Abstract:
AbstractThis article will discuss how two major contributing factors, functional staging and perceived authenticity, have had and continue to have a powerful influence on the sound of record production across geographical boundaries and throughout history. Functional staging is a concept building on the idea of phonographic staging developed by William Moylan and Serge Lacasse and related to Allan Moore's ‘sound-box’. The staging of sounds in the record production process is considered to be functional if the reason for their particular placement or treatment is related to the practicalities of audience reception rather than to aesthetics. It is not a question of whether the music has a function or not but whether that function has influenced the staging of the recorded music. Thus the divergence of staging techniques used in dance music and rock music that began in the 1970s can be seen as resulting from the different functions the music was put to by the different audiences. Music that is played back through large speakers in a club for the purpose of dancing needs to maintain the clarity of the rhythmic elements and so the ‘drier’ techniques of drum and percussive instrument mixing that characterise dance music developed. Rock, however, was more frequently played back in the smaller, less ambient, home environment and so reverberation was added to simulate the atmosphere of the large-scale venue. At the same time, a variety of culturally constructed notions of authenticity have developed within different musical audiences. Why is it that Queen felt the need to inscribe ‘no synthesisers were used in the making of this album’ on their early records and yet Brian May felt entirely comfortable constructing multiple layered performance ‘patchworks’ of guitar tracks? Why might the use of one type of technological mediation be considered more or less authentic than another? Using examples taken from recordings from all around the world and from ‘art’ and ‘popular’ forms of music, this article will explore how audience-led cultural trends in recording and production practice have resulted in the particular ‘sounds’ of different recorded music genres.

Abstract:
The present study aimed at evaluating histopathological changes in gills of Rhamdia quelenjuveniles submitted to different concentrations of ibuprofen (0.0; 0.5; 5.0 and 50.0 mg/L); concentrations were determined from LC50acute assays of 5.0mg/L(120h). For each concentration four replicates were made, with four copies each. The experiment had a total duration of 120 hours in a chronic toxicological trial. After completion, the animals were euthanized, laparotomized, and the gills were localized, isolated and fixed in 10% formaldehyde, preserved in 70% alcohol and subsequently submitted to histological routine technique for paraffin inclusion. Fivecuts (6μM) per specimen were made and stained with Hematoxylin and Eosin (H&E). The Average Alteration Score(AAS) and Histopathological Alteration Indices (HAI) were determined according to the degree of the lesion and then classified in stages I, II and III which produced respectively Lamellar derangement, Lamella’s epithelium rupture and aneurysm with more expressive alterations. For all concentrations the drug led to histological alterations for Rhamdia quelen, putting in evidence damages that go from moderate to severe in the gills of the analysed fishes. Ibuprofen caused branchial lesions in R.quelenwith a progressive effect in high concentrations, suggesting that the presence of this drug could cause abnormalities and favour mortality in this species.

Abstract:
The aim of the study was to determine the effectiveness of the drug "Cobazin" on the productivity of bee colonies during the increase in strength (increase in the number of young bees in the bee colony) in spring and autumn.
 Materials and methods. Control and experimental groups were formed to test "Cobazin" feed supplement. It was fed 5 times with an interval of 5 days. Bee colonies from the control group received a pure sugar solution (1:1) in the amount of 0.5 liters per bee colony, and the experimental group received a sugar solution of 0.5 liters per bee colony (1:1) with the addition of the drug "Cobazin" in the amount of 2.5 g.
 Indicators characterizing the development of bee colonies were determined by a measuring frame of 5x5 cm in size, for 12 days to cover the total number of eggs laid by the queen bee during feeding. Four measurements were performed during the study. The results of the control and experimental groups were compared and the significance of the differences between them was established. There was a comparison of the difference between groups of bees both fed pure sugar syrup in a ratio of 1:1 and syrup with the addition of the drug "Cobazin".
 Results. It is proved that during the feeding period the number of brood in bee colonies that received feed supplement "Cobazin" increased significantly by 76 % (71 % in the group that received only sugar solution without this drug). Live weight of queen bees in colonies that were stimulated with "Cobazin" was actually 2 % higher compared to the control group.
 It was found that at the end of the bee season, bee colonies fed in May with the feed supplement "Cobazin" had significantly higher values of strength, number of sealed brood and live queen bees compared to those who received pure sugar solution. The brood in the control group was 25 % lower than in the experimental group.
 Conclusions. For the first time it was proved that the use of feed supplement "Cobazin" in combination with sugar solution has a positive effect on the features that characterize the development of bee colonies (strength, number of sealed brood) and their health productivity. The positive impact is observed not only during feeding, but also in subsequent periods of colony development.

The purpose of this paper is to investigate the biological properties of mangosteen in relation to health promotion effects.Design/methodology/approachSeveral research papers from well-known database (such as PubMed, Google scholar, Scopus and Sciencedirect) were reviewed without considering publication-times to understand the biological properties of mangosteen.FindingsMangosteen and its xanthone exerted diverse biological activities such as anti-oxidant, anti-inflammatory, anti-allergy, anti-bacteria, anti-fungal, anti-malaria, anticancer and anti-diabetes.Originality/valueBased on these studies, mangosteen is beneficial dietary supplement of overall human health.

Sex involving illicit drugs is currently receiving close academic attention. For the most part, research in this area is public health orientated and focuses on the experiences of men who have sex with men engaging in ‘chemsex’. In the current article, I use in-depth interview data from 14 participants across a range of gender and sexual identities to undertake a queer criminological analysis of sex on drugs. Taking Gayle Rubin’s foundational Thinking Sex as a starting point, I argue that participants’ narratives construct a sober/drug-involved sex hierarchy in which the former is afforded a higher status. I then utilise critical work on drug ‘effects’, Tim Dean’s work on ‘raw sex’ and notions of stigma to interrogate assumptions underpinning this hierarchy. I argue that the perceived artificial nature of intoxicated intimacy, the unmediated appeal of sober sexual intimacy and stigma connected to certain drugs and certain sex-on-drug-practices underlie participants talk. I conclude that ‘thinking sex on drugs’ demonstrates the productive potential of criminological engagement with queer theory, and vice versa.

Fundamentally shaped by queer and trans activism and labor, Los Angeles’ cannabis markets offer an opportunity to understand how “diverse economies,” as defined by Gibson-Graham, are dynamic, contingent political projects that require contending with power and difference. With data from nearly four years of ethnographic observation and 70-plus interviews, I analyze how numerous Black, Latinx, Native, and Asian and Pacific Islander queer women and transgender economic actors in cannabis have developed labor relations, collective institutional forms, and reciprocal exchange to make cannabis dispensaries a space of care and solidarity. Starting with AIDS crisis-era medical marijuana activism, queer economic actors have built affective relations at the scale of the body with patients, owners, and each other in ways that transcend profit imperatives and bridge across difference. More recently, in the face of economic exclusion and the pervasive gendered division of intimate labor, queer and trans workers of color have turned to the body as a scaffolding for collective action across scales. Drawing from resurgent social movement unionism in the region, they have led intersectional campaigns to protect more-than-capitalist elements of the industry and challenge the carceral state’s drug war. Bridging feminist economic and political geography allows insight to the spatially and temporally contingent nature of diverse, queer economies and their embedding in broader relations of racial, carceral, and homonormative capitalism. At the same time, such an approach centering the active politics of diverse economies surfaces the potentialities for multi-scalar movements to develop and sustain alternatives to capitalism.

On February 26, 1988, rookie New York City police officer Edward Byrne was shot dead while guarding a material witness in a drug trafficking case in South Jamaica, Queens. This article considers how state narratives and visual rhetoric emerging from Byrne’s murder emboldened the police power and a revanchist campaign aimed at “taking back the streets” secreted under the war on drugs. As such, this case powerfully illustrates a disparate politics of death and the ways that the state enlists thanatopolitical power in order to reaffirm and reproduce its sovereign authority. Such a reproduction or reanimation of power registers as the state’s ability to unleash violence unequivocally and unequally upon poor and marginalized communities, as later demonstrated by the legal and proper police murder of Sean Bell, a resident of South Jamaica, Queens killed by NYPD agents in 2006.

The present report is the case of a 26-year-old man, born with Usher syndrome. The patient had had a significant hearing impairment since birth and had developed retinitis pigmentosa. He had originally been diagnosed with a depressive disorder and treated with antidepressants, with no subsequent improvement in his mental state. Following a deterioration in his mental state he was admitted for reassessment at the Queen Elizabeth Psychiatric Hospital, Birmingham, and antidepressants were stopped. It subsequently became apparent from observations, interviews with the patient and information from the patient’s carers and relatives that he had a psychotic illness. Treatment was started with the antipsychotic drug risperidone, after which he showed significant improvement. The association between Usher syndrome and psychosis is discussed

BackgroundMedication errors in pediatric care are common and clinically important. The greatest risk of error occurs during drug prescription and administration of medicines. Medication errors can be prevented if potential sources of error are identified and addressed.ObjectiveExplore and describe the physicians' and nurses' views on the risks regarding pharmacological treatment of pediatric inpatients. Find out what the obstacles might be for the staff in their work with prescription, preparation and administration of medicines, and what might constitute help and improvements to reduce medical errors.MethodWe used three different methods to identify and describe the risks of pharmacological treatment at the department of pediatrics at Queen Silviás Children's Hospital in Gothenburg; reports from the medical error report system MedControl, targeted interviews with nurses and doctors and a questionnaire survey to all doctors and nurses.ResultsThe study revealed three main problem entities; difficulties related to drug prescription and the IT-system for computerized prescriptions, a need for improved support systems for drug prescriptions and administration as well as hazards relating to work environment such as disturbances, lack of time, and high staff turnover.ConclusionDrug treatment for children is a delicate matter that places high demands on everyone involved. A pressured work environment, incomplete IT systems and gaps in supports system for drug prescription and administration are unnecessary risks. To overcome these difficulties we propose extended local prescription and administration support, coordination of procedures between wards and development of IT systems adapted to the specific needs in pediatrics.

Royal jelly (RJ) is a glandular secretion produced by worker honeybees and is a special food for the queen honeybee. It results in a significant prolongation of the lifespan of the queen honeybee compared with the worker honeybees through anti-inflammatory, anti-oxidant and anti-microbial activities. Consequently, RJ is used as cosmetic and dietary supplement throughout the world. In addition, in vitro studies and animal experiments have demonstrated that RJ inhibits cell proliferation and stimulates apoptosis in various types of malignant cells and affects the production of various chemokines, anti-oxidants and growth factors and the expression of cancer-related molecules in patients with malignancies, especially in patients treated with anti-cancer agents. Therefore, RJ is thought to exert anti-cancer effects on tumor growth and exhibit protective functions against drug-induced toxicities. RJ has also been demonstrated to be useful for suppression of adverse events, the maintenance of the quality of life during treatment and the improvement of prognosis in animal models and patients with malignancies. To understand the mechanisms of the beneficial effects of RJ, knowledge of the changes induced at the molecular level by RJ with respect to cell survival, inflammation, oxidative stress and other cancer-related factors is essential. In addition, the effects of combination therapies of RJ and other anti-cancer agents or natural compounds are important to determine the future direction of RJ-based treatment strategies. Therefore, in this review, we have covered the following five issues: (1) the anti-cancer effects of RJ and its main component, 10-hydroxy-2-decenoic acid; (2) the protective effects of RJ against anti-cancer agent-induced toxicities; (3) the molecular mechanisms of such beneficial effects of RJ; (4) the safety and toxicity of RJ; and (5) the future directions of RJ-based treatment strategies, with a discussion on the limitations of the study of the biological activities of RJ.

IntroductionIt was observed anecdotally that several trauma patients were not receiving routine oral meds due to being ‘Nil-by-mouth’. Our aim was to improve perioperative administration of regular medication for patients admitted with fractured neck of femur (NOF #) on the day of surgery.MethodsData was collected from drug charts of those admitted to the Queen Alexandra Hospital, Portsmouth with NOF # in 2019. Good practice was mostly observed, however areas for improvement were identified in anticoagulation, cardiac and diabetic medication. In addition, the pre-op guidelines and drug trolley posters were not always available and were out of date. Change was implemented by updating the NOF # guidelines using up-to-date guidance.The ‘Nil by Mouth’ drug trolley posters were re-designed and were re-distributed to every drug trolley on the orthopaedic wards and the nursing teams were informed of the changes.ResultsFollowing re-audit, improvement was made in the following areas. Anticoagulation administration now had a 100% compliance. Rate-limiters administration had improved, as had the administration of antihypertensives (CCB). Parkinson’s medication, inhalers and alcohol withdrawal medication had continued to be consistently given. The reasons for omitting medication was documented. No improvement was made in the following areas: Gastric protection, oral analgesia, anti-anginals and more data was required to assess glycaemic management.ConclusionsOptimisation of these high-risk patients requires a multi-faceted approach but by improving basic aspects of care such as peri-operative medication, it is hoped that overall mortality and morbidity are reduced. Results: 33 patients were recruited with median age 45 years (IQR 33-61), and 13 (39%) female. Median pre-treatment oxygen saturations were 84% (IQR 76-87%). Oxygen delivery devices were often shared with other patients (n=10, 33%) and the flow rate was often unknown (n=14, 47%), mostly because of broken equipment (n=8, 57%). Median duration of oxygen therapy was 3 days (IQR 1-7). Death occurred in 16 (49%). Hazard ratios for short oxygen therapy were reduced in patients who had a chest radiograph performed (HR 0.08, 95% CI 0.02–0.30), in ex-smokers (HR 0.01, 95% CI 0.00-0.22) and in never smokers (HR 0.03, 95% CI 0.00 – 0.78). Conclusions: Delivering oxygen therapy in lower-middle income countries is challenging; broken equipment and shared delivery devices prevented titration of flow rates. Patients were relatively young and at a high risk of death. Patients with a chest radiograph received oxygen for longer than those without. Knowledge of oxygen therapy durations will allow careful assessment of the oxygen supply need at the hospital level.

AbstractBradford Cox's physical appearance has mystified audiences, critics, and fans since the inception of his American indie rock band Deerhunter in the early 2000s. As the band's frontman, Cox is 6’4” with an exceptionally thin, angular frame, physical effects associated with Marfan syndrome. Yet many critics and fans do not recognize that he has a disability per se, instead speculating about possible anorexia, drug abuse, mental illness, and even his gender and sexual orientation. Through analysis of Cox's reception and creative output, I argue that the simultaneous fetishization and normalization of his body is due to its resonance with two overlapping countercultural discourses: the current idealization of thinness in indie rock as it both extends and departs from an earlier tradition of freakery and androgyny in punk. I show that Cox resists what he views as the masculine heteronormativity and performative apathy of indie rock through “freakish” sartorial reference to his androgynous punk idols PJ Harvey, Joey Ramone, and Patti Smith, an aesthetic that for Cox is vitally queer. Cox uses his solo musical output to further convey his alienation as a queer-disabled male artist: through his lyrics, album art, and specific vocal affectations and production techniques, he establishes a continuity across the visual and sonic registers of his identity to ultimately achieve a sense of belonging.

This study aims to investigate the effects of the three potassium channel genes KCNA1, KCNA2, and KCNV2 on increased susceptibility to epilepsy as well as on responsiveness to antiepileptic drugs (AEDs). The pharmacogenetic and case-control cohort (n = 595) consisted of 296 epileptic patients and 299 healthy individuals. Epileptic patients were recruited from the Pediatric Neurology clinic at the Queen Rania Al Abdullah Hospital (QRAH) in Amman, Jordan. A custom platform array search for genetic association in Jordanian-Arab epileptic patients was undertaken. The MassARRAY system (iPLEX GOLD) was used to genotype seven single nucleotide polymorphisms (SNPs) within three candidate genes (KCNA1, KCNA2, and KCNV2). Only one SNP in KCNA2, rs3887820, showed significant association with increased risk of susceptibility to generalized myoclonic seizure (p-value < 0.001). Notably, the rs112561866 polymorphism of the KCNA1 gene was non-polymorphic, but no significant association was found between the KCNA1 (rs2227910, rs112561866, and rs7974459) and KCNV2 (rs7029012, rs10967705, and rs10967728) polymorphisms and disease susceptibility or drug responsiveness among Jordanian patients. This study suggests that a significant association exists between the KCNA2 SNP rs3887820 and increased susceptibility to generalized myoclonic seizure. However, the present findings indicate that the KCNA1 and KCNV2 SNPs do not influence disease susceptibility and drug responsiveness in epileptic patients. Pharmacogenetic and case-control studies involving a multicenter and multiethnic approach are needed to confirm our results. To improve the efficacy and safety of epilepsy treatment, further studies are required to identify other genetic factors that contribute to susceptibility and treatment outcome.

The present study evaluated the stress induced anti-ulcer effect of tulsi leaves in albino rats.Methods: Albino rats were randomly, allocated to different experimental groups and aqueous leaf extract of Tulsi (Ocimum sanctum) is given for 7 days. Gastric ulcers were induced by cold restrain methods and results were compared with control using distilled water and standard drug Ranitidine.Results: The Anti-ulcer effect of aqueous extract of Tulsi was significant in a dose dependent manner with cold restraint model. The percentage of ulcer protection of Tulsi leaf extract at dose 100mg/kg is 68.85% and at dose of 200mg/kg showed 65.66% and the standard drug Ranitidine showed 78.23% ulcer protection which is significant (p <0.05).Conclusions: The study showed that when Tulsi (Ocimum sanctum) is given at dose 100-200mg/kg for 7 days i.e., pre-treatment in rats for anti-ulcer effect, it caused a significant reduction in the ulcer scores and ulcer index when compared with control group, as evidenced by the gross appearance and histological findings of gastric mucosa in rats. There were few signs of mucosal injury and the percentage of damage was less compared to control group. Serosal surface revealed very few dilated blood vessels and petechial hemorrhages and mucosal surface had few ulcers of varying sizes.

His neurological training was at University College London Hospitals and the National Hospital for Neurology and Neurosurgery, Queen Square. He also spent 1 year at L’Hopital Salpetriere, Paris. He has achieved international recognition for his work on Parkinson’s disease and abnormal movement disorders. He is an original member of the Highly-Cited Researchers ISI Database with an h-index of 130. Founder member of the international Movement Disorder Society, he was elected President (2004–2006) and co-edited the Movement Disorders Journal between 1995 and 2003. In 2006, he was awarded the Movement Disorders Research Award by the American Academy of Neurology and he was awarded the Association of British Neurologists Medal in 2015.At the age of thirty-two he was appointed to the consultant staff at the National Hospitals, The Middlesex, and Whittington Hospitals and in 1987 was elected a Fellow of the Royal College of Physicians. He introduced me to the idea that nothing happens by chance and that novel scientific discoveries rely heavily on personally distinctive actions. He also helped me to understand that art is a complementary source of truth that enlists inventiveness to transport science beyond the acquisition of fact.

The aim was to explore what happened during a five-year period to a group of patients with no previous psychiatric admissions, who were detained for the first time in a psychiatric hospital under part II of the Mental Health Act 1983. Subjects (n=189) who were detained under Section 2 or 3 in 1996 and 1997 in Queen Elizabeth Psychiatric Hospital, Birmingham were identified from computerised inpatient admission records. The mean age was 40.0 years (range 21-65). Nearly half of the cohort had a diagnosis of psychoses. Asians had the highest proportion of psychoses (64%) compared to African-Caribbean patients (48%) and White patients (41%). Almost half of the sample had a further admission, with Asian and African-Caribbean patients more likely to be readmitted than their white counterparts. Nearly a third of the cohort had a further compulsory detention. On re-admission, African-Caribbean subjects were more likely to be detained compared to Asian and White subjects. Fifty per cent had a further compulsory admission within a year of the index admission. The high number of psychoses in the Asian group may be due to their psychosis being more serious at presentation as a result of denial of illness and an increased co-morbidity with drug use in Asian men.

Introduction: The distal radioulnar joint (DRUJ) is commonly affected in rheumatoid arthritis and is associated with significant functional morbidity. The aim of our study is to review our results with matched hemi-resection interposition arthroplasty in patients with DRUJ arthritis. Methods: This was a retrospective study of 39 patients with 51 wrists that were treated at Queen Mary Hospital in Hong Kong from 1989 to 2007. All patients underwent matched hemi-resection interposition arthroplasty and dorsal wrist synovectomy. Long arm hinged elbow brace was used for three weeks followed by intensive rehabilitation up to twelve weeks. The indicators of outcome included range of motion assessment, pain, wrist stiffness, grip of strength and need for revision assessed during follow-up. Statistical analysis was performed with student t-test. Results: The average age of patients was 50.5 years (25 to 77 years) and there was a 35:4 female to male ratio. The average follow up was 4.5 years ranging from 1 to 18 years. Associated extensor tendon ruptures were found in 31.4% patients. The average increase in supination was from 73 degrees preoperatively to 81 degrees at long term follow up (p = 0.10 at 1 year and 0.13 at long term follow-up). The average increase in pronation was from 68 degrees preoperatively to 74 degrees on long term follow up (p = 0.57 at 1 year and 0.02 at long term follow-up). There was evidence of painless, relatively stiff but functional wrist in 37.25% of patients. There was an increase in grip strength from an average of 6.1 kilogram force preoperatively to an average of 11.5 kilogram force at follow-up (p = 0.004 at 1 year and 0.15 at long term follow-up). Complete relief of ulnar sided pain was seen in 43 wrists (84%), partial relief was seen in 7 wrists (13.7%) and no relief was found in one wrist (1.9%). Conclusions: DRUJ arthroplasty is a rewarding procedure and most of the patients obtain pain free movement.

BackgroundSexually transmitted infections (STI) prevention in the Northwest Territories (NWT), Canada is an urgent concern as STI prevalence is seven-fold the national average. The study objective was to explore factors associated with sexual activity and condom use among adolescents in the NWT.MethodsWe conducted a cross-sectional survey with youth aged 13–18-years-old in 17 NWT communities. We use Poisson regression models with a robust sandwich error variance to estimate adjusted relative risks estimates of the likelihood of experiencing the primary outcomes of sexual activity (vaginal/anal/oral sex) and consistent condom use (oral/anal sex) in the past 3 months by gender.ResultsParticipants (n=607; mean age: 14.2 years; SD: 1.5) included adolescent cisgender girls (n=302; 49.5%), cisgender boys (n=298; 48.9%) and transgender persons (n=7; 1.2%). Most identified as Indigenous (n=444; 73.1%) and 14.0% (n=85) as lesbian, gay, bisexual or queer sexuality (LGBQ+). Among sexually active individuals (n=115), less than half (n=54; 47.0%) reported past 3 month consistent condom use. In adjusted analyses among girls, sexual activity was associated with age, STI knowledge, and alcohol/drug use; LGBQ+ identity and alcohol/drug use were associated with reduced likelihood of condom use. Among boys, sexual activity was associated with age and alcohol/drug use; LGBQ+ identity was associated with increased likelihood of condom use.ConclusionsFindings demonstrate sexual activity among adolescents in the NWT varies by gender, age, and alcohol/drug use. Consistent condom utilisation was low, particularly for those using alcohol/drugs. Gender-tailored STI prevention strategies with Northern adolescents should address alcohol/drug use and build protective factors.

There are complex historical connections between sexual minoritisation and desires to chemically alter bodily experience. For gay men, drug and alcohol use can be a creative or experimental response to social marginalisation – and not necessarily a problematic one in every instance. Numerous studies have found that infection with HIV and other sexually transmissible infections (STIs) is more likely among gay and men who have sex with men (MSM) who use recreational drugs than those who do not, but the causal nature of these relations is uncertain. Sexualised drug use is associated with a range of other problems, including dependence, mental health issues, accident and overdose. A growing body of work in the Alcohol and Other Drugs (AOD) field demonstrates the action of drugs and their purported effects to be a product of their relations with various other actors, contexts and practices. Given these contingencies, it is impossible to predict the future of drugs or their effect on the sexual health of gay and MSM with any degree of certainty. This article outlines some of the conditions most likely to mediate such futures in the medium term. Public funding for lesbian, gay, bisexual, transgender and queer drug issues should not remain restricted to questions of HIV prevention and sexual health. It should be expanded to equip sexual health and AOD service providers with the cultural and sexual literacy to mitigate stigma and allow them to respond constructively to drug problems among sexual and gender minorities as a matter of priority.

Presentamos el caso de un paciente varón de 48 años de edad, con diagnóstico de diabetes mellitus tipo 2, no controlada, de diez años de evolución, a quien se le diagnosticó tuberculosis pulmonar mediante signos clínicos, radiográficos y cultivo en esputo positivo para Mycobacterium tuberculosis, sensible a drogas antituberculosas de primera línea. Recibió isoniacida, rifampicina, etambutol y pirazinamida. Dos meses después de iniciado el tratamiento presentó hipersensibilidad a medicamentos, con los siguientes signos y síntomas: rash dérmico generalizado, prurito generalizado, anemia Coombs positiva, eosinofilia y síntomas sistémicos, compatibles con el síndrome DRESS (drug rash with eosinophilia and systemic symptoms). Ante ello, se suspendió la medicación antituberculosa y se instaló tratamiento con antihistamínicos y corticoides sistémicos, con remisión y mejoría de síntomas. Posteriormente, recibió un esquema individualizado de tratamiento para tuberculosis consistente en medicamentos mínimamente hemato-hepatotóxicos, similar al indicado en pacientes inmunosuprimidos. Desde entonces presenta baciloscopias negativas.

Background: Workers with lived experience of problematic alcohol and other drug (AOD) use are increasingly recognized as integral to the AOD field. However, little is known about the prevalence or characteristics of AOD workers with lived experience across the general AOD workforce, in Australia or internationally. This study aimed to (1) investigate the prevalence of lived experience in AOD workers, (2) build an initial profile of workers with lived experience, (3) identify areas where appropriate support mechanisms may be warranted, and (4) generate recommendations for future work. Method: Nongovernment organization AOD workers from New South Wales, Australia, were invited to participate in a purpose-designed, online survey. Measures included demographic and workforce characteristics, work-related psychosocial factors, and health, quality of life, and AOD use. Descriptive analyses compared responses from workers with and without lived experience on key variables. Results: Two hundred and sixty-eight workers responded. Workers with lived experience comprised 43% of the sample; were more likely to be older; male; identify as lesbian, gay, homosexual, or queer; have lower salary; report discrimination in the workplace; abstain from alcohol; report opioid use; and experience less support outside work. Conclusion: This is the first Australian study to examine the profile of AOD workers with lived experience. Workers with lived experience constituted a substantial proportion of the AOD workforce. Analogous to other countries, comprehensive, appropriately tailored workforce development and support policies are required. Future research should build on these findings by extending to a broader population base, including government workers.

The aim of the present study was to determine the chemical composition and evaluate the acaricidal activity of Thymus algeriensis essential oil (TAEO) against Varroa destructor. This ectoparasitic mite is a pest of the honey bee Apis mellifera. The essential oil from the aerial parts of T. algeriensis, obtained by hydrodistillation, was obtained in a yield of 2.8± 0.2%, w/w. The TAEO was analyzed by GC and GC/MS. Thirty-four compounds were identified, representing 99.3% of the oil. The main constituents were carvacrol (48.4%), γ-terpinene (14.9%), p-cymene (14.7%), and thymol (5.6%). Four lots were constituted at the level of an apiary in order to study the dynamics of the Varroa destructor and its host, Apis mellifera. After diagnosis by the biological method “install of diapers”, the lots were treated at different doses of TAEO (0.1, 0.3 and 0.5%). TAEO was sprayed on top of the hives. The results show that TAEO at 0.5% resulted in a decrease in the rate of infestation of Varroa destructor, causing a mortality rate of 32.6% without negative effect on the nesting of the queen. The essential oil of T. algeriensis could be used as a bioacaricidal agent.

The use of pharmacological agents in pregnant females poses a major clinical challenge due to the marked physiological changes that may modify the pharmacokinetics of drugs and to the potential effects on the fetus. The purpose of this paper is to review briefly our knowledge on the use of antibacterial drugs during pregnancy and to provide information for the judicious selection of an antimicrobial treatment for use in pregnant bitches and queens. The risk to the fetus is a result of the ability of a drug to reach the fetal circulation and to produce toxic effects. The placenta functions as a barrier that protects the fetus due to the presence of transporters and metabolising enzymes; however, during pregnancy, the presence and activity of both enzymes and transporters may change. Antimicrobial agents that have been shown to be safe for use during pregnancy include betalactams, macrolides, and lincosamides. Pharmacotherapy during pregnancy in all species may affect adversely the developing fetus; therefore, it should be avoided when possible.

Abstract This research reports on newspaper representations of PrEP, a HIV-prevention drug recently made available on a trial basis to at-risk individuals in England. Using corpus-assisted queer critical discourse analysis, we investigate the linguistic representations of the users of PrEP within three leading British newspapers from across the political spectrum between 2014–18. We find that users of PrEP are most frequently positioned as ‘men who have sex with men’ or ‘gay men’, a representation that we argue limits public awareness of HIV itself, and of available HIV prevention. Furthermore, while the most left-leaning newspaper in our corpus focuses on the human benefit of PrEP, the most right-leaning newspaper takes a moralistic stance which frames gay men as risk-taking and therefore less deserving of healthcare funding than other groups. We therefore argue that certain representations of PrEP’s beneficiaries are implicitly homophobic, and that most representations are unhelpfully restrictive.

In José Revueltas' novel El apando (1969), the possibility for transgression is greatly diminished by the system. In such circumstances, the only possibility of transgression by means of the body is through drug consumption and erotic acts. This article examines the lack of literary criticism regarding the erotic, the homoerotic and the expression of sexual desire, other than in the sense of the society's values. The notion of act-space, together with concepts from cultural and queer theory serve as a critical instrument for this analysis. This essay evaluates critically the narrative discourse, clearly tinted with homophobia and a patriarchal patronage incapable of dealing with the most intimate aspects of self-expression. Under the patriarchy, that self-expression is occluded and controlled by the same ideologies, which again, are signs of homophobia. / En la novela El apando (1969) de José Revueltas, la posibilidad de transgredir se reduce a base los parámetros impuestos por el sistema. Ante tal situación, la única posibilidad de transgredir corporalmente que permanece se presenta a través del consumo de droga y la expresión erótica. El presente ensayo examina por qué existe una carencia de crítica en relación al erotismo, al homoerotismo y a la expresión del deseo sexual si no se estudian estos tres puntos de interés exclusivamente a base de los valores sociales que imperan. Una lectura que va contracorriente, en combinación con la noción de acto-espacio, se integra a conceptos culturales y de teoría queer para forjar un instrumento de investigación. Este análisis evalúa críticamente el discurso narrativo que está teñido por sentimientos de homofobia que sustancian a un patriarcado incapaz de aceptar los aspectos más íntimos de la auto-expresión. Esta auto-expresión bajo el patriarcado se ve controlada y obstruida por las mismas ideologías que ejemplifican, una vez más, la homofobia dentro de dicho sistema.

AimFunctional neurological disorders (FND) are one of the most common presentation in neurology clinics, causing a significant disability and economic burden. Cognitive behavioural therapy (CBT) has one of the best available evidence in managing FND, although access remains limited. Queen Square, London neuropsychiatry experts have established an excellent model for a CBT based, Guided Self Help (GSH) programme, which is preparatory to a multidisciplinary inpatient treatment. It has been shown to have good outcomes. This study was designed to ascertain the feasibility and acceptance of this QGSH model, in an Exonian cohort of FND patients, whilst piloting its stand-alone version, without the inpatient component. Additionally, the study explores the need and types of modifications required for the stand-alone adaptation of QGSH.MethodConsecutive patients referred to Exeter FND Service, between February to June 2020, who had internet access, were offered the QGSH pilot. Patients with a primary mental disorder concurrent drug/alcohol misuse or risk of self-harm or suicide were excluded. Ethics approval was not required. The QGSH intervention constitutes of 11 modules focussing on specific elements crucial to FND management along with homework tasks, delivered by the author, under supervision by QGSH experts. Patients completed Pre and Post-intervention questionnaires as well as structured feedback.ResultsThree successive patients with varied FND symptoms were recruited to the pilot between February and June 2020. The baseline health status of these patients was worse as compared to EQ-5D-5L population norms with significant baseline psychiatric comorbidity. Outcome measures used before and after QGSH intervention included PHQ 9, GAD 7, EQ-5D-5L and a locally devised symptom severity questionnaire. Necessary modifications were made to the program based on the patients informal feedback and structured formal feedback was sought in the end.ConclusionAll patients derived some benefit from QGSH and certain modifications were suggested in patient feedback to improve engagement. Despite study limitations, especially small size and the impact of Covid 19 pandemic during the intervention; QGSH model appears acceptable and feasible in an Exonian cohort, however, some modifications are recommended for the stand-alone version to succeed. The recommendations will be presented.

Despite the clinical success of Truvada-as-PrEP in preventing HIV transmission for several years, it has still not yet been widely adopted by the MSM community. Clinicians insist that the drug is safe, and there are avenues to pay for the drug even for the uninsured—and yet, there is hesitation. Since the 1980s, HIV/AIDS narratives have been linked to what Susan Sontag (1989) calls an “imputation of guilt,” and more broadly, the creation of a connection between queer sexuality and social stigma whose inertia is difficult to escape. How is the way people talk about PrEP framed by this discursive history? Can the community’s reluctance be understood in those terms, and addressed by deeper understandings of how the disease and its treatment are conceived? This article takes a discourse analysis approach to examine the narratives surrounding PrEP, in particular those that have circulated in online and print media in the USA. These are contextualized by the evolution of conversations surrounding the disease and its treatment, from “death sentence” to chronic illness to medical condition. Following Kover (1998), HIV/AIDS is positioned here as an object of the terrifying sublime, that can only be dealt with indirectly, while PrEP represents a (possible) shift in strategies for coping with the disease, from avoidance to confrontation, with all that that entails.

Peaches are one of the most preferred seasonal fruits, and a reliable source of nutrients. They possess biologically active substances that largely differ among varieties. Hence, revealing the potential of several late season peaches is of present interest. Three commonly consumed varieties (“Flat Queen”; “Evmolpiya”; “Morsiani 90”) were studied in terms of nutritive and phytochemical content, as well as antioxidant activity with the use of reliable spectrophotometric and High Performance Liquid Chromatographic (HPLC) methods. An analysis of the soil was also made. The phytochemical data were subjected to principal component analysis in order to evaluate their relationship. The “Morsiani 90” variety had the highest minerals concentration (2349.03 mg/kg fw), total carbohydrates (16.21 g/100 g fw), and α-tocopherol (395.75 µg/100 g fresh weight (fw)). Similar amounts of TDF (approx. 3 g/100 g fw) were reported for all three varieties. “Flat Queen’s” peel extract was the richest in monomeric anthocyanins (2279.33 µg cyanidin-3-glucoside (C3GE)/100 g fw). The “Morsiani 90” variety extracts had the highest antioxidant potential, defined by 2,2-diphenil-1-picrylhydrazyl (DPPH), ferric-reducing antioxidant power (FRAP) and cupric ion-reducing antioxidant capacity (CUPRAC) assays.

RÉSUMÉLes cliniques « Brown Bag » permettent aux pharmaciens participants de rencontrer les personnes âgées afin d'examiner les médicaments en leur possession et de corriger les situations qui pourraient mener à des réactions toxiques (DRAPEs — drug-related adverse patient events). Cent vingt-trois patients ont été en contact avec 30 pharmaciens lors des cliniques pilotes tenues dans sept communautés de la Colombie-Britannique. En moyenne, chaque patient a apporté plus de neuf médicaments (prescrits ou non-prescrits) et les cliniciens ont identifié 482 problèmes potentiels de médication (les médecins traitants de 39 patients ont été immédiatement contactés). Plus de 80 pour cent des patients ont considéré les cliniques « Brown Bag » comme étant utiles et ont manifesté le désir de participer à nouveau lors de futures cliniques. Les pharmaciens cliniciens, bien qu'en faveur des cliniques, sont cependant réticents à ré-offrir leur services de façon bénévole. Lors des cliniques de rappel conduites de six à 11 mois plus tard, les patients avaient oublié une bonne partie de l'information obtenue lors des premières sessions.

Feline mammary fibroepithelial hyperplasia (FMFH) following a single injection of depot medroxyprogesterone acetate (MPA) was observed in eight intact young queens. The repository compound is marketed as a veterinary product by a local pharmaceutical company with an indication for contraception in cats. The drug was administered according to the recommended doses and injection frequencies. Serum hormone assays performed immediately before neutering and 3 weeks after neutering detected persistently high levels of progesterone suggesting that depot MPA was still exerting its influence. No corpora lutea were found in those cases ruling out ovaries as the main site of progesterone. Immunohistochemistry performed on the hyperplastic mammary glands detected progesterone receptors in the nuclei of ductal cells, and growth hormone (GH) and insulin-like growth factor-I (IGF-I) in the cytoplasm of ductal epithelium. Overdosing should be considered here as the animals received at least 10 mg/kg of depot MPA in a single injection. Progestin-induced local synthesis of GH and IGF-I in mammary epithelial cells is suggested as one of the pathogenic mechanisms involved in the development of FMFH.

440 Background: Sorafenib (Sor) is the only approved systemic therapy for advanced HCC yet the safety and efficacy for its use in patients (pts) who developed recurrence after liver transplantation (LT) has not been established. There is an unmet need for treatment option in this group of pts. Methods: This is a retrospective analysis of pts treated with Sor for recurrent HCC post-LT in Queen Mary Hospital, Hong Kong. Results: During year Jan 2008 to Feb 2014, 25 post-LT pts with HCC recurrence were treated with Sor. Tacrolimus and sirolimus were the immunosuppressant of choice during the same treatment period in 76% (19/25) and 40% (10/25) pts respectively. All pts had Child-Pugh A liver function and ECOG performance status 0-1. Majority of pts were chronic hepatitis B carriers (88%) and male (88%). Median time to recurrence was 13.2 months (range 3.4-71.1). Intrahepatic recurrence and extrahepatic recurrence were found in 52% and 80% of pts respectively. Up to 68% of pts received the standard dose of 800 mg/day. The most common adverse events (AEs) were hand-foot syndrome (44%) and diarrhea (28%). G3 transaminase elevation was found in 4 pts (16%), in which 2 had histological evidence of acute graft rejection. Both cases were associated with tailing down of tacrolimus upon tumor progression, and the liver function recovered after adjustment of immunosuppressant. Nine pts (36%) required dose adjustment. The median maintenance dose was 400 mg/day. The median duration of treatment was 4.44 months (95% CI 2.53-6.44). Median time to progression was 6.21 months (95% CI 2.83-9.63). Median overall survival was 13.6 months (95% CI 7.69-19.61). Ten (40%) pts also received other subsequent systemic therapy. Conclusions: Sor given in combination with tacrolimus and sirolimus did not lead to unexpected graft rejection. The drug is in general tolerated in this population and toxicities appeared to be similar to non-transplant pts. Further study is required to evaluate for drug interaction and efficacy.
