Abstract:
The amino acid L-isoleucine has been cocrystallized with seven selected D-amino acids including D-methionine [L-isoleucine–D-methionine (1/1), C6H13NO2.C5H11NO2S, amino-acid side chain R = —CH2—CH2—S—CH3] and a homologous series from D-alanine [L-isoleucine–D-alanine (1/1), C6H13NO2.C3H7NO2, R = —CH3] through D-α-aminobutyric acid [L-isoleucine–D-α-aminobutyric acid (1/1), C6H13NO2.C4H9NO2, R = —CH2—CH3] and D-norvaline [L-isoleucine–D-norvaline (1/1), C6H13NO2.C5H11NO2, R = —CH2—CH2—CH3] to D-norleucine [L-isoleucine–D-norleucine (1/1), C6H13NO2.C6H13NO2, R = —CH2—CH2—CH2—CH3] with linear side chains, and D-valine [L-isoleucine–D-valine (1/1), C6H13NO2.C5H11NO2, R = —CH—(CH3)2] and D-leucine [L-isoleucine–D-leucine (1/1), C6H13NO2.C6H13NO2, R = —CH2—CH—(CH3)2] with branched side chains. All the crystal structures are divided into distinct hydrophilic and hydrophobic layers. In the five complexes with amino acids with linear side chains the polar parts of the D- and L-amino acids are related by pseudo-glide-plane symmetry, and four of them have remarkably similar molecular arrangements. The D-valine and D-leucine complexes, on the other hand, are structurally quite different with the polar parts of the D- and L-amino acids related by pseudo-inversion. Differences in the hydrogen-bond pattern in the two molecular arrangements are discussed.

Using the properties of Gauss sums, the orthogonality relation of character sum and the mean value of Dirichlet L-function, we obtain some exact computational formulas for the hybrid mean value involving general Kloosterman sums K ( r , l , λ ; p ) and certain Hardy sums S 1 ( h , q ) ∑ m = 1 p − 1 ∑ s = 1 p − 1 K ( m , n , λ ; p ) K ( s , t , λ ; p ) S 1 ( 2 m s ¯ , p ) , ∑ m = 1 p − 1 ∑ s = 1 p − 1 | K ( m , n , λ ; p ) | 2 | K ( s , t , λ ; p ) | 2 S 1 ( 2 m s ¯ , p ) . Our results not only cover the previous results, but also contain something quite new. Actually the previous authors just consider the case of the principal character λ modulo p, while we consider all the cases.

Abstract:
Abstract Severi varieties and Brill–Noether theory of curves on K3 surfaces are well understood. Yet, quite little is known for curves on abelian surfaces. Given a general abelian surface S with polarization L of type {(1,n)} , we prove nonemptiness and regularity of the Severi variety parametrizing δ-nodal curves in the linear system {|L|} for {0\leq\delta\leq n-1=p-2} (here p is the arithmetic genus of any curve in {|L|} ). We also show that a general genus g curve having as nodal model a hyperplane section of some {(1,n)} -polarized abelian surface admits only finitely many such models up to translation; moreover, any such model lies on finitely many {(1,n)} -polarized abelian surfaces. Under certain assumptions, a conjecture of Dedieu and Sernesi is proved concerning the possibility of deforming a genus g curve in S equigenerically to a nodal curve. The rest of the paper deals with the Brill–Noether theory of curves in {|L|} . It turns out that a general curve in {|L|} is Brill–Noether general. However, as soon as the Brill–Noether number is negative and some other inequalities are satisfied, the locus {|L|^{r}_{d}} of smooth curves in {|L|} possessing a {g^{r}_{d}} is nonempty and has a component of the expected dimension. As an application, we obtain the existence of a component of the Brill–Noether locus {{\mathcal{M}}^{r}_{p,d}} having the expected codimension in the moduli space of curves {{\mathcal{M}}_{p}} . For {r=1} , the results are generalized to nodal curves.

Abstract:
A proven effective preventive measure against cervical cancer is a full-fledged HPV vaccination course conducted before the start of the patient’s sexual life. Its cost is essential for the realization of the possibility of using the vaccine on a voluntary basis. However, the most important argument for making a decision about vaccination is the awareness of patients about its expediency, effectiveness and safety. P u r p o s e : to identify the level of awareness of the population about the primary prevention of cervical cancer by vaccination against HPV. M a t e r i a l s a n d m e t h o d s . According to a specially developed questionnaire, an anonymous survey of adolescents and their parents (legal representatives) on the organization of HPV vaccination was conducted. The quota sample for the survey included 723 respondents who applied to women’s consultations and children’s polyclinics in St. Petersburg (a total of 19 medical organizations). R e s u l t s . In fact, only 59,7% of respondents are informed about diseases caused by HPV, and another 5,1% of those who took part in the survey are mistaken about this. Only one in three (36,4%) respondents know about the safety of currently used vaccines, and the majority of respondents – 50,8% noted that they do not know anything about this issue. Among the respondents, 87,8% indicated that given the high importance of vaccination to prevent the development of oncological diseases of the female genital area, HPV vaccinations should be carried out free of charge to citizens within the framework of the National Vaccination Calendar and the calendar of preventive vaccinations for epidemic indications. F i n d i n g s . A sociological study showed an unsatisfactory level of knowledge of the population on the issues of primary prevention of malignant neoplasms of the female genital organs.

The square-planar nickel(II) title complex, [Ni(C24H36F2N6)](ClO4)2 or [NiL](ClO4)2 (L = 3,10-bis(4-fluorophenethyl)-1,3,5,8,10,12-hexaazacyclotetradecane) was synthesized by a one-pot reaction of template condensation and its X-ray crystal structure was determined. The nickel(II) ion lies close by a twofold axis and the complex displays whole-molecule disorder. Ligand L, a hexaazacyclotetradecane ring having 4-fluorophenethyl side chains attached to uncoordinated nitrogen atoms, adopts a trans III (R,R,S,S) configuration. The average Ni—N bond distance is 1.934 (9) Å, which is quite similar to those of other nickel(II) complexes with similar ligands. The nickel(II) ion is located 0.051 (7) Å above the least-squares plane through the four coordinated N atoms. The average C—N bond distance and C—N—C angle involving uncoordinated nitrogen atoms are 1.425 (12) Å and 118.0 (9)°, respectively, indicating a significant contribution of sp 2 hybridization for these N atoms. The intermolecular N—H...O, C—H...O/F hydrogen bonds of the complex form a network structure, which looks like a seamless floral lace pattern.

Abstract:
Abstract. Lovejoy and Varotsos (2016) (L&V) analyse the temperature response to solar, volcanic, and solar plus volcanic forcing in the Zebiak–Cane (ZC) model, and to solar and solar plus volcanic forcing in the Goddard Institute for Space Studies (GISS) E2-R model. By using a simple wavelet filtering technique they conclude that the responses in the ZC model combine subadditively on timescales from 50 to 1000 years. Nonlinear response on shorter timescales is claimed by analysis of intermittencies in the forcing and the temperature signal for both models. The analysis of additivity in the ZC model suffers from a confusing presentation of results based on an invalid approximation, and from ignoring the effect of internal variability. We present tests without this approximation which are not able to detect nonlinearity in the response, even without accounting for internal variability. We also demonstrate that internal variability will appear as subadditivity if it is not accounted for. L&V's analysis of intermittencies is based on a mathematical result stating that the intermittencies of forcing and response are the same if the response is linear. We argue that there are at least three different factors that may invalidate the application of this result for these data. It is valid only for a power-law response function; it assumes power-law scaling of structure functions of forcing as well as temperature signal; and the internal variability, which is strong at least on the short timescales, will exert an influence on temperature intermittence which is independent of the forcing. We demonstrate by a synthetic example that the differences in intermittencies observed by L&V easily can be accounted for by these effects under the assumption of a linear response. Our conclusion is that the analysis performed by L&V does not present valid evidence for a detectable nonlinear response in the global temperature in these climate models.

Assessing patients’ expectations and perceptions of health care delivery is challenging. To understand the quality of health care delivery, a study was conducted related to how patients assess the expected and perceived quality of health care delivery in a district hospital. Modern methods of measuring patient satisfaction and its interpretation using statistical analysis methods allow solving problems of this type. P u r p o s e : development of approach to improvement of quality of medical services provision using results of measurement of satisfaction with quality of medical services provided to patients in inpatient conditions of regional level, using SERVQUAL technique; substantiation on built regression model of influence of certain aspects of patients’ perception of quality satisfaction. M a t e r i a l s a n d m e t h o d s . The materials of the study are presented in the form of the results of a medical and sociological study of 418 patients who received medical services in one of the medical organizations at the regional level. Methods of statistical analysis of patient questionnaire results, factor, regression analysis, Importance-Performance Analysis (IPA) were used. R e s u l t s . An analysis of the data showed that 54.3% of patients were quite satisfied with the services. A regression equation is constructed to calculate the degree of patient satisfaction depending on a number of aspects of health care delivery. According to the IPA method, a schedule of four-quadrant distributions of patient assessments has been drawn up, on the basis of which it is possible to form measures to increase patient satisfaction with medical services. F i n d i n g s . The use of the SERVQUAL technique allows you to measure satisfaction with the quality of medical services, and the use of the IPA technique allows you to interpret it and obtain useful information regarding the quality of medical services as an element of patient feedback in the quality management system of a medical organization.

Abstract:
An analytical procedure regarding the determination of selenium(IV) by anodic stripping voltammetry exploiting the in situ plated bismuth film electrode is described. Since organics are commonly present in untreated natural water samples, the use of Amberlite XAD-7 resin turns out to be quite important to avoid problems such as the adsorption of these compounds on the working electrode. The optimum circumstances for the detection of selenium in water using differential pulse voltammetry techniques were found to be as follows: 0.1 mol L−1 acetic acid, 1.9 × 10−5 mol L−1 Bi(III), 0.1 g Amberlite XAD-7 resin, and successive potentials of −1.6 V for 5 s and −0.4 V for 60 s, during which the in situ formation of the bismuth film on glassy carbon and the accumulation of selenium took place. The current of the anodic peak varies linearly with the selenium concentration ranging from 3 × 10−9 mol L−1 to 3 × 10−6 mol L−1 (r = 0.9995), with a detection limit of 8 × 10−10 mol L−1. The proposed procedure was used for Se(IV) determination in certified reference materials and natural water samples, and acceptable results and recoveries were obtained.

Abstract:
The development of infection in the field of surgical intervention is inextricably linked with financial costs and the public reputation of the institution. The medical and preventive institution needs to allocate additional funds from the hospital budget for diagnostic studies, as well as for medicines for the treatment of frolicking purulent-septic complications. As a result, medical and preventive institutions experience annual additional economic losses. P u r p o s e of the study is to assess the economic damage caused to medical and preventive institutions due to the development of infections in the field of surgical interventions. M a t e r i a l s a n d m e t h o d s . In the course of this study, an analysis of domestic and foreign literature sources on the issue of economic damage from purulent-septic complications in surgical practice was carried out. R e s u l t s . The damage caused by the development of infections in the area of surgical intervention is quite significant, and even the introduction of new technologies and therapies does not relieve patients from their possible appearance. The estimated costs of infection of the surgical intervention area vary depending on the region, the type of operation and the degree of infection. Economic analysis in this case is necessary to study the activities of organizations from the point of view of evaluating their work in order to monitor and subsequently better manage the processes of supplying the institution itself with material resources, as well as the use of internal reserves and their rational use. F i n d i n g s . The distribution of funds in medical and preventive facilities should be carried out rationally to ensure the full-fledged activity of the institution. In the absence of measures taken to prevent the development of nosocomial infections, infections of the surgical intervention area may occur after a certain period of time after the patient is discharged from the hospital, thereby contributing to the spread and contamination of healthy people, which from a socio-economic point of view is unprofitable.

Reading acquisition is a complex process, which includes several concepts from the simplest and primary such as decoding signs, to the fullest, as we see Martins (2012), Fernandes (2010), who consider reading much more than the simple action of translating language codes. This research used the qualitative method in order to be exploratory, aiming to provide greater familiarity with the problem in order to make it explicit or build hypotheses, involving literature review and analysis of the activities, which encouraged understanding the phenomenon under study. The research also used the technique of observation and systematization of data collection, in order to identify the determinants of the phenomenon, which favor the occurrence of the same.Fieldwork was in a municipal public school in the city of Recife, in Pernambuco. At first there were, initially, six students in this research, but as appropriate to what the research proposed, the number changed to four, this enabled a transformation of the students’ role in the reading classes. The weak points in the learning of the reading process were identified, and the activities with specific reading strategies, that were used as a support to remedy the difficulties presented by the students of the third grade of elementary school 1. Production was analyzed in order to verify whether or not there is the deletion of the coda in /r/, /s/ and /l/medial and final, for language deletion of syllable codas performed in readings. After the intervention process it was found that with the application of consistent reading strategies, participants developed a production of syllabic coda in /r/, /s/ and /l/ medial and final. And identify the degree of complexity of the reading of the focus on students and the reading of the focus on students and the reading level of each student was at the beginning during the process and the final phase of the development of research.

Abstract:

AbstractIn EMC, it is essential to understand the concept of transmission lines and when to apply it. Abstract:
Abstract Human’s motion and its mechanisms had become interesting in the last years, where the medecine’s field search for rehabilitation methods for handicapped persons. Other fields, like sport sciences, professional or military world, search to distinguish profiles and ways to train them with specific purposes. Besides, recent findings in neuroscience try to describe these mechanisms from an organic point of view. Until now, different researchs had given a model about control motor that describes how the union between the senses’s information allows adaptable movements. One of this sense is the proprioception, the sense which has a quite big factor in the orientation and position of the body, its members and joints. For this reason, research for new strategies to explore proprioception and improve the theories of human motion could be done by three different vias. At first, the sense is analysed in a case-study where three groups of persons are compared in a controlled enviroment with three experimental tasks. The subjects belong to each group by the kind of sport they do: sedentary, normal sportsmen (e.g. athletics, swimming) and martial sportmen (e.g. karate, judo). They are compared thinking about the following hypothesis: “Martial Sportmen have a better proprioception than of the other groups’s subjects: It could be due to the type of exercises they do in their sports as empirically, a contact sportsman shows significantly superior motor skills to the members of the other two groups. The second via are records from encephalogram (EEG) while the experimental tasks are doing. These records are analised a posteriori with a set of processing algorithms to extract characteristics about brain’s activity of the proprioception and motion control. Finally , the study tries to integrate graphic tools to make easy to understand final scientific results which allow us to explore the brain activity of the subjects through easy interfaces (e.g. space-time events, activity intensity, connectivity, specific neural netwoks or anormal activity). In the future, this application could be a complement to assist doctors, researchers, sports center specialists and anyone who must improve the health and movements of handicapped persons. Abstract:
DIC is a clinical syndrome which is initiated by exces sive thrombin generation, fibrin formation and fibrin deposition in organs, thus resulting in multiple organ failure. It is a frequent, often fatal complication in intensive care patients. We evaluated a recently developed ELISA (Behringwerke) for TAT complexes in 50 healthy volunteers and in 43 consecutive intensive care patients referred by the attending physician on the clinical suspicion of DIC. The ELISA is based upon thrombin-antibody coated test tubes and measurement of bound TAT complexes with peroxidase-conjugated AT III antibody. The method has a detection limit of 0.7 μg TAT/L, a normal range of 0.97-5.4 μg/L, The intra- and inter assay v.c. at 3.3, 11.2 and 42.4 μg/L TAT levels were 5, 7, 8% and 9 , 9 and 12% resp. Patients suspected of DIC were in a clinically septic condition (n=33), confirmed by positive bloodcultures in 18, malignancies (n=5), multiple trauma (n=3) or solutio placentae (n=2). Of the 43 patients 29 had increased TAT levels: mean 21.5, range 5.4-62.5. TAT levels correlated (p <0.05) with concurrently assayed plasma fibrinogen (r=0,543), Factor V (r=0,539), platelet count (r=0,307), AT III (r=0,317), and with plasminogen (r=0,365) (Spearman rank correlation test). The accuracy of the TAT-test for the diagnosis of DIC was assessed in all patients. DIC was assumed if patients fulfilled 3 of the 5 following laboratory criteria, i.e. AT III <0.7 U/ml, factor V <0.6 U/ml, fibrinogen <1.5 g/L, platelet count <100x109/L or a positive fdp/FDP test. The sensitivity and specificity of the TAT-test was 86 and 46% respectively, the positive and negative predictive values were 60 and 77%.

BALEŽENTIENĖ, Ligita. "ALLELOPATHIC ACTIVITY OF INVASIVE SPECIES SOLIDAGO CANADENSIS L." In Rural Development 2015. Aleksandras Stulginskis University, 2015. http://dx.doi.org/10.15544/rd.2015.037.

Abstract:
Solidago canadensis L. (Asteraceae) spread throughout the world and also is enlisted in the National list of invasive species (2012). Here the allelopathic activity of the species was assessed in terms of further understanding of their distributions over the range of native spread. The invasion success of S. Canadensis was tried to base on the estimation of the total phenolics content (TPC) in the plant aqueous leachates at different growth stages. Allelopathic impacts (total phenolics content, total concentration and dynamic, conventional coumarine units, CCU) of S. canadensis were examined during 2012-2013 in Laboratory of raw materials, agro and zootechnics research, Aleksandras Stulginskis University. The plants were sampled in spring (May, rosette), summer (June, flowering) and autumn (September, seed maturity) for preparing the aqueous extracts. The biochemical (allelopathic) characteristics of S. canadensis aqueous extracts were examined at different plant growth stages. Principal (0–9) and secondary (0–9) growth stages as per universal BBCH scale description and coded using uniform two-digit code of phenologically similar growth stages of all mono- and dicotyledonous plant species. The TPC ranged between 0.968 mg ml-1 to 23.591 mg ml-1 depending on the plant ontogenetic stage, plant part and extract concentration (r = -0.7). Due to accumulated allelochemicals, the invasive S. canadensis, might acquire distribution advantage in new territories outside the native habitat range, through the inhibitory effects on germination of native plant species.

Abstract:
Recently, we have developed an enzyme-linked immunosorbent assay (ELISA) for determination of thrombin-antithrombin III complex (TAT) in numan plasma (1). The test system follows the sandwich principle and uses two different antibodies directed against human thrombin and human antithrombin III, respectively. The antibodies bind selectively to the corresponding antigen moieties of TAT. The assay was calibrated with definite concentrations (2.0 to 60 μg/l) of preformed purified TAT added to TAT-poor plasma. Plots of absorbance 492 nm against TAT concentration revealed a linear correlation (r = 0.98). The lower limit of sensitivity of the assay was 0.5 μg/l. Mean coefficients of variation of 4 % (intraassay) and 7.5 % (interassay) were found for TAT concentrations between 2 and 60 μg/l. A reference range from 0.85 to 3.0 μg/l was calculated from TAT concentration in plasma samples from 88 healthy donors (mean value ± SD: 1.45 ± 0.4 μg/l). In plasma samples from patients with pulmonary embolism confirmed by lung-scan (n = 17), TAT concentrations between 10 and 20 μg/l were measured. In patients with deep vein thrombosis confirmed by phlebography (n = 15), TAT were found up to 7 - 13 μg/l. Patients with septicaemia associated with a consumption coagulopathy (n = 10), and patients with acute hepatic failure (n = 5) showed markedly increased TAT values. In plasma samples from patients with various metastatic malignant diseases, TAT concentrations of 30 to 60 μg/l could be measured. From these data we conclude that measurement of TAT can be a sensitive parameter for specific detection of a latent activation of the clotting pathway.

Abstract:
Neutron image obtained from a small digital neutron imaging system, always has characteristics of low contrast, blurred edges and serious noise. It is disadvantageous to further analyse information about the sample’s internal structure, so it is essential for the observer to process the degraded image to improve its visual quality. In order to avoid the noise amplification problem of the original Richardson-Lucy (R-L) algorithm, which is adopted to recover degraded image, a restoration algorithm by combining R-L algorithm with Steering Kernel (S-K) algorithm for neutron image is presented in this paper. First S-K algorithm is applied to restrain the noise of the blurred noisy neutron image, as well as improving the signal-to-noise ratio of the image, and then R-L algorithm is used to reconstruct the blurred noisy image. The proposed algorithm is able to make up for the deficiency of R-L algorithm in dealing with the noise amplification problem, which is caused by the repeated iteration, while retaining the details of the image characteristics as much as possible. Comparative experimental results show that the algorithm can obtain satisfactory restoration visual effect for neutron image. The details of the work done are described in this paper.

Гарлицька, Т. С. Substandard Vocabulary in the System of Urban Communication. Криворізький державний педагогічний університет, 2018. http://dx.doi.org/10.31812/123456789/3912.

Abstract:
The article is devoted to substandard elements which are considered as one of the components in the system of urban forms of communication. The Object of our research is substandard vocabulary, the Subject is structural characteristics of the modern city language, the Purpose of the study is to define the main types of substandard vocabulary and their role in the system of urban communication. The theoretical base of our research includes the scientific works of native and foreign linguists, which are devoted to urban linguistics (B. Larin, M. Makovskyi, V. Labov, T. Yerofeieva, L. Pederson, R. McDavid, O. Horbach, L. Stavytska, Y. Stepanov, S. Martos). Different lexical and phraseological units, taken from the Ukrainian, Russian and American Dictionaries of slang and jargon, serve as the material of our research. The main components of the city language include literary language, territorial dialects, different intermediate transitional types, which are used in the colloquial everyday communication but do not have territorial limited character, and social dialects. The structural characteristics, proposed in the article, demonstrate the variety and correlation of different subsystems of the city language. Today peripheral elements play the main role in the city communication. They are also called substandard, non-codified, marginal, non-literary elements or the jargon styles of communication. Among substandard elements of the city language the most important are social dialects, which include such subsystems as argot, jargon and slang. The origin, functioning and characteristics of each subsystem are studied on the material of linguistic literature of different countries. It is also ascertained that argot is the oldest form of sociolects, jargon divides into corporative and professional ones, in the structure of slangy words there are common and special slang. Besides, we can speak about sociolectosentrism of the native linguistics and linguemosentrism of the English tradition of slang nomination. Except social dialects, the important structural elements of the city language are also intermediate transitional types, which include koine, colloquialisms, interdialect, surzhyk, pidgin and creole. Surzhyk can be attributed to the same type of language formations as pidgin and creole because these types of oral speech were created mostly by means of the units mixing of the obtruded language of the parent state with the elements of the native languages.

Abstract:
As a student undertaking a Longitudinal Integrated Clerkship (LIC)1 based in a GP practice in a rural community in the North of Scotland, I have been lucky to be given responsibility and my own clinic lists. Every day I conduct consultations that change my practice: the challenge of clinically applying the theory I have studied, controlling a consultation and efficiently exploring a patient's problems, empathising with and empowering them to play a part in their own care2 – and most difficult I feel – dealing with the vast amount of uncertainty that medicine, and particularly primary care, presents to both clinician and patient. I initially consulted with a lady in her 60s who attended with her husband, complaining of severe lower back pain who was very difficult to assess due to her pain level. Her husband was understandably concerned about the degree of pain she was in. After assessment and discussion with one of the GPs, we agreed some pain relief and a physio assessment in the next few days would be a practical plan. The patient had one red flag, some leg weakness and numbness, which was her ‘normal’ on account of her multiple sclerosis. At the physio assessment a few days later, the physio felt things were worse and some urgent bloods were ordered, unfortunately finding raised cancer and inflammatory markers. A CT scan of the lung found widespread cancer, a later CT of the head after some developing some acute confusion found brain metastases, and a week and a half after presenting to me, the patient sadly died in hospital. While that was all impactful enough on me, it was the follow-up appointment with the husband who attended on the last triage slot of the evening two weeks later that I found completely altered my understanding of grief and the mourning of a loved one. The husband had asked to speak to a Andrew MacFarlane Year 3 ScotGEM Medical Student 2 doctor just to talk about what had happened to his wife. The GP decided that it would be better if he came into the practice - strictly he probably should have been consulted with over the phone due to coronavirus restrictions - but he was asked what he would prefer and he opted to come in. I sat in on the consultation, I had been helping with any examinations the triage doctor needed and I recognised that this was the husband of the lady I had seen a few weeks earlier. He came in and sat down, head lowered, hands fiddling with the zip on his jacket, trying to find what to say. The GP sat, turned so that they were opposite each other with no desk between them - I was seated off to the side, an onlooker, but acknowledged by the patient with a kind nod when he entered the room. The GP asked gently, “How are you doing?” and roughly 30 seconds passed (a long time in a conversation) before the patient spoke. “I just really miss her…” he whispered with great effort, “I don’t understand how this all happened.” Over the next 45 minutes, he spoke about his wife, how much pain she had been in, the rapid deterioration he witnessed, the cancer being found, and cruelly how she had passed away after he had gone home to get some rest after being by her bedside all day in the hospital. He talked about how they had met, how much he missed her, how empty the house felt without her, and asking himself and us how he was meant to move forward with his life. He had a lot of questions for us, and for himself. Had we missed anything – had he missed anything? The GP really just listened for almost the whole consultation, speaking to him gently, reassuring him that this wasn’t his or anyone’s fault. She stated that this was an awful time for him and that what he was feeling was entirely normal and something we will all universally go through. She emphasised that while it wasn’t helpful at the moment, that things would get better over time.3 He was really glad I was there – having shared a consultation with his wife and I – he thanked me emphatically even though I felt like I hadn’t really helped at all. After some tears, frequent moments of silence and a lot of questions, he left having gotten a lot off his chest. “You just have to listen to people, be there for them as they go through things, and answer their questions as best you can” urged my GP as we discussed the case when the patient left. Almost all family caregivers contact their GP with regards to grief and this consultation really made me realise how important an aspect of my practice it will be in the future.4 It has also made me reflect on the emphasis on undergraduate teaching around ‘breaking bad news’ to patients, but nothing taught about when patients are in the process of grieving further down the line.5 The skill Andrew MacFarlane Year 3 ScotGEM Medical Student 3 required to manage a grieving patient is not one limited to general practice. Patients may grieve the loss of function from acute trauma through to chronic illness in all specialties of medicine - in addition to ‘traditional’ grief from loss of family or friends.6 There wasn’t anything ‘medical’ in the consultation, but I came away from it with a real sense of purpose as to why this career is such a privilege. We look after patients so they can spend as much quality time as they are given with their loved ones, and their loved ones are the ones we care for after they are gone. We as doctors are the constant, and we have to meet patients with compassion at their most difficult times – because it is as much a part of the job as the knowledge and the science – and it is the part of us that patients will remember long after they leave our clinic room.
