Full textBackground: There are few studies focusing on the patients’ perspective of psychiatric care. However, this perspective is important, as it offers insight in the care received from the patient’s view point. Aim: The aim of this study was to describe how the patients in a psychiatric ward perceive the caregiving behavior of the ward’s nurses. Method: Semi-structured interviews analyzed through qualitative content analysis. Result: Analysis of the data resulted in four main categories describing aspects of good and less good caregiving behavior as perceived by the patients. The categories were: The importance of being seen and validated by the staff, The staff’s attitudes and behavior towards the patients, Wishes and expectations regarding the staff and How non-verbal communication affects the way the care giving behavior is perceived. Conclusion: The most prominent aspect of good caregiving behavior was described as the nurse’s ability to take the time to listen to the patient, and the most prominent aspect of a less good caregiving behavior was described as insufficiency in this ability.

Full textBackground: The nursing profession has been through huge changes from 1950th until today. The nurse in psychiatric care has a central role in caring of the patient, so why not get further educated? The literature describes the specialist nurse role often as multifaceted and complex but at the same time unclear, but what does the specialist nurse in psychiatric care think of their role today? Aim: The aim of the study was to illuminate how the specialist nurse experienced her role in psychiatric care. Method: A qualitative descriptive design. Manifest content analysis was made according to Graneheim and Lundman (2004). The sample consisted of eleven specialist nurses in psychiatric care. All participants were women between the age of 31-59 years. One focus group and six individual interviews were performed by using an interview guide containing semi structured questions. Results: Four categories emerged: (Non)specific tasks, Lack of scientific tradition, Holistic view, Pass on the knowledge. The result shows that the specialist nurse role is independent, lack of specific tasks, has expended knowledge in psychiatric care and lack of terms in the profession. The specialist nurse had a positive attitude towards science but did not take partition in it that much. The specialist nurse had an ability to see the patient in a holistic view and described a collaboration- and an educational role. Conclusion: The specialist nurse role in psychiatric care regard themselves as important and having a high level of competence and at the same time an unclear role. Clinical significance: In health care, that is more specialized than ever the specialist nurse in psychiatric care is needed, therefore the academy and the employer should clarify the role so that the competence is taken care of and nurses stimulates to get further education.

Full textThreats and violence are common in psychiatric care and are a significant problem for both patients who are under treatment and for the nurses who work within the psychiatric healthcare. The nurses within the psychiatric healthcare has a responsibility to handle the situations with patients were elements of threat and violence are a reality without inflicting unnecessary suffering. It requires knowledge and experience but also an understanding for the patients situation and previous experiences. Research that concerns working environment within the psychiatric care shows that occurrence of threat and violence may result in working environmental problems for the nurses. The consequences that may occur among the nurses are psycho social problems with a result of demised work pleasure. Nurses who face threat and violence at their jobs has often feelings of guilt and self accusations. The feelings of failure affect the possibility of a genuine contact with the patient when nurses feel a lack of ability of coping with their jobs. This study is aimed to try to describe psychiatric nurses’ experiences of threatening meetings between them and patients. Data has been collected through qualitative interviews. The analysis has been done on the basis of a content analysis. All the subjects of the interviews have a specialist education within psychiatric care and works on a psychiatric clinic. The result shows that nurses who have the ability to show care and respect and also being able to be present in threatful situations have the best possibility to achieve a genuine dialog with the patient. When nurses are feeling fear and insecurity at its most they tend to be even more forced in their attitude against the patient. The threatful meeting threatens the important relation between the patient and the nurse who has the difficult task to achieve the dialog which is based on respect and presence towards the patient despite the fact that the meeting can evolve into a threat full meeting. One conclusion of the study is that a frightened and insecure nurse has a diminished capability to offer a genuine contact with the patient. The second conclusion is that the best possibility to prevent a threatful situation is before a meeting turns into a threatful situation between the patient and the nurse.

Full textTitle: Working under fear- a literature review about nursing staff’s experiences of threat and violence in psychiatric care. Background: Reports show that threat and violence is a common phenomenon in healthcare. It appears in every part of healthcare, but some units are at more risk than others, for example psychiatric care. The verbal and physical violence creates an insecure work environment and can cause serious consequences for both personnel and patients. Aim: To describe psychiatric nursing staff’s perceptions of threat and violence in psychiatric care. Methods: A literature review based on eight qualitative articles. These articles were examined, analyzed and compiled. Results: Threat and violence caused strong emotional reactions, like fear and anger, among nursing staff that influenced the work and their private life. The unsafe work environment composed a threat to the nursing staff’s professional role and self image. It was percieved that violence were unavoidable, and that nursing staff were expected to accept violence as a part of the job. Violence were handled and prevented with different strategies, while manegement support were lacking. Conclusion: Violence against nursing staff in psychiatric care are a relatively unexplored subject in need of more research. Nursing staff are suffering both physical and psychological damage due to violence in the workplace, which also effects the care given to patients and the nursing staff’s own health. Full textIt is common in most areas of psychiatric healthcare that nurses come into contact with threatening and aggressive patients. In order to ensure that healthcare units shall be able to meet the needs of patients and at the same time be certain that the work environment is safe for nurses, it is important to precision what threats and violence actually mean. If threatening and violent situations can be identified before they lead to physically or psychologically damaging encounters, this will save the both patients and nurses unnecessary suffering. The Purpose: was to bring to light nurses’ definitions and experiences of threats and violence and at the same time describe how they perceive daily working enviroment within psychiatry. The Method: the work was built on twenty nurses presently working in various psychiatric clinics. We made use of a data collection method that is built upon semi-structured interviews where we there after, in a sytematic and step by step process, classified the data through qualitative content analysis in order to more easily identify and condense pertinent sentence concepts. We created codes which were then placed into three categories. The Results: are split up into categorys: The precision of threats of violence, the experience of threats, and violence in the workplace. These categories are presented by the codes: threat, violence, sexual harassment, direct threats, the effects of threats, violence in combination with compulsory psychiatric care, the consequences of threats and violence, the acceptance of threats and violence, safety measures, the psychiatric nurses’ work enviroment, and the support of the nurses’ colleagues and the care organisation, were made apparent during the interviews. The Discussion: The majority och nurses enjoy their job and feel safe working at psychiatric clinics and wards, despite that incidents of threats and violence do occur.

Full textBackground: Asylum seeking patients often suffer from mental health problems related to stress during flight and harsh living conditions. Being responsible for the care, nurses are responsible to help people with what they themselves have difficulties performing or are unable to do themselves. Due to language difficulties and cultural conditional expectations the caring of patients within this group can sometimes be suffering. In order to work effectively with asylum seeking patients in psychiatric care, it is required to possess a thorough knowledge of how nurses describe their work with asylum seeking patients. Aim: The purpose of this paper is to describe nurses' experiences of caring for asylum seeking patients in psychiatric care. Method: The study is a qualitative study with the inductive approach in which semi-structured interviews were conducted with five nurses working in psychiatric care. The collected data were analyzed according to qualitative content analysis. Results: Data analysis resulted in two categories with five and two subcategories each. The first category describes the nurses' descriptions of the obstacles and opportunities in relation to asylum seekers patients with subcategories: cultural differences, personal impact and the need for guidance, the need of information, approaches, and patient social networks. The second category describes nurses' descriptions of the obstacles and opportunities in relation to external instances, with subcategories; interpreter and asylum process. Discussions: The results are discussed from Leininger’s Sunrise Model and other relevant research. Sunrise model provides support for nurses in the meeting with patients with a different cultural background, which can be important to work holistically.

Full textBACKGROUND: The studies show that violence has increased over the years. Risk factors and risk behaviors have been identified and the reason for the patient to experience illness can be many. Research in this field has so far focused on the underlying causes of the patient's aggressive behavior and has been conducted for many years. Increased knowledge of the nursing experience of caring for patients with aggressive behavior in psychiatric care can help to develop nursing care adapted to the situation, in order to promote health. AIM: The aim of the study was to explore nursing care and nurse’s perceptions of caring for patients with aggressive behavior in psychiatric care. METHOD: The systematic review was based on both qualitative and quantitative studies. RESULTS: The analysis resulted in three main categories and 12 subcategories. The main themes that emerged were: the nurse in the interaction with the patient, organizational and personal factors as well as subcategories that dealt; diversion of threatening situations, personal space, information and assessment, setting limits, risk factors associated with aggression, lack of support from managers, staffing, personnel security, experience, and communication skills, loyalty towards patient and work, hopelessness and disappointment and coping. CONCLUSION: The results show that nurses experience the care of patients with aggressive behavior in psychiatric care in different ways. The nurse's experience may affect the patient's health. For the nurse to work with health promotion it is required tools and skills to handle aggression in an adequate way. Increased skills are perceived to have an impact on the quality of care for the patient and are experienced to affect the number of violent incidents. A supportive management structure and good working environment is perceived to be important and could contribute to more and more nurses to remain in their workplace in psychiatric care.

To enjoy work, and be satisfied in the work you do, is important to most people. Work scientific research shows that dissatisfaction with physical or the psychosocial work environment often gives negative effects on the performed work. The purpose of our study is to chart the experience in work satisfaction and psychosocial work environment amongst nurses within psychiatric care. It is an empiric cross section study. The participants in the study contains nurses who are either employed on a conditional tenure or have a temporary post (>3 months) in policlinics and wards in general adult psychiatric care (n=126). In the study a PAK/AK questionnaire (psychosocial work environment/ work satisfaction-survey) was used. The form includes 49 questions which were analyzed in the Statistical Package for Social Sciences (SPSS). The outcome shows that there is a considerable dissatisfaction with salary, advantages, and possibilities for personal development. Nurses who work in policlinics are more satisfied compared to nurses who work in wards. They feel that they have possibilities to decide how the work should be performed, possibilities to learn new things and they feel that they have support from the employer when it comes to education. Almost everyone thinks that it is important to use their capacity and competence in their work. The participants are relatively satisfied with the work climate created by the employer. The most negative answer as to workload is about the psychological strain, which is to be tired and exhausted after work, and stress. The frequency of psychosomatic pain is low, but the work described as difficult in a mental aspect. The study shows no connection between how work satisfaction can affect the nursing - an important aspect in the nurses work.
Att trivas på sitt jobb och vara tillfredsställd i det arbete man utför är viktigt för de allra flesta. Den arbetsvetenskapliga forskningen har visat, att missnöje med den fysiska eller psykosociala arbetsmiljön oftast ger negativa effekter för verksamheten. Syftet med studien var att kartlägga upplevelsen av arbetstillfredsställelse och psykosocial arbetsmiljö hos sjuksköterskor inom psykiatrisk vård. Studien är en empirisk tvärsnittsstudie där deltagarna utgörs av hela populationen sjuksköterskor (n =126) som är tillsvidareanställda eller anställda på vikariat (>3 månader) inom öppen och sluten allmän vuxenpsykiatrisk vård inom ett landsting i Sverige. I studien användes frågeformulären PAK/AK (psykosocial arbetsmiljö-/arbetstillfredsställelsekartläggning). Formulären omfattade 49 frågor som bearbetades och analyserades i Statistical Package for Social Sciences (SPSS). Resultatet visar att det finns ett ansenligt missnöje med lön och andra förmåner, även utvecklingsmöjligheter visar på otillfredsställelse. Sjuksköterskor inom öppenvård uppger att de i signifikant högre grad än kollegorna inom slutenvård har möjlighet att lära sig nya saker i arbetet och att själva bestämma hur arbetet ska utföras. De allra flesta tycker att det är viktigt att få användning för sina anlag och sin kompetens i arbetet. Man är också relativt nöjd med arbetsledningsklimatet. Frekvensen av psykosomatiska besvär är låg, men arbetet beskrivs som mentalt påfrestande. Sjuksköterskor inom öppenvård upplever i högre grad än kollegorna inom slutenvård arbetet som psykiskt tungt. Studien visar dock ingen koppling mellan arbetstillfredsställelse och hur det kan påverka patientens omvårdnad - en viktig aspekt av sjuksköterskans arbete.

Full textBackground: Quality of care is a multi-faceted concept that cannot be described in a unilateral way. Quality of care can be seen from many different perspectives, where the patient perspective is one. The concept of participation appears as central when the quality of care is described. The patient's involvement in the development of health and medical care is regulated by law and the patient is seen as a natural part of the development work. Previous studies show that patients emphasized interpersonal process as most important, for how care quality was perceived. Purpose: The aim is to compile research from a patient perspective regarding the quality of care in psychiatric care. Methodology: A literature study based on 8 reviewed qualitative articles. The analysis was carried out in accordance with Evans' four-way method. Key concepts were identified and then themes and sub-themes were created that generated the result. Result: The quality of care according to the patients is a combination of several factors. The most prominent are the staff who care about good treatment and where the patients may be involved. The care environment is also an important factor for the patients to experience quality of care and then it includes a safe and aesthetic environment. The result is presented in two themes, Staff who care and A quiet, beautiful and safe environment with four sub- themes; Treatment, Participation, Aesthetics and Security. Conclusion: Staff with an ability to create a meeting that involves participation in safe forms in a beautiful environment is a prerequisite for patients in the psychiatric care to experience quality of care. The literature study contributes with a compilation of knowledge about what patients within psychiatric care experience is quality of care, why it can be used to promote development, patient involvement and the staff's ability to give patients a good quality of careregardless of the health context.

Full textBackground: Person-centered care (PCV) is increasingly used in both somatic and psychiatric care, with predominantly positive effects for the individual. Clinical studies are ongoing, but another way of evaluating methods is to follow how indicators from patient registries develop after implementation. Purpose: In this study, the indicators are analyzed based on the knowledge that the psychiatric clinic at Gotland has introduced PCV 2016 and has thus had the opportunity to closely monitor the indicators' development. Method: Through descriptive analysis, this study has identified results that indicate that there are changes in indicator development over time that can be linked to the introduction of PCV in 2016. Outcome: The clearest result is the reduction in the number of individuals and inpatient care. The reduction of Individual plans is clear and should be analyzed in more detail with a gender perspective. Results in areas of safe care are not clear, but by no means negative, with low numbers maintained. Conclusion: The results in this study indicate that there are changes in indicator development over time that can be linked to the introduction of PCV in 2016.

Full textBackground: Mental health problems have increased since the 1990s and there is a correlation to the need for psychiatric care. Patients with psychiatric disorders occur in the general healthcare and the patient group has an increased risk of somatic diseases. This puts pressure on the educational level for treating patients with psychiatric disorders in general hospitals. Aim: The purpose of this literature review is to highlight the nurses' experience of caring for patients with psychiatric illness in general care. Method: A literature review with scientific articles, which have been found with relevant keywords on different databases and reviewed with the help of review templates. Results: 14 reviewed articles, 10 qualitative and 4 quantitative were used in the literature review. In the results, seven categories could be identified, these were Lack of time, Lack of environment, Competence and knowledge, Vulnerability, Communication and Emotional aspect. The results showed a lack of knowledge among nurses, also the majority have negative feelings to nursing patients with psychiatric illness. Lack of time was a major obstacle to nurses' ability to care and the care environment did not allow good care for patients with psychiatric illness. Conclusion: Nurses need more resources to care for patients with psychiatric disorders in the somatic care to achieve good patient care. Furthermore, education in psychiatrics for nurses is required and a change in the care environment needs to be initiated to promote the care of patients with psychiatric diseases.

Full textBackground: There are between 30 000 and 40 000 persons living with Schizophrenia in Sweden and every year between 1500 and 2000 persons experience their first psychotic episode. These persons often have difficulties in coping with everyday life and are in need of multiprofessional care. Previous research shows that persons with psychotic disorders experience that they are not sufficiently involved in their care and many decisions about the care are made without their involvement. Aim: The aim was to review and synthesize research regarding persons experiences of participation in mental health care for psychosis. Method: The current literature review study consists of 16 scientific qualitative articles which are included in the result. Analysis of data was conducted withBettany-Saltikov and MchSherrys method for qualitative litterature analysis. Result: The result comprises three categories with eight subcategories. The experiences of participation in mental health care is described in the categories: Personal abilities, Power dynamics and Hierarchy and Availability. Conclusion: Depending on the circumstances the distribution of power between the person and the healthcare staff should be dynamic. The mental health care should have more focus on cooperation between the person and the healthcare staff instead of drug regimen.

Full textBackground: Most acts of violence in a psychiatric ward are directed towards the nursing staff. Aggressive behaviour often stems from feelings of frustration and powerlessness in patients while committed and therefore having limited liberties. To avoid an escalation of a hostile situation, it is important that the nursing staff applies the kind of encounter that will yield a positive outcome. Aim: The aim of this literary study is to elucidate the possible encounter the nursing staff can apply when encountering a hostile patient in psychiatric care. Method: The literary study is based on twelve scientific articles. The articles were analysed by means of content analysis in which different categories emerged and were subsequentially labelled. Result: The study resulted in three main category labels and six sub-category labels. The main category labels are: Situation Adaption and Flexibility, Creating Assurance and The Importance of Self-Determination.
Conclusion: More time and resources are needed for training nursing personnel in psychiatric care to adapt to situations and rely on their own abilities and knowledge.

Bakgrund: Att vara inskriven enligt lagen om psykiatrisk tvångsvård, LPT, innebär att självbestämmandet tas ifrån en. Eftersom den nya lagen skapades för att skydda patienters integritet undersöker detta arbetet patienternas upplevelser av tvångsvård. Syfte: Syftet var att beskriva patienters erfarenheter av tvång med fokus på tvångsvård inom den psykiatriska vårdenheten. Metod: En litteraturstudie där sju vetenskapliga artiklar ingick. Systematisk granskning av artiklarna genomfördes. Resultat: Resultatet utgörs av tre kategorier: de som uppfattar tvång i störst utsträckning vilket var främst vita, högutbildade och kvinnor. Negativa erfarenheter av tvångsvård vilket inkluderade att ingen lyssnade eller tog en på allvar. Slutligen positiva erfarenheter av tvångsvård vilket bland annat innebar att ha någon att prata med och få viss kontroll över sin vård. Diskussion: Självbestämmande och kommunikation är viktiga faktorer som spelar en stor roll när det kommer till upplevelser av tvångsvård. Slutsats: Många faktorer spelar in vid upplevelser av tvångsvård och tvång. Både negativa och positiva upplevelser kom fram. Många problem hade kunnat undvikas om personalen hade pratat med patienterna och visat empati. Fler studier i ämnet behövs för att personalen ska kunna förstå patienterna och i slutändan ge en bättre vård.
Background: To be admitted according to the law of coercive measures in psychiatric care, LPT, one is stripped of one’s self-determination. Since the new law was created to protect patients' integrities, the aim of this study was to determine how patients' experienced coercive measures. Purpose: The aim was to describe patient´s experience of coercive measures with focus on psychiatric coercive care within a psychiatric care unit. Method: A literature review in which seven scientific articles were included. Systematic review of the articles was used. Results: Three categories were found: Those who experience coercive measure the most which was mainly white people, women and those with a high education. Negative experience of coercive measures which included not being listened to and taken serious. Finally positive experience which included having someone to talk to and being given some control over one’s own care. Discussion: Self determination and communication are key factors when it comes to experiences of coercive measures in psychiatric health care. Conclusion: There are many factors that correlated with experience of coercive measures and coercion. Both negative and positive experiences were mentioned. Many problems could have been avoided if only the staff had talked to the patients and showed empathy. More studies are needed to make the staff understand patients and ultimately give them better care.

Full textBackground: All medical care performed in Sweden is optional, even in the psychiatric care. Although the care can sometimes be coercive by law and therefore against the patients will. When the nurse limits the autonomy of a patient by coercive measures the nurse may experience it as it ethically difficult even if it is for the best interest of the patient. Purpose: The purpose of the literature study is to highlight nurses experiences when performing coercive measures against patients with psycothic diseases in psychiatric closed care, from an ethical perspective. Method: A literature study with ten qualitative studies was processed and analyzed by the authors. From the analysis the authors were able to make categories out from the finds in most of the articles. Result: When performing coercive measures the nurses experienced ethical dilemmas. By relation, purpose and reflection the nurses experienced that the autonomy of the patients could be respected and treated better. Discussion: By using the categories from the result as a basis the ethical experiences of performing coercive measures were discussed. A main concept that described the relationship between nurse and patient was paternalism. The HSL and LPT- laws were discussed and put in conflict against eachother.

Full textBackground: People with mental illness have poorer health and increased mortality from somatic diseases compared with the general population. The diseases can also lead to difficulties in coping with everyday life and reduced quality of life. It is very important that the nurse gets a clear picture of what affects the patient’s quality of life. There are a number of different instruments for measuring health and quality of life, e.g. The Schedule for the Evaluation of Individual Quality of Life - Direct Weighting (SEIQoL-DW). Aim: To examine individual quality of life amongst patients with mental illness with the assistance of SEIQoL-DW. The second aim was to study the feasibility and applicability of the instrument. Method: Patients in psychiatric care filled in the SEIQoL-DW-form with support of the health care staff. Then both the patient and the health care staff filled in a questionnaire about the instrument's usability. The material was analyzed quantitatively and qualitatively. Results: Patients participating in the study estimated Relationships/Social network as the most important area of life. The area that generally was perceived to work best was Family, Work had the lowest level. Most of the patients and all of the health care staff considered the instrument to be relevant and a good support for the dialogue. About half of the patients considered the form to be time-consuming. Conclusion: Although quality of life is something individual, many of the patients stated the same areas of life as important. The instrument is user-friendly to a great extent, but the patient may need assistance to complete the form.

Full textFamily members and next of kin are a central and natural partner who, together with the patient, constitutes the most important experts in their life circumstances. Collaboration with family members may include prevention of illness, acceleration of recovery, shorter care time and fewer incisions for the relative with mental illness. Only a few studies describe the professionals´ perspective on collaboration with family members. Aim: The aim of the pilot study was to describe the psychiatric nurse's experiences of collaboration with family members in psychiatric care. Method: The study has a qualitative approach and data was based on four interviews with psychiatric nurses based on open questions. Data has been analyzed with qualitative content analysis. Result: The study resulted in the categories: Opportunities for collaboration, Experiences of working collaboration and Experiences of challenge for collaboration. Conclusion: Psychiatric nurses had an interest in collaboration with family members in psychiatric care. Collaboration was considered positive in patient work but was not always possible to practice. Collaboration was found to facilitate patient work and lead to improved relationships between family members and patient. There were several difficulties that constituted interference to collaboration. It requires that the psychiatric care unit provide support in raising competence and has an attitude aimed at collaboration with family members for quality improvement. The result may be important for improvement purposes in order to develop effective collaborative relationships with family members in psychiatric care.

Full textBackground: The shortage of specialist nurses in psychiatry is great, but the skills are still not required by the employers. Proposals to reduce the number of specialist courses have been made, of which psychiatry is one of them. Objective: The aim of the study was to investigate how nurses assess specialist nurses´ competence in psychiatric care. Method: A quantitative web based study was conducted with a total of 110 nurses with experience from psychiatric care. Results: The specialist registered nurse is judged to have a higher competence than registered nurses, but with equal tasks and responsibilities. Self-esteem among specialists are higher. Experience is of great importance for the competence. Conclusion: The competence of specialist registered nurses was assessed to be higher than that of registered nurses, but work and responsibilities are the same. There is a general need for employers to more clearly utilize the competence of specialist nurses. The visibility of the specific competence can then in the long run result in more psychiatry specialist nurses.

Full textBackground: Priorities in primary- and psychiatric care differs, generating problems with integration, when patients are referred between care facilities. People with somatic and mental illness often experience care as inadequate. Patients feel more satisfied with the care when there is a cooperation between the care units. Aim: The aim was to describe how adult patients with comorbidity perceive the cooperation between somatic and psychiatric care. Method: The study is a structured literature study with an inductive approach. Result: Three main themes emerged; negative aspects of care cooperation, positive aspects of care cooperation and cooperation from a holistic perspective. It was perceived as positive if parties involved took responsibility for nursing and a non-fragmented care organization that had close cooperation between care units was perceived as positive. Lack of continuity in patient care created frustration and for patients with mental illness fragmented care interventions were an obstacle. If psychiatric-and somatic care cooperate so that access to both instances within the same unit is possible, it is beneficial to the patients. Conclusion: There are both organizational and personnel-related factors in need of improvement. Good cooperation, clear guidelines and a holistic care perspective were the most important findings of this literature study.

It is a conscious search forunderstanding. Reflection provides the opportunity to develop professional awareness ofone ́s own way of functioning and reacting. Through systematic reflection, the individual candevelop and link theory to practice. The individual must consciously evaluate theirexperiences in order to learn from them. Aim: The purpose of this study is to elucidate nurses ́ opportunities for self-reflection inpsychiatric careMethod: An integrative literature study was used as a method. Data were obtained from thedatabases Cinahl Plus, PubMed and PSYCinfo. A total of six articles form the basis for theresult. The material has been analyzed using the Whittemore and Knafls (2005) method. Results: Nurses need to create space for self-reflection. The study showed that employersneeded to enable self-reflection to be exercised in the workplace and employees needed toappreciate that self-reflection could serve as an aid in psychiatric nursing. It was alsoeducators, in an educational situation, were interested in the individual ́s learning and hadknowledge of how to teach self-reflection. Conclusion: Opportunities for self-reflection are time, knowledge and a permissiveenvironment where the nurse or nurse student can and dare to ask questions and where there isa common norm that values self-reflection in the workplace.

Abstract Aims: To explore whether and in that case how nurses question women in the psychiatric care about violence in close relationships and how nurses respond to women who have been exposed to domestic violence. Method: A qualitative descriptive interview study with twelve nurses from four psychiatric wards was carried out in Uppsala. Informants were chosen from a convenient selection. The interviews with semi-structured interview questions were recorded on tape, transcribed and analyzed with content analysis. Results: There is no routine to ask all female patients if they are victims of violence. It happens only in cases where the suspicion arises or is confirmed. It is individually whether and how the nurse asks the woman if she is subjected to violence in a close relationship, depending on patient and nurse. Nurse handles the violence exposed woman by supporting her to share their experiences. Then consult and discuss with their nurse colleagues and other professionals for further follow-up and approach. Conclusions: The study has in accordance with previous research further highlighted the need for more education for nurses on how to handle women who are victims of violence in close relationships. More research should be done in Sweden to develop and implement standardized questionnaire for nurses in all health care.

Full textABSTRACT Background: Previous research shows that nursing students benefit greatly from participation in patient care in their education and future careers. Patients in health care generally have a positive attitude to student participation. However, research on how patients in psychiatric care perceive student participation is very limited. Aim: To investigate how patients in psychiatric care experience student involvement in their care. Method: Quantitative study, a questionnaire study. Patients from three psychiatric departments at a university hospital in central Sweden were asked to respond to survey questions. Result: The result showed 655 completed questionnaires. Of the respondents, 95 % stated that it was important that students get to meet patients during their training. Seventy-six percent were comfortable with students participating in the health care visits and 61 % were comfortable with meeting students for individual conversation. Female patients were less comfortable with male students compared to female students and they were also less comfortable if the student was very young. For male patients either the sex or the age of the student was of any significance to how comfortable they were with student participation. Patients who had met students previously were generally more comfortable with student participation. Conclusion: Patients in psychiatric care has a positive approach to student participation, for female patients the student’s gender has an impact on how comfortable they are. Patients want to be consulted on student participation in a timely and respectful manner. If supervisors are trained in what is important for patients to know before they decide whether a student may participate or not, it will further increase opportunities for student participation. Full textBackground:Transferring results from research to clinical praxis seems to be very complex. In spite of extensive research about implementation guidelines there are insufficient facts about what strategies are the most effective. Specialist nurses within psychiatric care can with his/her specific competence improve the general care of psychiatric patients. Purpose: The aim with study was to examine health care personnel´s experiences of and work with the Bergen model within psychiatric health care wards in Stockholm Läns Landsting. Method:Qualitative interviews were chosen as method. Data was analyzed with content analysis. Results: The results showed that the health care personnel found the Bergen model to be an essential tool that could be used daily, in working with the patients. It is shown to be useful in situations where threats and violence occur. The categories that emerged from the content analysis were: Unclear staff management, A preventive undertaking, A caregiving perspective, and Communication and Repetition. The majority of the interviewees expressed inadequate staff management during threatening and violent situations and there was no structure during these situations. It was requested from the majority of the participants that the nurse was the one to take charge and to fill the leading role. Conclusions: in conclusion, the results from this study shows that the Bergen Model has contributed to help with the prevention of violence and threats. It has given the health care personnel a better decision-making capacity in threat full situations with patients in psychiatric wards. This study shows that a leader is needed in threatening and violent situations, and that the nurse was preferred to be the one to take on the leading role. Clinical significance: There are a lot of different factors that can affect the way the results from research can be used in clinical praxis. The biggest challenge is to find solutions to implement existing research into clinical praxis in a simple way. The results from this study can also show a need for an evaluation of the Bergen Model, pertaining to clarifying each and everyone´s responsibilities.

Full textBackground: The specialist nurse has knowledge on how to improve and develop the care of every patient and lead staff in the nursing process. It's not always the role of the psychiatric specialist nurse is obvious, the role is usually not visible or confirmed in the daily work. Aim: To describe the nurse's experience of the benefits of the specialist competencies in psychiatric care. Method: An interview study with a qualitative approach. Six specialist nurses in psychiatric nursing with a master's degree who worked in psychiatric care for a minimum of two years in their current profession were interviewed for this study. Results: The informants experienced the education giving them necessary skills for the work, increased professional pride, and a holistic approach in the treatment of patients, increased scientific awareness and significance for the leadership. On the other hand informants experienced that their competences and skills were unknown to other team members and not confirmed and looked after by the organization. Discussions: Studies describing the factors that prevent or promote specialist nurse's ability to use its expertise in psychiatric care is needed.
