The purpose of the study was to examine self-reported psychiatric nurse performed tasks and self-perceived competence. The study is based on individually conducted interviews with 19 psychiatric nurses working in psychiatric inpatient ward. The authors have used a proportional stratified sample. The results are presented in five categories; psychiatric nurse competence, specific tasks in the areas of competence, work outside the areas of competence, tasks psychiatric nurse lack and role conflicts and related subcategories. In psychiatry nurse competence includes duties as supervisor and nursing manager to plan the work in the department, care planning, documenting, and pharmaceutical administration. The specific competence in the field proved to be, inter alia, support calls, motivational talks and teaching. The nurses would also have a comprehensive approach and make observations. Tasks outside the competence demonstrated that they are helping the doctors with various administrative tasks, and some counselors work. Tasks that nurse do mainly lack; are more activities for patients, more patients, better cooperation with neighbors and family care, and increased ethical awareness in the nursing care and documentation work. The nurses also experienced role conflict in their work as most were due to various ambiguities in the working group. The authors conclude that a better working instruction, better division of responsibilities between the different professions and a more structured care of patients would reduce the lack of clarity and uncertainty of psychiatry nurses.

Full textBackground: The competence of the specialised nurse is a development of the knowledge and skill obtained in the elementary nursing education. The need for specialised nurses increases but as the work tasks of specialised and general nurses do not differ considerably, this may lead to the specialised nurses leaving the clinical area of work. The requested expertise will then be at risk of disappearing. On these grounds and as a way of clarifying the role of the specialised nurse in psychiatric in-patient care, it is of interest to observe how the specialised nurse is perceived and what the expectations of her competence are. Aim: to describe doctors' conceptions of the competence of the specialised nurse in psychiatric in-patient care. Method: Eight qualitative semi-structured interviews were analysed using a phenomenographic approach. Results: The main findings were summarised in five categories: The role of the specialised nurse is unclear and unseen; The specialised nurse executes special tasks; The specialised nurse possesses in-depth competence; Specialist skill leads to personal development and finally The specialised nurse is an ambassador. Conclusion: There is a need for an increased understanding of the competence of the specialised nurse in psychiatric in-patient care. In order for this to happen a clearer descripton of what it entails to work as a specialised nurse in in-patient care is requested.

Full textObjective:The purpose of this study was that from an ethical perspective illuminate how the psychiatric nurse creates a relationship with the patient in a threatening situation. Method: An empirical study with a qualitative approach. Open interviews with four psychiatric nurses formed the basis for content analysis in accordance with Graneheim and Lundman (2004). Findings: Four themes emerged, i.e. the nurse as a professional caregiver, the nurse as the patient's advocate, the threatening encounter and the ambiguous encounter. Conclusion: The psychiatric nurse creates a relationship with the patient in a threatening situation, explained by Lévinas ethics, by experiencing feelings of compassion and sorrow for the patient's fate, by taking a permanent responsibility approaching the patient in listening and trust, by having support of colleagues in this listening, expanding the responsibility for the patient, the Other and by, when coercion is used, be convinced that it is the patient's best interests. On the one hand the nurse acts in accordance with general patterns, on the other she shows her deep understanding of the Other's otherness, which is expressed in the concept of spirituality: the Infinite according to Lévinas.

Full textAim: The study focuses on advanced trained psychiatry nurse’s experiences of transcultural nursing in psychiatric care. Method: The study was conducted using a qualitative method of individual semistructured interviews. Nurses working in psychiatric care could share experiences of transcultural nursing. Results: In the result, one finding was that nurses perceive differences oflanguage as a barrier in the care, because communication is central in psychiatriccare. Religious and cultural beliefs and practices may also have an impact on the meeting with a patient from a different culture. There is opportunity for enhanced mutual understand when relatives are involved in the care. They can provide more information as well as support to the patient. Interpreters are used to solve communication problems; however they are not engaged as often as they should, due to the cost. Conclusion: That nurses, to a large extent, have been able to take into account patient’s wishes and thereby been able to offer the best possible treatment optionsto the patient. The study finds that nurses in general need a wider knowledge of transcultural nursing.

Full textBackground:Nurses working in psychiatric care experience difficulty in articulating their professional identity. The background describes the role of the nurse in psychiatric care from a historical and contemporary perspective. The background shows that nurses tend to leave the psychiatric care when they feel their skills are not utilized, which in turn also affects the patients. Aim: The aim of the study was to describe how nurses working in psychiatric care understand their professional identity Methods: Five nurses where interviewed with the help of unstructured interviews. A qualitative design was used for the study. Result: The results are presented in the theme a complex professional identity and the three categories: nurturing unique people, to be supervisors and their own development as well as eight subcategories Discussions: The results are discussed from Imogene King's conceptual role, organizations, authority, power, status and stress, and on the basis of gender theory perspective.

Full textBackground: Depression is seen as part of bipolar disorder when the individ suffers from both depressive and manic or hypomanic episodes or as a unipolar syndrome only with depressive episodes. Physical activity is defined as a body movement resulted from the muscles´ activity. It leads to increased energy consumtion. It has been linked to reducing the risk of depression, of isolation and to improving the feeling of well-being. It is not an easy task to motivate elderly to practise physical activity. The nurse has an important role in implementing physical activity in this patientgroup´s care. The psychiatric nurse´s responsiblity is to ensure that patients are offered the support that can help to their recovery. Aim: The purpose of this study is to describe the nurses´ perceptions of physical activity as part of the care of the elderly depressed patients who are treated within psychiatric care. Method: Six semi - structured interviews were done with nurses working on a ward specialized on taking care of elderly suffering from mental illnesses. Inductive content analysis inspired from Granheim och Lundman (2012) has been used to analyse the interviews. Results: Two categories resulted from the analysis: "support based on promoting physical activity" and "health impact". Three subcategories (practical, emotional and social support) are included in the category "support based on promoting physical activity". Subcategories "physical impact", "psychological impact" and "social impact" belong to the category "health impact". Discussions: The results were discussed in relation to Barker´s tidal model (2005) with emphasis on three of his concepts (self-domain, others´ domain and the world domain). The psychiatric nurse has the opportunity to promote physical activity for these patients by offering them practical, emotional and social support.

Full textBackground: Previous research has shown the positive effects that physical activity has in patients with psychosis, highlighting how antipsychotics induce weight gain and increase the risk of patients developing metabolic syndrome. Despite studies clearly demonstrating the importance of physical activity and the positive effect it has on this patient group, physical activity is still low on the agenda in the healthcare treatment and management of psychosis. Patients with psychosis often live a sedentary life, where isolation is a part of the symptomatology. At the same time, prescribed medical treatments often result in side effects that have detrimental effects on the long-term physical health of patients. Recent research demonstrates that physical activity contributes to patients’ physical health as well as their mental health, significantly contributing to their recovery. One way to improve the prognosis of patients with psychosis, often related to poor physical health, is to work with patients to promote all aspects of good health, including physical activity – a role that often fall upon nurses. Aim: The aim of the study was to illustrate nurses' experiences of physical activity in the nursing of patients with psychosis. Method: This is a qualitative study performed using inductive content analysis and is based on the interviews of 13 nurses working in Swedish outpatient care. Results: The result is presented as an overall theme, consisting of three main categories and eight subcategories. The result shows that the promotion of physical activity for nurses involves active and attentive listening, and demonstrates the time-consuming aspect of this work, especially as symptomatology and other needs still needs to be addressed. It is a work that can contribute to both positive change and new opportunities for patients. Conclusion: Physical activity is a complementary treatment that should be given increased priority and resources to further contribute to the improved healthcare for patients with psychosis.

Full textBackground: Supervision can give the nurse the possibility to reflect, let of steam, process emotions and reduce stress and feelings of inadequacy. Nurses can get increased self-confidence and a better coherence with each other but to have group supervision can be a challenge because it demands openness and commitment. Aim: The aim of this study was to describe nurses experience of clinical group-supervision in psychiatric dependency care clinics. Method: The study has a qualitative design with semi-structured interviews for data collection. Nine nurses were interviewed and the material was analyzed with qualitative content analysis. Results: The results of the study is represented in four main categories; to reflect on the caring paradigm through supervision, to create a nursing team through supervision, to understand what is expected from the nurse through supervision and to get to know your self better through supervision. The main categories has all together nine appurtenant subcategorys. Discussions: The result of the study is discussed with Antonovskys KASAM (comprehensebility, manageability and meaningfulness) and other literature and scientific articles. The result of the study showed that the nurses could use the supervision to reduce the risk of burnout and feelings of incapacity and frustration, increase the trust to each other in the working group, increase autonomy and self-confidence and to handle the feelings that arises in the meeting with the patient.

Full textBackground: Borderline Personality Disorder (BPD) is characterized by dramatic, emotional mood swings, lack of impulse control, failing self-image/identity and unstable relationships which manifests in self-harm behaviour, substance abuse and an increased suicide risk. It’s quite common that the patient will have developed less successful strategies to cope with their emotions and BPD is to a great extent, associated with comorbidity where anxiety is one of the most frequent factors. Dealing with these patients is often described as challenging and the emotional rollercoaster which is often associated with BPD poses many times as the main reason for frustration within the staff. Method: A qualitative interview study and a performed content analysis of collected data, eight individual semi-structured interviews with the intent to take part of the psychiatric nurse’s experiences. Aim: The aim of the study has been to describe the experiences of psychiatric nurses in their encounters with patients with BPD and anxiety. Results: The analysis of the collected data resulted in three categories and six subcategories. The categories are as follows: Challenges within the patient group, factors which affect the care relationship and support and education. It is revealed that the patient’s characteristic traits and anxiety disorders characterizes the psychiatric nurse’s attitude and treatment and a substantial part of the medical care consists of helping the patient develop sound strategies for anxiety management. Discussion: Supervision/guidance is seen to have a significant effect involving the psychiatric nurse’s attitude and therefore, approach to patients with BPD.

Full textThe specialist nurse’s competence within the psychiatric inpatient care - An important resource that needs more space Aim. To illuminate the specialist nurse’s perception of significance of the own competence within the psychiatric inpatient care. Background. There is shortage of competence and well utilized resources to meet the patients’ needs within the psychiatric inpatient care. The competence of specialist nurses is underestimated and obscured as there is an absence of nursing focus. Clarifying the significance of the specialist competence could imply better care. Design. A qualitative design with a phenomenographic approach. Method. Fourteen semi-structured interviews with specialist nurses in patient-oriented inpatient nursing at three psychiatric clinics in north Sweden were conducted during November 2015-January 2016. The interviews were analysed by using a qualitative analysis. Result. The specialist nurse’s competence enables a person-centred psychiatric nursing and shares knowledge within the team. Enabling more patient-oriented and development related work could increase the significance of specialist competence. The specialist competence needs to be visualized and nursing highlighted as a treatment alternative. Increasing the significance of the specialist competence could affect the quality of care and influence the patient’s care positively. Conclusion. The specialist nurse’s competence is important within the inpatient care but needs more space to enable high quality care. The result demonstrates the significance to stress the specialist nurse’s position within psychiatric inpatient care. This could attract specialist nurses to stay within inpatient care and enable the competence to come to use for the patients who needs it the most.

Full textBackground: The professional identity of psychiatric nurses is perceived as complex and obscure as it is based both on the nurse´s own values of herself and her professional role as well as on external expectations. Previous studies have shown that clinical supervision is significant for nurses´ experiences of work satisfaction. However, the research is insufficient concerning the impact of clinical supervision on the professional identities of psychiatric nurses. Aim: The aim of the study was to describe psychiatric nurses´ experiences of clinical supervision and it´s significance for their professional identity within psychiatric inpatient care units. Method: The study was carried out with a qualitative design where eight psychiatric nurses were interviewed through semi-structured interviews. The results were analyzed through qualitative content analysis with an inductive approach. Results: The results are presented through the following three main categories and are sorted according to Imogene King´s theory of systems: To be formed as a psychiatric nurse – personal system, To aim for the same goal – interpersonal system and The significance of clinical supervision for the profession of nurses – social system. Discussions: The results are discussed according to Imogene King´s theory of systems as well as relevant scientific research.

Full textThreats and violence are common in psychiatric care and are a significant problem for both patients who are under treatment and for the nurses who work within the psychiatric healthcare. The nurses within the psychiatric healthcare has a responsibility to handle the situations with patients were elements of threat and violence are a reality without inflicting unnecessary suffering. It requires knowledge and experience but also an understanding for the patients situation and previous experiences. Research that concerns working environment within the psychiatric care shows that occurrence of threat and violence may result in working environmental problems for the nurses. The consequences that may occur among the nurses are psycho social problems with a result of demised work pleasure. Nurses who face threat and violence at their jobs has often feelings of guilt and self accusations. The feelings of failure affect the possibility of a genuine contact with the patient when nurses feel a lack of ability of coping with their jobs. This study is aimed to try to describe psychiatric nurses’ experiences of threatening meetings between them and patients. Data has been collected through qualitative interviews. The analysis has been done on the basis of a content analysis. All the subjects of the interviews have a specialist education within psychiatric care and works on a psychiatric clinic. The result shows that nurses who have the ability to show care and respect and also being able to be present in threatful situations have the best possibility to achieve a genuine dialog with the patient. When nurses are feeling fear and insecurity at its most they tend to be even more forced in their attitude against the patient. The threatful meeting threatens the important relation between the patient and the nurse who has the difficult task to achieve the dialog which is based on respect and presence towards the patient despite the fact that the meeting can evolve into a threat full meeting. One conclusion of the study is that a frightened and insecure nurse has a diminished capability to offer a genuine contact with the patient. The second conclusion is that the best possibility to prevent a threatful situation is before a meeting turns into a threatful situation between the patient and the nurse.

Full textBackground: In previous research, it appears that the culture of care, teamwork, person- centered care and that the patient is receiving safe care are important for good psychiatric care. The previous research also highlights the patients’ experiences of good nursing. This subject is highly relevant in psychiatric care and in constant development therefore this needs further exploration. Purpose: To describe nurses’ experiences of what is important for good psychiatric care for adult patients in inpatient care and in psychiatric residential care. Method: Data were collected by semi-structured interviews with 10 nurses in psychiatric inpatient care and residential care in Sweden. Data has been analyzed with a qualitative content analysis with an inductive approach. Results: Reported in four categories: To have the patient in focus, to feel valuable, to have practical conditions for conducting good nursing and to interact with colleagues. These categories have a total of 14 associated subcategories. Conclusion: In order to provide good psychiatric care, the nurses competence need to be utilized and caring science needs more time and space. The nurses’ feelings need more attention and to be valued through reflection. The psychiatric care needs knowledge and continuous development to avoid habitualpatterns. Good teamwork, openness and good attitudes are required in the team. In order to perform good nursing, the nurse needs to have a holistic view and see the person behind the diagnosis.

Full textBackground: Previous researches shows patients who come in contact with a psychiatric ward after suicide attempt often experience feelings of shame and fear. The first meeting is determined by how the time at the ward develops. If a patient experience not being taken seriously or heard it usually leads to feelings of escaping or withdrawing. Aim: The purpose of the study is based on scientific literature describing patients and nurses' experiences in receiving and providing care after a suicide attempt. Method: Qualitative research review included eight scientific studies from the databases CINAHL and PsychINFO. Data were analyzed with the aid of Evans four steps analysis. Results: The results show that patients have a need to feel that they are being taken seriously in order to alleviate suffering. The result also shows that nurses are experiencing difficulties in meeting with the suicidal patient. The nurse's attitude and commitment to the patient has a central role in the patient's encounter with psychiatric care after a suicide attempt. Discussion: The meeting between the patient and the nurse is reflected in the nurse's attitude and commitment to the patient, by taking the patient seriously suffering can be alleviated and the patient's well being can be increased. Through dedication supported the patient and by the elimination of negative emotions experienced nurse can reach the patient in a way that allows a patient to be inspired to change their lives.

Full textBackground : Psychiatric care includes, in addition to reducing the psychiatric symptoms of disease, health promotion efforts to support the patient's development toward positive aspects of health. Care must be based on a health promotion perspective in which the patient's own experiences attention. Psychiatric nursing can be described from different perspectives. One perspective conveys security, confidence, and the second type of care is described as primarily meant security. Common keywords for psychiatric nursing missing. Aim: The aim of this study was to highlight concepts that psychiatric nurses in psychiatric care use in nursing and thus possibly contributing to the common values of what is psychiatric nursing. Methods: A qualitative empirical study with descriptive design was carried out.. Results: The result culminating in three categories and seven subcategories. The results showed that there are common characteristics, concepts and ideas about psychiatric nursing, if no recognized concepts. Participants agreed that the psychiatric care can be complex and is to have a holistic perspective, to nurture the whole person. Conclusion: Support, strengthen and validate the patient is seen as central to the psychiatric care, as well as to see the patient from a holistic perspective. Respect and encouragement to participate in their own care, as well as being safe and unpretentious. To see the person behind the disease, as well as see the disability instead of the diagnosis is important to provide adequate care.

Full textBackground: The ACT model developed in the 70's and 80's when many psychiatric clinics were closed in the United States. The original ACT model can be seen as a full service model but has since been modified and adapted to the current conditions of society and health care. As it appears today, several models have been developed based on the ACT model but can be distinguished by which components are included. The model has shown positive results regarding patient participation and recovery. The nurse has a wide professional background and is considered to be an important part of the team. With their skills, they have the basic knowledge to perform medical and psychiatric care, but several studies have shown that there is a need for specialist educated nurses, as many of the patients are severely ill. Aim: The aim of this study was to describe the psychiatric specialist nurse's experience of their competence and role in teams working under the ACT model. Method: A qualitative design has been selected. Data collection was done with semi-structured interviews. A qualitative content analysis with inductive approach was used to process the material. Results: The content analysis resulted in two main categories, skills in the team and the versatile nurse, with six associated subcategories. Discussions: The result was discussed based on the two main categories, skills in the team and the versatile nurse. In support of the discussion, relevant topics and literature were used as well as Phil Barker's ten obligations as a starting point.

Full textBackground: In psychiatric care, threats and violence against personnel are commonplace. Studies also show that nurses are particularly vulnerable because the nurse handles drugs and also usually the person who leads the work in coercive measures. Social support at the workplace and adequate support from managers are emphasized in order to be able to process these situations. Previous studies suggest that the negative consequences of threat and violence situations can be reduced by adequate support, but there is a need for additional knowledge about how this support should be designed for nurses.Aim: In the present study, the aim has been to describe nurses' experiences of support in connection with threat and violence situations. What situations require the need for support? What support have they received? Does the support for threat and violence situations affect the nurse's work? Method: The present study has been conducted as an interview study with six informants, of which two are specialist nurses in psychiatric care and the age range is 34 years to 50 years old. Prior to the study The Department of Health Care Sciences at Ersta Sköndal Bräcke University College approved it. Results: Threats and violence from patients with neuropsychiatric diagnoses as well as personality disorder diagnoses considered the informants to be particularly serious and the need for support. Colleagues gave the best support in the form of companionship, conversation and humor. Support from managers in the form of being taken seriously and receiving follow-up focused on the employee's feelings is lacking. Being able to meet people with severe psychological problems requires that you yourself be mentally stable and well-off, which is an effect of obtaining adequate support. Discussions: The results of the present study are discussed against Aaron Antonovsky's theory Sence of coherence. Hopefully, the present study can form the basis for identifying areas of improvement in clinical activities and maintaining an awareness of the subject. In the future, it would be interesting to extend the scope of the study to include more nurses and also nurses in psychiatric outpatient care.

Registrered nurses who take care of this group of patients facing some difficulties in their caring. To optimize the caring there is a essential need of comprehensive for the nurse´s experiences of refugees with mental illness. Aim: The aim of the study is to describe the experiences of registred nurses to care of refugees with mental illness. Method: A study with a empirical approach, qualitative interviews with five registrered nurses in a psychiatric clinic. All data were analysed through a content analysis. Results: The results demonstrated five different experiences of the nurses: "experiences of caring of the refugees, "experiences of the need of skills development", "experiences of culture clash", "experiences of powerless" and "experiences of compassion". Conclusion: Two of five dimensions of experiences are new compare to earlier studies´ approaches: experience of powerlessness and experiences of compassion among the nurses in psychiatric care. Compassion of the nurses contributed to a tension in their own mental health. A professional supervision by the employer is necessary to facilitate the nurses´ care of the refugees. Further research: To minimize the experiences, for example powerlessness, the research should exspand on national and international basis.

Full textFamily members and next of kin are a central and natural partner who, together with the patient, constitutes the most important experts in their life circumstances. Collaboration with family members may include prevention of illness, acceleration of recovery, shorter care time and fewer incisions for the relative with mental illness. Only a few studies describe the professionals´ perspective on collaboration with family members. Aim: The aim of the pilot study was to describe the psychiatric nurse's experiences of collaboration with family members in psychiatric care. Method: The study has a qualitative approach and data was based on four interviews with psychiatric nurses based on open questions. Data has been analyzed with qualitative content analysis. Result: The study resulted in the categories: Opportunities for collaboration, Experiences of working collaboration and Experiences of challenge for collaboration. Conclusion: Psychiatric nurses had an interest in collaboration with family members in psychiatric care. Collaboration was considered positive in patient work but was not always possible to practice. Collaboration was found to facilitate patient work and lead to improved relationships between family members and patient. There were several difficulties that constituted interference to collaboration. It requires that the psychiatric care unit provide support in raising competence and has an attitude aimed at collaboration with family members for quality improvement. The result may be important for improvement purposes in order to develop effective collaborative relationships with family members in psychiatric care.

Full textBackground: Inpatient aggression is common as a phenomenon in psychiatric inpatient care. Inpatient aggression arises for several reasons. It disturbs the nurses in their work assignments and causes negative emotions, which can be reflected in the interaction with the patients. Aim: To describe nurse's experiences of caring for aggressive patients in psychiatric inpatient care Method: An interview study was conducted with eight nurses at two psychiatric clinics at a hospital in a big town in Western Sweden. The material was analysed through qualitative content analysis. Results: The analysis resulted in four main categories. The first category is uneasiness, with subcategories: to feel anxiety, to feel stress, to feel sad and to feel offended. The second category is fear, with subcategories: to feel fear for the aggressive patient and to feel fear for others sake. The third category is powerlessness, with subcategories: to lose control, to feel frustration and to work with a ticking time bomb. The fourth category is work to create security policies with subcategories: understanding the patients' situation and courage to remain in an aggressive encounter. Conclusion: Inpatient aggression is often associated to symtoms and/or drug abuse, which also was confirmed in our study. In most scenarios the nurses had a deeper understanding for the patients behavior and thereby a higher tolerance towards inpatient aggresion. The nurses claimed that beeing available to the patients was very important, since that created an atmosphere of security for both parties and it was assumed a presumtion for a good caring relationship.

Full textBackground: Studies show a need for development in mental health service for salutogenic approaches. Few systematic health promoting activities exist and a holistic perspective, where the patient’s individual resources are catered, should be the focus. This study intends to provide a better knowledge of how the work with the patient’s resources look like. Aim: The aim of this study was to examine how psychiatric nurses identify, value and use the resources of patients with mental illness. Method: A qualitative design was chosen. The data collection was done by interviews, in which six psychiatric nurses participated. Open-ended questions were used to answer the research questions. Data was analyzed according to qualitative content analysis, using an inductive approach. Results: The participants described that the patient’s resources were identified in sessions where the emphasis was laid on building a therapeutic alliance, information gathering and motivational interviewing. The resources that the participants valued resulted in two categories, internal versus external resources. To have a social network, to have awareness of one’s situation and motivation to change were all examples of meaningful resources according to the participants. To formulate resource diagnosis and to integrate these with the interventions in the care plan were described as effective tools in order to look after the patients’ resources. All participants experienced significant improvements in terms of managing their resources. For instance, there was no agreed understanding of how to utilize resources, the work was perceived as too problem focused and a preference for a more process focused way of thinking was expressed. Discussion: The study’s results were discussed against Antonovsky’s salutogenic perspective, which emphasises the importance of viewing the healthy aspects in patients as well as the factors that contributes to good health. Many studies highlight that the salutogenic perspective and KASAM can be applied in practice and is useful for the recovery process in mental health.
