The development of biomarkers, of targeted therapies, have extensively modified practices related to diagnosis, prognosis and treatment Patient care evolved from evidence-based medicine, relying on vast clinical trials, into a more personalised approach based on numerous biological data. Nowadays, gene therapy, the use of stem oells, genome analysis, "omics", carry hopes for this renewed knowedge. Beyond these emblem.tic fields of research, we can reflect upon the existence, nature and evolution of biological culture shared by medical practitioners and researchers alike. According to Michel Morange molecular biology is a set of techniques and discoveries that constitutes a new way of looking at the living as stocki!YJ and transmitting information, especially genetic information. This new conoeption creates explicative power of the main characteristic of the living in medicine: clinical heterogeneity in sickness. By referring to a new level of action which was inaccessible, it becomes possible to imagine new ways for explanation and treatment The ·one gene, one protein" relationship is extended into "ore gene, one protein, one disease" by researchers wishing to make up for a "molecular pathology". How, then, are molecular biology data integrated into medical knowledge and practices?

Full textIn this manuscript we try to solve the following problem: Does the French cancer announcement guideline is a witness of the patient's status evolution in the French health System? Through a historical, epistemological and ethical study of the medical information and the disease announcement concepts, we observe that the cancer announcement guideline actually reflects the changing role of the patient in the French medical System (patient becomes informed-accompanied), the recent direction taken by the system's organization (a power redistribution within the health System) and gives the keys to the systemic recommendations development suited to this emerging System (ethical reforms and epistemological consideration)

Full textAntonio Guaineri (ca;1390-158) and his son Thedore (1449-ca.1509), both from Pavia, enjoyed a full medical career. While the first was physician of Amadeus VIII, Duke of Savoie, then of John Jacob, Marquess of Monferrat, the second served French Kings Charles VII and Louis XII. Both reflect the emergence of the court doctor. By analysing their personal experience. Moreover, the study of the medical cases desribed in their treatises provides us whith a fresh perspective on fiftennth-century doctor-patient relationship. Finally, their books value not only doctors, but other healers as well, among whom legitimate healthcare professional or lay practitioners

Full textMedicine in France is currently undergoing a crisis, with respect to its establishedreferences and values, as a result of the major technical, sociological and scientific changes undergone in the twentieth century. This thesis aims to explore the ins and outs of this crisis through an historical and philosophical study of the emergence and development of modern and contemporary French medicine as a profession, scientific discourse and social practice. From the forming of the medical body to the emergence of an autonomous non-professional discourse, wedefend the idea following which the genesis and evolution of medical discourse, from the eighteenth century to the present day, rests on its ability to answer the foundational problem of the possibility a scientific and technical objectivation of the human subjectivity. Originally of an epistemological nature, this problem reveals itself, through our genealogy, to be rather of a philosophical, ethical and sociopolitical nature, which leads us to conceive a frame of reference by means of which to better understand the contemporary crisis underlying the modern doctorpatient relation. Finally, a case study of the correspondence of Enlightenment's medical doctor Samuel-Auguste Tissot (1728-1797) offers an essential viewpoint from which to reflect on the possibility and conditions of a medical epistemology that ensures the respect of the autonomy andidentity of all subjects, patients and practitioners alike
