Full textBakgrund: Suicid är ett stort problem i Sverige då det är den vanligaste dödsorsaken i åldersgruppen 15-44 år. Varje år dör ca 1500 människor i Sverige till följd av suicid. Sjuksköterskan spelar en viktig roll när det gäller omvårdnaden av suicidnära patienter. Trots detta finns det få studier som belyser mötet med suicidnära patienter ur sjuksköterskans perspektiv. Syfte: Syftet med studien var att beskriva sjuksköterskors uppfattningar om vad som är viktigt i mötet med suicidnära patienter. Metod: För att beskriva vad som är viktigt i sjuksköterskans möte med suicidnära patienter valdes en kvalitativ ansats med intervjuer som datainsamlingsmetod. Fem sjuksköterskor inom psykiatrin intervjuades. De transkriberade intervjuerna analyserades genom kvalitativ manifest innehållsanalys. Resultat: Två huvudkategorier framkom ur analysen: Viktiga faktorer i mötet med suicidnära patienter och Faktorer som försvårar mötet med suicidnära patienter. Ur resultatet framkom flera viktiga faktorer i mötet med suicidnära patienter. Sjuksköterskans bemötande visade sig ha en stor betydelse för den suicidnära patientens omvårdnad. Slutsats: I studien framkom det tydligt att hopp, lyhördhet, närvaro, bekräftelse och tydlighet är viktiga faktorer i mötet med suicidnära patienter. Sjuksköterskan kan genom ett bemötande som präglas av dessa faktorer skapa en relation där patientens hälsa och välbefinnande kan främjas.

Full textIt is common in most areas of psychiatric healthcare that nurses come into contact with threatening and aggressive patients. In order to ensure that healthcare units shall be able to meet the needs of patients and at the same time be certain that the work environment is safe for nurses, it is important to precision what threats and violence actually mean. If threatening and violent situations can be identified before they lead to physically or psychologically damaging encounters, this will save the both patients and nurses unnecessary suffering. The Purpose: was to bring to light nurses’ definitions and experiences of threats and violence and at the same time describe how they perceive daily working enviroment within psychiatry. The Method: the work was built on twenty nurses presently working in various psychiatric clinics. We made use of a data collection method that is built upon semi-structured interviews where we there after, in a sytematic and step by step process, classified the data through qualitative content analysis in order to more easily identify and condense pertinent sentence concepts. We created codes which were then placed into three categories. The Results: are split up into categorys: The precision of threats of violence, the experience of threats, and violence in the workplace. These categories are presented by the codes: threat, violence, sexual harassment, direct threats, the effects of threats, violence in combination with compulsory psychiatric care, the consequences of threats and violence, the acceptance of threats and violence, safety measures, the psychiatric nurses’ work enviroment, and the support of the nurses’ colleagues and the care organisation, were made apparent during the interviews. The Discussion: The majority och nurses enjoy their job and feel safe working at psychiatric clinics and wards, despite that incidents of threats and violence do occur.

Full textSjuksköterskor inom somatiken möter patienter med psykisk ohälsa i sitt dagliga omvårdnadsarbete. Syftet med studien var att belysa sjuksköterskans upplevelser av omvårdnad av en patient som utfört suicidal handling och vårdas på somatisk akutavdelning/akutmottagning. Författarna använde sig av en kvalitativ metod med intervjuer som datainsamlingsmetod. Tio sjuksköterskor intervjuades som arbetade inom akutsjukvård och hade minst ett års erfarenhet. Resultatet bestod av fyra teman; svårigheter i omvårdnaden, känslor i mötet med patienten, resurser och samarbete med psykiatrin. Resultat visade att allmänsjuksköterskan önskade mer psykologisk grundkunskap för att utveckla sin omvårdnad av den suicidala patienten, med exempelvis samtalsteknik, eftersom de ansåg att kommunikationen med patienten var utmanande. Även ett bättre tillvägagångssätt för att närma sig, bemöta och knyta an i en vårdrelation med patienterna. Samarbetet mellan psykiatrin och den somatiska vården var inte tillfredställande vilket gjorde sjuksköterskans omvårdnadsarbete svårare. Det behövdes en tydlighet i utförandet av omvårdnad och arbetsrutiner för patienter som vårdades inom både somatik och psykiatri.
Nurses’ within the somatic care are encountered with patients with mental illness in their daily work. The aims of the study were to enlighten nurses’ experience of caring for a patient that has committed a suicidal act and are cared for in a somatic acute ward. The authors used a qualitative method with interviews for collecting data. Ten nurses’ were interviewed that worked with acute care and had at least one year experience. The result contains four themes; difficulties with caring, feelings in meeting with the patient, resources and cooperation with psychiatry. Result showed that general nurses’ described a wish for more psychological knowledge to improve their care for the suicidal patient, for example dialogue technique, because they thought that the communication with the patient was challenging. Also a better way to get close, meet and connect in a relationship with the patient. The cooperation between psychiatry care and somatic care were not satisfying which made caring more difficult for the nurse. There were needs to clarify the caring and working routines for patients that are cared for in both the somatic and psychiatry care.

Full textThe basis of the study was that nurses in psychiatry is a profession who are faced with various moral dilemmas in everyday life. It was examined nurses' perceived moral dilemmas within their field which was psychiatric inpatient and outpatient care and public psychiatry to gain a deeper understanding of what experienced psychiatric nurses considered that general ethical dilemmas are and what thoughts they had about potential solutions. Seven nurses with further education in psychiatry were interviewed about their experiences with ethical dilemmas. It was used Interpretative Phenomenological Analysis (IPA) using verbatim transcriptions of the semi-structured interviews. The two main themes which were identified were conflicts of loyalty and cooperation in hierarchical structures. Ethical dilemmas were not experienced by the respondents mostly by the compulsory treatment, respectively coercion - as it could be expected - but in situations where they felt forced to choose between loyalty to the patient, organization or herself as professional. The hardest perceived dilemmas were those that forced the nurses to act against their own values. When emotions were involved, dilemmas were remembered strongest. The working team and the doctor on the ward was considered as part of the solution of moral dilemmas. On the other hand they were experienced equally as triggering factor for moral dilemmas, especially when nurses got aware of the role they play within the hierarchical structure. The study contributes to potential solution approaches to counteract the negative consequences that moral dilemmas entails both for personnel and for patients.

Full textSwedish health care is working with many immigrants and asylum seekers whom are suffering from mental disorders. In the psychiatric care, communication is especially important, and if patients and health care staff do not speak the same language health care staff will front communication barriers. With this qualitative literature study useful communication strategies has been identified as well as important components for good communication. These are: the attitude of the health care staff, information and support and help. It is important that nurses and other health care staff are interested, that they are listening, shows on empathy, and on cultural knowledge within the psychiatric care. Patients wish to get encouraged to keep in contact with friends and to participate in group activities. They need help to develop interests and to see possibilities. When the communication is poor, as in language barriers, important information might be omitted. This is dangerous and sometimes annoying for both health care staff and the patient. Taking care of the patient will take longer time and the quality will be poorer. The most used strategies for communication has been identified as: professional interpreters on sight, professional phone interpreters, bilingual health care staff, and relatives as interpreters, other verbal strategies and non-verbal strategies. Health care staff and patients experience these strategies as effective in different ways. Even though there are professional interpreters, they are not always used due to reason as time deficiency, too little knowledge about how to contact an interpreter and also for financial issues. Often relatives have to interpret, and in these situations important information might be omitted and own thoughts or opinions put forward. Some patients though, prefer the relatives to translate, since they not always trust the confidentiality of the interpreter. As a theoretical framework, Antonovskys’ SOC, Sense of Coherence, has been used in this literature study.

Full textAnorexia Nervosa är en komplex sjukdom som kräver en individualiserad vård. Då patienter med anorexi ofta kännetecknas av att vara ambivalenta angående att tillfriskna, behövs ett gott samarbete och en god relation med sjuksköterskorna. Syftet med studien är att undersöka vad god omvårdnad innebär för patienten och sjuksköterskan vid vård och behandling av Anorexia Nervosa. Arbetet är utformat till en litteraturstudie grundad på tio vetenskapliga artiklar. I resultatet, som är tvådelat, framkommer sju teman vilka är självständighet, socialt stöd, isolering, en utmaning, relationen, omtanke och respekt samt tillit. I resultatet framkom det att patienter prioriterade ett gott stöd från sjuksköterskorna genom att finnas tillhands, visa omtänksamhet samt vara icke-dömande. Sjuksköterskorna ansåg att vägen till god omvårdnad var; empati, tillit, acceptans samt ömsesidig respekt. Slutsatsen blev att en god relation mellan patienten och sjuksköterskan var av stor betydelse för att nå framgång i behandlingen av Anorexia Nervosa. Förhoppningen med denna studie är att sjuksköterskor ska kunna få en inblick och ta lärdom av det presenterade materialet, för att på så sätt förbättra omvårdnaden av flickor med diagnosen Anorexia Nervosa.
Anorexia Nervosa is a complex disease that requires an individual treatment. Patients with Anorexia are known to be ambivalent when it comes to making a recovery and therefore need strong teamwork and a good relationship with the nurse. The aim of this study is to examine what good care means for the patient and the nurse when it comes to treating Anorexia Nervosa. The essay is a literature review based on ten scientific articles. In the result, which is divided in two parts, seven themes arise and these are: independence, social support, isolation, a challenge, the relationship, caring and respect and trust. In the result it was revealed that the patients prioritize support from the nurses, showing that they care and don’t judge the patients. The nurses describe compassion, trust, acceptance and mutual respect as important parts in the given care. The conclusion is that a good relationship between the nurse and patient means a great deal to achieve success when treating Anorexia Nervosa. Hopefully this study gives nurses an ability to learn from the presented material, and be able to improve the care of girls suffering from Anorexia Nervosa.

Full textBackground: The shortage of specialist nurses in psychiatry is great, but the skills are still not required by the employers. Proposals to reduce the number of specialist courses have been made, of which psychiatry is one of them. Objective: The aim of the study was to investigate how nurses assess specialist nurses´ competence in psychiatric care. Method: A quantitative web based study was conducted with a total of 110 nurses with experience from psychiatric care. Results: The specialist registered nurse is judged to have a higher competence than registered nurses, but with equal tasks and responsibilities. Self-esteem among specialists are higher. Experience is of great importance for the competence. Conclusion: The competence of specialist registered nurses was assessed to be higher than that of registered nurses, but work and responsibilities are the same. There is a general need for employers to more clearly utilize the competence of specialist nurses. The visibility of the specific competence can then in the long run result in more psychiatry specialist nurses.

Full textBackground: In Sweden, 14-20 percent of the population has reduced psychiatric health. Self-care is actions done to maintain health and is crucial in psychiatric nursing care. Nurses meet people with reduced psychiatric health in care situations and therefore knowledge about how self-care should be used in the care of the psychiatric patient is important. Aim: The aim of this study was to describe nurses’ experience of how self-care is applied in psychiatric nursing care. Method: A qualitative interview study was conducted in out- and incare psychiatric care in a middle-size Swedish municipality. Nine interviews was transcribed and thereafter analyzed with a conventional content analysis. Result: The nurses was found to work from a professional approach in order to educate and individualize care. They described that the concept of self-care was rarely used but that they ascribed many actions of nursing care to self-care. The nurses described that insight, responsibility, motivation and participation were the determinant abilities of the patient for a functional self-care. Yet, nurses believed that a greater faith for the medication and lack of competence of the care givers contributed to neglection of the patients’ self-care when problems often were short-term solved and the patients remained in psychiatric care. Conclusion: In the group of interviewed nurses, the general opinion was that the concept of self-care not has been embraced in psychiatric nursing care. According to the nurses, it results in patients remaning in psychiatric care during an unjustified long time and that the course of illness becomes extended.

Full textBackground: Mental illness can range from mild anxiety to more serious illnesses such as psychosis, bipolar disorder and more severe depression. Today, primary care has a great responsibility for patients with mental illness. Shortcomings have been identified regarding health care staff not having the appropriate knowledge in order to deal with these patients. Cooperation between different professions is important both inside and outside primary care. A functioning team around the patient is important when creating good care, and the nurse's core competencies are highlighted, which means that the nurse must work towards collaboration in teams and person-centered care. Aim: The aim was to illustrate the nurse ́s coordinating function in the care of patients with mental illness in primary care. Method: Systematic literature study with inductive approach. Results: The results are based on 11 scholarly articles, which illustrate the nurse's caring of patients. The result presents into two categories. Firstly, the meaning of a close collaboration between specialist healthcare and primary care, which also should emerge in the team around the patient. Collaborations also lead to an increase in knowledge about mental illness. It emerges that if the nurse can create continuity and perform follow-up around the patient, a more person-centered care arises. Conclusion: The study shows that care becomes more accessible when the role of the nurse is to provide support in terms of coordinating and continuous care.

Full textBackground: Previous researches shows patients who come in contact with a psychiatric ward after suicide attempt often experience feelings of shame and fear. The first meeting is determined by how the time at the ward develops. If a patient experience not being taken seriously or heard it usually leads to feelings of escaping or withdrawing. Aim: The purpose of the study is based on scientific literature describing patients and nurses' experiences in receiving and providing care after a suicide attempt. Method: Qualitative research review included eight scientific studies from the databases CINAHL and PsychINFO. Data were analyzed with the aid of Evans four steps analysis. Results: The results show that patients have a need to feel that they are being taken seriously in order to alleviate suffering. The result also shows that nurses are experiencing difficulties in meeting with the suicidal patient. The nurse's attitude and commitment to the patient has a central role in the patient's encounter with psychiatric care after a suicide attempt. Discussion: The meeting between the patient and the nurse is reflected in the nurse's attitude and commitment to the patient, by taking the patient seriously suffering can be alleviated and the patient's well being can be increased. Through dedication supported the patient and by the elimination of negative emotions experienced nurse can reach the patient in a way that allows a patient to be inspired to change their lives.
