Full textThis research presents and compares peripheral pediatric doses measurements in radiation therapy, for of thyroid, heart, breast, abdomen, ovaries and testicles regions, obtained with TLD LiF: Mg, Ti (TLD-100) powder, placed on three humanoid phantoms with sizes of children as two, five and ten years old, made of high density polyethylene and completely filled with water. It at linear accelerator Varian Clinac 600CD was employed to irradiate the heads of the three phantoms, with X-rays beams generated by a potential of 6 MV, with three different skull plannings: (a) IMRT sliding window with multileaf collimator rotated 0°, (b) IMRT sliding window with the multileaf collimator rotated 90° and (c) conformal planning (3D). The doses applied were 300 cGy and 600 cGy with five coplanar fields for all cares planning. The results show that the doses obtained for the all irradiations are very low compared to deterministic dose values deterministic for the regions of greater interest in radiological protection and that IMRT 90º provides more shielding for leakage radiation which would be directed to patient's body, offering advantages when compared to conformal planning and IMRT 0º.

Full textThis work performs an algorithmic study of optimization of a conformal radiotherapy plan treatment. Initially we show: an overview about cancer, radiotherapy and the physics of interaction of ionizing radiation with matery. A proposal for optimization of a plan of treatment in radiotherapy is developed in a systematic way. We show the paradigm of multicriteria problem, the concept of Pareto optimum and Pareto dominance. A generic optimization model for radioterapic treatment is proposed. We construct the input of the model, estimate the dose given by the radiation using the dose matrix, and show the objective function for the model. The complexity of optimization models in radiotherapy treatment is typically NP which justifyis the use of heuristic methods. We propose three distinct methods: MOGA, MOSA e MOTS. The project of these three metaheuristic procedures is shown. For each procedures follows: a brief motivation, the algorithm itself and the method for tuning its parameters. The three method are applied to a concrete case and we confront their performances. Finally it is analyzed for each method: the quality of the Pareto sets, some solutions and the respective Pareto curves
O presente trabalho realiza um Estudo Algor?tmico para Otimiza??o do Plano de Tratamento da Radioterapia Conformal. Inicialmente s?o apresentadas: uma vis?o geral sobre o c?ncer, o tratamento com radioterapia e no??es sobre a intera??o do feixe de radia??es ionizantes com a mat?ria. Uma proposta para Otimiza??o do Plano de Tratamento Radioter?pico ? desenvolvida de modo sistem?tico. ? apresentado o paradigma de problemas multicrit?rio, os conceitos de Pareto otimalidade e Pareto Domin?ncia. Um modelo Gen?rico de Otimiza??o para o Plano de Tratamento Radioter?pico ? proposto. Full textINTRODUCTION: In Radiotherapy, the dose distribution accuracy in heterogeneity correction calculations is directly related to the choice of calculation algorithm. There are many calculation algorithms commercially available. They vary in accuracy and processing time. This study aimed to quantify the accuracy of ten different calculation algorithms in lung equivalent material and to analyze the impact of the algorithm choice in the dose distribution in Radiotherapy applied to lung tumors. METHODS: It was used plates of water (solid water RW3) and lung (cork) equivalent materials to determine the Percentage of Depth Dose (PDD) and transversal profile inside the heterogeneity (cork). The measurements were performed in a Clinac Varian 6EX, with 6 MV photon beams and two field sizes (5 x 5 cm2 and 10 x 10 cm2), through irradiation of radiochromic films Gafchromic EBT3 and ionization chamber Scanditronix Wellhofer CC13. Treatment planning of 25 patients - 11 with tridimensional (3D) technique and 14 with Stereotactic Body Radiation Therapy (SBRT) technique - were performed, first without heterogeneity correction and, by keeping the Monitor Units (MU), the calculations were then performed with the different algorithms/methods of heterogeneity corrections and the results were compared with the initial planning. It was analyzed the target volume and organs at risk doses. RESULTS: The measurements performed in phantoms revealed that algorithms based on the convolution principle (Eclipse® Pencil Beam Convolution with correction methods Batho, Batho Modified and Equivalent TAR; XiO® Clarkson and Convolution e iPlan® Pencil Beam) presented significant dose differences in the cork region, overestimating the measurement, with a overdose higher than 8%. More advanced algorithms, as Eclipse® AAA and Acuros XB, XiO® Superposition and iPlan® XVMC, presented deviations below to 3% in the heterogeneity region. The profile analysis showed, similarly, that the second class of algorithms presents better performance in medium with low electronic density, like cork. The penumbra width presented deviations below to 1 mm for the more sophisticated algorithms against differences up to 4.5 mm between the convolution based algorithms. The dose distribution analysis in lung treatment planning showed that all the calculations performed with heterogeneity corrections presented doses higher than the calculation without heterogeneity corrections. The target volume dose-volume histogram (DVH) had a higher impact compared to the organs at risk. The calculation performed with convolution based algorithms presented dose distributions comparable, although different from the calculation performed without heterogeneity correction. Eclipse® AAA, Acuros XB, XiO® Superposition and iPlan® XVMC presented dose distribution similar, however Eclipse® Acuros XB and iPlan® XVMC are still more similar. The SBRT treatment planning presented higher deviations from the calculation with no heterogeneity correction, compared with the 3D treatment planning. CONCLUSIONS: The different calculation algorithms available have different accuracies in low density mediums. These differences have impact in the dose distributions in lung treatment planning, being the impact higher for the SBRT technique. Between the evaluated algorithms there is, at least one of each manufacturer, that presented acceptable performance in lung equivalent material and it should be the choice in lung treatment planning calculation

O presente estudo consiste na análise da viabilidade econômica e financeira da implantação da radioterapia de intensidade modulada (IMRT) no tratamento do câncer de próstata e comparar seus benefícios clínicos frente aos procedimentos similares custeados pelos sistemas de saúde públicos e privados. Para tanto, foram analisadas as distribuições de dose de radiação preconizadas nos planejamentos radioterápicos com as técnicas de radioterapia conformacional tridimensional (3D-CRT) e de intensidade modulada (IMRT), prescrita para 20 pacientes com câncer de próstata tratados em um serviço de radioterapia privado. Para verificação da qualidade da distribuição de dose de radiação no volume de tratamento, foram analisados os índices de homogeneidade (IH) e de conformidade (IC) de cada um dos planos radioterápicos estudados. Em comparação com a técnica 3D-CRT, a técnica de IMRT permitiu uma melhor cobertura e conformação da dose prescrita no volume alvo de planejamento (PTV), porém distribuída de forma mais heterogênea. Além disso, avaliou-se os custos financeiros para aquisição dos equipamentos de irradiação e manutenção dos tratamentos, bem como os valores de repasses feitos pelos sistemas de saúde para os procedimentos de radioterapia em próstata.
The present study consists in analyzing the economic and financial viability to establish the intensity modulated radiotherapy (IMRT) in the treatment for prostate cancer and compare its clinical benefits to similar procedures funded by public and private health systems. In order to do so, the radiation absorbed-dose distributions recommended in the radiotherapy plans with three-dimensional conformal radiotherapy (3D-CRT) and intensity-modulated radiotherapy (IMRT) techniques, prescribed for 20 patients with prostate cancer treated in a private radiotherapy clinic, were evaluated. To verify the quality of the radiation dose distribution in the treatment volume, the homogeneity index (HI) and the conformity index (CI) of each radiotherapy plans studied were analyzed. In contrast to the 3D-CRT technique, the IMRT technique allowed a better coverage and conformation of the radiation dose prescribed in the planning target volume (PTV), but distributed in a more heterogeneous way. It was also evaluated the financial costs for the acquisition of the irradiation equipment and for the maintenance of the treatments, as well as the values of the on-lending made by the health systems for the radiotherapy procedures for the prostate.

Full textLos tratamientos con radiación contra el cáncer han evolucionado procurando aumentar su eficiencia para favorecer la recuperación de los pacientes. La complejidad de estos tratamientos de radioterapia impone requisitos a los sistemas de dosimetría de radiación ionizante. Por ello, es necesario contar con sistemas avanzados de dosimetría, que permitan satisfacer los requerimientos dosimétricos actuales. Con ese objetivo, se desarrollaron e investigaron materiales y metodologías para realizar mediciones dosimétricas en técnicas de radioterapia novedosas, dando soluciones a situaciones que exceden las posibilidades de los sistemas tradicionales.
Radiation treatments for cancer have evolved with the aim of increasing their efficiency. The complexity of these radiotherapy treatments imposes requirements on ionizing radiation dosimetry systems. Therefore, it is necessary to have advanced dosimetry systemsto fulfil current dosimetry requirements. With this objective, materials and methodologies were developed and researched to perform dosimetry measurements in novel radiotherapy techniques, providing solutions to situations that exceed the capabilities of traditional systems.
