Abstract:

Abstract:
The aim of this study is to investigate students in classes 1-2-3-4, studying Pyschological Guidance and Counselling (PGC) at a university in T.R.N.C. (Turkish Republic of Northern Cyprus) in their relationships with their parents and their attitudes towards marriage. The study includes 325 students and was conducted during the spring of 2015-2016. ‘Personal information forms’, ‘İnönü Marriage Attitudes Scale, ‘Attitudes towards Parents Scale I and II’ have been used in order to collect research data. The viewpoints of the student PCG group, who work with child and family, have been particularly focused in connection to all of these. The findings revealed that female students’ attitudes towards their mothers are better compared to that of male students. However, in terms of gender, no connection was found between attitude towards their father and their attitude towards marriage. As female PCG students’ attitude towards their mother increases, so too has their attitude towards their father and their attitudes towards marriage. For the men whose parents are still together (not separated), there is no link between their strong attiudes towards their parents and their attitude towards marriage. It has come to light that female students, whose parents are separated, have a stronger attitude towards their mother and father. No connection was found between the students’ parents being married or having separate living arrangements and the students’ gender. Male students with parents who live separately have a stronger approach towards their their mother and father.

Abstract:
Aim:Changes in mood and anxiety constantly follow nearly all clinical categories which we encounter in working with adolescents. While recent studies that followed the therapeautic effects of antidepressants on children and adolescents report results which argue against the use of antidepressants on children and adolescents, our experiences show that within the therapeutic process it is often necessary to include medication, keeping in mind their developmental processes as well as the risks and protection i factors in potential psychopathological phenomena. The study is directed towards the confirmation of the therapeutic effects of Tianeptine in the treatment of adolescents.Method:60 adolescents (ages 15 - 23) took Tianeptine in a daily dosage of 37,5mg and were followed for the duration of 3-6 months. Tianeptine monotherapy was applied for 31,8% of adolescents, while we involved 68,2% of adolescents in some form of pscyhotherapeutic work. Clinical monitoring was conducted with the HRDA scale. Psychological testing (MMPI, PIE) was done at the beginning of therapy and after three months. Results:Statistically, a less significant depression and anxiety resulted in both clinical and pyschological tests following the utilisation of Tianeptine in both monotherapeutic as well as combined psychotherapeutic techniques. Conclusions:Our results confirm the therapeutic effects of Tianeptine on symptoms of depression and anxiety in adolescents. We want to point out the efficacy of a rational application of antidepressants for adolescents, especially when the same is utilised as part of a complex medical psychotherapy treatment of adolescents.

Abstract:
The older population is at a high risk of Vitamin-D deficiency due to decreased cutaneous synthesis and dietary intake of Vitamin-D. Results from previous studies examining associations between serum 25- hydroxyvitamin D (25(OH)D) concentrations and depressive symptoms are equivocal. Hence this study was undertaken in order to find the association between vitamin D and symptoms of depression, anxiety and stress in a cross-sectional analysis of a population-based sample of elderly residing in Bangalore city. The objectives of this study are 1. To assess Serum 25(OH) Vitamin-D in the elderly. 2. To assess symptoms of depression, anxiety and stress in the elderly with different levels of Vitamin-D. This study involved 80 healthy elderly subjects with consideration of inclusion and exclusion criteria. Written informed consent was taken. For each subject, fasting blood sample of 4ml was collected for Serum 25(OH) Vitamin-D assessment. Anthropometric measurements were taken. 24-hour dietary recall, General History Questionnaire and Mental health symptoms were assessed using the 21-item Depression Anxiety Stress Scales (DASS-21). Associations between serum 25(OH)D concentrations and total DASS-21 scores and subscale scores of depression, anxiety and stress were explored using negative binomial regression, adjusting for age, race, BMI and physical activity. The comparison of Serum Vitamin-D levels and DASS-21 components showed that the mean scores for Depression, anxiety and Stress were significantly higher in the Vitamin-D deficient participants, indicating that the Vitamin-D deficient participants had worst Psychological consequences. It can be concluded that a significant correlation was found between Serum Vitamin-D deficiency and poor psychological health. Based on the results of this study, psychological health for elderly can be improved by providing regular examinations and checking their Serum Vitamin-D levels and supplementing Vitamin-D to the elderly to enhance their quality of Life.

Background and aims: Anxiety and depression are common in patients with chronic obstructive pulmonary disease (COPD). The degree of lung function may not explain anxiety and depression. The aim of our study was to assess the psycholog- ical aspects of COPD, to test the BODE index (a composite score of body mass, obstruction, dyspnea and exercise capac- ity), and to evaluate the association between atypical cyto- logic findings of sputum, bronchoalveolar lavage (BAL) and the pyschological components of the disease. Methods: COPD was classsified according to the GOLD stages based on forced expiratory volume in 1 second (FEV1) in 60 stable patients. The BODE index was calculated for grading COPD. The Hospital anxiety and depression (HAD) scale was used to appraise the anxiety and depression symptoms. Cytologic examination of sputum and BAL samples were per- formed in each patient. The cytologic findings were classified as normal, mild, moderate or severe atypia. Results: The overall prevalance of anxiety and depression symptoms was 41.7% and 46.7% respectively. The prevalance of these symptoms increased with increasing BODE stages and correlated well with the severity of atypical BAL cytology results (p < 0.001). Dyspnea and reduced exercise capacity were the predominant mechanisms leading to anxiety and depression symptoms associated with COPD. Conclusions: We conclude that the BODE index is superior to GOLD stratification for explaining anxiety and depression symptoms in COPD. BAL cytologic findings, which reflect the distal parenchymal lung structure, correlated significantly with the presence of the anxiety and depression symptoms.

Abstract:
Abstract: Scoliosis is one of the spine deformities in which a person’s spine is bend laterally associated with or without a rotation of the vertebrae. There are several variations in the etiology, onset, prognosis, and therapy of scoliosis, but the effects of untreated scoliosis are the same, as follows: pain, and disturbances in balance, cardiopulmonal function, emotion, behaviour, and ADLs (activities in daily living). The most common symptoms of scoliosis are abnormal deformity of spine which can cause pain, decrease of quality of life, disability, comestic disturbance, functional limitation, pulmonary problems, probabilty of progression at adulthood, dan pyschological disturbance. In physical examination, midline shift of spinous process, asymmetry of the back, rib hump, asymmetry of the scapula, hip, shoulder and pelvis, as well as leg length discrepancy must be carefully examined. Besides that, a radiological examination showing the Cobb angle is also important to do. The therapy of scoliosis includes observation, rehabilitation therapy namely physical modalities, orthoses/brace, exercise, or invasive therapy such as operation. Additionally, the prognosis of scoliosis depends on sex, curve angle at first recognition, type and rotation of curve, and age of onset.

Abstract:
Systemic Lupus Erythematosus (SLE) is more frequent in women, with a female-to-male ratio ranging from 2-6:1 prior to puberty and 3-8:1 following menopause up to 8-15:1 during their fertile years [1]. SLE commonly begins when women are in their 20s, during the prime of their child-bearing years when they are often beginning to plan their families [2], and may have enormous impact on their childrearing. Although rates of infertility are not felt to be elevated among women with SLE, secondary amenorrhea has been identified in 13-17% of women with SLE who are naïve to cyclophosphamide, compared with a prevalence 1-5% in a healthy population [3]. One reason may be related lower levels of anti-Mullerian hormone [4] and higher levels of elevated anti-corpus luteum antibody levels in female patients with SLE [5]. According to one study, 64% women with SLE had fewer children than originally planned. This is likely a result of many factors including disease and medication impact on fertility and fear of disease flare-up with pregnancy. Moreover, many socioeconomic challenges accompany the disease, particularly concerns about the impact of SLE on child welfare and family life, a feature shared by many other chronic illnesses. One study reported that patients with SLE who chose to have less children than they had previously desired described concerns about inability to care for a child, damage from medications, and genetic transmission of their disease leading to the decision to pursue fewer pregnancies [6,7]. Anxieties regarding transmission and impaired ability to take care of children are among the primary worries of patients with lupus [8]. Nevertheless, this generally does not reflect a major concern of medical practitioners, leading to gaps in communication and discordant goals of care [9]. Despite intact fertility among SLE patients, there is morbidity associated with pregnancy. One study of 13,555 participants illustrated a maternal mortality 20-fold higher among women with SLE compared with healthy age-matched controls [10]. The rate of miscarriage is reported as 21.2% compared with 14% in a normal population. While the percentage of live births ranges from 85 to 90, pregnancy is considered a high-risk situation for female SLE patients [11]. Rate of stillbirth is 5 to 10 fold higher in patients with SLE than in the general population [12]. Preeclampsia is more common in SLE and may occur in up to 20% of lupus related pregnancies [13]. There is also increased risk for fetal morbidity, particularly preterm birth (12%) among SLE pregnancies compared with 4% in controls), intrauterine growth restriction, and neonatal lupus [11,14]. One third of pregnancies end in caesarian section [15]. Pregnancy morbidity is most strongly associated with increased disease activity in the six to 12 months prior to and during pregnancy, especially in cases with renal involvement [16,17]. Other risk factors in pregnancy include presence of hypocomplementemia, elevated levels of anti-DNA antibodies, antiphospholipid antibodies, and thrombocytopenia [18,19]. Moreover, pregnancy and the period immediate following delivery is a well-known time for lupus flare-ups [20]. While the hormonal influence on pregnancy is not fully understood due to the complicated interwoven hormonalinflammatory pathways, a disruption in the balance of Treg’s and Th17 helper cells and elevated IFN-γ appear to be players in generating poorer pregnancy outcomes [21,22]. Other maternal complications are related to the hypercoagulability of pregnancy augmented to the increased coagulation risk in SLE in general. During pregnancy, the risk of venous thromboembolism in patients with SLE is 62 out of 10,000 compared with 7.22 of 10,000 in the general population. Moreover, the risk of pulmonary embolism is significantly increased with an odds ratio of 9.76 [23]. In addition, the risk for stroke is 6.5-fold higher than that of healthy pregnant women [24]. In addition to the effect that SLE itself may impose on pregnancy and delivery, certain related medications are teratogenic. Moreover, cyclophosphamide can actually impair fertility, primarily by causing premature ovarian failure [25,26]. Accordingly, providers are advised to offer child-bearing women GnRH analogue therapy prior to initiation of cyclophosphamide [27]. Furthermore, observational studies have shown that most assisted reproductive techniques are safe and equally effective among women with SLE. There are no official guidelines regarding any specific protocol to be used among SLE patients aside from antithrombotic prophylaxis among women with antiphospholipid antibodies [28,29]. Among those patients who seek contraception, most options are available to women with SLE. Women with antiphospholipid lipid antibodies, even without a history of clotting or obstetric complication, and women with additional clotting risk factors including migraines and smoking, should be advised against use of combined hormones. However, aside from this advisement, most other contraceptive methods have proven to be safe in patients with SLE [30]. Nonetheless, despite vigorous research demonstrated the safety and benefits of contraception in patients with SLE, effective methods of birth control are widely underused. One study reported 55% of SLE patients had unprotected sex occasionally and another 23% engaged in unprotected sex most of the time [31]. Another glaring study found that 55% of patients with SLE using contraceptives regularly were using less-effective barrier methods only, even while on teratogenic medications [32]. These findings highlight the immense obstacle that patients with SLE face in receiving comprehensive care that meets their needs during their fertile years. Over the last decade, there is a growing understanding of the importance of early, open, and continual discussions on the topic of family planning between providers and patients. The ACR and EULAR have devised recommendations for providers to help stratify patients and offer appropriate counseling regarding contraception, conception, and assisted reproduction [33,34]. Despite the progress that has been achieved, future studies are warranted to determine how to best approach these patients and best counsel them through the complicated, interrelated pyschologic and medical issues that accompany SLE during the child-bearing stage.
