Full textCollaboration between health care providers has been greatly encouraged in recent decades to improve effectiveness and efficiency of health services. However, to this day, little is known of private sector physiotherapists’ interprofessional practices. This thesis had three main objectives that were addressed by conducting a three-part mixed-methods qualitative/quantitative project. The main objective of the first part, a qualitative descriptive study, was to explore experiences and perceptions of private sector physiotherapists regarding their interventions with adults with low back pain. Results of semi-structured interviews with 13 physiotherapists showed that, for physiotherapists, interprofessional practices included processes such as sharing information and referring, they were influenced by factors related to patients, providers, organizations, and wider systems and were mostly viewed as having positive effects. The second and third parts of the project were quantitative. Their main objectives were respectively to draw portraits of 1) the organizations where physiotherapists work in the private sector in Québec (part 2), and 2) of the interprofessional practices of these physiotherapists in the context of their interventions with adults with low back pain (part 3). Data were obtained through a cross-sectional provincial survey with 327 randomly-selected physiotherapists (participation: 67,7 %) working in 243 organizations. Parts 2 and 3 allowed to describe physiotherapists’ interprofessional practices, to identify physiotherapists’ perceptions regarding these practices, as well as to explore organizational and provider-related variables associated with the intensity of interprofessional practices. In addition, part 3 was associated with a methodological contribution by the adaptation of an instrument measuring the intensity of the interprofessional practices and the evaluation of a few of its psychometric properties, the results having indicated it was a promising instrument. This thesis offers new knowledge of physiotherapists’ interprofessional practices, an understudied subject, which is of interest to physiotherapists and other providers, managers, decision-makers and professional boards and associations, with the goal of improving or extending interprofessional practices involving physiotherapists when relevant.

Full textThe present doctoral thesis aims to further the understanding of violence in romantic relationships during adolescence through the adoption of a person-centered approach. The primary aim of this thesis is to identify the different patterns of physical and psychological violence experienced in romantic relationships during adolescence, for both genders separately, through latent class analysis. Additionally, it aims to establish whether the violence patterns differ on certain associated factors, through an analysis of variance. A sub-sample of 2 730 adolescents (aged 14 to 18 years) having inflicted or suffered an act of physical or psychological violence during the last year were selected among a representative sample of high school students in the province of Quebec. Four distinct violence patterns were identified for the sample of girls and boys. The identified patterns differ in terms of type of violence suffered as well as frequency of violent acts, but all are characterized by mutual violence. Thus, the present doctoral thesis demonstrates the usefulness of adopting a person-centered approach to study this phenomenon, and proposes different options for intervention.

Full textThis thesis examines the private charitable sector for English-speakers in Quebec City from 1759 to 1900. It provides an overview of poor relief associations, the needs they addressed, and the gaps that remained. The role of private charities increased over the period studied, and that of the state decreased. Compassion toward the poor also increased, leading to new types of charitable organizations for the underclass. Despite this, the prison system served as a refuge to fill gaps in the private charitable sector. More specifically, this study demonstrates how changes in ethno-religious relations shaped the charity network. In the first half century after the Conquest of Quebec, British authorities supported the Catholic charitable infrastructure established during the French regime, which was unusual within the British Empire. After 1815, as immigration from Britain and Ireland increased, lay private voluntary associations emerged, including many that involved elite cooperation across religious and linguistic lines. Instances of cooperation decreased from 1835 to 1855 due to rising ethnic boundaries caused by the defeat of Patriote republicanism, an increase in religious practice, the establishment of separate confessional schools, and a new type of Irish-Catholic nationalism following the Great Famine. In the latter half of the nineteenth century, the private charitable sector became sharply divided into three parallel networks with hardly any overlap: one for Francophone Catholics, one for English-speaking Irish Catholics, and one for English-speaking Protestants. Two core institutions founded in the 1850s, Saint Bridget’s Asylum and the Ladies’ Protestant Home, cemented the divide. Rare attempts to challenge these boundaries resulted in tension and even violence. Despite these divisions, there was a greater mutual respect of established boundaries among communities than in most North American cities.

Full textThis research aimed to better understand the emotional difficulties (depression, anxiety, burnout, etc.) of Quebec’s child welfare clinicians and to explore the impact of these difficulties on their professional relationships (with the children and their families, colleagues and managers). By highlighting their subjective experience and experiential knowledge, the research allows to explain the emotional difficulties in a work context outside of the dominant psychiatric and psychological perspective. A sociological framework based on the concept of emotional labor as well as the clinical sociology theories of work was developed. This conceptual perspective shed’s light and articulates the contextual, organizational and individual factors that impact emotional difficulties experienced by clinicians in a child welfare setting. The research methodology is qualitative, based on semi-directed interviews at the CISSS and a CIUSSS from two different administrative geographic regions in the Québec province. The population sample is composed of thirty-one psychosocial clinicians (Professional Social Workers, Criminologists, Psychoeducators and Bachelor Professional Psychologists) working in child welfare organizations. The research results demonstrate that there are three (3) main protection factors at work (organizational mandate, work diversity, peers relations). Furthermore, the following factors were identified as impacting their emotional difficulties; three (3) contextual factors, nineteen (19) organizational factors and four (4) individual factors. From these categories two of them are structural as therefore external to individual control. As a result, targeting individual factors to explain clinicians emotional difficulties restricts the possibility of modifying their situation. Specifically, participants identified that the latest health care reform including the CISSS and CIUSSS service structure changes led to significant consequences that deeply affected their work organization and their clinical practice in the context of child welfare context. Also, the results show clinical functional impacts on their professional relationships. Negative impacts on their relationships with families and children were reported by all clinicians (decrease of intensity in service delivery, hurtful relationships with clients, lack of empathy and helpful relationship, decrease of work quality and objective and analytical abilities of clinicians). For 90% of the participants, emotional difficulties had negative impact on their relationships with their managers (anger, frustration, trustfulness, avoidance and lack of confidence) and 68% with their colleagues (isolation and withdrawal, negative emotional contamination, decrease in collaboration and lack of mutual aid). Overall, this research demonstrates the importance of protecting clinicians in a children protection mandate. Also, it shows that the emotional work that clinicians must produce, according to the requirements of the institutions involved (employer, academia and professional orders) is at the of all professional relationships in a child welfare context. This research allows in this regard to highlight the favorable conditions of emotional work, which can act as a protective filter in child welfare services. The research also allows, finally, to extend the typology of emotional work.

Full textWe conducted a descriptive cross-sectional study to estimate the proportion of consultations in family medicine reporting risk communication, and clarification of values and preferences during the medical decision-making process. In 238 clinician-patient dyads (238 patients and 71 clinicians), we observed 63% (95% confidence interval [CI] 54% - 70%) of visits where risk communication, value clarification and preference elicitation occurred. We also observed that six factors were associated with the presence of these two elements during the discussion with: 1) new therapeutic options (OR = 3.54; 95% CI 1.32 - 9.48); 2) treatment options (OR = 3.56; 95% CI 1.52 - 8.36); 3) presence of five health decisions or more (OR = 5.00; 95% CI 1.5 - 16.9), 4) postponing a decision (OR = 4.92; 95% CI 1.35 - 17.87); 5) the longer visits (OR = 1.03; 95% CI 1.002 - 1.07) and the collaborative decision-making style of health professionals (OR = 8.78; 95% CI 1.62 - 47.71). Interventions directly targeting those that are modifiable should be considered to increase risk communication and values clarification during the process of medical decision making in primary care.

Full textUnderstanding the historical and contemporary relationship between aboriginal peoples and the governments of Canada and Quebec is necessary if a rapprochement between these adversarial positions is to be achieved.
This paper explores the legal and historical basis of aboriginal rights, focussing on self-government and the fiduciary relationship between aboriginal peoples and the Crown. It discusses international law principles under which Quebec will seek recognition as an independent state and the relevance of aboriginal rights to that recognition. Finally, it urges that the current debate provides an opportunity to establish a new partnership between Quebec and aboriginal peoples, to their mutual benefit.

Full textThe purpose of this thesis is to study the reconfiguration of the relationship between work and training. It seeks to highlight the process by which cross-curricular competencies entered Quebec's primary and secondary school system in the early 2000s. In an analysis combining a homology approach inspired by Lucy Tanguy's work and a study of social representations based on the model developed by Pascal Moliner, the aim of this thesis is to understand the change in the relation between work and training, which has been achieved by the introduction of transversal skills. It aims to demonstrate the influence of new productive systems and work flexibility practices on the development of a new conception of primary and secondary education. A conception designed to "secure" the career paths of future workers by instilling competences aimed at adaptability and "employability". On the other hand, this "security" would not be exclusively motivated by changes in the labor market, but by the development of a contemporary worldview based on uncertainty.

Full textIn Quebec, most children go to school at 5 years old when they enter kindergaten. For the majority of children, entry to school will be no problem. For others, the transition will be more difficult and the difficulties encountered at the beginning of schooling will often have consequences that will affect them throughout their school path. In order to meet students’ needs, several support measures can be put in place, for both students and teachers, including collaboration between different professionals (Belmont & Verillon, 2004). Previously, the collaboration was done outside the classroom by co-intervention. As things change, it is now being carried out more and more within the class-group by co-teaching. Most co-teaching research was conducted at the primary and secondary levels and focused on defining the concept and evaluating the effectiveness of this type of teaching and its set up (Friend et al., 2010). Most research doesn’t focus on actual practices of teachers, especially in the kindergaten classroom. The purpose of this case study is to 1) identify and analyze the interventions made within the group in order to take into account the specific needs of students at risk, handicapped or in difficulty of adaptation or learning. It also wants to 2) describe and to understand how kindergaten co-teaching works by observing coteaching teaching-learning situations. This research was conducted with two kindergaten teachers from the Quebec-Chaudière-Appalaches region doing co-teaching on a full-time basis. The teaching-learning situations were observed between January and May 2018 every two weeks. The data were collected through semi-structured interviews, filmed observations as well as selfconfrontation interviews. The results of this research show that full-time co-teaching in kindergarten is a form of collaboration that promotes inclusive education and pedagogical differentiation of both learning and teaching, not only the students’ needs in difficulty, but also all students in the classroom. Full textLes problèmes d'intégration et de coordination des services, particulièrement en première ligne, perdurent depuis plusieurs années. Les professionnels de la santé de la première ligne travaillent souvent en "silos" et les services offerts aux citoyens suivent rarement un processus de continuité. Intégrer des psychologues au sein des équipes cliniques de première ligne pourrait contribuer à améliorer la continuité, particulièrement entre les services médicaux et psychologiques. La présente étude vise la compréhension du rôle des facteurs individuels, professionnels et organisationnels sur la capacité qu'ont les psychologues à s'intégrer dans des équipes multidisciplinaires de première ligne.
L'étude prend appui sur le modèle de collaboration interdisciplinaire de Sicotte et ses collaborateurs (2002). Une méthode qualitative a été privilégiée, soit une étude multi-sites. Une unité de médecine familiale (UMF) en centre hospitalier et une UMF en Centre de santé et de services sociaux (CSSS) constituent les deux principaux sites à l'étude, soit deux situations de première ligne qui pourraient bénéficier de l'implication de psychologues. Des entrevues individuelles ont été réalisées, en face à face, auprès de médecins, d'infirmiers(ères), de travailleurs sociaux, de psychologues, de résidents en médecine et de gestionnaires (n=19). Les principaux constats révèlent l'existence d'un réel besoin d'informer les professionnels de la santé de la première ligne sur ce qu'est le travail en équipe interdisciplinaire et en quoi consiste l'apport du psychologue au sein d'une équipe de première ligne. Les entrevues révèlent également que la distinction entre les rôles du psychologue et du travailleur social est loin d'être claire.

Full textAujourd’hui, devenir grand-parent se vit d’une tout autre façon que pour les générations précédentes : la grand-parentalité est désormais reconnue contingente, généralement comme une chance, et on y évolue dans des situations de solidarité familiale intergénérationnelle diversifiées qui se transforment avec l’interprétation de nouveaux rôles de parents et de grands-parents. Ce mémoire vise à comprendre comment la solidarité familiale intergénérationnelle se transforme avec la naissance d’un premier enfant et petit-enfant et comment, dans ce nouveau contexte, les parents et grands-parents se trouvent engagés dans des pratiques de solidarité dans lesquelles – et par lesquelles – le rôle grand-parental prend forme. L’analyse d’entretiens croisés réalisés auprès de onze parents et de neuf grands-parents révèle que la naissance d’un enfant est perçue comme un moment charnière qui introduit de nouvelles attentes induisant pour la plupart un renforcement de la solidarité familiale intergénérationnelle, autant par une multiplication des pratiques concrètes de solidarité que par l’expérience de normes et de sentiments nouveaux. Le rôle grand-parental se profile dans la continuité du rôle parental exercé précédemment, en réponse aux nouveaux besoins des parents. Il s’articule également aux rôles tenus par les parents auprès des enfants, selon un principe de consolidation des fonctions parentales, ou de différentiation par rapport à celles-ci. Mais si le rôle grand-parental se définit en grande partie dans son articulation au rôle parental, il se compose également dans la filiation d’idéaux personnels qui renvoient à des souvenirs, s’appuient sur des modèles et se dessinent en opposition à des contre-exemples. C’est donc entre les attentes que posent ces conceptions idéalisées et les possibilités concrètes qu’offre la situation de solidarité familiale intergénérationnelle que le rôle grand-parental prend forme.
In contrast with previous generations, becoming a grandparent nowadays is experienced utterly differently. Being a grandparent is now a contingent situation, generally perceived as a chance, which results in the growth of a plurality of situations of intergenerational family solidarity, evolving coextensively to the interpretation of the new roles of both parents and grandparents. This thesis aims to understand how intergenerational family solidarity is transformed with the birth of a first child and grandchild, and how, in this new context, parents and grandparents find themselves engaged in practices of solidarity, in which – and by which – the role of the grandparent is embodied. The analysis of cross interviews conducted with eleven parents and nine grandparents reveals that the birth of a child is perceived as a turning point, introducing new expectations leading to a reinforcement of intergenerational family solidarity. This was observed through a multiplication of tangible practices of solidarity where much of the experience of sentiments, dispositions and norms newly arouse. The role of the grandparent is located in the continuity of the role previously played as a parent, and is shaped in response to the new needs of the parents. This role is also integrated with the roles of the parents themselves vis-à-vis their children, following a principle of consolidation or differentiation of the parents’ functions. The role of the grandparent is strongly defined by its association with the role of the parent, and yet the grandparent’s role takes form through personal ideals that recall memories, role models, or stand in opposition to counterexamples. It is therefore between the expectations induced by these ideals and the tangible possibilities offered by the situation of intergenerational family solidarity that the role of the grandparent arises, thereby taking shape and form

Full textEvery year, more than 40,000 new immigrants land in Quebec, full of hope for a better life, and with high expectations with regard to their new home. These expectations were largely nourished by the Quebec government marketing efforts abroad to attract qualified immigrants. However, although more and more qualified, newcomers to Quebec are often faced with multiple barriers: non-recognition of their previous training and experience, lack of social networks, discriminatory practices, etc. Even with the help of the various integration groups that provide them with pertinent information, the newly arrived often find it difficult to enter the job market. With time, the ethnocultural networks play an increasingly important informational role and shape the immigrants experience along their migration path. The aim of this exploratory study is to show how the information received and perceived by qualified immigrants influence their migration project and shape their settlement process, thereby influencing their chances for a successful integration.

Full textThe general objective of this thesis is to study the forms of parental and teacher involvement in the school-family-community relationship and the impact of these forms on primary school achievement.. More specifically, it consists of three objectives, which correspond to the three articles of the thesis. The first objective, discussed in the first article, is the theoretical study of the links between the forms of parental involvement (IMPP), the socio-economic status of the family (SES), and the student's academic success. This objective is achieved through a metasynthesis based on the examination of meta-analyzes, the aim here being to identify the links that research has so far established between these three variables, parental involvement (IMPP), academic achievement and socio-economic status of the family (SES), taking into account the methodological difficulties of their study. In particular, the analysis found that different definitions of IMPP, SES, and success measurement tools contribute to differences in the outcomes of studies with respect to the impact of parental involvement on student achievement. The second objective of this thesis is to study the forms of IMPP from the SES of the family, the forms of teacher involvement (IMPE) and to identify the links with the achievements of primary school children. The reference frame is that of the bio-ecological model of Bronfenbrenner (1979) and the model of the shared influence of Epstein (2009). These two models allow the study of IMPP and IMPE from the activities of parents and teachers, the roles of parents and teachers, and interpersonal relationships. To do this, a quantitative approach was adopted. The study sample consisted of 239 students and parents, and 46 teachers. The results show that the forms of the IMPP, such as school-family communication and learning at home, are favored by the parents and at the same time that they do promote success. The study also confirms that the feeling of parental competence is an important vector in motivating parents to get involved in school and at home. Moreover, the results show that parents' education influences the success and the choice of the parents to get involved with their children more than family income. On the other hand, this study explores the forms of IMPE by asking the teachers what it actually put in place to solicit parental involvement at school and at home. For example, teachers promote family-school communication and information for home learning, which the goal of enabling parents to better help children at home. However, while the findings did not clearly reveal which forms of IMPE support success, this study helped to advance knowledge about the teachers’ role and actions in soliciting parent collaboration. The third objective aims at identifying the forms of IMPP and IMPE from the discourses of parents and teachers, and the strengths and weaknesses in the relationship between the actors. To do so, a qualitative approach was favored, using a sample of 14 primary teachers and 45 parents from 7 primary schools in the national capital, Québec. The results revealed the convergence in the forms of the IMPP and the IMPE as school-family communication and home studying. Moreover, by confronting the reasons, expectations and perceptions of the actors, the results revealed that the main reason for their motivation is the student's success. Generally, parents and teachers are sensitive to the same difficulties that surround their relationship: the distance between school and family, the lack of communication, the loss of parent's confidence in the teacher, lack of time etc. The partners recognized that the climate is generally conducive to collaboration. However, the strengths and weaknesses that emerged from the actors' discourse made it possible to nuance and better understand the tensions surrounding the relationship between parents and teachers.

Omnipresent in Quebec, issues about language are to be found in many themes, in religion, politics, education, publishing, the press, cinema, theatre, songs, radio and television ; they are essential to the nation’s cultural identity. Our subject deals with the French-speaking populations in France and Canada. 80 % of French Canadians are found in the province of Quebec. The linguistic problems are prominent and generally trigger a mutual sensitivity between Quebec and France. Our search begins at the end of nineteenth century and finishes in 1977, and is concerned with speech, thinking and opinions on French language, along with its evolution, transmission and representation at the heart of the relations between Quebec and France and French-speaking communities. The particular and unique history between these regions makes them the driving force behind the promotion and the spreading of French in the French-speaking communities and in the world

Full textThrough an ethnographic analysis, this master’s tries to understand the lived work experience of immigrant personal care attendants by studying the practices and professional and social relations found within long-term centers care in Quebec City. It focuses on the place of ethnicity in personal care attendant’s relations with colleagues and patients. This master’s shows that their lived work experience is often difficult: they cannot do their job like they would like to do it, because of the numerous organisational constraints surrounding their work. More so, their experience many difficulties in their interactions with their colleagues and patients. Ethnicity is at the core of this negative lived work experience: it is that by which social interactions are defined in this place of work, and it is a source of exclusion from their coworkers for immigrant personal care attendants. Nevertheless, it occupies a different place in social interactions between personal care attendants and the patients than it does between immigrant personal care attendants and their colleagues: ethnicity is not the primary source of negative care relation with attendant’s patients. Thus, analyzing the relation between personal care attendants and their patients leads to a more positive view of their work experience.

Full textThis thesis examines the link between psychological well-being in adolescence and engagement in sexual contact with nonromantic partners. We call such contact “casual sexual relationships and experiences” (CSREs). Although this association between psychological well-being and CSREs has been studied among young adults, especially among university students, studies on adolescents are scarce. Based on a representative sample of Quebec 14- to 18-year-old high school students from the province of Quebec, this thesis, consisting of two articles, explores gender differences in this association. It also explores differences between two forms of CSREs: friends with benefits relationship (FWB) and one-nigh-stand (ONS). The first article examines five psychological well-being predictors (psychological distress, suicidal ideation, self-esteem, alcohol use, and drug use) of engagement in CSREs among sexually active adolescents (N = 2,601). The second article examines the impact of CSREs on the same five psychological well-being measures six months after the engagement while controlling for prior well-being (N = 2,304). For the first article, multinomial regressions were conducted. For the second article, path analysis and a logistic regression were conducted. The results show that suicidal ideation, alcohol use and drug use are associated with greater engagement in CSREs for girls. Alcohol use is associated with greater engagement in CSREs for boys. CSREs have a small impact (small effect size) on psychological well-being among girls: their psychological distress and use of alcohol and drugs increase six months after such engagement. There is no change in psychological well-being among boys. Avenues of research and interventions are discussed.
