Full textLa validité des résultats provenant d’études s’intéressant à l’impact des facteurs nutritionnels sur la santé dépend directement de la validité des outils d’évaluation alimentaires utilisés. Les différentes méthodes d’évaluation alimentaire ont chacune leurs forces et leurs faiblesses et sont toutes sujettes à de nombreux biais. Le rappel de 24 heures traditionnel (R24H), une méthode où un interviewer demande au participant de rapporter tous les aliments et boissons consommés au cours de la dernière journée, est de plus en plus utilisé en recherche en raison d’études de validation utilisant des biomarqueurs ayant démontré que des R24H répétés pouvaient fournir des données d’apports alimentaires de haute qualité et ce, avec un biais minimal. Néanmoins, cet outil est considéré coûteux puisqu’il dépend de professionnels formés à la fois pour son administration et pour le codage des réponses dans des bases de données nutritionnelles. Le développement d’outils d’évaluation alimentaire Web apparaît comme une solution économique dans un contexte où il est de plus en plus facile de rejoindre les Canadiens par une application Web. C’est dans ce contexte qu’un rappel de 24 heures automatisé et auto-administré sur une plate-forme Web, le R24W, a été développé à l’INAF, Université Laval. L’objectif de ce mémoire est de décrire le développement de ce nouvel outil d’évaluation alimentaire et de discuter de l’impact de son utilisation dans la recherche en nutrition.
The validity of results from studies assessing the impact of nutritional factors on health depends directly on the quality of dietary assessment tools. All dietary assessment methods have strengths and weaknesses and are subject to bias. The 24-hour dietary recall (24HDR), where an interviewer asks the participant to report all foods and beverages consumed during the last 24 hours, is increasingly used in epidemiology due to validation studies using biomarkers that have demonstrated that repeated 24HDR could provide high quality dietary intake data with minimal bias. However, this tool is considered expensive since it depends on trained professionals for both its administration and the coding of responses in nutritional databases. The development of web-based assessment tools appears as an economical solution in a context where it is increasingly easy to reach Canadians through a web application. It is in this context that an automated, self-administered web-based 24-hour recall, the R24W, was developed at INAF, Laval University. The objective of this study is to describe the development of this new dietary assessment tool and to discuss the impact of its use in nutrition research.

Full textPregnancy has long been recognized as a determinant of the mother’s and infant’s future health. Among other factors, the mother's diet is known to have a great impact on many pregnancy-related outcomes. In fact, inadequate dietary intakes have often been associated with a high risk of developing adverse pregnancy outcomes, such as gestational diabetes. The evaluation of potential associations between diet and health variables may, however, be affected by an imprecise assessment of pregnant women’s actual dietary intakes. Multiple administrations of a 24-hour dietary recall have the potential to accurately assess the diet of pregnant women, but its traditional version is often criticized for the time it requires and the many biases induced by the presence of an interviewer. To overcome this problem, new standardized and fully automated food assessment methods have been developed. These were recognised as valid in the general population, however, their precision during pregnancy has not yet been fully assessed. This thesis assesses the validity of the R24W, a Web-based 24- hour dietary recall, among pregnant women. Eventually, this validation will allow to use this dietary assessment tool in future epidemiological studies as well as for the nutritional management of pregnant women in a clinical context.

Full textIntroduction: Several diet quality indicators have been developed based on nutrient intakes, foods or food groupings, or a combination of nutrient and foods. Measures of overall diet quality consider numerous aspects of dietary intake. It is within this framework that the Canadian Healthy Eating Index (C-HEI) was adapted to Canadian dietary and nutritional guidance using Canada’s Food Guide and Nutrition recommendations for Canadians. Objectives: To determine the links between the C-HEI using the mean of three 24-hour diet recalls, and certain health outcomes at entry into the study (T1). The analyses were carried out using data collected from participants in the Quebec Longitudinal Study on Nutrition and Successful Aging (NuAge). Methodology: Bivariate analyses (Spearman correlation, cross-tabulation and chi-square) were carried out to assess relationships between nutritional and anthropometric variables, the number of chronic disease and the C-HEI. Results and Discussion: Participants met or exceeded most Dietary Reference Intakes (DRI). The C-HEI did not show consistent relationships with individual nutrients (rs= 0.14-0.52, p<0.01). However, selected sub-scores of the C-HEI reflect attainment of Canada’s Food Guide recommendations, showing a strong association with dietary fiber intakes (rs= 0.51) and nutrition recommendations addressing percent energy as fat (rs=-0.60) and as saturated fat (rs= -0.59), p<0.01. Energy and protein adequacy increased from Q1 to Q 4 of C-HEI (p<0.05). In addition, proportions of subjects with anthropometric measures associated with a health risk decreased from quartiles Q1 to Q4 (p<0.05), attesting to links between good diet quality and lower health risks associated with overweight and obesity. Conclusion: The C-HEI discriminates overall diet quality based on dietary data collected from 24HR, providing further evidence on the links between the C-HEI and selected nutritional and anthropometric parameters as applied to quantitative dietary data obtained from community-dwelling older adults.
