Full textENGLISH ABSTRACT: This paper aims to provide an understanding of the psychodynamic implications of battering on the victims of this form of abuse. Three dominant approaches to trauma (one descriptive, one explanatory and one phenomenological) are briefly discussed. Available empirical data is then explored to ascertain whether the empirical research correspond to these dominant theories. The research indicates that the theories all highlight different aspects of battering and all have important implications for treatment.
AFRIKAANSE OPSOMMING: Hierdie werkstuk poog om die psigodinamiese implikasies van vroue mishandeling te verstaan. Drie dominante modelle van hierdie vorm van trauma (een beskrywend, een verduidelikend, en een fenomonologies) word kortliks bespreek. Full textENGLISH ABSTRACT: Violence against women, commonly known as women abuse, is currently evolving into a serious social problem. In South Africa thousands of women are victims of violence inflicted by their intimate partners. Although violence against women is a serious violation of human rights, it is often not recognised as such. Violence against women in intimate relationships often results in women experiencing serious physical, psychological and reproductive health problems. When living or trapped in abusive relationships, women are exposed to injuries, ailments and diseases, wounds, chronic pain, infertility and internal bleeding. In spite of the fact that governments, including the South Africa government, have committed themselves to addressing women abuse through legislation, large numbers of women still experience extraordinary high levels of violence. However, many women neglect to report incidents of violence. Many women remain in such relationships, legislation aimed at protecting their rights not withstanding. The aim of this study was to explore why women neglect to report incidents of violence and to explore women's experiences of violence. Seven coloured women between the ages of 21 and 38 years and living in a small rural town in the Western Cape participated in the study. In-depth interviews were used to obtain information from these women. The extent of the problem of women abuse worldwide and in South Africa is illustrated by research conducted internationally as well as locally. Radical feminism is used as a theoretical framework to interpret the causes of women abuse, the 'acceptance' of women abuse by the community and society and the reasons why women choose not to report incidents of abuse. The analysis and the interpretation of the data is done against the background of social practices embedded in the patriarchal structuring of society. It was found that women's experiences and lives are predominantly structured in terms of their sex and that women comply with expectations set by their intimate partners, family and friends. In order to maintain their relationships, secure a household income and to protect their children against poverty, women remain silent about the abuse they suffer. The study also found that women's responses to abuse relate to the sources and options available to them.
AFRIKAANSE OPSOMMING: Geweld teen vroue wat algemeen bekend staan as vrouernishandeling, is tans besig om in 'n ernstige sosiale probleem te ontwikkel. Binne die Suid-Afrikaanse konteks is duisende vroue die slagoffers van geweld binne intieme verhoudings. Alhoewel vrouernishandeling 'n ernstige skending van menseregte is, word dit nie altyd as sodanig erken nie. Mishandeling binne intieme verhoudings lê dikwels ten grondslag van vroue se fisiese, sielkundige en reproduktiewe gesondheidsprobleme. Vroue wat vasgevang is in sulke verhoudings word blootgestel aan beserings, kwale en siektes, wonde, kroniese pyn, onvrugbaarheid en interne bloeding. Ten spyte van die feit dat verskeie lande se regenngs, insluitende Suid-Afrika, hulself verbind het tot 'n stryd teen vrouernishandeling met behulp van wetgewing, ervaar groot getalle vroue wêreldwyd nog steeds buitengewone vlakke van geweld. Baie vroue versuim egter om insidente van mishandeling te rapporteer. Baie vroue bly in intieme verhoudings ten spyte daarvan dat wetgewing wat gerig is op die beskerming van hul regte in plek is. Die doel van hierdie studie was dus om vas te stel waarom vroue versuim om insidente van mishandeling te rapporteer en om insig te verkry in die ervaringswêreld van mishandelde vroue. Sewe Kleurling vroue tussen die ouderdomme van 21 en 38 jaar en woonagtig in 'n klein plattelandse dorpie in die Wes-Kaap het aan die studie deelgeneem. 'n Kwalitatiewe benadering is gevolg en data is met behulp van in-diepte onderhoude ingesamel. Die omvang van vrouernishandeling wêreldwyd en binne die Suid-Afrikaanse konteks word geïllustreer aan die hand van internasionale en plaaslike navorsing oor die verskynsel. Radikale feminisme word as teoretiese vertrekpunt geneem in die interpretasie van die oorsake van vroumishandeling, die 'aanvaarding' van die verskynsel deur die gemeenskap en samelewing en die redes waarom vroue nie insidente van geweld rapporteer nie. Die analise en interpretasie van data is gedoen teen die agtergrond van sosiale praktyke binne die konteks van patriargale strukturering van die samelewing. Die studie bevind dat vroue se ervarings grootliks gestruktureer word in terme van hul geslag en dat hulle hul gedrag in ooreenstemming bring met die verwagtings van hul intieme maats, vriende en familie. Ten einde hulle verhoudings in stand te hou, 'n huishoudelike inkomste te verseker en hul kinders teen armoede te beskerm, besluit vroue om 'n stilswye rondom hul mishandeling te handhaaf. Die studie toon dat vroue se response verband hou met die bronne en opsies wat tot hul beskikking is.

Full textENGLISH ABSTRACT: This study investigated the outcome of cognitive restructuring for adult survivors of childhood sexual abuse in terms of clinically significant change. Twenty-six participants were assessed for depression, State anxiety, State anger, State guilt and self-esteem before and after 10 weekly sessions of group Rational-emotive behaviour therapy and at follow-up after 8 weeks. In contrast to a previous analysis of the data in terms of statistical significance indicating significant improvements on all variables from pre- to post-treatment, an analysis based on clinically significant change showed a differentiated treatment effect. Cognitive restructuring was found to be highly effective in facilitating recovery on measures of anxiety, depression and anger, but less effective for guilt and self-esteem. Only 3 participants (11.54%) recovered on all five variables, while 10 participants (38.46%) showed recovery on at least four variables. Relationship to perpetrator and pre-treatment irrational evaluative beliefs (measured by means of the Survey of Personal Beliefs) were found to be the best predictors of treatment outcome. A poor response to treatment was associated with the perpetrator being a close family member and with more Otherdirected Shoulds, while recovery was associated with the perpetrator being a friend or stranger and with more Awfulizing, Self-directed Shoulds and Negative Self-worth beliefs.
AFRIKAANSE OPSOMMING: In hierdie studie is die uitkoms van kognitiewe herstrukturering, as behandelingsprosedure VIr volwasse slagoffers van kindermolesteuring, in terme van kliniese betekenisvolheid ondersoek. Sesen- twintig deelnemers is voor en na behandeling (10 weeklikse groepsessies van Rasioneelemotiewe gedragsterapie) en by opvolg na 8 weke in terme van depressie, Staat-angs, Staat-woede, Staat-skuldgevoelens en selfwaarde geassesseer. In 'n vorige analise van die data in terme van statistiese betekenisvolheid is betekenisvolle verbetering na behandeling op al die veranderlikes gevind. 'n Heranalise van die data in terme van kliniese betekenisvolheid toon egter 'n gedifferensieerde behandelingseffek. Daar is gevind dat kognitiewe herstrukturering hoogs effektief was om herstel in terme van angs, depressie en woede te fasiliteer, maar minder effektief was ten opsigte van skuld en negatiewe selfwaarde. Slegs 3 patiente (11.54%) het klinies betekenisvol op al vyfveranderlikes verbeter, terwyl 10 pasiente (38.46%) op minstens vier veranderlikes herstel het. Die pasient se verhouding tot die molesteerder en irrasionele kognisies voor behandeling (gemeet met die Survey of Personal Beliefs) was die beste voorspellers van behandelingsuitkoms. 'n Swak respons op behandeling het verb and getoon met 'n naby familielid as molesteerder en met meer Ander-gerigte Eise, terwyl herstel geassosieer het met 'n vriend of vreemdeling as molesteerder en met die irrasionele kognisies van Katastrofering, Self-gerigte Eise en Negatiewe Selfbeoordeling.

Full textENGLISH ABSTRACT: Seeing that breast cancer is the most common cancer in women and that it has such a high incidence that every woman should be aware of it, time has come for the theology, and especially for pastoral care, to provide some answers on the possible problems that accompany the illness. Treatment of breast cancer almost always include a degree of surgical intervention through which part of or a whole breast, and in extreme cases even the underlying tissue, is being removed. This procedure, to some extent, influences a woman's experience of her physique which in turn has an effect on the intimate space of her marriage. The mastectomy patient may experience that the operation, with all the psychological affects it has, disturbs her inner life rythm and that results in her relationships also being pressurised. The crisis of a mastectomy should be handled as a relationship crisis within a marriage, in which the husband can play an important therapeutic role. The husbands contribution will be greatly effected by: a) His commitment to the relationship and b) The correlation between his male identity, his sexuality and his perceptions of the female physique. Within the crisis of mastectomy, the woman's body-image should not be separated from her faith identity and the quality of her marriage - there exists a dynamic interaction between these three, which means that the handling of the crisis of a mastectomy is directly dependent on the quality if her loving relationship and on the way in which she experiences her faith. The faith factor plays an important role in the handling of the crisis, by creating a distance between the trauma of the loss and the way in which she experiences her identity. Pastoral care can play a vital role in the handling of the crisis of a mastectomy by means of guidance and support, as it proclaims the active presence of God through the marrid body of Jesus Christ. The husband of the mastectomy patient can, in his therapeutic role, be guided to be much more sensitive for the emotional needs of his wife, in regard to aspect of sexuality. This would give her the support she needs and work constructively towards the goal of healing.
AFRIKAANSE OPSOMMING: Aangesien borskanker die algemeenste kanker onder vroue is en dit sulke geweldige afmetings aangeneem het dat elke vrou daarmee moet rekening hou, het dit tyd geword dat die teologie, en met name die pastoraat, ditself ook ten opsigte van die moontlike problematiek wat daarmee saamgaan, moet verantwoord. Die behandeling van borskanker behels in die meerderheid van gevalle 'n mate van chirurgiese ingrepe waardeur 'n deel van of die hele bors, en in ekstreme gevalle ook die onderliggende weefsel, verwyder word. Hierdie prosedure oefen, in 'n mindere of meerdere mate, 'n invloed op die vrou se belewing van haar liggaamlikheid uit wat weer na die intieme band van die huwelik kan deurwerk. In die verwerking van die krisis vervul die geloofsfaktor 'n deurslaggewende rol om afstand tussen die emosionele trauma van die verlies en die ervaring van identiteit te skep. Die pastoraat kan 'n belangrike funksie vervul ten opsigte van begeleiding en ondersteuning in die verwerking van die krisis van 'n mastektomie deurdat dit God se aktiewe betrokkenheid by die situasie, aan die hand van die liggaamlikheid van Jesus Christus, verkondig. As terapeut kan die eggenoot begelei word om, op die gebied van die seksuele, baie sterker op die emosionele behoeftes van die mastektomiepasiënt ingestel te wees, om haar sodoende te ondersteun en in die proses van heling mee te werk.
