Full textAnalysis of personality changes always was important field of clinical psychology. It has a special meaning in modern times, as global changes depend not only on circumstances, but on the changes of individual too. Inner factors such as pathological conditions and inner personality structures are analyzed in the perspective of personality transformation. Depth presumptions of personality changes, outer factors and psychotherapy process are important part of our research which is represented for the habilitation procedure. Special attention is paid for trauma experience and its healing process. Effective psychotherapy depend on successful therapeutic relationship. Conditions of formation, structure and processes of therapeutic relationship are one more subject of our research. We discuss some methodological questions in our publications (case analysis and scientific research, combination of quantitative and qualitative analysis)- those questions are new in Lithuanian clinical psychology. Personality changes and its research is interesting and perspective field – we suppose its development could include further research of effectiveness of psychotherapy and dynamic of personality identity.

Full textLittle is known about predictors of treatment selection for survivors of intimate partner violence (IPV). This study aimed to identify the relationship between patient demographics, positive cut-off score of PTSD, anxiety, depression, and somatization, number of lifetime traumas, diagnosis, length of treatment, and clinician’s selected evidence-based therapy (EBT). A secondary data analysis of 319 adults who sought services at a community mental health clinic who reported IPV was conducted. Neither Patient demographic characteristics or screen positive cut-off score for PTSD, anxiety, depression, or somatization was indicative of selected EBT. Patient demographic characteristics of age, female gender, race/ethnicity, employment disability, marital status, and education were found to be significant pre-treatment predictors of attending more sessions of treatment. There was no association between clinician’s treatment selection of EBT and patient diagnosis or patients number of lifetime traumatic experiences. A dramatic need for future research regarding predictors of selected EBT are discussed.

Full textA literatura evidencia a escassez de estudos que caracterizem o processo terapêutico em psicoterapia existencial. Neste trabalho, procedemos à caracterização dos segundos seis meses de quatro processos terapêuticos de orientação existencial com duração de aproximadamente 48 sessões. Como medida de processo utilizamos o PQS (Psychotherapy Process Q-set), procedendo-se à cotação das sessões através da audição das mesmas. Como medida de outcome utilizamos o CORE-OM (Clinical Outcome in Routine Evaluation – Outcome Meassure), com medições no pré e póstratamento. Os dados emergentes da análise das quatro díades sugerem concordância com os fundamentos da psicoterapia existencial. Verifica-se uma atitude empática, responsiva e envolvida por parte dos terapeutas, numa atmosfera marcada pela compreensão e aceitação do modo de ser dos pacientes. Verifica-se o enfoque nas situações actuais da vida dos pacientes, remetendo-nos para o aqui e agora. Os resultados revelam a existência de indicadores de mudança em duas das quatro díades analisadas, assinalando-se a manutenção da atitude dos terapeutas nos dois grupos de processos. Nos processos terapêuticos com indicadores de mudança, assinala-se a existência de uma relação terapêutica com baixa expressão da ambivalência, onde se evidencia um sentimento de confiança e segurança, indo de encontro aos fundamentos da psicoterapia existencial. Nos processos sem indicadores de mudança, verifica-se uma atitude defensiva por parte dos pacientes, fazendo-nos reflectir sobre o papel da aliança terapêutica como factor facilitador da mudança psicoterapêutica. Discutimos o papel do terapeuta, do paciente, e da relação terapêutica, estabelecendo os paralelismos necessários entre a investigação em psicoterapia e os fundamentos da psicoterapia existencial.
ABSTRACT: There is a scarcity of studies that characterize the therapeutic process in existential psychotherapy. In this work, we characterize the second six months of four therapeutic processes of existential orientation, lasting approximately 48 sessions. We used PQS (Psychotherapy Process Q-set) as process measures, and produced quotations by listening to the sessions. As outcome measure we used CORE-OM (Clinical Outcome in Routine Evaluation – Outcome Measure), with pre- and post-treatment measures. The data that emerged from the analysis of the four dyads suggest concordance with the fundaments of existential psychotherapy. We verified an empathetic, responsive and committed attitude by the therapists, in an atmosphere marked by understanding and acceptance of the patient's way of being. There was a focus on the current situation of the patient's lives, referring them to the here and now. The results revealed the existence of change indicators in two of the four analyzed dyads. with a maintenance of therapist attitudes in the two groups of processes. In the therapeutic processes with change indicators, we point out the existence of a therapeutic relationship with low expression of ambivalence, with an evident sentiment of trust and security, in accordance with the fundaments of existential psychotherapy. In the processes without change indicators, the patients had a defensive attitude, leading us to reflect upon the role of the therapeutic alliance as a facilitating agent of psychotherapeutic change. We discuss the role of the therapist, the patient and the therapeutic relationship, establishing the necessary parallels between research in psychotherapy and the fundaments of existential psychotherapy.

Full textENGLISH ABSTRACT: Sexual relationships in the psychotherapeutic setting have always been regarded as unethical and sexual attraction between therapist and client has generally proved to be awkward and anxiety provoking for those involved. Although research indicates that these relationships do occur, and that sexual feelings between therapists and clients are not infrequent, there is little conceptual clarity on the dynamics thereof. Consequently, the available guidelines and ethical rules regulating such sexual contact generally take the form of “thou shalt not…” and provide limited assistance to therapists in managing such situations in a manner that protects the integrity of the therapeutic relationship. This review assumes the presence of the psychodynamic phenomenon of transference in all psychotherapeutic relationships, and constructs sexual attraction and contact as the product of the sub-concept of erotic transference. Against this background, the review presents the dynamics of erotic transference in terms of its nature, impact and management. On this basis, current ethical codes and training issues are reviewed, guidelines for proper prevention and management are suggested and important directions for future research are identified.

Full textThis study traces the description of Paradox as a therapeutic measure by numerous therapists since Erikson first used the paradox in the form of positive reframing during the thirties. Major theoretical definitions of the paradoxical communication are reviewed. The therapeutic Paradoxical intervention is categorized into a series of relatively circumscribed intervention strategies, each of which shares theoretical similarities but emphasizes different aspects of the paradox. The paradoxical intervention has been utilized in a number of ways depending on the epistemological premise of the therapist, and the theory of change accepted within this epistemology. The study initially discusses the paradoxical intervention as designed as an isolated intervention within a linear cause and effect paradigm. Within a context of power and control, the paradoxical intervention was used by therapists of the Mental Research Institute (Watzlavick et a1. 1977) to emphasize and maintain the hierarchical distinction between the therapists and patient. The Strategic approaches including those of Fisch, Weakland and Segal (1982) Haley (1963) and Papp (1979) involve direct and planned influence of persons and are based, in large part, on an analysis of the power relationships that they aim to modify (Golann 1988.) According to Bogdan (1982) the Milan approach even with its systemic circular approach, involves the use of the paradoxical intervention as a means of simultaneously influencing the behavior of all the members of a family. The paradoxical intervention is discussed within a circular cybernetic paradigm following De Shazer's (1982) explanation of a therapeutic intervention. De Shazer (1982) sees the paradoxical intervention as one type of intervention within a pattern of isomorphic interventions, and stresses the necessity for this intervention to follow the family interaction patterns. De Shazer developed the concept of Isomorphism and used this as a prescriptive tool for guiding the therapists efforts to promote change, by intervening from a different angle, based on the description of family patterns. Where the family's interactive patterns are paradoxical in nature, a paradoxical intervention would be appropriate. With this emphasis on a more encompassing pattern of interventions based on patterns of client-family interactions, the significance of the feedback processes within the relationship between therapist and client in the design and utilization of the paradoxical intervention is stressed. The study concludes that the psychotherapeutic relationship between the therapist system and the client system can be seen to contextualize and provide meaning to the paradoxical intervention.

Full textPrevention, in Mental Health, encloses three important levels: Primary, Secondary and Tertiary. These three levels consubstantiate three interventions, each with specific characteristics. I served as apprentice academic for the Masters Degree in Nursing, with specialization in Mental Health and Psychiatry which presents three modules with different intervention levels: Promotion of healthy life styles, Diagnose; Development of competencies in units of adult’s intervention, and one last optional module. Module I, was developed in an Integrated Project of Community Support which belonged to IDT; module II occurred in Santo Tirso/ Trofa, location site for Magalhães Lemos Hospital; the last, and optional module, was performed in the Infancy and Adolescent Psychiatrics Department of Centro Hospitalar do Porto. For each module, where developed specific goals, with activities of support, and described which competencies were developed. Being adolescence a period of transition for adult life, it is also a period for experimenting, taking risks, and also be exposed to vulnerabilities. It is important for a specialised nurse to know how to promote healthy life styles, be able to intervene early in Mental Health, and also to promote the modification of risk factors and protecting factors, never forgetting family as a major importance factor to protect the patient. During an intervention for treatment of complex situations, it is important for the nurse to know how to establish a therapeutic relationship and a relationship of aid with the patient and its family and being able to perform Psychotherapeutic Interventions adapted to the patient needs which can occur in a group context, individually, and also in its family.

Full textEquine-Assisted Psychotherapy (EAP) is a form of Animal Assisted Therapy (AAT) used to treat individuals’ psychological problems. EAP is an interactive process in which a licensed mental health practitioner, a credentialed equine professional and suitable equines work together to address psychotherapy goals. Since the 1990s, research on EAP has grown rapidly in the United States of America (USA) and Europe, however research in the South African context is lacking. This descriptive and exploratory research study explored and described the experiences of licensed mental health practitioners and/or credentialed equine specialists who have included EAP within their practise by focusing on the role that equines play within the psychotherapeutic process. The knowledge of this study was derived from a Constructivistic epistemology. Constructivism argues that humans attain and build knowledge and meaning from their experiences. Participants were selected based on purposeful criterion sampling. Only licensed mental health practitioners or credentialed equine specialists who included EAP within their practise, registered at the Equine-Assisted Psychotherapy Institute of South Africa (EAPISA) or the Equine-Assisted Growth and Learning Association (EAGALA) were considered for participation. Semi-structured, face-to-face interviews were conducted with six participants. A post-interview follow-up email was used to gain additional feedback from each participant. Finally, data was analysed using thematic analysis. As EAP is a fairly new modality of therapy within South Africa, the contribution of this study is to add to the growing body of professional EAP literature. Thematic analysis identified seven main themes: shifting dynamics in the therapeutic relationship, setting the scene for storytelling, the equine as an intermediary and therapeutic tool, what the equine sees is what you get, instantaneous but lasting results, variations and similarities in approach and activities, the emotive motives of EAP practitioners. These themes are discussed in relation to the research findings. Findings of this study showed substantial agreement in what the equine brings to the psychotherapeutic session: unique equine attributes, opportunities for metaphorical learning, and relational aspects. The participants demonstrated a consensus in the belief that EAP can be beneficial to a large spectrum of psychotherapeutic populations in South Africa. A recommendation for future research is for larger, international studies that explore the viewpoints of psychotherapists who practice equine therapies in other countries in order to expand the knowledge base and address the competency and standards discussion in the EAP field.
