Abstract:
With Chiptech’s current growth rate and size it has become necessary to develop a Quality Management System to enable repeatability, meet customer demands, and protect Chiptech from staff turnover. ISO 9001 was identified as a base for development, with the imperative that the system identified and developed must deliver value for Chiptech. Several frameworks were investigated, along with journal articles and discussions with industry members in order to determine the aspects that would deliver value, and determine the key success factors. Two factors were identified as critical: employee involvement, and the utilisation of metrics – both of which were leveraged for the project results and recommendations. The systems developed have already proved they offer benefits, however, in order to maintain performance Chiptech must a) keep evaluating the measured results, b) ensure that quality forms an integral part of the organisational culture and, c) continue the systematic approach of continual improvement.

Abstract:
In order to achieve accreditation for their services, pathology departments must meet international recognised standards and comply with national guidelines, issued by accrediting bodies such as Clinical Pathology Accreditation (United Kingdom Accreditation Service), the Medicines and Healthcare products Regulatory Agency and the Human Tissue Authority in United Kingdom. Objectives: An evaluation of existing Quality Management System (QMS) and implementation of a quality standard and a centralised electronic document management software (Q-pulse) in a pathology department of a district hospital. Method: A service evaluation utilising an adapted version of the Context, Input, Process, and Product model, (with the addition of the implementation stage). This consisted of context evaluation (an evaluation of the department’s decentralised QMS), input evaluation (an assessment of the various types of electronic document management system (EDMS), implementation (the implementation of a centralised QMS and EDMS), progress evaluation (monitoring, documentation and assessment of the progress of the QMS) and finally impact evaluation (measuring the impact of the newly implemented system). Data collection included quality audits, non-participant observation and qualitative interviews. The implementation strategies involved technical support training and feedbacks. Results: The implementation of a continuous QMS and EDMS within the department, using the adapted CIPP model, increased the involvement of all staff in quality matters. It enabled the department to improve quality control processes, procedures and performance. Staff and quality teams now meet regularly and multidisciplinary workings groups have been put in place to continual improve the quality matters. The level of compliance and awareness to standards and guidelines has increased through education and training of staff in within the QMS, and there is an increase in top level management involvement in quality activities within the department. As a result of this evidence of compliance, the department’s Quality management system, was accorded, an unconditional accreditation, by the accrediting bodies. (Clinical Pathology Accreditation (United Kingdom Accreditation Service), the Medicines and Healthcare products Regulatory Agency). Conclusion: The quality management system is functioning in all the departments. All staff involved in the evaluation processes, judged it to be useful for them and the department. Further research is required to establish the longevity of the implemented products and other modules within Q-Pulse.

Abstract:
Quality management (QM) is viewed as concepts, principles, or practices within which prescriptive views and empirical facts play roles in constructing and operating the industry to improve the performance. The growth of Building Maintenance (BM) as a proportion of the construction industry’s output has led to increasing awareness of the need to manage buildings effectively. In the Kingdom of Saudi Arabia, the cost of construction projects in Riyadh City 2014 is around SR 181 billion, and that figure does include the operation and maintenance projects that cost SR 10 billion in 2014. However, this segment of the industry faces several challenges in the Kingdom. This work draws on five Quality Management Concepts (QMCs) (Total Quality Management, Six Sigma, Lean Management, Lean Six Sigma, and ISO 9001) to underpin the research principles, methodology, and implementation. From this research, The primary aim of this research is to investigate the Quality Management System (QMS) required to improve Saudi public Building Maintenance (BM) practices through the implementation of the most suitable and effective Quality Management Concepts (QMCs). The nature of BM is examined in detail in the literature review, to ensure the subsequent collection of appropriate knowledge and information from the empirical interviews and focus group discussions. The first qualitative exercise relates to interviews conducted to collect information to examine the current BM processes in public departments, with a view to ascertaining underlying problems and assess awareness and implementation of QMCs. This was followed by a second qualitative technique, the focus group, intended to explore the most suitable and effective QMCs for implementation in BM departments. After that, the QMS was developed and then validated by focus group method a second time. In this study, thematic analysis is used for both qualitative methods. The most significant problems facing the public BM sector were identified and then categorised into three major groups: (1) top management problems, (2) human resource problems, and (3) technical problems. The main results of the study emphasise that ISO 9001 is the most suitable foundation for quality management of BM and it is found to be an effective baseline on which the BM process can be improved. It was established that there should be specific guidelines for QM in BM (quality management system) which have been developed in this research. The QMS is customised to provide the information required to improve current practices in BM industry. It was confirmed by the evaluation and validation that the developed quality management system can generate positive outcomes, lead to better management, clear responsibilities and improve communication.

Integrated Quality Management System (IQMS) is an appraisal system of the KwaZulu-Natal Department of Education. The appraisal system intended to develop educators on an on-going basis. The primary aim of this investigation was to establish the effect that IQMS has on educator efficacy. As an introduction to the study, the requirements for educator efficacy were reviewed. In so doing, the various characteristics for effective teaching-learning situation and of an “ideal” educator were highlighted. The didactical behaviour of successful educators was also in focus. The requirements for educator efficacy and the effects of IQMS on educator efficacy were investigated using a literature and empirical study. The study revealed that IQMS has a positive effect on educator performances. For the purposes of the empirical investigation, a structured questionnaire was used. Four hundred and six (406) questionnaires were completed by educators from the primary and secondary schools from the Pinetown District in the Ethekwini Region of KZN and these questionnaires were analysed. The data was processed and interpreted using descriptive and inferential statistics.

Successful quality management systems proceeding and continual development, that determine systems maturity, is not self-contained phenomenon. Persuading for quality management systems maturity, organization must create suitable conditions for that. Organization must determine maturity factors and tend for these factors suitable impact to quality management system. Information factors are ascribable to factors, that determinate quality management systems maturity. But still now, the whole of information factors is not institutionalized sustaining by information since theoretical base. Characteristics of these information factors are defined insufficiently too. Defined uncertainty direct, why organizations can’t create suitable conditions for quality management systems maturity. These problems incite the research purpose – indicate the whole of informational factors, that impact quality management systems maturity, and clarify theirs influence to system development.

Siekdamos kokybės vadybos sistemos brandos organizacijos privalo sukuti tam tinkamas sąlygas: identifikuoti brandos veiksnius ir rūpintis jų tinkama įtaka. Informaciniai veiksniai yra priskiriami veiksnių, lemiančių kokybės vadybos sistemos brandą, kategorijai. Tačiau iki šiol, informacinių veiksnių visuma, įtakojanti kokybės vadybos sistemų brandą, nėra identifikuota remiantis informacijos mokslų teorine baze. Nepakankamai apibrėžtos ir veiksnių charakteristikos, užtikrinančios tinkamą įtaką kokybės vadybos sistemoms. Abstract:
Managers today are faced with a bewildering choice of Management Standards that are being promoted to improve personal and organisational performance by a wide range of Standards-setting bodies. Standards-based management is a well-researched field, but all the research concentrates on individual Standards such as ISO 9000 and 14000 without identifying the influences of other Management Standards. This research seeks to extend the debate about Standards-based management and to encourage other researchers to consider it as both an entity and a phenomenon and to note its divergence from other management theories. A taxonomy of Management Standards is presented to enable the principal Standards to be categorised and a definition of a Management Standard is proposed to enable the distillation of the considerable body of literature into more manageable proportions. A chronology of Management Standards development is tabled and compared with the evolution of Quality Management and Total Quality Management (TQM) and the possible future development of Management Standards is examined. The literature research confirmed that Standards-based management approaches had increased very significantly, despite a lack of empirical research to show that this method of management yielded uniform improvements. It identified a trend for integrating Management Standards and creating a “super” Management Standard that would incorporate all the functions of management within an organisation. The Scottish Quality Management System (SQMS) is an integrated Management Standard that incorporates many of the key functions of management within an audited Management Standards framework that its architects claim is based on the principles of TQM. While originally developed for the Scottish training provider network in 1993, it has spread internationally and is currently in use by circa 600 organisations in several countries. It is the only example of a Management Standard of this kind, and despite its longevity, extensive coverage and significant public investment, it has not attracted any previous research interest. The SQMS project was ambitious, seeking to impose a major new Standard on a diverse and predominately unsophisticated population of organisations within a tight time frame. The field research surveyed the entire SQMS registered organisation population using a questionnaire that was based upon the principles of ISO 9004: 1994 and some preliminary research. The questionnaire was designed to examine attitudes and motivation, as well as gauging the effectiveness of the SQMS Standard as determined by an analysis of the improvements that could be directly associated with its implementation. The survey achieved over 70% response rate and the resulting data set was comprehensive and the analysis robust. An additional element of the research compared the results of the questionnaire response analysis with the perceptions of the SQMS Auditors who had audited a majority of the respondent organisations. The field research showed that the SQMS population was very experienced and had been working with SQMS for well over three years. This is a period that the literature research indicated was the minimum time required for the benefits of quality improvement initiatives to become measurable. In the absence of any clear published objectives for the implementation of SQMS, a number of likely objectives were postulated and these objectives were measured against the results of the research. The results of the research suggest an attitude of compliance, rather than improvement as the underlying mind-set of the SQMS organisations and an absence of quality improvement planning or measurement of key performance indicators. This attitude was no different among organisations that had also adopted ISO 9000 and IiP. SQMS accreditation had not shown to provide any marketing or promotional benefit in the same way as ISO 9000 and it had not led to increased market share or profitability. Few of the possible benefits of applying a Standard of this nature had been realised by the majority of organisations and there was little evidence of tangible organisational improvement. The Standard could not be deemed to have met its proposed objectives and a comparison with an accepted model of TQM did not find sufficient compatibility to designate SQMS as a tool for TQM implementation. The interviews with SQMS Auditors identified major discrepancies between their perceptions and the views of the organisations that they audited. The admission by over half of the respondent organisations that they produced evidence purely to satisfy audits raised questions about the effectiveness of the Standard and its audits and the competence of auditors to audit “super” Standards. The novelty of the research is examined and suggestions for future research proposed.

ENGLISH SUMMARY: Total quality management (TQM) can be defined as a systemic approach on a global level, based on process management of continuous quality improvement by all human resources within the business or company environment, with the specific intent to satisfy the implicit expectations of all stakeholders in the specific business environment. Various factors play a role in the active drive towards a quality-driven learning environment. These factors include increasingly changing market forces, changes in customer requirements and the very way in which quality is perceived by the employees within a company. The above statement raises some important questions, such as • What is the quantifiable value of quality, and • Why is it very easily ignored by various companies? The reason for such questions is the significant shift needed in the thought patterns of management, difficulty in abandoning misconceptions about TQM and difficulty in learning from own mistakes and those of others. It seems that the biggest barrier to the implementation of a total quality system is the misconception that quality will immediately be perfect and is a quick solution which is self-sustaining. TQM is, in fact, not a model that is built in concrete, but a journey consisting of sequential steps. As with any staircase, it can only be sustainable if it is solidly founded on factors such as managerial commitment, drive, fairness, motivation and mobilisation of human resources.

Many top performing businesses, which achieve superior levels of success and sustainability, have a sound, implemented, and well maintained, Quality Management System (QMS). The correlation between business success and an implemented management system has been shown in numerous papers. This research, which culminates in a quantitative measure of QMS performance, was conducted at Eskom’s Koeberg Nuclear Power Station (KNPS). The power station is the operating leg of the Koeberg Operating Unit (KOU). The researcher is a QMS lead auditor in the KNPS Quality Assurance Department. A program of audits is planned based on the KOU quality and safety manual and the national regulatory licencing requirements. The audit monitoring program is then implemented over a three year period and considers all the management system processes which impact on nuclear safety and business performance. The individual audits each consider ISO 9001 criteria in context of the business area audited. Each major business area (e.g. design, maintenance, etc.) within the power station adheres to all generic ISO 9001 QMS clauses and considerations, such as documentation management, records management, etc. Each process or business area audit is thus effectively a QMS audit. The audit results, when combined are therefore a representative measure of the overall organisational QMS performance. The potential value to be gained from the audit results and data accrued over the monitoring period has not been optimised to maximise the return on investment to Eskom. The research problem statement thus proposes that the performance measurement capability of the quality management system at Eskom's Koeberg Power Station is insufficient. This diminishes management's ability to identify business risk resulting from management system deficiencies, which impacts negatively on business performance. The research question seeks to determine how the performance measurement capability of the QMS can be improved to assist management in identifying business risk resulting from quality management system deficiencies in order to improve business performance. The research objectives are supported by the literature study, which identifies the quality management methods currently used in order to measure and subsequently improve business performance. It also shows how QMS performance measurement, when deconstructed and analysed can provide the required insight for supporting management decision making. The research approach is considered inductive in that a theory is developed based on the collection and the analysis of that data. Applied research, will thus serve as the basis of the research methodology as it is considered the most appropriate research approach, based on the need to answer practical questions around the measurement of QMS performance philosophy. The research shows that by introducing additional theming and severity data into the secondary audit findings data, it is possible over time to extract high level strategic direction information when analysing the additional metadata. The dimensions and value of the QMS Performance measuring instrument are: Ø A cause and effect theming philosophy of audit findings providing an additional context to business improvement advice to management. Ø The provision of a QMS process deficiency locator / identifier which targets management action areas for improvement. Ø The provision of a quantitative measure of the management system performance, providing a reference from which to improve. By providing a quantifiable measure of an organisations QMS performance, a reference point is provided to gauge QMS performance and also render a definitive measure to enable performance improvement of the business.

There is a growing tendency in the application of the quality management system to the construction industry. Within this perspective, some quality management standards, like ISO 9001, are utilized to assure the quality in projects. Application of this system to water resources projects is also of importance since they are very large systems having various components for which quality management would improve the overall efficiency. The aim of this thesis is to examine the implementation of the quality management in the design and construction processes of a sample irrigation network in order to evaluate its benefits by the cost of quality, which is assumed to be the most effective performance measure. To this end, the prevention and appraisal costs and failure costs, which constitute the cost of quality,are identified separately and their interactions are investigated on a case study in which the causes of these failures are analyzed and quantified in the form of graphs.

Abstract:
Intelligent Traffic Systems (ITS) can manage on-road traffic efficiently based on real-time traffic conditions, reduce delay at the intersections, and maintain the safety of the road users. However, emergency vehicles still struggle to meet their targeted response time, and an ITS is vulnerable to various types of attacks, including cyberattacks. To address these issues, in this dissertation, we introduce three techniques that enhance the service quality and reliability of an ITS. First, an innovative Emergency Vehicle Priority System (EVPS) is presented to assist an Emergency Vehicle (EV) in attending the incident place faster. Our proposed EVPS determines the proper priority codes of EV based on the type of incidents. After priority code generation, EVPS selects the number of traffic signals needed to be turned green considering the impact on other vehicles gathered in the relevant adjacent cells. Second, for improving reliability, an Intrusion Detection System for traffic signals is proposed for the first time, which leverages traffic and signal characteristics such as the flow rate, vehicle speed, and signal phase time. Shannon’s entropy is used to calculate the uncertainty associated with the likelihood of particular evidence and Dempster-Shafer (DS) decision theory is used to fuse the evidential information. Finally, to improve the reliability of a future ITS, we introduce a model that assesses the trust level of four major On-Board Units (OBU) of a self-driving car along with Global Positioning System (GPS) data and safety messages. Both subjective logic (DS theory) and CertainLogic are used to develop the theoretical underpinning for estimating the trust value of a self-driving car by fusing the trust value of four OBU components, GPS data and safety messages. For evaluation and validation purposes, a popular and widely used traffic simulation package, namely Simulation of Urban Mobility (SUMO), is used to develop the simulation platform using a real map of Melbourne CBD. The relevant historical real data taken from the VicRoads website were used to inject the traffic flow and density in the simulation model. We evaluated the performance of our proposed techniques considering different traffic and signal characteristics such as occupancy rate, flow rate, phase time, and vehicle speed under many realistic scenarios. The simulation result shows the potential efficacy of our proposed techniques for all selected scenarios.

Abstract:
Background: Health is influenced by many determinants, including the social and physical environment, economic factors, and individual characteristics. Diet and nutrition play a critical role in health, both in the prevention and treatment of non-communicable diseases, with dietary risk factors alone causing 5.3% of the total disease burden in Australia. Dietetic care has been shown to improve consumer outcomes, demonstrating its role in addressing the burden caused by poor diet. While quality has been extensively explored in tertiary and, to a lesser degree, in primary healthcare settings, there is limited research exploring quality in primary care dietetics. The overarching aim of this doctoral research program was to explore quality management in primary care dietetics through the lens of complexity science. Five aims were addressed in four sequential stages of research using a qualitatively driven mixed methods design. The five aims were: i) to describe and evaluate the literature on quality improvement strategies used to enhance health outcomes achieved by dietetic care that is delivered in the primary care setting (Stage 1); ii) to explore primary care dietetics through the lens of complexity science (Stage 2); iii) to explore how healthcare consumers describe nutrition care (Stage 3); iv) to explore how healthcare consumers and professionals describe dietetic care (Stage 3); and v) to develop a theoretical approach to quality management for primary care dietetic services (Stage 4). The research was designed and executed from a pragmatic philosophical positioning and, consequently, focused on exploring quality with a solution- and action-orientated purpose.

Abstract:
Quality is often claimed as the 3rd dimension of any project: the success of a project depends on the management of time, cost and quality. However, quality is a much more elusive substance and its management can be problematic. This thesis examines some of the models proposed for managing quality in projects and considers their relevance via a number of case studies. The present study aims to provide a foundation from which a methodology for the management of quality in projects can be evolved. The general definition of “quality” is still discussed and its interpretation in the specific area of project management is open to debate. In this thesis it has been found useful to consider three levels of quality management in the project environment, broadly equivalent to those usually accepted in operations management: quality control, quality assurance and total quality management. Various methods of quality control have been employed in projects for many years. The emphasis of the present study is on the project management equivalents of quality assurance and total quality management and to examine whether they fulfil the true need. A major element of successful quality management is the establishing of constructive methods of feedback. Feedback is also often claimed to be a vital ingredient of successful project management: learning from past successes, and failures. A conduit to provide feedback is often problematic due to the very nature of projects and their finite lifespan. Mainstream thinking believes that standard quality systems employed in the production and operation environment can be incorporated effectively in project management thus increasing operational consistency and reducing the risk of activity or project failure. However, is the model of quality embedded in these systems relevant to projects with their characteristics of uniqueness and long time scales? Initial debate assumed that existing quality management systems would be of benefit in projects, which exhibited a lower level of uniqueness and were closer to an operations management perspective. A literature review followed to gauge the level of importance attached to quality systems and its role in the project process. This determined that there was a need to investigate what existing quality systems, contributed in a purely project environment and what impact they had on improving project success especially in regard to the uniqueness of the projects and to the size of the project organization involved. In addition, the impact an industry-standard quality system had on project organizations compared to project organizations that did not possesses any formal system. To compare and contrast the conventional approach to quality in projects, the complementary areas of systems thinking and system dynamics were explored. Examining an alternative field to quality management was beneficial in providing a different perspective on how systems can be modelled evaluated and applied to real-world applications. This part of the research contributed significantly to the formation of the ideas and opinions on the way in which the concept of quality should be promoted in project management. In particular, the identification of mental models and the use of graphical representations to describe, illustrate and model the tangible and intangible entities found in most types of system. The use of a case study methodology was seen as the most valid way of attaining a holistic view of the complete project process and exploring the salient issues surrounding quality and projects. The fieldwork carried out to facilitate this goal, comprised of a restricted number of in-depth case studies, which encapsulated complete projects. An essential part of this process was the use of participant observation and in part action research, as these approaches increased the access to the available qualitative data whilst being mutually beneficial to the research and to the organisations involved. The scope of the case studies carried out was governed by a number of constraints: • The availability of suitable projects. • The timespan of the available projects matching that of the research, consequently the projects studied are on a relatively small scale. • The organizations in which the action research process could be a valid exchange of services. • The reluctance of organizations in certain industries to allow access to data on projects in particular activities that had led to project failure. The alternative models and techniques offered by systems thinking and system dynamics were explored to see if they could deliver more insights into the diverse aspects of project quality and how feedback in systems can be effectively represented. From the four case studies carried out, it is evident that there is a need for a fundamental revision on how quality is both defined and measured in project management. There is a need for greater emphasis on the acquisition and retention of knowledge by project organizations including the ability to disperse that knowledge by a practical and useful medium. Existing quality management systems still exhibit their origins, which do not take into account the uniqueness and instability of the project environment. In practice, the demand for registration to an industry-recognised quality standard appears to discriminate against the smaller project organization. The impact on the larger organizations is no less significant due to the creation of ‘underground’ parallel working practices, which are a significant waste of resources. The veneer of compliance to a global standard does not help the project organization learn or accumulate knowledge. In conclusion, this thesis proposes that project management needs an alternative methodology to provide a more practical method of project feedback, to enhance the ability of future projects. This thesis puts forward a foundation for this methodology based upon the valuable attributes of the models identified during the research in addition to the value of the case studies compiled. The aim for the implementation of a practical quality system has to be based on a reappraisal of what its purpose is. Therefore, it is proposed that the objective of any system would be to capture knowledge, store and redistribute that knowledge in a manner that makes a positive contribution to future project success. Emphasis is placed on increasing success by the acquisition of knowledge, in contrast to the traditional quality concepts of decreasing risk by the control of activities. In essence a shift from existing mechanistic systems towards more neurocybernetic systems. The increasing capabilities of communication and information technologies make the practicalities of creating this type of system perfectly feasible.

Abstract:
The introduction of standardized quality systems and their development into quality management systems offers the opportunity to evaluate the organizational quality of business entities, regardless of their industry. For professional purchasers it has become common practice to demand evidence of the successful application of a quality management system-a necessary step in the process of pre-qualifying potential suppliers. It has become common practice to have the introduction and operation of a quality management system as a minimum requirement for contract as part of a value chain of potential clients or contracting authorities as a future or continuous supplier or service provider. Moreover, this has to be approved. To do so, purchasers can perform supplier audits or ask for evidence provided by certificates. Typically, certificates stating conformity to the requirements of a quality management system are issued by professional audit service companies, so called certification bodies. This study helps to remedy the dearth of studies concerning the effectiveness of audits conducted by certification bodies. This qualitative study uses a limited number of case studies to analyse and assess the approaches and techniques used by auditors for certification or re-certification audits. Additionally, this study attempts to understand whether auditors involved have taken into account the paradigm change of the early 2000s. To this end, auditors involved in the case studies were interviewed. The interviews were dedicated to understanding the cognitive processes of data analysis and the resulting judgements which led to the issuance of certificates stating conformance to the relevant standards. The study was completed on quality management system audits founded on the ISO 9001:2008. This standard establishes the foundation for various further industry-dedicated standards, for example the IRIS or the ISO/TS 16949. Cases studies were performed out in small sized companies and recorded through audio tape recordings, as well by notes concerning auditors’ observed actions. These were transcribed and coded. Analysis of the data indicates that auditors have a detailed idea of what is to be found, regardless the type of business an auditee conducts and the aims of a company. The structure, policy, and individual processes of the audited company did not seem to be of interest. Additionally, audits were performed on the results of audited processes more so than on the progress of the process action and their input/output relation. The interviews shed light on auditors’ understandings of the underlying concepts of the ISO 9001:2008. The results show that the auditors examined still have problems with applying these concepts for their audit practices. Furthermore, the study shows the relationship between power and independence of auditors and their impact on effectiveness. Consequently, the study shows that a certificate issued by a professional audit service does not guarantee conformance to the ISO 9001:2008.

ENGLISH ABSTRACT: The processes of monitoring, modelling and managing the water quality of a catchmerit system including all its unique complexities and interrelationships requires an innovative tool or set of tools to help water managers with their decision making. Numerous methods and tools have been developed to analyse and model the real world. However, many of these tools require a fair degree of technical expertise and training to operate correctly and their output may have to be translated or converted to meaningful information for decision-making using a further set of analytical and graphical display tools. A more appropriate technique for management would be to combine all these functions into a single system. The objective of this research was to develop one such tool, an integrated water quality information system (WQIS). A review of the literature revealed that there has been extensive research and development of tools for the management of individual aspects of water resource distribution, augmentation and quality. However, these tools have rarely been integrated into a comprehensive information system offering decision support to a wide variety of river users and managers. Many of the literature sources also noted that a process of interactive development and integration (i.e. including the intended users in the decision of which components to include, the interface design and the graphical display and output) was vital to ensuring the information system becomes an integral part of the users routine work and decision-making. The WQIS was developed using the recommendations from numerous knowledgeable persons in response to questionnaires, interviews and a prototype demonstration. It includes the results of hydrodynamic river and reservoir simulations and the ability to perform operational river scenario testing. However, the development process is continual and always evolving based on the current or local requirements of water managers. These further developments and research needs are discussed in more detail in the conclusion.
AFRIKAANSE OPSOMMING: Die proses om die waterkwaliteit van 'n opvanggebied, met al die unieke kompleksiteite en onderlinge verhoudings van so 'n stelsel te monitor, modelleer en bestuur, vereis 'n innoverende instrument om waterbestuurders te ondersteun in hul besluitnemings. Talle instrumente en metodes vir die ontleding en modellering van die werklikheid is reeds ontwikkel. Die gebruik van hierdie instrumente vereis gewoonlik 'n redelike mate van tegniese kundigheid en opleiding. Dit mag verder nodig wees om die uitvoer van sulke instrumente te vertaal en/of om te skakel na betekenisvolle inligting vir besluitneming deur die gebruik van bykomende analitiese en grafiese vertoon instrumente. 'n Meer toepaslike bestuurstegniek sou wees om al die funksies in 'n enkele stelsel te kombineer. Die doel van hierdie navorsing was om een so 'n instrument, naamlik 'n geïntegreerde waterkwaliteit inligtingstelsel (WQIS), te ontwikkel. 'n Hersiening van bestaande literatuur het getoon dat daar omvattende navorsing en ontwikkeling van instrumente gedoen is vir die bestuur van individuele aspekte van waterbronverspreiding, waterbronaanvulling en waterkwaliteit. Integrasie van hierdie instrumente, in 'n uitgebreide stelsel wat besluitnemingsondersteuning aan 'n verskeidenheid riviergebruikers en bestuurders bied, kom egter selde voor. The diffusion of a Quality Management System (QMS) to improve business performance depends upon the quality management strategy used. The aim of Eskom’s QMS strategy was to create a sustainable quality drive throughout the company to improve customer and stakeholder satisfaction. This research project’s objective is to determine how the diffusion of a QMS can improve the electricity supply industry in the Western Cape division of the electricity sector in South Africa. The key research objectives within the research study are:  to assess what effect a QMS has on a sustainable long-term solution for energy supply;  to assess how a QMS contributes to the establishment of world class processes and practices;  to determine how a QMS affects quality culture;  to determine what factors of a QMS play a role in a continual improvement process, and  to determine what barriers affect a sustainable QMS implementation. The research question is: “how can the diffusion of a QMS bring improvement to the electricity supply industry?” A quantitative research methodology was employed in the study. The responses to a survey questionnaire were analysed and conclusions were drawn. The research finding is that diffusion of a QMS improves the electricity industry by positively affecting attitudes towards the challenge of securing a sustainable long-term energy supply, improving processes and practices, engendering a quality culture and contributing towards continual improvement. At the same time, certain barriers to the implementation of a sustainable QMS were identified.

As the Indonesian construction industry is now promising to become the engine of national economic development, achieving quality and higher performance, not only domestically but also in the international market, are becoming crucial issues. Implementing quality management systems (QMSs) based on ISO 9001 are important in helping Indonesian construction companies become more competitive, for it is acknowledged that quality in construction is a major concern in the global construction industry. However, the possession of ISO 9001 certification does not reflect the presence of a well-operated QMS, which is capable of giving customer and project end-users satisfaction. The review of literature found that there is a significant correlation between a company.s organisational culture and the quality performance of contractors. While research into this area has involved many researchers, there is no critical mass of information specifically related to the Indonesian situation. Studies based on contemporary perspectives of the characteristics of the Indonesian construction organisational culture using Cameron and Quinn.s Organizational Culture Assessment Instrument (OCAI) and its relationship with the contractor.s ISO 9001 practices, have not been previously undertaken. This thesis research, therefore, investigated the culture profiles in Indonesian construction organisations, together with the current implementation of ISO 9001 and their performance during implementing QMSs, as information in these areas has a bearing on the poor performance and low levels of competitiveness of Indonesian construction companies. Questionnaire surveys were distributed to selected representative grade 7 civil engineering contractors located in the two provincial capitals of Makassar and Manado, and the national capital, Jakarta, in order to collect responses designed to examine the effectiveness of their QMSs implementation. The survey also aimed to identify current problems within the systems, and examine the performance of companies while implementing their QMSs. The questionnaire contained questions to assess the organisational culture profiles of Indonesian construction companies, adopting the OCAI. The survey results were then used to analyse the influence of different organisational culture profiles on QMSs implementation among respondent companies. The results from the questionnaire survey supported the development of a Culture-based Quality Management System Improvement Implementation Framework, designed to help Indonesian construction companies identify some typical barriers associated with impacting on effective QMSs implementation; to assist them to develop cultural values that can drive effective QMSs implementation; to undertake effective QMSs practices; and to recognise the potential results and longer-term benefits obtainable from implementing effective QMSs. A Focus Group Discussion was conducted with the assistance of a panel of professional construction practitioners, policy makers and academic experts, which further considered and validated the usefulness and applicability of the framework. Based on the outcome from this discussion and on the results of the earlier data analysis, a final version of the framework was developed to assist Indonesian construction companies in improving their capability of construction project delivery, and thereby contribute to providing or improving the competitive advantage of Indonesian construction companies in the local, national and global construction market.

Esta dissertação propõe um processo de integração e implantação dos sistemas de gestão da qualidade, meio-ambiente e segurança e saúde ocupacional, os quais, atualmente, operam de forma isolada numa empresa. O negócio da empresa em questão é a produção de gases industriais e pretende que o processo de integração considere a cultura da organização e os esforços movidos nesta direção. A metodologia utilizada para a integração da gestão dos sistemas já é de uso e conhecimento das organizações e da academia e foi selecionada conforme equipe experiente e designada para este propósito, partindo do princípio que não se optaria pela reestruturação e sim pela adequação dos sistemas já existentes. São ainda comentados os pontos fracos identificados no programa de implantação dos sistemas isolados e que deveriam ser considerados quando da integração dos mesmos, assim como as necessidades que direcionam a integração e a expectativa com relação a responsividade, abrangência, resultados coletados, além da mudança cultural promovida. A estratégia é considerada diferencial na decisão da integração dos sistemas e a metodologia Seis Sigma a ferramenta que possibilita a mensuração de custos e a implantação da filosofia da melhoria contínua na busca em exceder as expectativas do cliente.
This dissertation proposes an integration process and the implementation of quality management systems, environmental and safety policies, and occupational health. These systems are presently operating separately in the company. The company´s core business is the production of industrial gases and its internal policies establish that the integration process should take into consideration the culture of the organization and also the efforts already taken to move in this direction. The methodology applied for the systems integration management is under knowledge domain and it is being used by organizations and by the academy. It was selected and adopted by a skilled team assigned for this purpose, and it was defined that, instead of restructuring, adjustments would be made to existing systems. There are also comments about weak points identified in the program of implementation of isolated systems that should be considered for the integration process, as well as the needs that drive the integration and expectations related to responsiveness, scope of work, collected results and also the outcome in terms of cultural change. The strategy is considered relevant when deciding for systems integration and for the methodology Six Sigma, which is the tool that allows costs measurement and the implementation of the philosophy of continuous improvement to exceed customer´s expectations.

Grounded in the Design Science Research Approach in Information Systems, the goal of this research project was to conceptualise, design, and develop, an innovative framework for Programme Alignment, Implementation, and Reporting (PAIR) in order to facilitate student success at a South African higher education institution (University ABC) . For this study, the researcher applied PAIR to two academic departments at University ABC. Construction of the framework was interpretively considered and informed by the Diffusion of Innovations Theory of Everett Rogers (1995). The Blackboard Learn® Goals Area (BbGA), a feature embedded in the official Learning Management System (LMS) of University ABC, was incorporated into PAIR as it afforded the two departments mentioned above the opportunity to provide proof of evidence that programme outcomes were constructively aligned with course content, course assessment, student digital activities, the tracking of students’ performance, and the monitoring of students’ progress. It was argued that PAIR could also provide a basis for professional programme outcomes in the departments mentioned above when aligned to their respective Professional Boards’ accreditation criteria requirements. It was found that, within a decentralised higher education institution (such as University ABC) where an annual quality review of programme and module outcomes alignment and reporting in departments is not compulsory, that PAIR could facilitate such a process as part of a faculty’s and a department’s teaching goals. It was also concluded that PAIR could afford South African higher education institutions in general the opportunity to contextualise outcomes coverage and achievement reporting to improve programme quality and to inform intervention strategies to advance effective student learning and student success.

Abstract:

In today’s society we are surrounded by large amounts of information, quick decisions and high expectations to perform successfully in everything we do. As a statistical agency, Statistics Sweden is responsible for producing some of the information that is used for decision-making in society and is therefore under constant internal and external pressure to perform well. The responsibility to produce high-quality statistics to all customers and users is not simple. What is the quality of the statistics produced? How do we assure and control the quality of the statistics? Do we use our resources efficiently? These are important questions, which need to be addressed.

One way of addressing these and other issues is to work with quality in a systematic fashion. Thus there is a need for a Quality Management System, i.e., a systematic way to handle quality issues of all kinds in all parts of the organization, and to continue the journey towards the vision of being a “world class statistical agency”.

This Masters thesis is a description and discussion of the efforts to find a suitable Quality Management System. The thesis starts with a discussion about the vague quality concept, continues with a description of numerous frameworks, methods and systems related to quality management as well as their pros and cons and ends with a recommendation for Statistics Sweden. The recommendation is to use the EFQM Excellence Model as a quality framework, Six Sigma as a tool-box for improvement projects and modern internal auditing methods for evaluation and follow-up. Finally, issues related to the implementation of the system are discussed.

Abstract:

Active Pharmaceutical Ingredient (API) repackagers play a niche, yet critical role in the expansive pharmaceuticals industry. Many repackagers have sub-optimally implemented quality management systems (QMS) leading to sub-optimal performance and elevated risks. This study sets out to demonstrate a strategy to strengthen the case for quality for this industry sector. Dr. Deming's Plan-Do-Study-Act cycle serves as the study's framework, while Phil Crosby's Quality Is Free philosophy forms the theoretical basis. An archival measure of FDA Warning Letters concerning APIs has been performed to bring a considerable failure cost factor to the forefront. A statistical analysis has been performed on data available from the study's pilot firm to demonstrate relationships between quality-related and revenue performance indicators in order to present a bottom-line approach preferred by management. Results from the study indicate a linkage between QMS factors and business outcomes and call for improved understanding, increased support, and a push towards QMS maturation.

Abstract:
A detailed framework is essential to facilitate quality system implementation. In this study, we have offered a cost effective do-it-yourself approach to quality management. We have proposed a quality system implementation framework for small-medium sized organizations to enable their transition from a no-quality system to an ISO 9001 quality management system. The proposed framework is validated using a case study of a small door manufacturing company. The findings reveal several setbacks experienced during quality system implementation and suggests means to overcome them using a proposed seven step framework. This study also advises an effective maintenance tool to facilitate continuous improvement in organizations after implementing a quality management system. The study results will be useful for quality practitioners, managers, consultants and engineers, especially in small companies and discloses several benefits that can be achieved by employing the proposed framework in any organization irrespective of its size and nature.

Neste contexto, as organizações apostam cada vez mais na gestão da qualidade, encarando-o como uma ferramenta eficaz para fazer face à competitividade nacional e internacional. Não menosprezam toda a atividade relacionada com a inovação, mas consideram que é fundamental uma base com qualidade, permitindo assim atingir patamares superiores. O presente relatório visa abordar a metodologia e o processo de implementação das normas internacionais a nível geral. O conhecimento adquirido ao longo do percurso profissional no desenvolvimento de um software, bem como em outras competências intrínsecas, permitiram idealizar o desenvolvimento de uma plataforma capaz de gerir e implementar um sistema de gestão de qualidade. com o intuito de que este seja capaz de evoluir e de ser mantido de forma organizada. Com a implementação da ISO 27001, neste contexto, e após a análise de soluções existentes no mercado e da estrutura de implementação e certificação das normas internacionais, surgiu a necessidade de apresentar uma solução que vise facilitar este mesmo processo e que também ajude na manutenção de um sistema de gestão. Para este desenvolvimento foi necessário efetuar um levantamento de alguns sistemas que já existiam nesta área, sendo que as soluções encontradas no mercado não satisfaziam a necessidade de um sistema de gestão de qualidade. Apesar destas permitirem gerir este tipo de sistemas de qualidade, não estão focadas no processo de implementação e certificação das ISO. Deste modo, o QMS4.0 tem como objetivo minimizar esta lacuna identificada e facilitar o processo interno das empresas.
Technological developments in industry and in the methodologies used make it imperative for organizations to adopt new mechanisms and new technologies and to reflect on greater efficiency. In this context, organizations are increasingly focusing on quality management, seeing it as an effective tool to address national and international competitiveness. They do not underestimate all innovation-related activity, but considering that, a quality base is fundamental, thus allowing to reach higher levels. This report aims to address the methodology and process of implementation of international standards at the general level. The knowledge acquired along the professional career in software development as well as in other intrinsic skills allowed us to idealize the development of a platform capable of managing and implementing a quality management system in order to be able to evolve and to develop. be kept in an organized manner. With the implementation of ISO 27001 in this context, and after the analysis of existing solutions in the market and the structure of implementation and certification of international standards emerged the need to present a solution that aims to facilitate this same process and that also helps in maintaining a system management. For this development, it was necessary to make a survey of some systems that already existed in this area, and the solutions found in the market did not satisfy the need for a quality management system. Although these allow the management of such quality systems, they aren’t focused on the ISO implementation and certification process. This QMS4.0 aims to minimize this identified gap and facilitate this internal process of companies.

Abstract:
This study explored the views and experiences of School Management Teams (SMTs) when implementing Integrated Quality Management System (IQMS) in three primary schools in the Nkangala Region of the Mpumalanga Province, South Africa. A qualitative research approach was adopted and the data were collected by means of semi-structured interviews and document retrieval in schools. The data were then analysed by the constant comparative method. All the participants in this study expressed the view that IQMS is a teacher appraisal system with a potential to develop educators in schools provided both evaluators and evaluatees are honest with the evaluation process and are able to identify their areas of development. However the experience of the principals and deputy principals with the implementation of IQMS in schools reveals that educators inflate their scores and do not identify areas where they need development. The Development Support Groups (DSGs) do not conduct authentic evaluation since they serve the purpose of helping educators to qualify for salary progression. Participants recommended external evaluation as a solution to subjective ratings since external evaluators have a potential to provide objective and credible evaluation because they are unfamiliar with educators and do not experience the pressure to maintain collegiality in schools. A key recommendation is that the Department of Education should conduct effective training for the stakeholders involved with the implementation of IQMS in schools.

As to improve the quality of domestic public constructions, the Public Construction Commission of Executive Yuan enacted the “Quality Management System of Public Constructions” in October, 1993. With trying to carry out the system, Government promised the improvement of domestic public constructions. The system is based on the three-grade quality management system, which is composed of the construction quality assessment held by superintendent organizations, the construction quality assurance system enforced by sponsoring agencies, and the construction quality control system held by contractors. With the most effective control measures, we were able to perform effective surveillances on materials, equipments, and construction assignments during the process of the construction, then achieve the purpose of improving the quality of public constructions. Due to the fact that the intermediate and small-scaled “People’s livelihood Constructions” are closely related to people’s lives, the quality of constructions would be the indication of how people are able to personally experience the quality of constructions and the policy enforcement efficiency of the government. Therefore, by what means the local district government enforcing the quality control system and improving the quality of people’s livelihood constructions, is the task that should not be ignored. This paper is based on the experience of Taipei County Government’s enforcement on the quality management system of people’s livelihood constructions, and it provides reference to other county governments on the establishment of the quality management system of people’s livelihood constructions. By means of forming the specialized department of quality management which drafts the quality management system and enforcing strategies, and with the well use of Turnkey and project management on bidding invitation systems, we are able to redeem the insufficient manpower of construction sponsoring agencies. Through the propaganda of quality management, we establish general senses of quality with companies. Also with conducting trainings of quality education, we will improve the professional abilities of construction sponsoring agencies. Also, in order to encourage the fine companies of taking responsibility of Taipei County’s constructions, we will hold a fine company election to discuss and elect excellent construction companies and excellent companies of technical services. We expect that by using the methods shown above would carry out the quality management system and improving the quality of people’s livelihood constructions. Eventually, we discuss the people’s surveillance system of public constructions in which we enlarge people’s participation and concern about people’s livelihood constructions, and assure the quality of people’s livelihood constructions.

People attached gradually importance to quality management since the 1980s. Some organizations realized their quality management but couldn’t improve quality management performance. Some studies advocated to create quality culture was important for effective quality management but few quantitative studies verified it. The purposes of this study include: 1.explore the effective factors of quality management system and quality culture; 2.explore the cause-effect relations among quality manage- ment system, quality culture and quality management per- formance; 3.explore the effects of quality management system, quality culture, and their interactive effects on quality management performance. For achieving these purposes, three questionnaires were developed, and surveyed the top five hundred manufacturers in Taiwan area. Data is analyzed with SPSS discriminant analysis to discriminant the organizational variables by quality management system, and canonical correlation analysis to analyze the relationship between environmental variables and quality management system; to discriminant the organizational variables by quality culture, and to analyze the relationship between environ- mental variables and quality culture. Using LISREL path analysis to analyze paths among quality management system, quality culture, and quality management performance and the fitness of models, and SPSS hierar- chical regression analysis to analyze the main and interactive effects of quality management system and quality culture on quality management performance. The results of this study revealed that the companies with middle number of employees attach importance to ‘humanity assumptions’; the companies with the most number of employees attach importance to ‘quality concepts’ and ‘behavior criteria’. The companies are not good at both quality management system and quality culture when their competitive environment is mild, customer’s requirement are easy to quality, and the government’s decrees are free. The fitness of model is good when quality culture as the intermediary variable. ‘quality planning’ and ‘improve- ment action’ influence significant ‘humanity assumptions’, ‘quality concepts’, and ‘behavior criteria’; ‘humanity assumptions’ and ‘quality concepts’ influence significant ‘people performance’; ‘behavior criteria’ influence signi- ficant ‘result performance’ and ‘total quality performance’. The most important influence factor of quality management performance is quality management system, the second is quality culture and the last is the interaction between quality management system and quality culture. To achieve good quality management performance must have good quality management system and go through the quality culture. The results of this study could be help- ful to the quality management practices and the constru- ction of quality management model in disciplinary study.

The general problem was that vast resources (time, money, etc.) have been invested in the Integrated Quality Management System. Besides the generally positive feedback it was not yet known for certain to what extent IQMS contributed to the perception of improved educator performance and the problems which existed with the implementation. The objectives of this dissertation were to determine what the perceived impact of the Integrated Quality Management System on Educator Performance was and prove the validity of a proposed model of factors related to Educator Performance. In order to construct a theoretical frame of reference of the existing knowledge, an extensive literature review of the Performance Management theory in the private sector was followed by a review of the Department of Education publications about the practical implementation of these principles in the management of educator performance in schools by means of the Integrated Quality Management System. Both qualitative (a pilot study consisting of several in depth interviews with educators and principals) and quantitative research (a questionnaire based on the literature study, objectives and hypothesis and using a five point Likert scale) methods were used to determine the impact ofthe implementation of Integrated Quality Management System on Educator Performance in South African public schools. The results from the four hundred and twelve respondents were analysed with the aid of the EXCEL and SPSS computer programmes. It was found that the implementation of the Integrated Quality Management System has contributed significantly to all areas of perceived Education Performance, i.e. structure, staff development, motivation, class visits, feedback, goal setting, problem solving and forms. The IQMS was negatively related to disciplinary management. The non parametric nature of the data could not allow for statistical techniques such as multiple regressions to be run, but the proposed model still revealed itself as valid in the factor analysis. It was concluded that the Integrated Quality Management System has done what it was meant to do: it had a positive impact on perceived Educator Performance. Furthermore, it was found that the factors: structure, staff development, motivation, class visits, feedback, goal setting, problem solving, accurate scores and disciplinary measures had a significant relation with perceived Educator Performance. It was recommended that the success story of the implementation of the Integrated Quality Management System be made public, neutral educators be brought on board, the accuracy of the scores be upgraded and even more structure added to the staff development programme. It was also recommended that School Management Teams be trained in motivating staff, class visits be increased and feedback to staff on their performance be improved. It was also recommended that School Management Teams be trained in goal setting programmes and the implementation of the Educator Performance Model.

Developed till now, this standard can not only provides a completely quality management system but also testify whether the products and services meet the customers’ needs. The purpose of compiling and publishing ISO9004: 2009 Standard is to help the success of organization; the standard is mainly to assist the organization to achieve the goal of sustained success in a complex, demanding and everchanging environment. The purpose of this research is to study the methodology of establishing the Preventive Quality Management System of the IC Industry and to find out the goodness of the system for reference of Quality Management improvement.. In order to keep the impartial position , I Select the Quality system 2001/6/29Common tool-ISO check list as the Quality System check criteria , and I also choose 10 IC suppliers in Asia with different scale as the evaluation sites for comparison ,the result are summarized as following : 1. IC assembly / test Industry Quality system Implement situation reach 98.6% at process document compliance and 91.1% at operation compliance. 7.5% major deviation are related to Quality management deficiency. 2. On IC Industry''s supplier control verification and IQC are Quite conscientious. 3. The performance of Quality system control is not related to company''s scale and QC resources. 4. Weakness on corrective action and preventive problem , moreover the statistic application is not enough . 5. The training and education need further Improve. 6. Process control document and record retention still has lot form for improvement.
