Abstract:
Background: Multiple pregnancies are associated with an increased risk of obstetric complications as well as perinatal morbidity and mortality especially in developing countries. The present study aims to understand the maternal and perinatal outcomes of multiple and singleton pregnancies delivering at Rajendra Institute of Medical Sciences, Ranchi, India. Aim of study was to investigate the maternal and fetal outcome in twin pregnancies in Rajendra Institute of Medical Sciences (RIMS), RanchiMethods: This comparative prospective study was conducted in the department of obstetrics and gynecology, Rajendra Institute of Medical Sciences, Ranchi from the period of 1st April 2015 to 30th September 2016. Consecutive sampling was done till the sample size of 75 was reached for both twin and singleton pregnancies. It included all women admitted in antenatal ward and labor room with clinical or ultrasound diagnosis of twin pregnancy after 28 weeks of gestation. Results: The incidence of twins in this study was 1.85%. Mean maternal age was 25.25±4.5 years for twin pregnancies and 23.53 ± 3.3 for singleton pregnancies. Twins were seen more in multigravida (70.7%) as compared to primigravida (29.3%). Preterm labor (74.7%), anemia (44%) and hypertensive disorders (32%) and PPH (13.33%) were the most common complication in twin pregnancies. Significantly higher rate of LSCS were seen in twin pregnancies (32.67%) as compared to singletons 18.67%. Mean weight of first twin was 2.03±0.52kg and for second twin it was 1.98±0.51kg. There was higher incidence of moderate to severe asphyxia, IUGR and higher rate of NICU admissions in twins as compared to singletons. Perinatal mortality rate of monchrionic pregnancy was 30% and it was 10.2% for dichorionic pregnancy which shows a significant association of perinatal mortality rate and chorionicity. Conclusions: Good antenatal care, with increased rest and nutritional supplementation, early detection of fetal and maternal complications together with thorough intranatal and postnatal vigilance, can lower both maternal and fetal dangers.

Abstract:
The publication of T. Rajendra’s book is very timely. The author’s main argument is that for understanding the contemporary migration crisis and fair distributions of responsibilities between citizens and migrants, current relations between locals and newcomers should be put into historical context. Heated debates and ambiguous attitudes towards refugees and migrants in receiving countries create, according to Rajendra, different, sometimes diametrically opposed narratives, which often contain historical inaccuracies and even barefaced lies. She thinks that justice to migrants requires correcting these inaccuracies, fiction, and falsifications and creating narratives more truly describing relations between citizens and migrants. Hence, these inaccurate and incomplete narratives should be replaced with fuller narratives faithful to reality. Rajendra concludes that an adequate answer to ethical questions about citizens’ responsibilities to migrants and potential migrants and the bases of these responsibilities requires not only new narratives but also new accounts on justice grounded on the ideas of responsibilities rather than the ideas of human rights that are typically used in the migration discourse. She calls it “justice as responsibility to relationships”. T. Rajendra’s book provides a fresh look at the migration crisis and the problem of undocumented migrants and offers a new approach to the reimagination and reconstruction of narratives about immigration — this complex and contradictory reality of our days.

Abstract:
Background: Breast carcinoma is the second most common carcinoma in women and accounts for 22% of all female cancer, which is more than twice the prevalence of cancer in women at any other site. Triple negative breast cancer, i.e., negative expression of oestrogen and progesterone receptors and HER2/neu receptors and accounts for approximately 10-17% of all breast carcinomas, is biologically aggressive, resistant to conventional cytotoxic chemotherapy treatment, and is associated with reduced survival compared to other subtypes of breast cancer. Methods: History, local examination, Various investigations like: FNAC of breast lump and axillary lymph node, USG of breasts and axilla, USG of whole abdomen and pelvis, Chest X-ray PA View etc, ER/PR and HER-2/neu status on the specimen sent for HPE were done. The prognostic implications on ER, PR and HER-2/neu receptors were assessed indirectly with the help of Nottingham prognostic index (NPI)).Results: A statistically significant correlation of ER/PR receptor status was found with tumour size, no. of lymph nodes, tumour grade and NPI, whereas HER2/neu receptor status had a statistically significant correlation with tumour size and no. of lymph node involved. Incidence of triple negative breast cancer in this Institute is 20%.Conclusions: ER, PR and HER2/neu receptor status is highly important predictor in cases of carcinoma breast which necessitates routine evaluation of these receptor statuses for better management of disease.

Abstract:
This paper examines Rajendra Bimal’s “The Lankuri Tree Will Blossom Again” is a post-conflict narrative which describes a poor and insurgency affected family living in the very hinterland of Nepal and their traumatic experience: fleeing away from home for safety, leaving the very adorable daughter to an unknown person and determining to live with the situation going on during the Maoist war. The story has been analyzed from the author’s narrative of adversity and trauma along with some thematic description. The themes include: perception of impending violence, trauma in the lives of commoners and gradual development from severe stress to determination to face the challenge. Nepal’s ten years long insurgency, a great loss in the country in the name of bringing peace and solidarity, has taken the lives of thousands of people and millions of others have been mentally and physically wounded. Hence, they are in trauma and their testimony has been a significant inscription of the history for those who want to know how innocent and common people were targeted. Here, I examine the story from personal trauma to the conflict generated consequences in the lives of affected people using content analysis of the personal, group, and socio-political factors associated with Bimal’s understanding of the People’s War, and of violence. Discussing the expected findings, this article centers on the common people’s fear of war and embedded peace in the country. Rajendra Bimal’s “The Lankuri Tree Will Blossom Again”, as a traumatic testimony of the insurgency, has been analyzed in this article to bring out the facts.

Abstract:
The field experiment was conducted at the Agricultural Research Farm, Banaras Hindu University, Varanasi, India during two consecutive rainy (kharif) and winter (rabi) seasons of 2015-16 and 2016-17 in split plot design to study the effect of integrated nitrogen management (INM) [100% recommended dose of nitrogen (RDN: 120 kg N/ ha), 75% RDN + 25% N through Azolla microphylla, 75% RDN + 25% RDN through NADEP compost, 75% RDN + 25% RDN through sewage sludge and 75% RDN + 25% RDN through vermicompost] in aromatic rice (Oryza sativa L.) varieties (HUR-4-3, HUR-105, NDR-6093 and Rajendra Kasturi) under system of rice intensification and its residual effect on productivity of succeeding wheat (Triticum aestivum L.). The results of the study revealed that the variety HUR-4-3 and the INM with 75% RDN + 25% N through Azolla microphylla were found superior in terms of enhanced growth, yield attributes, yield, nutrient uptake, milling quality and length-breadth ratio (L/B) before cooking as compared to rest of the varieties and integrated nitrogen management, respectively. It was found that soil microbial population get improved when 75% RDN was coupled with 25% N through Azolla microphylla. Further, higher gross income, net return and output-input ratio were attributed to the variety Rajendra Kasturi (177.47×103 `,₹130.35×103 `/ha and 3.79, respectively) and the application of 75% RDN + 25% N through Azolla microphylla (162.21×103 `,₹117.14×103 `/ha and 3.6, respectively). Moreover, varieties did not show marked effect on yield of the succeeding wheat while significant yield enhancement was evident as a result of nitrogen management in preceding rice with 75% RDN + 25% N through Azolla microphylla.
