Rat bite fever is rare in Western countries. It can be very difficult to diagnose as blood cultures are typically negative and a history of rodent exposure is often missed. Unless a high index of suspicion is maintained, the associated polyarthritis can be mistaken for rheumatoid arthritis. We report a case of culture-positive rat bite fever in a 46-year-old female presenting with fever and polyarthritis. The clinical presentation mimicked rheumatoid arthritis. Infection was complicated by discitis, a rare manifestation. We discuss the diagnosis and management of this rare zoonotic infection. We also review nine reported cases of rat bite fever, all of which had an initial presumptive diagnosis of a rheumatological disorder. Rat bite fever is a potentially curable infection but can have a lethal course if left untreated.

SUMMARY Rat bite fever, caused by Streptobacillus moniliformis, is a systemic illness classically characterized by fever, rigors, and polyarthralgias. If left untreated, it carries a mortality rate of 10%. Unfortunately, its nonspecific initial presentation combined with difficulties in culturing its causative organism produces a significant risk of delay or failure in diagnosis. The increasing popularity of rats and other rodents as pets, together with the risk of invasive or fatal disease, demands increased attention to rat bite fever as a potential diagnosis. The clinical and biological features of rat bite fever and Streptobacillus moniliformis are reviewed, providing some distinguishing features to assist the clinician and microbiologist in diagnosis.

Painless rat bites in patients with diabetic neuropathy are very rare occurrence and may result in foot ulcers, amputations, or rat bite fever. Infective endocarditis complicating rat bites are extremely rare and almost exclusively a complication of rat bite fever caused by Streptobacillus moniliformis (a common microbial flora of the rat mouth). To the best of our knowledge, this is the first report of Staphylococcus aureus (another common flora of rat teeth) native valve endocarditis complicating rat bite.

Abstract:
A 71-year-old man was admitted with confusion having been discovered on the floor of his unkempt home. Social services would later confirm that the home was infested by rats. An infection of unclear origin was suspected, and he was immediately started on broad spectrum antibiotics. He subsequently developed a swollen left knee. Two knee aspirates showed a raised white cell count but no microbiological or cytological diagnosis. Finally, samples sent for 16S rDNA PCR identified Streptobacillus monilliformis. Fastidious organisms and early treatment with antibiotics can limit microbiological diagnosis through standard laboratory analysis. Newer techniques such as sequence-based testing can overcome these limitations; however, they are not widely available and require the clinician to have a high index of suspicion. Crucially, the patient continued to be treated for his clinical sepsis despite initial laboratory results being negative and conflicting specialist opinions.

4 year old patient was admitted to the Emergency Department of Hospital Regional de Cusco with 17 days history of a rat bite. One week later, the patient developed fever and rash, which lasted for 3 days. Fever, an abscess on the forehead with lymphangitis, and cervical lymphadenopathy were observed on admission. The VDRL test was positive but FTA-Abs was negative. He was diagnosed of rat bite fever and treated with IV penicillin G with a favorable response. The case is discussed and pertinent literature is reviewed.

In the United States, rat-bite fever is a rare systemic illness principally caused byStreptobacillus moniliformis, an organism found in the nasopharyngeal flora of rodents. Infection through direct exposure to rat excreta such as saliva, urine, or feces can lead to fever, rash, and an asymmetric migratory polyarthritis. As rodents are becoming more popular as pets, more pediatric cases are being documented. We report a pediatric case of delayed onset septic arthritis in the left wrist and right knee due toS. moniliformisfrom a rat bite. Previously reported pediatric case studies of suppurative arthritis due toS. moniliformishave only involved the hip. This case study demonstrates the importance of a thorough exposure history and consideration of zoonotic infections as a cause of septic arthritis in a pediatric patient that requires antibiotics and surgical intervention.
