Abstract:
Abstract Taking care of a paraplegic may contribute to the caregiver’s fatigue. Sixty family caregivers participated in our study, out of which 30 provided care for paraplegics in hospital, and 30 for paraplegics at home. The Orientation to Life Qestionnaire (SOC-29) was used to measure individual sense of coherence, The Life Orientation Test - Revised for dispositional optimism, The Polish Resiliency Assessment Scale for resiliency, and The Daily Life Fatigue Questionnaire for daily life fatigue. In order to collect data about caregivers an individual examination was applied. People with higher personal resource levels such as sense of coherence, optimism and resiliency are characterized by less severe daily life fatigue.

Abstract:
Background: Non specific low back pain (NSLBP) has been identified as one of the commonest rheumatic disorders in prevalence surveys in Asia-Pacific. Objective: Development of a core questionnaire for identification of risk factors of NSLBP at community level. Methods: Following steps were followed: 1) item generation from literature survey, existing Nordic questionnaires and patient focus group discussions, 2) development of a preliminary APLARCOPCORD English questionnaire, 3) translation into target language, back translation and development of a synthetic target language version, 4) adaptation of the synthetic target language version through tests of comprehensibility, content validity test-retest reliability, and 5) finalization of the English questionnaire. Results: 45 items were generated. A preliminary English questionnaire was developed. Conclusion: The developed English questionnaire will serve as an efficient tool for identification of risk factors of NSLBP in Asia-Pacific communities.DOI: http://dx.doi.org/10.3329/kyamcj.v2i2.13263KYAMC Journal Vol.2(2) January 2012, 187-192

Abstract:
The therapeutic strategy for a discharged patient who suffered a miocardial infarction implies a complexity of therapeutic measures and recommendations that guide the mid- and long-time outcome. A main factor of this process is represented by the thrust the patient has in the physician, physician being the person who prescribes and adjusts the treatment, and decides upon the investigations that need to be done. This is a dinamic process, the status of a patient who suffered a miocardial infarction being able to vary at any time during this evolution, especially in associated pathologies. The patient may be evaluated by multiple doctors, the therapeutic strategy being established by a cardiologist, general practitioner, or a team composed of cardiologist and general practitioner. However, the one who knows this best is the patient. This study is exclusively addressed to the patients (structured as a qestionnaire), future discussion being based on their answers. In this context, following the guideline recommandations concerning monitorization, recovery and the quality of life of the patient suffering from miocardial infarction, best result are obtained when the therapeutic strategy is established by a team formed from cardiologist and general practitioner.

Abstract:
Abstract Introduction A quarter of women Veterans (WVs) receiving VA healthcare meet diagnostic criteria for both insomnia disorder and posttraumatic stress disorder (PTSD). Cognitive Behavioral Therapy for Insomnia (CBT-I) is effective at improving sleep among individuals with comorbid psychiatric conditions; however, no studies have examined the impact of CBT-I in women with insomnia plus PTSD. The current analyses examined changes in sleep symptoms, quality of life (QoL), and mental health symptoms from pre- to post-CBT-I in WVs with and without PTSD. Methods This was a secondary analysis of 75 WVs with insomnia (32 with probable PTSD), who received CBT-I within a behavioral sleep intervention study (NCT02076165). Measures completed at baseline, posttreatment, and 3-month follow-up included: insomnia severity (Insomnia Severity Index, ISI), sleep quality (Pittsburgh Sleep Quality Index, PSQI), PTSD symptoms (PTSD Checklist-5, PCL-5; probable PTSD=total score ≥33), depressive symptoms (Patient Health Qestionnaire-9, PHQ-9), and mental and physical quality of life (Short Form Health Survey, SF-12). One sample T-tests examined changes in ISI, PSQI, PHQ-9, PCL-5, and SF-12 from baseline to posttreatment and baseline to follow-up. Two samples T-tests compared change scores in ISI, PSQI, PHQ-9, and SF-12 between participants with and without PTSD. Results There were significant improvements in ISI (p≤.001), PSQI (p≤.001), PHQ-9 (p≤.001), PCL-5 (p=.001), and SF-12 mental (p≤.001) and physical (p=.03) from baseline to posttreatment and 3-month follow-up (p≤.001-.01). There were no significant change score differences between WVs with and without PTSD from baseline to posttreatment (p=.06-.98) or 3-month follow-up (p=.09-.93). Conclusion CBT-I appears to be an effective treatment to improve insomnia symptoms among WVs with and without PTSD, and may reduce psychiatric symptoms as well. These findings suggest WVs with comorbid insomnia and PTSD benefit from CBT-I. The appropriate sequencing of CBT-I and PTSD treatments remains potentially important, but unstudied.
