Abstract:
It has been suggested that in environments where there is greater fear of litigation, resident autonomy and education is compromised. Our aim was to examine failure rates on American Board of Surgery (ABS) examinations in comparison with medical malpractice payments in 47 US states/ territories that have general surgery residency programs. We hypothesized higher ABS examination failure rates for general surgery residents who graduate from residencies in states with higher malpractice risk. We conducted a retrospective review of five-year (2010–2014) pass rates of first-time examinees of the ABS examinations. States’ malpractice data were adjusted based on population. ABS examinations failure rates for programs in states with above and below median malpractice payments per capita were 31 and 24 per cent ( P < 0.01) respectively. This difference was seen in university and independent programs regardless of size. Pearson correlation confirmed a significant positive correlation between board failure rates and malpractice payments per capita for Qualifying Examination ( P < 0.02), Certifying Examination ( P < 0.02), and Qualifying and Certifying combined index ( P < 0.01). Malpractice risk correlates positively with graduates’ failure rates on ABS examinations regardless of program size or type. We encourage further examination of training environments and their relationship to surgical residency graduate performance.

This article appeared in the print version of this issue in a shortened form, which can be found in Bulletin May 2021, Volume 103, Issue 3, pp144-148. DOI: 10.1308/rcsbull.2021.60. Preserved records suggest that the first licences to practise surgery in England and Wales were issued by craft guilds from the 14th century and probably earlier. A brief chronicle is presented of the training, examining, qualifying and licensing of surgeons by The Royal College of Surgeons of England and its direct predecessors, now a part of history. The context in which these organisations were founded and evolved is outlined along a timeline of almost 700 years. The landmark Medical Acts of 1858 and 1886 are briefly reviewed. Mention is also made of the abolition of the two non-university qualifying examinations that were still being offered in England at the end of the 20th century and of the sale of both jointly owned Examination Halls.

Abstract:
Abstract Background Residents deemed at risk for low performance on standardized examinations require focused attention and remediation. Objective To determine whether a remediation program for residents identified as at risk for failure on the Emergency Medicine (EM) Written Board Examination is associated with improved outcomes. Intervention All residents in 8 classes of an EM 1–3 program were assessed using the In-Training Examination. Residents enrolled in the Focused Board Intervention (FBI) remediation program based on an absolute score on the EM 3 examination of <70% or a score more than 1 SD below the national mean on the EM 1 or 2 examination. Individualized education plans (IEPs) were created for residents in the FBI program, combining self-study audio review lectures with short-answer examinations. The association between first-time pass rate for the American Board of Emergency Medicine (ABEM) Written Qualifying Examination (WQE) and completion of all IEPs was examined using the χ2 test. Results Of the 64 residents graduating and sitting for the ABEM examination between 2000 and 2008, 26 (41%) were eligible for the program. Of these, 10 (38%) residents were compliant and had a first-time pass rate of 100%. The control group (12 residents who matched criteria but graduated before the FBI program was in place and 4 who were enrolled but failed to complete the program) had a 44% pass rate (7 of 16), which was significantly lower (χ2 = 8.6, P = .003). Conclusions The probability of passing the ABEM WQE on the first attempt was improved through the completion of a structured IEP.

Abstract:
Abstract Background To determine objective resident characteristics that correlate with Ophthalmic Knowledge Assessment Program (OKAP) performance, as well as to correlate OKAP performance with Accreditation Council for Graduate Medical Education (ACGME) milestone assessments, written qualifying examination (WQE) scores, and oral board pass rates. Methods Review of administrative records at an ACGME-accredited ophthalmology residency training program at an urban, tertiary academic medical center. Results The study included data from a total of 50 resident physicians who completed training from 2012 to 2018. Mean (standard deviation) OKAP percentile performance was 60.90 (27.51), 60.46 (28.12), and 60.55 (27.43) for Years 1, 2, and 3 examinations, respectively. There were no statistically significant differences based on sex, marital status, having children, MD/PhD degree, other additional degree, number of publications, number of first author publications, or grades on medical school medicine and surgery rotations. OKAP percentile scores were significantly associated with United States Medical Licensing Examination (USMLE) Step 1 scores (linear regression coefficient 0.88 [0.54–1.18], p = 0.008). Finally, continuous OKAP scores were significantly correlated with WQE (r s = 0.292, p = 0.049) and oral board (r s = 0.49, p = 0.001) scores. Conclusion Higher OKAP performance is correlated with passage of both WQE and oral board examinations during the first attempt. USMLE Step 1 score is the preresidency academic factor with the strongest association with success on the OKAP examination. Programs can utilize this information to identify those who may benefit from additional OKAP, WQE, and oral board preparation assistance.

Abstract:
The researcher looked into the factors that affected the performance of Rizal Technological University CPA board examinees on the 2012 licensure examinations to determine the intervention necessary to improve the university’s review programs. Descriptive research design was used wherein twenty-four Bachelor of Science in Accountancy graduates who took the May and October 2012 board examinations were surveyed using a validated researcher-made questionnaire. Documentary analysis was used for evaluating qualitative data. Results revealed that: student factors (interest in the accounting program, time spent for studying lessons and availing of library/internet resources in research activities) and home/family factors (family financial support, parental involvement in studies, motivation and encouragement) highly influence performance; school factors (adequacy of relevant library books and materials, adequacy of technology hardware and software, good accounting program and good accounting faculty) were perceived to have average influence; and scholarships, grade weighted averages and attendance to review courses handled by prestigious review schools were found to be positive factors in passing the board examinations. Based on the findings, the researcher recommended that the regular assessment through qualifying examinations be conducted and that the pre-board/mock examinations in the review classes be strengthened.

Abstract:
The American Board of Surgery In-Training Examination (ABSITE) score is used by general surgery training programs as a measure of the medical knowledge component of the Accreditation Council of Graduate Medical Education competencies. Poor performance on ABSITE (<35%) has correlated with high failure rates on the American Board of Surgery Qualifying Examination. We previously demonstrated an improvement in ABSITE scores over a 1-year period through the initiation of weekly reading assignments and review examinations. We sought to determine whether this educational endeavor would result in a sustained improvement in ABSITE scores over several years. ABSITE scores from two successive, 3-year time periods (1999-2001, Group 1, 2002-2004, Group 2), were compared. For Group 1, no formal reading assignments or preparatory exams were given. For Group 2, weekly reading assignments and examinations were administered by the faculty. For Group I, the mean ABSITE score was 60 per cent, versus 75 per cent ( P < 0.01) for Group 2. In Group 1,13 out of 76 scores (17%) were below 35 per cent, whereas in Group 2, only 4 out of 74 scores (5%) were below 35 per cent ( P = 0.03). Sustained improvement in ABSITE scores and fewer scores <35 per cent were achieved over a 3-year period with an educational program of weekly topic-specific reading assignments and written review examinations.

Abstract:
The graduates it produces often determine the quality of education offered by an institution. In like manner, the quality of graduates produced by an institution is measured by their performance in licensure examinations. The study determined the level of student’s academic performance, aspirations, attitudes towards accounting and study habits. It also determined the performance of the graduates in the Certified Public Accountant (CPA) licensure examination, as well as the relationship of the academic performance, aspirations, attitudes towards accounting and study habits and the performance in the licensure examination. One hundred twenty-four (124) respondents were considered in the study which utilized the descriptive-correlational method. A survey questionnaire determined the level of academic performance, aspirations, attitudes and study habits. Official results of the CPA Licensure Examination were retrieved from the Professional Regulation Commission. The study employed frequency counts, weighted means, ranking and regression analysis as statistical tools. The respondents had good academic performance, a high level of aspiration, a favorable attitude towards Accounting and good study habits. Performance of graduates is above the national passing percentage, though, graduates do not make it in the first attempt. Graduates performed best in the Theory of Accounts while the weakest performance is in Practical Accounting I. Academic performance, attitudes towards accounting and study habits are significantly related to the performance in the examination while the level of aspirations was not related. The study recommended the emphasis of students’ persistence to work toward their goals, study habits and passion in accounting in admitting prospective students. Regular assessment through a qualifying examination is also recommended to improve performance.
