Facing to the increasing trend of aged people how to improve the quality of life become an important issue at the present time. Since most of discussion emphasis on the " physical health status" and less on "mental health status" about aged people, this study try to figure out the influence factors of the health-related quality of life included physical and mental status about the community aged people. This study focus on the citizen more than 65 years old who lived in Tze-nen county Puli town. Adopting systemic sampling method to select 150 people in which 5 of them are cognitively impaired, therefore 145 samples are in effectiveness. The statistical method applied included Multiple Regression Analysis and Multivariate Analysis of Covariance. The results of this study included "macro" and "micro" parts. After control other variables, the formal part :concluded (Ⅰ)physical domain :the more chronic health problems and worse self-perceived health and more physical function disorder and higher depression degree, the worse of living quality of the aged people. (Ⅱ) mental domain, the older and worse self-perceived health and more physical function disorder and higher depression degree, the worse of living quality of aged people, (Ⅲ)social domain, junior high school education or above and married and higher depression degree, the worse of quality of life of aged people, (Ⅳ) environmental domain, worse self-perceived health and more physical functional disorder and higher degree of depression degree, the worse of quality of life of aged people. The later part finding: age, marriage status, higher educational, number of chronic health problems, self-perceived health, number of physical function disorder and degree of depression are influence factors about "the concepts of overall quality of life". This study also figures out that: on the age of "mental domain" and other domain are contrast trend; the older people the better the living quality they owned. The depression factors significant influence all the domain factors. In additions, to added up all the factors of living quality instead of separated analysis maybe misleading the message. Under the limited resource the health policy of aged people emphasis on the physical and mental disorder group. How to improve quality of life under the familiar environment is an important issue at this moment.

With the high incidence rate of diabetes and the considerable medical care costs generated from poor disease control, it would be a blessing for patients if the disease can be diagnosed and aggressive treatment can be applied as soon as possible. The Diabetes Health Promotion Center cares for diabetic patients by employing a multi-disciplinary team to provide patients quality medical care service, thus highlighting the importance of the Center and its professional team. This study aims to understand the journey of the Diabetes Health Promotion Center formed by primary care clinics in Puli Township, and conduct an in-depth investigation on the professional relationships and partnerships of the Center as well as its current execution status and difficulties. Based on the research purpose, qualitative interview was adopted and a total of nine interviewees were invited to participate. The research results can be generalized as follows: A.The Diabetes Health Promotion Center must invest money and time in personnel education and training, but the personnel also have to be familiar with the current status of diabetic patients before building harmonious doctor-patient relationships; regarding personnel retention, the Center has faced difficulties, such as being hard to agree on certain concepts, non-locals are unable to hold their positions for long, etc. B.When primary medical institutions were forming Diabetes Health Promotion Team, for hardware, relevant apparatus must be purchased additionally and inspection resources must be self-connected; because chronic illness care used hard copies, the software had to be equipped with an exclusive information aiding system for smoother operations; however, in addition to the manpower and cost burdens generated under the NHI Review System and the troubling administrative works of the reviews, the Center also has to face practical challenges such as pressure from prescribing diabetic medications. C.The operation of the Diabetes Health Promotion Team will have specific functions and divisions as well as multi-disciplinary care; by fully utilizing the advantages of team care through doctors, nurses, dieticians and pharmacists, the Team will enter the community, immerse in the community to let people gain correct prevention and control concepts; however, the dilemma they face in non-metropolitan areas is how to attract people to participate in activities. Another issue they must face while in the medical field is dealing with whether or not they will be acknowledged by authority figures. Finally, under the active care of the Team, the primary medical institutions will become model institutions in diabetes care, and the quality of primary care will also be elevated. In the end, researcher suggests that governmental departments should provide relevant human resource integration of diabetes health educators when the primary medical institutions are forming the Diabetes Health Promotion Center, as well as to help them establish relevant software and equipment to perfect the resources these primary care require for them to offer patients more assistance.

Abstract Owing to the economic advancement and the achievement of medical care, in requirements of life, people are more concerned with their satisfaction in spirit rather than essential physical need. They even expect higher quality of life. The discussion related to the quality of life is now becoming one of the predominant studies in all fields. Base on disease characteristics of our citizens, this study focuses on Diabetes Mellitus(DM), the fourth leading cause of death in Taiwan, to explore the factors influencing health-related quality of life(HRQOL)for people suffering from DM. Furthermore, in the light of health policy and hospital management, feasible suggestions for prevention and treatment of DM will be made. The questionnaire named as WHOQOL-BREF Taiwan version, developed by Yao Kai-Ping et al., is used in this study as the measurement of HRQOL, which is divided into four domains: physical domain, psychological domain, social domain-Taiwan version and environmental domain-Taiwan version. By way of adopting purposive sampling in PuLi township, Nantou county, 262 valid samples consisted of 130 DM patients and 132 non-DM patients are included. Data on medical resource consumption is collected from the claim data of national health insurance of a local teaching hospital in PuLi township, Nantou county between March 2003 and April 2004. Results of this study indicate that the reliability (Cronbach’s α) of the questionnaire is higher than 0.70 except for the social domain (slightly lower than 0.70), revealing good internal consistency. Comparing four domains in HRQOL of DM patients with non-DM ones, the result shows the scoring in physical domain of non-DM patients is significantly higher than DM patients. This demonstrates that having or not having DM indeed affects the HRQOL. Moreover, after generalizing about factors affecting HRQOL of DM patients with statistical analysis, education level and total medical expenditure are the main affecting factors; others, e.g. gender, age or marital status, are not remarkably significant. Base on the above, it is suggested to adjust the health education flexibly according to the education level of DM patients under considerations in time and prime cost of hospitals. In the treatment aspect, it is recommended setting up an effective disease management model through the cooperation of doctors, nurses and patients, in order to lessen disease manifestation and complications. In this way medical resource consuming can be reduced, and patients would be able to acquire improved health and better quality of life. Due to the advance of medical science, human lives can be prolonged. And when living standards of human being are promoted, people begin to think how to live in better lives rather than just live; therefore, health-related quality of life has become implementation of the National Health Insurance (NHI) in 1995, rapid increase in health expenditure has caused unbalanced financial status. How to control the growth of health care utilization has become a critical but political issue. Based upon these reasons, this study aims at exploring the relationship between utilization of health resource and health-related quality of life (HRQoL). In other words, how to improved HRQoL but control health resource utilization is the theme of this study. World Health Organization quality of life instrument: WHOQOL-BREF Taiwan version was used to assess quality of life of 520 residents of PuLi township of Nantou County in this study. Their health resource utilization data were also collected from claim databases of PuLi Christian Hospital. After matching these two datasets, the relationship between health resource utilization and quality of life was examined; and factors which associated with quality of life and resource utilization were also studied by apply multiple regression analyses. Result of the study indicates that quality of life and health resource utilization was negative correlated. In other words, an increase in health resource utilization may associate with poorer quality of life. Another important finding of this study is that self-perceived healthy status may result in better quality of life without increasing in consuming health resources. In other words, health authority of governments should spend more on health education so that citizens in Taiwan may have better understanding and consciousness of their health, which will result in less consuming health care resource and improve their quality of life. As medicine and science keep progressing, it’s not as difficult as before to keep healthy. People begin to pay attention to other events rather than just health, amongst which “quality of life” is a particularly serious issue. This study aims at analyzing the differences of health-related quality of life (HRQL) between aborigines and non-aborigines, and examining the factors affecting their HRQL. The research tool of this study is WHOQOL-BREF Taiwan version, and this research is a cross–sectional study which conditionally sample residents living in Puli Township of Nantou county. 255 aborigines and 670 non-aborigines were included in this study. Results indicate that there are differences of HRQL between aborigines and non-aborigines, particularly in the environment domain. Factors affect HRQL including: gender, marriage, family structure, social-economic status (SES), balance between revenue and expenditure, daily sleeping time, daily sporting time, self-perceived health status, self-reported happiness level, and the frequencies in receiving medical treatment last year. The affecting factors responsible for the observed differences in HRQL between aborigines and non-aborigines are “chronic illness” and “sick frequency”. It’s apparent that the impact of suffering chronic illness in aborigines is greater than that in non-aborigines. Based upon these results, this study suggests that health authority should review their policies to establish aborigines’ correct health conceptions. Additionally, it is also suggested that hospital administrators in remote areas to respect the aboriginal culture, to follow the example of “Community Health Center” to substitute for existing IDS services, and to avoid inducing health service demands.

Due to the development of medicine today, people desire for longer life and better quality of life. However, without proper adaptation of stress, the change of environments and infra-structure of healthcare industry may lead to psychological diseases or substance abuse, especially alcoholism. This study evaluates the effect of alcohol-dependency on quality of life and medical care utilization. Results of this study will be used to provide useful information for making better health policy and management. The evaluation tool for alcohol-dependency in this study was the Michigan Alcoholism Screening Test (MAST). Both the MAST questionnaire and the WHOQOL-BREF questionnaire-Taiwan version, translated by Yao Kai-Ping et al., were used in the study. WHOQOL questionnaire measures health related quality of life(HRQOL) to four domains: physical domain, psychological domain, Taiwan social domain and Taiwan environmental domain. 519 people lived in PuLi township, Nantou county were selected by applying purposive sampling. Their medical resource utilization data were also collected from Puli Christian Hospital and neighboring clinics during March 2003 to April 2004. Results of the study indicate that the reliabilities (Cronbach’s α) of the WHOQOL-BREF(Taiwan version) questionnaire and MAST were higher than 0.6 in all domains. This indicates good internal consistency in existence with these two questionnaires. Although it can not confirm a significant correlation between alcohol-dependency, quality of life and medical care utilization, it was found that there were high correlations between alcohol-dependency level and gender, age, religion, race, smoking and betel nut chewing. Besides, age is found to be the most important factor with was associated with medical care utilization. Age, religion, education, profession, social economical status, race and medical care utilization were main factors influencing alcohol-dependency.Based on results described above, author suggests that the Bureau of National Health Insurance should consider to adopt new strategies to improve existing payment mechanism and set up new treatment institutions to encourage proper alcoholism cure. At the same time, increasing the knowledge of health–promotion and reducing the non-essential medical utilization would improve people’s health and provide them with better quality of life.
