Abstract. The Metropolitan District of Quito is located on or very close to segments of reverse blind faults, Puengasí, Ilumbisí–La Bota, Carcelen–El Inca, Bellavista–Catequilla and Tangahuilla, making it one of the most seismically dangerous cities in the world. The city is divided into five areas: south, south-central, central, north-central and north. For each of the urban areas, elastic response spectra are presented in this paper, which are determined by utilizing some of the new models of the Pacific Earthquake Engineering Research Center (PEER) NGA-West2 program. These spectra are calculated considering the maximum magnitude that could be generated by the rupture of each fault segment, and taking into account the soil type that exists at different points of the city according to the Norma Ecuatoriana de la Construcción (2015). Subsequently, the recurrence period of earthquakes of high magnitude in each fault segment is determined from the physical parameters of the fault segments (size of the fault plane and slip rate) and the pattern of recurrence of type Gutenberg–Richter earthquakes with double truncation magnitude (Mmin and Mmax) is used.

Abstract:

Background: Sinonasal inverted papilloma is an unusual tumor of the nasal cavity and paranasal sinuses with potential for malignant transformation. The objective was to review the experience mostly using a similar open surgical approach.Methods: Twenty-eight patients were included. Mean age was 54, 57% of male gender. five patients were previously operated elsewhere, twenty-five had unilateral complete nasal obstruction and mean duration of symptoms was 8months. A computed tomography was performed in all patients. The tumor was located within the nasal cavity in 9 cases, limited to the ethmoid sinus complex in one, extended to the maxillary sinus in 16 and to the nasopharynx or palate in one, each. Mean size was 5.2cm. Twenty-seven cases were benign and one was a transitional carcinoma originated in an inverted papilloma. Surgical resection of the tumor was performed through a Denker’s approach in all the benign cases except a lateral rhinotomy in one recurrent lesion of limited ethmoid involvement. A partial maxillectomy was performed in the patient with malignancy. Minor postoperative complications occurred in 3 patients which was nasal bleeding, a cellulitis of the cheek with exophtalmus and an oro-antral fistula.Results: After a mean follow-up of 33.6months, recurrent benign tumors were easily resected through the same approach in 5 patients. All patients have currently no evidence of disease.Conclusions: Denker’s approach was a very useful, rapid and safe surgical approach for treatment of middle size or extended sinonasal inverted papillomas and remains an alternative to current endoscopic resection.

Abstract:

Introduction Branchial cleft anomalies are the second most common congenital anomaly in children. However, some lesions may not develop clinically and are not diagnosed until adulthood. The recent literature of branchial cysts (BCs) in the adult population is really scanty. For this reason, we analyzed the clinical and surgical management of the adult population treated for a BC at a tertiary care general hospital. Methods A retrospective review of the clinical records of all the patients with histological diagnosis of BC who were surgically treated at the Social Security Hospital in Quito, Ecuador, was performed. Fifty-one patients (27 women) with congenital anomalies of the 2nd (43 patients with cysts) and 3rd (6 patients with cysts and 2 with fistula) branchial arches were diagnosed and treated. Diagnosis was made on clinical grounds and by computed tomography scan. Results The 43 patients with a 2nd branchial cleft cyst underwent complete surgical excision through a wide mid-neck transverse cervicotomy. The 6 cases of 3rd branchial cleft cyst underwent surgical resection through a lower-neck transverse incision, and the 2 patients with clinical fistula in the lower aspect of the neck were operated on via an elliptical incision around this external fistula opening. Postoperative evolution was uneventful in all patients. Conclusions Branchial cysts can occasionally be diagnosed in adult patients in the setting of a general hospital population. A correct clinical and imaging assessment was diagnostic in most patients. Complete surgical resection was curative in all our patients, and postoperative complications were exceptional.

Quito como capital del Ecuador es parte de esta investigación a través de un estudio descriptivo con técnicas de observación y revisión bibliográfica en los emprendimientos locales y grandes empresas.Entre los elementos hallados se destaca que desde la década de los ochentas las franquicias y cadenas hoteleras internacionales rompieron el mercado con calidad y un alto desempeño que estimularon los porcentajes de ocupación corporativa. Adicionalmente el Ecuador posee un nuevo Reglamento de Alojamiento Turístico, vigente desde el 2015, donde ya se incorporan las categorías de: haciendas turísticas, lodges con la potencialización de infraestructuras patrimoniales y resorts con todos los servicios de alimentos, bebidas y lúdicos. Entre los resultados hallados se destaca que una de las mayores ventajas competitivas son las tarifas de hospedaje reconocidas como una de las más económicas de América. Además, el aporte que refleja la industria de alojamiento y servicios de comida ocupa el 27.7% de los ingresos por generación de empleo del sector y que la mayor concentración de empleabilidad está en la Zona Especial Turística “La Mariscal”. Finalmente se destaca la limitación en capacidades de inversión en los establecimientos pequeños en tecnología e innovación frente a las grandes franquicias y cadenas.

Abstract:

Background: Head and neck mucosal melanoma account for 2 to 8% of head and neck melanomas, the majority of which arises in the nasal cavity or paranasal sinuses. The aim of this report was to review our experience on sinonasal malignant melanomas (SNMM) treated over a long period of time at a tertiary referral hospital.Methods: Clinical records of all of the patients operated on for SNMM at the Social Security Hospital and private clinics in Quito, Ecuador, were reviewed. Nineteen patients had histologically proven diagnosis. Eleven patients were men and mean age was 64. The most common presenting symptoms were nasal obstruction and epistaxis. Most tumors were located at the nasal fossa. Extranasal extension was present in 11 patients. Twelve had stage III- IV tumors. All patients were treated by surgery. Postoperative radiotherapy was given to 7.Results: Local recurrence (11 events) occurred in 9 cases. These 11 events were treated with surgery in 8, eventually associated to radiation therapy (RT) and chemotherapy (CT). Eight out of these 9 patients died. Five overall survival was 46%. Death was related to local disease and distant metastases. All stage I tumor patients are alive.Conclusions: Most sinunasal melanomas have a poor prognosis, mainly attributed to initial advanced local disease, local recurrence and distant metastasis. Surgery is the mainstay treatment, followed by radiotherapy, according to the T category.

Abstract:

El siguiente trabajo es un resumen de la tesis doctoral en estudios del español de la Universidad de Salamanca, intitulada “Lo cortés no quita lo valiente”, un análisis de la cortesía en Quito y Salamanca. La dicha tesis busca identificar y analizar el uso de cortesía positiva y negativa, al momento de hacer pedidos, en ambas ciudades. Y, dentro de cada grupo, averigua diferencias por género, nivel de educación, edad y origen. Para recolectar la información se realizaron encuestas a 393 estudiantes en dos universidades de las antes mencionadas ciudades: 187 en la USAL (Universidad de Salamanca) y 206 en la PUCE (Pontificia Universidad Católica del Ecuador). Las encuestas incluyen situaciones cotidianas que se dan en diferentes niveles de confianza: mucha confianza (ambiente familiar o de amigos cercanos), poca confianza (amigos de amigos o de familiares) y cero confianza (en un almacén, una oficina o un taxi). Cada encuestado debía escoger entre una estrategia directa o una indirecta en una situación determinada. Se espera que el estudio arroje diferencias sustanciales en ambos lugares, no solo por la variedad dialectal: español andino y español europeo, sino también por la distancia social, el nivel de educación, el género y el origen, se ha incluido a estudiantes extranjeros en ambas ciudades.

Abstract:

El patrimonio arquitectónico moderno de Quito no ha sido identificado, evaluado ni protegido. El Instituto Metropolitano del Patrimonio de Quito ha preseleccionado 125 objetos arquitectónicos, pero ninguno consta en el catálogo final protegido. La presente investigación hace un repaso de los principios básicos de la modernidad desde las visiones globales para revisarlos, dirigidos hacia una mirada latinoamericana —desde la visión de Bolívar Echeverría y otros autores latinoamericanos— con el fin de determinar la necesidad de definir con mayor precisión los parámetros de valoración de los predios a ser patrimoniados. En la actualidad el Instituto Metropolitano de Patrimonio no cuenta con las herramientas teóricas ni críticas para realizar dicha tarea, producto de un profundo vacío de conocimiento en el tema de la modernidad local. Además, como se expone en el artículo, varias de las edificaciones actualmente patrimoniadas y que pertenecen a la etapa ya de la modernidad en la ciudad no tienen un respaldo teórico que avale su calidad patrimonial. Por tanto, se evidencia la necesidad de estudio de las particularidades del modernismo en términos de expresión cultural local como premisa y base de una posterior identificación, valoración y protección del patrimonio moderno de la ciudad.
