolically, by providing a framework built on the most prominent developmental theorists and researchers (Vygostky, Bowlby, Stern, Fonagy, Nelson, among others). The capacity for empathy and reflection across the developmental span is explored. Examples of encounters with children of all ages, school-aged children, preteens, and teenagers are offered and prefaced by comments about their respective developmental uniqueness and how this affects communication between child and therapist.
