Abstract Vascular access surgeons are being referred an increasing number of patients with either inadequate forearm veins or previous failed radiocephalic arterio¬venous fistulas (AVFs). In this situation the choice for secondary vascular access lies between a prosthetic graft AVF or the creation of an upper arm fistula using autogenous vessels. Prosthetic grafts have been favoured more in the USA but the various alternative autogenous elbow fistulas have proved more popular in European countries. It is difficult to make a direct comparison between these two general approaches as there are a number of different alternatives in each case and there is very little, if any, randomized controlled trial data on which to base decision making. Prosthetic grafts can be needled early and easily but the avail¬ able uncontrolled historical data suggest that in general they have higher rates of thrombosis and infection and the long-term patency rates are not as good as autogenous AVFs. Moreover, prosthetic graft materials are expensive.

Abstract Repeated haemodialysis only became feasible with the introduction of the external arteriovenous shunt described by Quinton, Dillard and Scribner in 1960. This procedure opened up a new era in the treatment of end-stage renal failure despite the fact that external shunts were prone to infection and thrombosis. These problems wrere largely solved by the introduction of the Brescia-Cimino internal radiocephalic arteriovenous fistula (AVF) in 1966. This operation has been so successful that it remains the first-choice procedure for vascular access some 30 years after its inception. Autogenous vein would appear to have many advantages over prosthetic graft materials and European vascular access surgeons tend to exhaust every available autogenous vein before considering prosthetic materials. In stark contrast to this, it is common practice in the USA to use prosthetic grafts as primary AV conduits.
