Les objectifs de cette thèse ont été d'étudier le rôle des tachykinines dans la transmission nerveuse au niveau des voies aériennes. Nous avons ainsi : 1) examiné le rôle des tachykinines dans la potentialisation par l'endotheline-1 de la contraction des bronches humaines isolées induite par la stimulation électrique des nerfs cholinergiques ; et 2) étudié chez le lapin, dans un modèle de reflux gastrooesophagien, le rôle des tachykinines dans la bronchoconstriction et l'extravasation plasmatique au niveau des voies aériennes induites par l'instillation d'HCI dans l'oesaphage. L'endotheline-1 accroît les contractions des bronches humaines isolées induites par la stimulation électrique de champ. .<br>1- The aim of this thesis is studying role of tachykinins in the airway neurotransmission. 2- This study demonstrated that, at least, in part, the ET-1 potentiation of cholinergic nervemediated contraction is mediated by tachykinin release, suggesting that, in addition to nerves, several type of cells, such as airway smooth muscle cell, may participate to neuropeptide production. 3- Intraoesphgeal (i. Oe. ) instillation of HC1 in anaesthetized rabbit induce a significant bronchoconstriction. . .

Le but de cette étude était d’analyser l’organisation des 3 composantes du bolus dégluti (air–mélange–baryte), d’évaluer leur temps de passage à travers la jonction œsogastrique, d’établir des comparaisons avec les bruits xiphoïdiens à partir d’acquisitions acoustiques et radiologiques simultanées chez des patients opérés d’un reflux gastroœsophagien(NF). Les surfaces des trois composantes et leurs temps de transit étaient mesurés, la durée du signal xiphoïdien et le nombre de groupe de vibrations étaient évalués. L’origine des bruits était déterminée. La décantation air-baryte était perturbée. Après NF, le passage du bolus était plus rapide. Avant NF, le bruit xiphoïdien était composé de groupes de vibrations plus nombreux et plus espacés. Après NF, la durée du bruit et le nombre de groupes de vibrations diminuaient. Le bruit xiphoïdien était généré par le passage de l’air et d’une partie du mélange à travers la jonction œsogastrique<br>The aim of this study was to assess the organization of the three bolus components (air– mixture–barium) and their transit time across EGJ with concurrent acoustic and radiological digitalised recordings in GERD patients treated by NF. The surfaces of the three bolus components in the oesophagus and their transit time across EGJ were measured he xiphoid signal duration and the number of vibration groups were evaluated The position of the three components at the EGJ were correlated with the vibration groups to determine the origin of sound on the acoustico-radiological sequences. The observation of radiological sequences shown that decantation air-barium were impaired. The passage of bolus was more quick after NF. The xiphoid sound was composed of vibrations groups which were more numerous and more spaced before NF. Sound duration and number of groups were decreased after NF. The xiphoid sound was generated by the passage of air and a part of mixture across EGJ

L'objectif de ce travail a été de faire le point sur les connaissances actuelles concernant les mécanismes physiopathologiques, les moyens diagnostiques et thérapeutiques du reflux duodéno-gastro-oesophagien et de rapporter nos travaux de recherche et d'évaluation clinique concernant cette entité pathologique. La première partie est consacrée au reflux duodéno-gastrique primaire et à la méthode de détection de la bilirubine par spectrophotométrie. Nous rapportons notre expérience acquise dans l'unité de chirurgie oeso-gastrique du Pr Collard à l'Université Catholique de Louvain (Cliniques Universitaires Saint-Luc) à Bruxelles. La deuxième partie est consacrée au reflux biliaire secondaire en cas de pathologie biliaire ou après chirurgie oeso-gastro-duodénale. Nous rapportons notre expérience concernant le reflux biliaire postopératoire en fonction de différents types de montages chirurgicaux après chirurgie gastrique. La troisième partie est consacrée aux lésions d'endobrachyoesophage. Le rôle du reflux biliaire dans la genèse et la transformation des lésions d'endobrachyoesophage est précisé ainsi que l'impact des différents montages chirurgicaux anti-reflux sur les lésions d'endobrachyoesophage. Abstract:
Le reflux gastro-oesophagien (RGO) est considéré un facteur d'aggravation de l'asthme. L'implication dans cette pathologie des systèmes nerveux cholinergique et neuro-sensoriel a été démontrée chez l'Homme et/ou chez l'animal. Nous avons étudié les effets de différentes substances dans un modèle d'étude des effets bronchiques du RGO chez le Cobaye. Nous avons ainsi montré, pour ce qui est du rôle des fibres neurosensorielles, que la nociceptine, un agoniste des récepteurs opioïdes du sous-type NOP exerce effet protecteur, de même que les cannabinoïdes, par la stimulation des récepteurs du sous-type CB2. Dans le domaine de la transmission cholinergique, les récepteurs muscariniques MI sont impliqués dans la protection bronchique par des effets inhibiteurs au niveau de la transmission ganglionnaire mais aussi au niveau vasculaire, en périphérie. En conclusion, nos résultats ouvrent des perspectives thérapeutiques nouvelles dans le traitement des manifestations bronchiques du RGO<br>Several evidence suggest that gastroesophageal reflux disease (GERD) is a potential trigger of asthma and a complicating factor in difficult-to-treat patients. Cholinergic and/or sensory nerves may be involved in this pathology. We have studied the role of these pathways in an experimental model of airway inflammation induced by GERD in the Guinea-Pig. We have shown that nociceptin, an agonist of the opioid NOP-receptor subtype and cannabinoids, through CB2 receptor subtype stimulation, may prevent inflammation induced by GERD. In addition, concerning the cholinergic system, we have shown that Ml receptor inhibition may prevent inflammation not only through inhibition of ganglionnic transmission but also by a direct effect on microvascular leakage. These results may suggest new therapeutic approches for the treatment of asthma aggravation induced by GERD

Abstract:
Le pronostic de l’adénocarcinome œsophagien (ADC) est très sombre, avec un taux de survie à 5 ans inférieur à 10 %, du à sa détection tardive. L’incidence de ce cancer est en constante augmentation depuis une trentaine d’année dans les pays occidentaux. L’ADC se développe sur un œsophage de Barrett, metaplasia induite par une exposition chronique de l’ œsophage au reflux duodéno-gastro-œsophagien. Notre équipe a précédemment montré que certains acides biliaires presents dans ce reflux régulent l’expression des deux mucines membranaires MUC1 et MUC4, ainsi que la PI3K et NF&#61547;B dans une lignée cellulaire d’adénocarcinome œsophagien humain. But du travail : Déterminer si le reflux duodéno-gastro-œsophagien induit une carcinogenèse œsophagienne dans un modèle in vivo, ainsi qu’identifier de nouvelles proteins associées à cette carcinogenèse. Matériel et méthodes : Un modèle de reflux chronique induit par chirurgie à été réalisé. L’expression des ARN ainsi que des protéines a été étudiée grâce aux techniques de RT-PCR, qRT-PCR, puces à ARN et immunohistochimie. Les propriétés biologiques de cellules issues d’un adénocarcinome œsophagien humain (OE33) ont été étudiées in vitro. Résultats : Tous les rats subissant un reflux montrent une inflammation ainsi qu’une néo-expression de gènes connus pour participer à la progression tumorale, plus particulièrement dans la différenciation, la prolifération, l’adhésion et la formation de métastases. Parmi ces gènes activés on retrouve les deux voies de signalisation PI3K et KF&#61547;B. De manière intéressante on observe une néoexpression des deux mucines membranaires Muc1 et Muc4 ainsi qu’une surexpression de deux protéines associées à la progression tumorale, S100a4 et Mcm6. Ces différents gènes participent à la régulation des propriétés biologiques des cellules d’adénocarcinome œsophagien. Conclusion : L’ensemble de ces résultats montre qu’une exposition chronique au reflux duodéno-gastro-œsophagien induit la progression vers un adénocarcinome œsophagien dans un modèle in vivo chez le rat. Nous avons mis en évidence des voies de signalisation ainsi que des proteins associées à la carcinogenèse œsophagienne, ces dernières pouvant être considérées comme de potentiels marqueurs ou cibles thérapeutiques dirigées contre l’adénocarcinome œsophagien. Its incidence has been increasing for the last 30 years. ADK develops on Barrett œsophagus, a metaplastic lesion induced by chronic exposure of œsophagus to the duodeno-gastro-œsophageal reflux. Moreover we showed previously that bile acids activate MUC1 and MUC4 mucins, PI3K and NF&#954;B signalling pathways in human adenocarcinomatous cells. Aim: To determine whether the reflux induces œsophageal carcinogenesis in an in vivo model, and to identify new tumor-associated proteins. Material and methods: A rat model of chronic reflux induced by surgery was established. Expression of RNA and proteins was studied using RT-PCR, qRT-PCR, micro-arrays and immunohistochemistry. Effects on cell biological properties were carried out in vitro in OE33 œsophageal adenocarcinomatous cells. Results: All rats with reflux showed inflammation and neoexpression of genes involved in tumor progression with alterations of markers involved in differentiation, proliferation, adhesion and metastasis in which key pathways such as PI3K and NF&#954;B were found activated. More importantly, Muc1 and Muc4 mucins were neoexpressed, and two new tumor-associate proteins, S100a4 and Mcm6, were overexpressed in tumors and showed in vitro effects on œsophageal cancer cell biological properties. Conclusion: Altogether, these data indicate that progression toward adenocarcinoma was effective following induction of a chronic reflux in rat œsophagus and that signalling pathways and new tumor-associated proteins were identified that may be new biomarkers and new therapeutic targets in œsophageal adenocarcinoma.
